Treatments for bulimia nervosa: a network meta-analysis

Background
Bulimia nervosa (BN) is a severe eating disorder that can be managed using a variety of treatments including pharmacological, psychological, and combination treatments. We aimed to compare their effectiveness and to identify the most effective for the treatment of BN in adults.
MethodsA search was conducted in Embase, Medline, PsycINFO, and Central from their inception to July 2016. Studies were included if they reported on treatments for adults who fulfilled diagnostic criteria for BN. Only randomised controlled trials (RCTs) that examined available psychological, pharmacological, or combination therapies licensed in the UK were included. We conducted a network meta-analysis (NMA) of RCTs. The outcome analysed was full remission at the end of treatment.
ResultsWe identified 21 eligible trials with 1828 participants involving 12 treatments, including wait list. The results of the NMA suggested that individual cognitive behavioural therapy (CBT) (specific to eating disorders) was most effective in achieving remission at the end of treatment compared with wait list (OR 3.89, 95% CrI 1.19–14.02), followed by guided cognitive behavioural self-help (OR 3.81, 95% CrI 1.51–10.90). Inconsistency checks did not identify any significant inconsistency between the direct and indirect evidence.
ConclusionsThe analysis suggested that the treatments that are most likely to achieve full remission are individual CBT (specific to eating disorders) and guided cognitive behavioural self-help, although no firm conclusions could be drawn due to the limited evidence base. There is a need for further research on the maintenance of treatment effects and the mediators of treatment outcome.

Inflammation of the mammary gland following bacterial infection, commonly known as mastitis, affects all mammalian species. Although the aetiology and epidemiology of mastitis in the dairy cow are well described, the genetic factors mediating resistance to mammary gland infection are not well known, due in part to the difficulty in obtaining robust phenotypic information from sufficiently large numbers of individuals. To address this problem, an experimental mammary gland infection experiment was undertaken, using a Friesian-Jersey cross breed F2 herd. A total of 604 animals received an intramammary infusion of Streptococcus uberis in one gland, and the clinical response over 13 milkings was used for linkage mapping and genome-wide association analysis. A quantitative trait locus (QTL) was detected on bovine chromosome 11 for clinical mastitis status using micro-satellite and Affymetrix 10 K SNP markers, and then exome and genome sequence data used from the six F1 sires of the experimental animals to examine this region in more detail. A total of 485 sequence variants were typed in the QTL interval, and association mapping using these and an additional 37 986 genome-wide markers from the Illumina SNP50 bovine SNP panel revealed association with markers encompassing the interleukin-1 gene cluster locus. This study highlights a region on bovine chromosome 11, consistent with earlier studies, as conferring resistance to experimentally induced mammary gland infection, and newly prioritises the IL1 gene cluster for further analysis in genetic resistance to mastitis.

Photoelectrochemical (PEC) water splitting for hydrogen production is a promising technology that uses sunlight and water to produce renewable hydrogen with oxygen as a by-product. In the expanding field of PEC hydrogen production, the use of standardized screening methods and reporting has emerged as a necessity. This article is intended to provide guidance on key practices in characterization of PEC materials and proper reporting of efficiencies. Presented here are the definitions of various efficiency values that pertain to PEC, with an emphasis on the importance of solar-to-hydrogen efficiency, as well as a flow chart with standard procedures for PEC characterization techniques for planar photoelectrode materials (i.e., not suspensions of particles) with a focus on single band gap absorbers. These guidelines serve as a foundation and prelude to a much more complete and in-depth discussion of PEC techniques and procedures presented elsewhere.

This tablet was found on 10th December, 1960, by Mr. Roger Wilson of Thrumpton, Notts, after ploughing had taken place on the lower slope of Red Hill in the parish of Ratcliffe-on-Soar, Notts. Roman pottery, bone counters and other objects have been found on Red Hill, which appears to be a Roman site. It is at the junction of the rivers Trent and Soar (where also Nottinghamshire and Leicestershire meet). The tablet was folded roughly into three when found. The material, discoloured by corrosion and of a browny copper colour, proved on analysis to be essentially lead (in an impure state). Its maximum measurements are 8·4 cm. in height, 5·7 cm. in width. Both sides have been lightly incised, perhaps with a needle, with Latin cursive writing. The hand is regularly formed and clear and shows skill on the part of the scribe. To judge from parallels from Egypt, it should be assigned to the end of the second or the early part of the third century A.D. For the earlier limit compare the receipt P. Lond. 730 (plate in Écriture Latine 26) of A.D. 167; and for the later, P. Oxy. VIII, 1114, a declaration of inheritance of A.D. 237 (plate ibid., pl. 7). Both of these, however, show a slope to the right, whereas in this tablet the slope is to the left in the older manner.

The seventeen pieces of wood which together form the tablet here described (Guildhall Museum E.R. no. 444) were found on 10th February, 1959, 15 feet below street level in a timber-lined pit on the site of Temple House, Queen Street, in the City of London. Ink writing on the wood was at once recognized. Even in the wet and sodden state of the tablet Latin words could be made out and a preliminary copy was taken. The surface which showed writing was twice photographed by Mr. M. B. Cookson, of the Institute of Archaeology of London University, using infra-red light. The other surface, on which no writing could be discerned, was not photographed, unfortunately, since when the wood was bleached some traces of ink appeared on this side too, but were subsequently lost. The wood was then dried out, using the carbo-wax process, in the laboratory of the Guildhall Museum; to everyone's disappointment the ink was found to have vanished, and it seems as though the gum adhesive of the carbon ink must have been interfered with. The account given here of the contents of the tablet therefore rests entirely on a preliminary examination of the wood while it was still wet and on the photographs. One of the latter, together with a line drawing, is reproduced in plate XI.

The find in Roman Britain of a Latin writing tablet bearing a still legible ink inscription is of so unusual a nature that it has seemed worthwhile to offer a provisional account of it to readers of the Journal. It is probable that more of the text will eventually prove readable than I have managed to decipher; for I have failed to obtain a continuous text. Apart from the difficulties inherent in reading Latin cursive writing, the faintness at many places of the ink, discoloration of the surface, and above all, the frequently misleading horizontal grain of the wood offer obstacles that I have failed to surmount. It is also possible that laboratory treatment will succeed at some future date in separating the ink more clearly from its background, at least for photographic purposes. Experiments to achieve this are in progress, but they must inevitably proceed cautiously and therefore slowly. I believe, however, that enough of the writing has been made out to establish with reasonable certainty the type of text concerned, and that further success in decipherment, while it will fill in details, will not change the essential framework into which those details are to be fitted. It has seemed right, in consequence, not to delay longer but to submit the results achieved so far to the judgment of colleagues and hope that their criticisms and suggestions will make good the writer's failings. The accompanying plate (XII) will supply a means of check and supplement.
