Use of clinical decision support for antibiotic stewardship in the emergency department and outpatient clinics: An interrupted time-series analysis

Objective:
To evaluate the impact of implementing clinical decision support (CDS) tools for outpatient antibiotic prescribing in the emergency department (ED) and clinic settings.
Design:We performed a before-and-after, quasi-experimental study that employed an interrupted time-series analysis.
Setting:The study institution was a quaternary, academic referral center in Northern California.
Participants:We included prescriptions for patients in the ED and 21 primary-care clinics within the same health system.
Intervention:We implemented a CDS tool for azithromycin on March 1, 2020, and a CDS tool for fluoroquinolones (FQs; ie, ciprofloxacin, levofloxacin, and moxifloxacin) on November 1, 2020. The CDS added friction to inappropriate ordering workflows while adding health information technology (HIT) features to easily perform recommended actions. The primary outcome was the number of monthly prescriptions for each antibiotic type, by implementation period (before vs after).
Results:Immediately after azithromycin-CDS implementation, monthly rates of azithromycin prescribing decreased significantly in both the ED (−24%; 95% CI, −37% to −10%; P < .001) and outpatient clinics (−47%; 95% CI, −56% to −37%; P < .001). In the first month after FQ-CDS implementation in the clinics, there was no significant drop in ciprofloxacin prescriptions; however, there was a significant decrease in ciprofloxacin prescriptions over time (−5% per month; 95% CI, −6% to −3%; P < .001), suggesting a delayed effect of the CDS.
Conclusion:Implementing CDS tools was associated with an immediate decrease in azithromycin prescriptions, in both the ED and clinics. CDS may serve as a valuable adjunct to existing antimicrobial stewardship programs.

Geophysical systems are often assumed to follow Gaussian probability density functions; however, deviations from Gaussian behaviour can shed light on the underlying dynamics. For the large-scale motion of the Arctic sea ice, such deviations have been interpreted as signatures of structure in dynamic flow fields. In this study, we use higher-order moments (skewness and kurtosis) to identify spatiotemporal changes in the Beaufort Gyre (BG) and the Transpolar Drift (TPD) sea-ice drift patterns. Higher-order moments of satellite-derived ice drift speeds are examined over the winter period of 2006–2017 to describe the persistence of features like the BG and TPD, and their variation over time. Index patterns indicate that the periphery of the BG can be identified by a combination of high positive skewness and high kurtosis in the ice drift time series on an annual basis.

Gravitational waves from coalescing neutron stars encode information about nuclear matter at extreme densities, inaccessible by laboratory experiments. The late inspiral is influenced by the presence of tides, which depend on the neutron star equation of state. Neutron star mergers are expected to often produce rapidly rotating remnant neutron stars that emit gravitational waves. These will provide clues to the extremely hot post-merger environment. This signature of nuclear matter in gravitational waves contains most information in the 2–4 kHz frequency band, which is outside of the most sensitive band of current detectors. We present the design concept and science case for a Neutron Star Extreme Matter Observatory (NEMO): a gravitational-wave interferometer optimised to study nuclear physics with merging neutron stars. The concept uses high-circulating laser power, quantum squeezing, and a detector topology specifically designed to achieve the high-frequency sensitivity necessary to probe nuclear matter using gravitational waves. Above 1 kHz, the proposed strain sensitivity is comparable to full third-generation detectors at a fraction of the cost. Such sensitivity changes expected event rates for detection of post-merger remnants from approximately one per few decades with two A+ detectors to a few per year and potentially allow for the first gravitational-wave observations of supernovae, isolated neutron stars, and other exotica.

We describe system verification tests and early science results from the pulsar processor (PTUSE) developed for the newly commissioned 64-dish SARAO MeerKAT radio telescope in South Africa. MeerKAT is a high-gain ( ${\sim}2.8\,\mbox{K Jy}^{-1}$ ) low-system temperature ( ${\sim}18\,\mbox{K at }20\,\mbox{cm}$ ) radio array that currently operates at 580–1 670 MHz and can produce tied-array beams suitable for pulsar observations. This paper presents results from the MeerTime Large Survey Project and commissioning tests with PTUSE. Highlights include observations of the double pulsar $\mbox{J}0737{-}3039\mbox{A}$ , pulse profiles from 34 millisecond pulsars (MSPs) from a single 2.5-h observation of the Globular cluster Terzan 5, the rotation measure of Ter5O, a 420-sigma giant pulse from the Large Magellanic Cloud pulsar PSR $\mbox{J}0540{-}6919$ , and nulling identified in the slow pulsar PSR J0633–2015. One of the key design specifications for MeerKAT was absolute timing errors of less than 5 ns using their novel precise time system. Our timing of two bright MSPs confirm that MeerKAT delivers exceptional timing. PSR $\mbox{J}2241{-}5236$ exhibits a jitter limit of $<4\,\mbox{ns h}^{-1}$ whilst timing of PSR $\mbox{J}1909{-}3744$ over almost 11 months yields an rms residual of 66 ns with only 4 min integrations. Our results confirm that the MeerKAT is an exceptional pulsar telescope. The array can be split into four separate sub-arrays to time over 1 000 pulsars per day and the future deployment of S-band (1 750–3 500 MHz) receivers will further enhance its capabilities.

