A framework for assessing clinical trial site readiness

Clinical trial processes are unnecessarily inefficient and costly, slowing the translation of medical discoveries into treatments for people living with disease. To reduce redundancies and inefficiencies, a group of clinical trial experts developed a framework for clinical trial site readiness based on existing trial site qualifications from sponsors. The site readiness practices are encompassed within six domains: research team, infrastructure, study management, data collection and management, quality oversight, and ethics and safety. Implementation of this framework for clinical trial sites would reduce inefficiencies in trial conduct and help prepare new sites to enter the clinical trials enterprise, with the potential to improve the reach of clinical trials to underserved communities. Moreover, the framework holds benefits for trial sponsors, contract research organizations, trade associations, trial participants, and the public. For novice sites considering future trials, we provide a framework for site preparation and the engagement of stakeholders. For experienced sites, the framework can be used to assess current practices and inform and engage sponsors, staff, and participants. Details in the supplementary materials provide easy access to key regulatory documents and resources. Invited perspective articles provide greater depth from a systems, DEIA (diversity, equity, inclusion, and accessibility) and decentralized trials perspective.
Emerging health data platforms: From individual control to collective data governance

Health data have enormous potential to transform healthcare, health service design, research, and individual health management. However, health data collected by institutions tend to remain siloed within those institutions limiting access by other services, individuals or researchers. Further, health data generated outside health services (e.g., from wearable devices) may not be easily accessible or useable by individuals or connected to other parts of the health system. There are ongoing tensions between data protection and the use of data for the public good (e.g., research). Concurrently, there are a number of data platforms that provide ways to disrupt these traditional health data siloes, giving greater control to individuals and communities. Through four case studies, this paper explores platforms providing new ways for health data to be used for personal data sharing, self-health management, research, and clinical care. The case-studies include data platforms: PatientsLikeMe, Open Humans, Health Record Banks, and unforgettable.me. These are explored with regard to what they mean for data access, data control, and data governance. The case studies provide insight into a shift from institutional to individual data stewardship. Looking at emerging data governance models, such as data trusts and data commons, points to collective control over health data as an emerging approach to issues of data control. These shifts pose challenges as to how “traditional” health services make use of data collected on these platforms. Further, it raises broader policy questions regarding how to decide what public good data should be put towards.

The Mental Capacity Act 2005 introduced a statutory framework for advance decisions, thus promoting autonomous healthcare decision making in advance of loss of capacity. In order to be valid and applicable, and so binding on healthcare professionals, advance decisions refusing treatment (ADRT) must specify treatments to be refused and the circumstances of refusal. Recent case-law indicates that a high level of specificity is required for advance refusals of life-sustaining treatment and, in contrast to a presumption of capacity for contemporaneous decisions, in some circumstances capacity must be demonstrated at the time of making the advance decision. In comparison with the ‘stringent’ requirements for ADRT, the more generic Advance Care Planning (ACP) is gaining more prominence in end of life decision making. The paper explores recent case-law and its impact on the effectiveness of ADRT in practice and compares with ACP.

The study examines the effects of nursing practices on the duration of lactation in middle-class American women. Thirty-two nursing mothers were followed for 2 years post-partum, data being collected at eight home visits by interview and by nursing records kept by the mothers. Those women who nursed frequently (>8/day) during exclusive breastfeeding remained amenorrhoeic longer than infrequent nursers, introduced supplements later and did not resume menses as promptly thereafter. They continued an hour or more of night nursing during supplemented nursing. Duration of exclusive nursing and night nursing after supplementation were the major influences on duration of amenorrhoea. Mothers' age, weight-for-height, and nursing frequency before supplementation showed no significant effect but night nursing after supplementation was a major factor in post-supplementation duration of amenorrhoea. Those women who both supplemented later and maintained an hour of night nursing after supplementing remained amenorrhoeic for 6–10 months longer than those who supplemented early and/or reduced subsequent night nursing.

Summary
Mrs. D is 75 years old and lives at home with her husband. She has a remote history of gastric ulcers and has mild renal insufficiency as a consequence of hypertension. She visits her family physician because of acute worsening of chronic arthritis in her right shoulder. She is having trouble lifting and carrying objects. Her family physician is considering treating Mrs. D with a non-steroidal anti-inflammatory drug.
Mrs. E is 80 years old and lives alone in an apartment. She is fully independent and has never had a serious illness. She prefers not to see doctors. She is admitted to hospital after falling on the stairs and suffering a fracture of the femoral neck. A consultant in internal medicine diagnoses critical aortic stenosis; this is confirmed by echocardiography. The anesthetist visits Mrs. E to discuss the proposed surgery and anesthesia. When he says that serious risks are associated with the surgery, Mrs. E says she does not want to know about them. She wants her hip fixed because she simply cannot live with reduced mobility. The anesthetist feels that he has a duty to disclose the risks of anesthesia.
What is disclosure?
Disclosure refers to the process during which physicians provide information about a proposed medical investigation or treatment to the patient. Disclosure, along with capacity, understanding, voluntariness, and consent, makes up the main elements of informed consent (Beauchamp and Childress, 2001).
