Recombinant Factor VIIa plus Surgery for Intracerebral Hemorrhage

Background:
Hyperacute surgical evacuation of intracerebral hemorrhage is associated with a high rebleeding rate. The peri-operative administration of rFVIIa to patients with intracerebral hemorrhage may decrease the frequency of post-operative hemorrhage, and improve outcome.
Methods:Patients receiving recombinant activated factor VII (rFVIIA) therapy immediately prior to acute surgery were collected at two centres. The intracerebral hemorrhage (ICH) score and ICH Grading Scale were determined, as was long-term outcome using the modified Rankin Scale. Residual/ recurrent clot was evaluated by comparing pre-operative to post-operative CT scans.
Results:Fifteen patients with intracerebral hemorrhage received 40-90 μg/kg of rFVIIa and underwent surgical hematoma evacuation at a median time of five hours following symptom onset. Median pre-operative clot volume was 60 ml, decreasing to 2 ml post-operatively. There were no thromboembolic adverse events. Thirteen patients survived, 11 (73%) were independent, and two (13%) had a moderate to severe disability. These outcomes were significantly better than expected based on the median ICH score (40% mortality) and based on median ICH Grading Scale (18% good outcome).
Conclusions:The pre or peri-operative administration of rFVIIa resulted in minimal residual or recurrent hematoma volume and may be an important adjunct to surgery in patients with intracerebral hemorrhage.

Distance sampling is a common and increasingly used method of estimating animal abundance in conservation and management programmes. The precision of a distance sampling survey can be estimated from the field data, but accuracy can only be evaluated by comparison with the true population size. For wild mammal populations, such opportunities are rare. The high-density badger Meles meles population at Woodchester Park in Gloucestershire, U.K. has been routinely and intensively live-trapped for the past 30 years. The estimates of abundance based on mark–recapture analyses of the live-trapping data provided a reliable baseline against which to evaluate distance sampling for a badger population. A distance sampling survey was carried out in autumn 2002 by using spotlighting along line transects. Owing to the impractical nature of spotlighting in closed habitats, such as woodland, surveys were conducted in open habitat only. A radio-tracking study provided behavioural data that were used to estimate the proportion of the population available for sampling in open habitats. This was incorporated as a multiplier into the DISTANCE analysis. The estimate of abundance from distance sampling was 76 badgers (% CV=42.2), using a multiplier based on radio-tracking data from autumn only. When the multiplier was derived from radio-tracking data from across the whole year, based on a larger sample of tracked badgers, the distance sampling abundance estimate was 77 badgers (% CV=25.9). These compared favourably to the mark–recapture estimate of 68 badgers (% C.V=7.4). The lack of precision in the distance sampling density estimate was largely owing to the inclusion of the multiplier. Distance sampling combined with data on habitat use can produce reliable estimates of badger abundance in areas of medium to high density, particularly in landscapes of primarily open habitat.

Regions of the brain involved in language and attention were studied using [18F]-fluorodeoxyglucose in PET. In nine chronic DSM–III schizophrenic patients who had persistent auditory hallucinations, ten who had recovered from hallucinations and ten normal controls. In none of the regions examined was metabolic activity significantly different in hallucinating patients compared with that in other groups. However, a pattern of seven significant correlations of metabolic activity between language regions and between frontal and parietal cortex characterised the hallucinating but not the other groups. Three of the seven correlations were significantly greater in hallucinating patients than in the two other groups, and six were greater in hallucinating patients than controls. Metabolism in Broca's region and its right-hemisphere homologue correlated positively and significantly in the hallucinating group, as it did in anterior cingulate and left superior temporal areas, and in right frontal and parietal areas. Hallucination ratings correlated with metabolism in the anterior cingulate region.
Polarization of IRAS Quasars and the Inner Structure of Active Galactic Nuclei

We have found that 4 new, bright IRAS quasars, out of 7 observed, have strong, non-variable, wavelength-dependent polarization. Three show degrees of polarization, pλ, increasing from infrared to UV wavelengths (Fig. 1), which implies a combination of a polarized, scattered spectrum and a much redder, unpolarized spectrum. Detailed IR and optical polarimetry and spectrophotometry of one, IRAS 13349+2438 (Wills et al.), shows a polarized flux spectrum, pλxFλ, (continuum and Pa α, Hα, and Hβ broad hydrogen lines) typical of unreddened, luminous quasars. This suggests that the path of scattered light from a central, luminous quasar is low in dust and that the polarization of the scattered spectrum is wavelength independent. The latter is most easily explained by electron scattering although the data do not exclude dust scattering. When this polarized flux spectrum is subtracted from the total spectrum, we are left with a very reddened line and continuum spectrum, E(B-V) = 0.3 to 0.7, which we attribute to the same luminous quasar seen through a thick dusty torus. The angle of polarization is parallel to the major axis of the r-band image, presumed to be that of the host galaxy. If the torus is in the plane of the galaxy, the axial ratio suggests a viewing angle of 40° to the plane of the torus. Fig. 2 illustrates the geometry. The appearance of the quasar at optical and UV wavelengths will depend strongly on viewing angle, suggesting that present samples of quasars selected by colours, optical flux density, or quasi-stellar appearance, may be seriously biased, with important consequences for studies of the space density and evolution of AGN.
