Objective
Inappropriate complementary feeding is one of the major causes of malnutrition in young children in developing countries. We developed an educational intervention, delivered by local health-care providers, aimed at improving complementary feeding practices and child nutrition.
DesignEight townships in Laishui, a rural area in China, were randomly assigned to the educational intervention or control group. A total of 599 healthy infants were enrolled at age 2–4 months and followed up until 1 year of age. In the intervention group, educational messages and enhanced home-prepared recipes were disseminated to caregivers through group trainings and home visits. Questionnaire surveys and anthropometric measurements were taken at baseline and ages 6, 9 and 12 months. Analysis was by intention to treat.
ResultsIt was found that food diversity, meal frequency and hygiene practices were improved in the intervention group. Infants in the intervention group gained 0·22 kg more weight (95 % CI 0·003, 0·45 kg, P = 0·047) and gained 0·66 cm more length (95 % CI 0·03, 1·29 cm, P = 0·04) than did controls over the study period.
ConclusionsFindings from the study suggest that an educational intervention delivered through local health-care providers can lead to substantial behavioural changes of caregivers and improve infant growth.

In birth certificate data for Massachusetts resident births from 1978 to 1982, 12–27% of births purportedly under 31 weeks of gestation were probably misclassified, i.e. had birthweight ≥2500g. Correcting for maldistribution of births removed 34% and 23%, respectively, of black and white births with reported gestational ages <36 weeks but with implausible weights. Percentages of unknown and incomplete reports of last menstrual period were also significantly higher for blacks. After adjustment, preterm black infants weighed less than whites at each gestational age. The proportion of infants <2500g born at term (≥37 weeks gestation) was higher (although not significantly) among blacks. These findings are consistent with hypotheses that low socioeconomic status negatively affects the rate of intrauterine growth.

The mission of public health is to assure the conditions in which people can be healthy and to reduce the occurrence of death and disability attributable to disease and injury. From the distinctive perspective of public health, the target is the health of the population as a whole, with a particular concern for vulnerable populations within the whole. Although public health is grounded in science, the mission and perspective of the field are shaped by the ever-evolving values of the society. Ethics and law are therefore constituent disciplines of public health policy and practice. One of the major challenges confronting contemporary practitioners of public health is the need to broaden and deepen their understanding of legal and ethical aspects of their work. This special issue of the Joumal of Law, Medicine & Ethics responds to this challenge. This article describes how the mission of public health has come to encompass the prevention and treatment of injury and highlights some of the political and ethical controversies now confronting the field.
