Studies on the effects of caffeine on health, while numerous, have produced inconsistent results. One of the most uncertain and controversial effects is on pregnancy outcome. Studies have produced conflicting results due to a number of methodological variations. The major challenge is the accurate assessment of caffeine intake. The aim of the present study was to explore different methods of assessing caffeine exposure in pregnant women. Twenty-four healthy pregnant women from the UK city of Leeds completed both a detailed questionnaire, the caffeine assessment tool (CAT) designed specifically to assess caffeine intake and a prospective 3 d food and drink diary. The women also provided nine saliva samples over two consecutive days for estimation of caffeine and a metabolite (paraxanthine). Caffeine intakes from the CAT and diary showed adequate agreement (intra-class correlation coefficient of 0·5). For saliva caffeine and paraxanthine measures, the between-sample variation (within the same woman) was greater than between-woman and between-day variation. However, there was still adequate agreement between these measures and the CAT. The CAT is a valuable tool that is now being used in a large prospective study investigating caffeine's role in pregnancy outcome.

Ovarian torsion is an uncommon gynecological emergency that requires prompt recognition and treatment. It may present with nonspecific signs and symptoms, and should be considered in any female with acute abdominal pain. The diagnosis is based on an awareness of the relevant risk factors, the clinical presentation, and a high index of suspicion. Timely investigation and management can make the difference between ovarian loss and salvage — an outcome of great importance in the population of reproductive age females.

This new project relies on the capabilities collocated at Los Alamos in the Trident laser facility of long-pulse laser drive, for laser-plasma formation, and high-intensity short-pulse laser drive, for relativistic laser-matter interaction experiments. Specifically, we are working to understand quantitatively the physics that underlie the generation of laser-driven MeV/nucleon ion beams, in order to extend these capabilities over a range of ion species, to optimize beam generation, and to control those beams. Furthermore, we intend to study the interaction of these novel laser-driven ion beams with dense plasmas, which are relevant to important topics such as the fast-ignition method of inertial confinement fusion (ICF), weapons physics, and planetary physics. We are interested in irradiating metallic foils with the Trident short-pulse laser to generate medium to heavy ion beams (Z = 20–45) with high efficiency. At present, target-surface impurities seem to be the main obstacle to reliable and efficient acceleration of metallic ions in the foil substrate. In order to quantify the problem, measurements of surface impurities on typical metallic-foil laser targets were made. To eliminate these impurities, we resorted to novel target-treatment techniques such as Joule-heating and laser-ablation, using a long-pulse laser intensity of ∼ 1010 W/cm2. Our progress on this promising effort is presented in this paper, along with a summary of the overall project.

Background
It is not clear if the frequency of deliberate self-harm (DSH) is the same in patients taking different pharmacological classes of antidepressant drugs.
AimsTo compare the frequency of DSH in patients who had been prescribed a tricyclic antidepressant (TCA) or a selective serotonin reuptake inhibitor (SSRI) prior to the DSH event.
MethodThis was a prospective study in 2776 consecutive DSH cases attending an accident and emergency department. The incidence of DSH in TCA-treated cases and SSRI-treated cases is expressed as number of DSH events per 10 000 prescriptions of each antidepressant.
ResultsSignificantly more DSH events occurred following the prescription of an SSRI than that of a TCA (P < 0.001). The occurrence of DSH was highest with fluoxetine and lowest with amitriptyline.
ConclusionsMerely prescribing safer-in-overdose antidepressants is unlikely to reduce the overall morbidity from DSH.

Size-monodisperse, stable 15 Å diameter silicon nanocrystals were synthesized in significant quantities using supercritical octanol as a capping ligand. The silicon nanocrystals exhibit an indirect band gap with discrete electronic transitions in the absorbance and photoluminescence excitation (PLE) spectra. The octanol-capped clusters show efficient blue band-edge photoemission with a luminescence quantum yield of 23 % at room temperature.
