The war in Ukraine raises concerns for potential hazards of radiological and nuclear incidents. Children are particularly vulnerable in these incidents and may need pharmaceutical countermeasures, including antidotes and cytokines. Searches found no published study comparing pediatric indications and dosing among standard references detailing pediatric medications for these incidents. This study addresses this gap by collecting, tabulating, and disseminating this information to healthcare professionals caring for children. Expert consensus chose the following references to compare their pediatric indications and dosing of medical countermeasures for radiation exposure and internal contamination with radioactive materials: Advanced Hazmat Life Support (AHLS) for Radiological Incidents and Terrorism, DailyMed, Internal Contamination Clinical Reference, Medical Aspects of Radiation Incidents, and Medical Management of Radiological Casualties, as well as Micromedex, POISINDEX, and Radiation Emergency Medical Management (REMM). This is the first study comparing pediatric indications and dosing for medical countermeasures among commonly used references for radiological and nuclear incidents.

Background
Psychiatry is facing major challenges during the current coronavirus disease 2019 (COVID)-19 pandemic. These challenges involve its actual and perceived role within the medical system, in particular how psychiatric hospitals can maintain their core mission of attending to people with mental illness while at the same time providing relief to overstretched general medicine services. Although psychiatric disorders comprise the leading cause of the global burden of disease, mental healthcare has been deemphasised in the wake of the onslaught of the pandemic: to make room for emergency care, psychiatric wards have been downsized, clinics closed, psychiatric support systems discontinued and so on. To deal with this pressing issue, we developed a pandemic contingency plan with the aim to contain, decelerate and, preferably, avoid transmission of COVID-19 and to enable and maintain medical healthcare for patients with mental disorders.
AimsTo describe our plan as an example of how a psychiatric hospital can share in providing acute care in a healthcare system facing an acute and highly infectious pandemic like COVID-19 and at the same time provide support for people with mental illness, with or without a COVID-19 infection.
MethodThis was a descriptive study.
ResultsThe plan was based on the German national pandemic strategy and several legal recommendations and was implemented step by step on the basis of the local COVID-19 situation. In addition, mid- and long-term plans were developed for coping with the aftermath of the pandemic.
ConclusionsThe plan enabled the University Hospital to maintain medical healthcare for patients with mental disorders. It has offered the necessary flexibility to adapt its implementation to the first and second waves of the COVID-19 pandemic in Germany. The plan is designed to serve as an easily adaptable blueprint for psychiatric hospitals around the world.

Treatment resistant schizophrenia (TRS) is one of the most disabling of psychiatric disorders, affecting about 1/3 of patients. First-line treatments include both atypical and typical antipsychotics. The original atypical, clozapine, is a final option, and although it has been shown to be the only effective treatment for TRS, many patients do not respond well to clozapine. Clozapine use is related to adverse events, most notably agranulocytosis, a potentially fatal blood disorder which affects about 1% of those prescribed clozapine and requires regular blood monitoring. This as a barrier to prescription and there is a long delay in access for TRS patients, of five or more years, from first antipsychotic prescription. Better tools to predict treatment resistance and to identify risk of adverse events would allow faster and safer access to clozapine for patients who are likely to benefit from it. The CRESTAR project (www.crestar-project.eu) is a European Framework 7 collaborative project that aims to develop tools to predict i) treatment response, particularly patients who are less likely to respond to usual antipsychotics, indicating treatment with clozapine as early as possible, ii) patients who are at high or low risk of adverse events and side effects, iii) extreme TRS patients so that they can be stratified in clinical trials for novel treatments. CRESTAR has addressed these questions by examining genome-wide association data, genome sequence, epigenetic biomarkers and epidemiological data in European patient cohorts characterized for treatment response, and adverse drug reaction using data from clozapine therapeutic drug monitoring and linked National population medical and pharmacy databases, to identify predictive factors. In parallel CRESTAR will perform health economic research on potential benefits, and ethics and patient-centred research with stakeholders.

Studies of the atomic phase of the interstellar medium, via the 21–cm spectral line of neutral hydrogen (H I), play a key rôle in our attempts to understand the structure and evolution of disk galaxies. We present here results from The H I Nearby Galaxy Survey (THINGS) and focus on the mass distribution as derived from the observed kinematics, and on the link between gas and star formation rate surface density, i.e., the Schmidt–Kennicutt law. Also, we briefly dwell on the wealth and wide variety of structures, often outlining what seem to be expanding shells surrounding sites of recent, massive star formation.

