Dysregulation of immune response in otitis media

Objective
Otitis media (OM) is a common reason for children to be prescribed antibiotics and undergo surgery but a thorough understanding of disease mechanisms is lacking. We evaluate the evidence of a dysregulated immune response in the pathogenesis of OM.
MethodsA comprehensive systematic review of the literature using search terms [otitis media OR glue ear OR AOM OR OME] OR [middle ear AND (infection OR inflammation)] which were run through Medline and Embase via Ovid, including both human and animal studies. In total, 82 955 studies underwent automated filtering followed by manual screening. One hundred studies were included in the review.
ResultsMost studies were based on in vitro or animal work. Abnormalities in pathogen detection pathways, such as Toll-like receptors, have confirmed roles in OM. The aetiology of OM, its chronic subgroups (chronic OM, persistent OM with effusion) and recurrent acute OM is complex; however, inflammatory signalling mechanisms are frequently implicated. Host epithelium likely plays a crucial role, but the characterisation of human middle ear tissue lags behind that of other anatomical subsites.
ConclusionsTranslational research for OM presently falls far behind its clinical importance. This has likely hindered the development of new diagnostic and treatment modalities. Further work is urgently required; particularly to disentangle the respective immune pathologies in the clinically observed phenotypes and thereby work towards more personalised treatments.

Gravitational waves from coalescing neutron stars encode information about nuclear matter at extreme densities, inaccessible by laboratory experiments. The late inspiral is influenced by the presence of tides, which depend on the neutron star equation of state. Neutron star mergers are expected to often produce rapidly rotating remnant neutron stars that emit gravitational waves. These will provide clues to the extremely hot post-merger environment. This signature of nuclear matter in gravitational waves contains most information in the 2–4 kHz frequency band, which is outside of the most sensitive band of current detectors. We present the design concept and science case for a Neutron Star Extreme Matter Observatory (NEMO): a gravitational-wave interferometer optimised to study nuclear physics with merging neutron stars. The concept uses high-circulating laser power, quantum squeezing, and a detector topology specifically designed to achieve the high-frequency sensitivity necessary to probe nuclear matter using gravitational waves. Above 1 kHz, the proposed strain sensitivity is comparable to full third-generation detectors at a fraction of the cost. Such sensitivity changes expected event rates for detection of post-merger remnants from approximately one per few decades with two A+ detectors to a few per year and potentially allow for the first gravitational-wave observations of supernovae, isolated neutron stars, and other exotica.

Background
Mastoiditis is an otological emergency, and cross-sectional imaging has a role in the diagnosis of complications and surgical planning. Advances in imaging technology are becoming increasingly sophisticated and, by the same token, the ability to accurately interpret findings is essential.
MethodsThis paper reviews common and rare complications of mastoiditis using case-led examples. A radiologist-derived systematic checklist is proposed, to assist the ENT surgeon with interpreting cross-sectional imaging in emergency mastoiditis cases when the opinion of a head and neck radiologist may be difficult to obtain.
ResultsA 16-point checklist (the ‘mastoid 16’) was used on a case-led basis to review the radiological features of both common and rare complications of mastoiditis; this is complemented with imaging examples.
ConclusionAcute mastoiditis has a range of serious complications that may be amenable to treatment, once diagnosed using appropriate imaging. The proposed checklist provides a systematic approach to identifying complications of mastoiditis.

Filamentary structures can form within the beam of protons accelerated during the interaction of an intense laser pulse with an ultrathin foil target. Such behaviour is shown to be dependent upon the formation time of quasi-static magnetic field structures throughout the target volume and the extent of the rear surface proton expansion over the same period. This is observed via both numerical and experimental investigations. By controlling the intensity profile of the laser drive, via the use of two temporally separated pulses, both the initial rear surface proton expansion and magnetic field formation time can be varied, resulting in modification to the degree of filamentary structure present within the laser-driven proton beam.

Ceramic clays are characterized by a method that stems from the IL/MA procedure proposed by Keeling (1958a, b) but which is more rapid to perform and more basic in concept. By thermogravimetric methods an estimate is made of both OH water and interlayer plus physically adsorbed water. These are plotted against each other in a manner similar to the IL/MA plot. The composition of the clay can be characterized uniquely on such a diagram by estimating either the quartz or the free silica contents of a sample, its silt, and < 2μ fraction, and plotting for each on a 100% clay basis their OH water, and interlayer and physically adsorbed water. Simple methods are given for assessing these factors. The determination of interlayer water content could form the basis of a method of plant control of clay composition in a way similar to that proposed by Keeling using moisture adsorption determinations.

Background
A range of endophenotypes characterise psychosis, however there has been limited work understanding if and how they are inter-related.
MethodsThis multi-centre study includes 8754 participants: 2212 people with a psychotic disorder, 1487 unaffected relatives of probands, and 5055 healthy controls. We investigated cognition [digit span (N = 3127), block design (N = 5491), and the Rey Auditory Verbal Learning Test (N = 3543)], electrophysiology [P300 amplitude and latency (N = 1102)], and neuroanatomy [lateral ventricular volume (N = 1721)]. We used linear regression to assess the interrelationships between endophenotypes.
ResultsThe P300 amplitude and latency were not associated (regression coef. −0.06, 95% CI −0.12 to 0.01, p = 0.060), and P300 amplitude was positively associated with block design (coef. 0.19, 95% CI 0.10–0.28, p < 0.001). There was no evidence of associations between lateral ventricular volume and the other measures (all p > 0.38). All the cognitive endophenotypes were associated with each other in the expected directions (all p < 0.001). Lastly, the relationships between pairs of endophenotypes were consistent in all three participant groups, differing for some of the cognitive pairings only in the strengths of the relationships.
ConclusionsThe P300 amplitude and latency are independent endophenotypes; the former indexing spatial visualisation and working memory, and the latter is hypothesised to index basic processing speed. Individuals with psychotic illnesses, their unaffected relatives, and healthy controls all show similar patterns of associations between endophenotypes, endorsing the theory of a continuum of psychosis liability across the population.

