Objective:
To better understand parents’ accounts of their prenatal and postnatal experience after prenatal diagnosis of CHD – particularly emotional processing and coping mechanisms – to identify strategies to improve support.
Methods:This single-centre, longitudinal qualitative study included pregnant mothers and their support persons seen in Fetal Cardiology Clinic at Vanderbilt Children’s Hospital from May through August 2019 for probable complex CHD. Twenty-seven individuals from 17 families participated in 62 phone interviews during pregnancy and postpartum: 27 conducted after the initial prenatal cardiology consultation, 15 after a follow-up prenatal visit, and 20 after birth. Applied thematic analysis approach was used to code and analyse transcribed interviews. Coding and codebook revisions occurred iteratively; intercoder reliability was >80%.
Results:Patients included mothers (16 [59%]), fathers (8 [30%]), and other support persons (3 [11%]). Initial fetal diagnoses included a range of moderate to severe CHD. Prenatally, parents sought to maintain hope while understanding the diagnosis; planning for the future rather than focusing on day-to-day was more common if prognoses were better. Postnatally, with confirmation of prenatal diagnoses, parents’ sense of control expanded, and they desired more active engagement in clinical decision making.
Conclusions:To enhance effective communication and support, understanding how parents conceptualise hope in relation to diagnosis and how that may evolve over time is critical. Expectant parents whose child has a significant risk of mortality may demonstrate hope by focusing on positivity. As prognostic uncertainty diminishes postpartum, the parental role on the team may shift, requiring clinicians to provide different support.

This experimental study explores the physical mechanisms by which a transverse jet’s upstream shear layer can transition from being a convective instability to an absolute/global instability as the jet-to-cross-flow momentum flux ratio $J$ is reduced. As first proposed in computational studies by Iyer & Mahesh (J. Fluid Mech., vol. 790, 2016, pp. 275–307), the upstream shear layer just beyond the jet injection may be analogous to a local counter-current shear layer, which is known for a planar geometry to become absolutely unstable at a large enough counter-current shear layer velocity ratio, $R_{1}$. The present study explores this analogy for a range of transverse jet momentum flux ratios and jet-to-cross-flow density ratios $S$, for jets containing differing species concentrations (nitrogen, helium and acetone vapour) at several different jet Reynolds numbers. These studies make use of experimental data extracted from stereo particle image velocimetry as well as simultaneous stereo particle image velocimetry and acetone planar laser-induced fluorescence imaging. They provide experimental evidence for the relevance of the counter-current shear layer analogy to upstream shear layer instability transition in a nozzle-generated transverse jet.

Schoenaster carterensis new species, is an asteroid-like ophiuroid (Echinodermata) from Upper Mississippian (Chesterian) shallow-water carbonates in the Ramey Creek Member of the Slade Formation in northeastern Kentucky. First described in the 1860s from Lower and Middle Mississippian rocks, Schoenaster Meek and Worthen, 1860 is not a well-known fossil genus, but the 39 specimens in this collection permitted further definition of the genus and extended its range by ca. 17 Ma into Late Mississippian (Chesterian) time. The number of specimens also permitted differentiation of growth stages based on average arm length and showed that arm length, disk perimeter, and disk area are interrelated in statistically significant ways. Although replaced by chert, the specimens are nearly intact due to rapid burial as rare constituents in habitat communities distributed among four once-contiguous habitats, including shoal, shoal margin, transitional, and basinal. Most of the ophiuroids were concentrated on firm grounds or hardgrounds in shoal and transitional environments, concentrations that probably reflect substratum stability and the ability to support the ophiuroid's generalist feeding strategy. Many fossil ophiuroid species are known from only a few specimens, severely limiting interpretations about their detailed taxonomy, individual variation, and ecology. In contrast, the greater number of specimens and extensive knowledge of geologic occurrence in this study permitted detailed interpretations regarding the taxonomic, intraspecific, and ecologic attributes of this species, which might be useful in the study of other fossil ophiuroids.

Children in the birth to 5 age range are disproportionately exposed to traumatic events relative to older children, but they are underrepresented in the trauma research literature as well as in the development and implementation of effective clinical treatments and in public policy initiatives to protect maltreated children. Children from ethnic minority groups and those living in poverty are particularly affected. This paper discusses the urgent need to address the needs of traumatized young children and their families through systematic research, clinical, and public policy initiatives, with specific attention to underserved groups. The paper reviews research findings on early childhood maltreatment and trauma, including the role of parental functioning, the intergenerational transmission of trauma and psychopathology, and protective contextual factors in young children's response to trauma exposure. We describe the therapeutic usefulness of a simultaneous treatment focus on current traumatic experiences and on the intergenerational transmission of relational patterns from parent to child. We conclude with a discussion of the implications of current knowledge about trauma exposure for clinical practice and public policy and with recommendations for future research.

Nitride-Arsenide materials were grown by molecular beam epitaxy (MBE) using a radio frequency (rf) nitrogen plasma. The plasma conditions that maximize the amount of atomic nitrogen versus molecular nitrogen were determined using the emission spectrum of the plasma. Under constant plasma source conditions and varying group III flux, the nitrogen concentration in the film is inversely proportional to the group III flux (i. e. the nitrogen sticking coefficient is unity). The relationship between nitrogen concentration in the film and lattice parameter of the film is not linear for nitrogen concentrations above 2.9 mole % GaN, indicating that some nitrogen is incorporated on other locations than the group V lattice sites. For films with these higher nitrogen concentrations, XPS indicates that the nitrogen exists in two configurations: a Gallium-Nitrogen bond and another type of nitrogen complex in which nitrogen is less strongly bonded to Gallium atoms. Annealing removes this nitrogen complex and allows some of the nitrogen to diffuse out of the film. Annealing also improves the crystal quality of GaAsN quantum wells.