We present several results from our analysis of dwarf irregular galaxies culled from The HI Nearby Galaxy Survey (THINGS). We analyse the rotation curves of two galaxies based on “bulk” velocity fields, i.e. velocity maps from which random non–circular motions are removed. We confirm that their dark matter distribution is best fit by an isothermal halo model. We show that the star formation properties of dIrr galaxies resemble those of the outer parts of larger, spiral systems. Lastly, we study the large scale (3–D) distribution of the gas, and argue that the gas disk in dIrrs is thick, both in a relative, as well as in an absolute sense as compared to spirals. Massive star formation through subsequent supernova explosions is able to redistribute the bulk of the ISM, creating large cavities. These cavities are often larger, and longer–lived than in spiral galaxies.

From January 1996 to April 2001, we treated eight patients with subdivided left atrium. Their ages at diagnosis ranged from newborn to 6.4 years. Prominent clinical symptoms were failure to thrive, respiratory symptoms, demand for oxygen, and congestive cardiac failure. Diagnosis was made by transthoracic echocardiography in all cases. Cardiac catheterization was necessary only in those patients who had associated cardiac anomalies, or suspected signs of pulmonary hypertension. In 7 patients, surgery was performed immediately after diagnosis, but one preterm infant died before operation due to neonatal sepsis and respiratory distress syndrome. In the postoperative period, one patient developed a severe capillary leak syndrome, and died due to irreversible congestive cardiac failure. The other 6 patients have all been followed up, with good results in the short- and intermediate-term at a mean of 34.3 ± 20.2 months. The infants were thriving, had a reduction in the frequency of infections of the respiratory tract, no significant arrhythmias, and showed early recovery from pulmonary hypertension and right ventricular hypertrophy.

Summary
Introduction: some historical remarks
The earliest detections of luminous X-ray sources (LX ≳ 1036 erg s−1) in globular clusters were made with the Uhuru and OSO-7 Observatories (Giacconi et al. 1972, 1974; Clark, Markert & Li, 1975; Canizares & Neighbours, 1975). About 10% of the luminous X-ray sources in our Galaxy are found in globular clusters. This implies that the probability (per unit mass) of finding a luminous X-ray source in a globular cluster is about two to three orders of magnitude higher than of finding one in the rest of our Galaxy (Gursky 1973; Katz 1975). Clearly, the conditions in globular clusters are very special in that they must be very efficient breeding grounds for X-ray binaries. For reviews that reflect the ideas in the late seventies and early eighties, see Lewin (1980), Lewin & Joss (1983), van den Heuvel (1983) and Verbunt & Hut (1987). At that time there was no evidence for a substantial population of binaries in globular clusters; e.g., Gunn and Griffin (1979) did not find a single binary in a spectroscopic search for radial velocity variations of 111 bright stars in M3.
Clark (1975) suggested that the luminous cluster sources are binaries formed by capture from the remnants of massive stars. Fabian, Pringle and Rees (1975) specified that they are formed via tidal capture of neutron stars in close encounters with main-sequence stars. Sutantyo (1975) suggested direct collisions between giants and neutron stars as a formation mechanism.
Hazardous Materials Incidents: A One-Year Retrospective Review in Central California

Objective:
This study describes the epidemiology of hazardous materials (hazmat) incidents in Fresno County, California, and analyzes the emergency medical services (EMS) response to these incidents.
Setting:The study area has a population of 635,000 people living in an area of 6,004 square miles.
Design:All Hazmat Emergency Response Team (HERT) reports and related prehospital, emergency department, and inpatient records from ljuly 1988 through 30 June 1989 were reviewed retrospectively.
Results:There were 107 hazmat incidents involving 156 materials consisting of pesticides (24.4%), miscellaneous chemicals (17.3%), corrosives (16.7%), petroleum products (13.5%), airborne toxins (10.2%), organic solvents (7.7%), unidentified chemicals (5.1%), infectious medical waste (1.9%), empty containers with radioactive warning symbols but without detectable radiation (1.3%), heavy metals (1.3%), and alkali metals (0.6%). In ten (9%) of the 107 incidents, 68 patients required on-scene evaluation and 26 patients were transported to emergency departments. Four of these patients required admission, three because of injuries due to vehicular accidents, and one because of a coincidental cere-brovascular accident. Five incidents produced multiple victims from exposures to airborne toxins, accounting for 63 (93%) of 68 patients. Spills of solid or liquid pesticides occurred in four incidents involving patients. Ambulance personnel and/or equipment became contaminated in three of these four incidents.
Conclusions:1) Ambulances should be dispatched selectively to hazmat incidents because only 9% of incidents involved patients. 2) training should emphasize personnel protection and proper patient decontamination to help prevent contamination of EMS personnel and equipment. 3) Preparation of EMS personnel should emphasize exposure to airborne toxins because these produced 93% of patients.