Background
Despite improvements in outcomes after completion of the Fontan circulation, long-term functional state varies. We sought to identify pre- and postoperative characteristics associated with overall function.
Methods and ResultsWe analyzed data from 476 survivors with the Fontan circulation enrolled in the Pediatric Heart Network Fontan Cross-sectional Study. Mean age at creation of the Fontan circulation was 3.4 plus or minus 2.1 years, with a range from 0.7 to 17.5 years, and time since completion was 8.7 plus or minus 3.4 years, the range being from 1.1 to 17.3 years. We calculated a functional score for the survivors by averaging the percentile ranks of ventricular ejection fraction, maximal consumption of oxygen, the physical summary score for the Child Health Questionnaire, and a function of brain natriuretic peptide. The mean calculated score was 49.5 plus or minus 17.3, with a range from 3 to 87. After adjustment for time since completion of the circulation, we found that a lower score, and hence worse functional state, was associated with: right ventricular morphology (p less than 0.001), higher ventricular end-diastolic pressure (p equals 0.003) and lower saturations of oxygen (p equals 0.047) prior to completion of the Fontan circulation, lower income for the caregiver (p equals 0.003), and, in subjects without a prior superior cavopulmonary anastomosis, arrhythmias after completion of the circulation (p equals 0.003). The model explained almost one-fifth (18%) of the variation in the calculated scores. The score was not associated with surgical centre, sex, age, weight, fenestration, or the period of stay in hospital after completion of the Fontan circuit. A validation model, using 71 subjects randomly excluded from initial analysis, weakly correlated (R equals 0.17, p equals 0.16) with the score calculated from the dataset.
ConclusionsRight ventricular morphology, higher ventricular end-diastolic pressure and lower saturations of oxygen prior to completion of the Fontan circuit, lower income for the provider of care, and arrhythmias after creation of the circuit, are all associated with a worse functional state. Unmeasured factors also influence outcomes.

Aphidius ervi Hal. parasitised natural populations of Acyrthosiphon pisum (Harris), Sitobion avenae (F.) and Microlophium carnosum (Buckt.) in Britain but parasitised few examples of S. avenae placed on trap plants in winter wheat, nettle-grass edge areas or lucerne-wheat strip crops. In laboratory experiments, Aphidius ervi reared from Acyrthosiphon pisum did not parasitise M. carnosum, and significantly fewer mummies were formed on S. avenae than on A. pisum. After rearing Aphidius ervi for 4–5 generations on S. avenae, similar numbers of mummies were formed on both hosts. The switch of hosts from Acyrthosiphon pisum to S. avenae was associated with the loss of certain parasitoid esterase enzyme bands revealed following electrophoretic separation of parasitoid proteins. Slow adaptation to new aphid hosts by polyphagous parasitoids such as Aphidius ervi may reduce the usefulness of reservoirs as a source of beneficial species.

An outbreak of Salmonella paratyphi B infection in the UK associated with a fish-and-chip shop is reported. The source of infection for the first three cases was believed to be a food handler who was infected overseas 6 years earlier. His wife whose faeces and urine were originally culture negative continued to run the shop but subsequently her faeces became positive on one occasion. She was considered to have been the source of two further cases, and secondary household spread of infection from these two cases resulted in one symptomatic and two asymptomatic infections. A second household contact of the proprietor also became a faecal excretor 2 months later. We recommend that food handlers living in households or in intimate contact with eases or carriers of S. paratyphi B should be put off work until all household contacts cease excreting the organism.

1. An attempt has been made to follow up a consecutive series of seventy-two patients for the presence of Phase 1 C.F. antibody approximately 2 years or more after the acute illness. Fifty-one of the series were tested.
2. Fifteen of the fifty-one patients had detectable amounts of antibody, generally in low titre.
3. The presence of Phase 1 antibody correlated well, in older age-groups, with the duration of convalescence following the acute illness. There was no correlation with the duration of fever.
4. All cases with Phase 1 antibody also had Phase 2 antibody, usually in slightly higher titre. In no case in which Phase 2 antibody was absent or present in low titre was Phase 1 antibody found.
5. It is suggested that the presence of Phase 1 C.F. antibody is an indication of past persistent infection. It cannot necessarily be concluded that it is an indication of present persisting infection.

On a New Palaeolithic Industry from the Norfolk Coast

The Glacial deposits along the Norfolk and Suffolk coasts are important to geologists and archaeologists, as some of the formations here, even those which are later than the Cromer Forest Bed Series, are earlier than those of many other districts. Finds of Palaeolithic implements in this area have not so far been abundant, but an industry was recorded by us in 1938 (see Nature, 19th November, 1938, p. 912), and it is now possible to give greater details about its mode of occurrence. The geological dating of this industry will be discussed first, and the implements will then be considered from the archaeological point of view.
