Objective:
PD patients commonly exhibit executive dysfunction early in the disease course which may or may not predict further cognitive decline over time. Early emergence of visuospatial and memory impairments, in contrast, are more consistent predictors of an evolving dementia syndrome. Most prior studies using fMRI have focused on mechanisms of executive dysfunction and have demonstrated that PD patients exhibit hyperactivation that is dependent on the degree of cognitive impairment, suggestive of compensatory strategies. No study has evaluated whether PD patients with normal cognition (PD-NC) and PD patients with Mild Cognitive Impairment (PD-MCI) exhibit compensatory activation patterns during visuospatial task performance.
Participants and Methods:10 PD-NC, 12 PD-MCI, and 14 age and sex-matched healthy controls (HC) participated in the study. PD participants were diagnosed with MCI based on the Movement Disorders Society Task Force, Level II assessment (comprehensive assessment). Functional magnetic resonance imaging (fMRI) was performed during a motion discrimination task that required participants to identify the direction of horizontal global coherent motion embedded within dynamic visual noise under Low and High coherence conditions. Behavioral accuracy and functional activation were evaluated using 3 * 2 analyses of covariance (ANCOVAs) (group [HC, PD-NC, PD-MCI] * Coherence [High vs. Low]) accounting for age, sex, and education. Analyses were performed in R (v4.1.2(Team, 2013)).
Results:PD-MCI (0.702± 0.269) patients exhibited significantly lower accuracy on the motion discrimination task than HC (0.853 ± 0.241; p = 0.033) and PD-NC (0.880 ± 0.208; p =0.039). A Group * Coherence interaction was identified in which several regions, including orbitofrontal, posterior parietal and occipital cortex, showed increased activation during High relative to Low coherence trials in the PD patient groups but not in the HC group. HC showed default mode deactivation and frontal-parietal activation during Low relative to High coherence trials that was not evident in the patient groups.
Conclusions:PD-MCI patients exhibited worse visuospatial performance on a motion discrimination task than PD-NC and HC participants and exhibited hyperactivation of the posterior parietal and occipital regions during motion discrimination, suggesting possible compensatory activation.

We give cases in which nearby cycles commute with pushforward from sheaves on the moduli stack of shtukas to a product of curves over a finite field. The proof systematically uses the property that taking nearby cycles of Satake sheaves on the Beilinson–Drinfeld Grassmannian with parahoric reduction is a central functor together with a ‘Zorro's lemma’ argument similar to that of Xue [Smoothness of cohomology sheaves of stacks of shtukas, Preprint (2020), arXiv:2012.12833]. As an application, for automorphic forms at the parahoric level, we characterize the image of tame inertia under the Langlands correspondence in terms of two-sided cells.
A computational simulation-based framework for estimating potential product impact during product design

The impact of engineered products is a topic of concern in society. Product impact may fall under the categories of economic, environmental or social impact, with the last category defined as the effect of a product on the day-to-day life of people. Design teams lack sufficient tools to estimate the social impact of products, and the combined impacts of economic, environmental and social impacts for the products they are designing. This paper aims to provide a framework for the estimation of product impact during product design. To estimate product impact, models of both the product and society are required. This framework integrates models of the product, scenario, society and impact into an agent-based model to estimate product impact. Although this paper demonstrates the framework using only social impact, the framework can also be applied to economic or environmental impacts individually or all three concurrently. Agent-based modelling has been used previously for product adoption models, but it has not been extended to estimate product impact. Having tools for impact estimation allows for optimising the product design parameters to increase the potential positive impact and reduce potential negative impact.

Aims
The pathophysiological entity of a persisting left-sided superior caval vein draining into the roof of the left atrium represents an extreme form of coronary sinus de-roofing. This is an uncommon, but well-documented condition associated with systemic desaturation due to a right-to-left shunt. Depending on the size of the coronary ostium, the defect may also present with right-sided volume loading. We describe two patients, both of whom presented with desaturation, and highlight the important anatomical features underscoring management.
Methods and ResultsBoth patients were managed interventionally with previous assessment of the size of the coronary sinus ostium through cross-sectional imaging. This revealed a restrictive interatrial communication at the right atrial mouth of the coronary sinus in both patients, which permitted an interventional approach, as the residual left-to-right shunt subsequent to closure of the aberrant vessel would be negligible. At intervention, test occlusion of the left superior caval vein allowed assessment of decompressing vessels before successful occlusion using an Amplatzer Vascular Plug.
ConclusionsPersistence of a left superior caval vein draining to the left atrium may be associated with an interatrial communication at the mouth of the unroofed coronary sinus. The ostium of the de-roofed coronary sinus can be atretic, restrictive, normally sized, or enlarged. Careful assessment of the size of this defect is required before treatment. In view of its importance, which has received little attention in the literature to date, we suggest an additional consideration to the classification of unroofed coronary sinus.

A detailed record of the smoking habits of 83 patients treated in hospital for neurosis was compared with national statistics derived from the General Household Survey and Statistics of Smoking in the United Kingdom, and with a control group of patients admitted for treatment of varicose veins. Neurotic patients were more likely to be smokers. They started to smoke at a younger age, smoked more cigarettes and were more likely to inhale deeply. It is concluded that neurotic patients have a greater exposure to the potentially toxic effects of cigarette smoking than non-neurotic individuals.