Monthly mean passive microwave-derived sea-ice motion maps for 36 winters (October–April) are used to examine pan-Arctic sea-ice drift speeds and patterns. The mean Arctic Ocean sea-ice motion consists of three well-known primary circulation regimes: the Beaufort Gyre (BG), transpolar drift (TPD), and a motion system from the Kara Sea (KS). The 36-year mean winter sea-ice drift pattern is used to identify the average boundaries between the circulation regimes mentioned above. Regression analyses of the ice drift speed anomalies show statistically significant positive drift speed trends in BG, TPD and KS. Non-significant trends are associated with negative trends of generally weak drift speeds north of the Canadian Arctic and over the Chukchi/East Siberian Shelf. The first three modes of Empirical Orthogonal Functions were found to explain 30.2%, 13.5% and 8.7% of the spatial variance in the mean winter ice drift patterns and highlight the large variability in the ice drift patterns.
Alternative Herbicides for the Control of Clethodim-Resistant Rigid Ryegrass (Lolium rigidum) in Clearfield Canola in Southern Australia

Rigid ryegrass is the most-troublesome, herbicide-resistant weed in cropping systems of southern Australia. Field experiments were undertaken at Roseworthy, South Australia, in 2013 and 2014, to identify effective herbicide options for the control of clethodim-resistant rigid ryegrass in Clearfield canola. PPI trifluralin + triallate followed by (fb) POST imazamox + imazapyr + clethodim + butroxydim had the lowest plant density of rigid ryegrass in 2014 and provided superior control compared with the standard grower practice of PPI trifluralin + triallate fb POST imazamox + imazapyr + clethodim in 1 of 2 yr. Propyzamide either alone or as a split application (PPI fb POST) or in combination with clethodim provided similar rigid ryegrass control to that of the standard grower practice (38 to 553 plants m−2). Rigid ryegrass treated with PPI dimethenamid-P, pethoxamid, pethoxamid + triallate, and PPI trifluralin fb carbetamide POST produced significantly more seeds than the standard grower practice, which would lead to reinfestation of subsequent crops. Canola yield responded positively to effective herbicide treatments, especially in 2014, when rigid ryegrass density was greater. PPI dimethenamid-P and pethoxamid alone or in combination with triallate and propyzamide were ineffective in reducing rigid ryegrass density and seed production to levels acceptable for continuous cropping systems.

Two field experiments were conducted during 2012 and 2013 at Roseworthy, South Australia to identify effective herbicide options for the management of clethodim-resistant rigid ryegrass in faba bean. Dose–response experiments confirmed resistance in both field populations (B3, 2012 and E2, 2013) to clethodim and butroxydim. Sequencing of the target site of acetyl coenzyme A carboxylase gene in both populations identified an aspartate-2078-glycine mutation. Although resistance of B3 and E2 populations to clethodim was similar (16.5- and 21.4-fold more resistant than the susceptible control SLR4), the B3 population was much more resistant to butroxydim (7.13-fold) than E2 (2.24-fold). Addition of butroxydim to clethodim reduced rigid ryegrass plant density 60 to 80% and seed production 71 to 88% compared with the standard grower practice of simazine PPI plus clethodim POST. Clethodim + butroxydim combination had the highest grain yield of faba bean (980 to 2,400 kg ha−1). Although propyzamide and pyroxasulfone plus triallate PPI provided the next highest levels of rigid ryegrass control (< 60%), these treatments were more variable and unable to reduce seed production (6,354 to 13,570 seeds m−2) to levels acceptable for continuous cropping systems.
Placental restriction in multi-fetal pregnancies increases spontaneous ambulatory activity during daylight hours in young adult female sheep

Intrauterine growth restriction (IUGR) has adverse effects on metabolic health and early life, whereas physical activity is protective against later development of metabolic disease. Relationships between birth weight and physical activity in humans, and effects of IUGR on voluntary activity in rodents, are mixed and few studies have measured physical activity in a free-ranging environment. We hypothesized that induced restriction of placental growth and function (PR) in sheep would decrease spontaneous ambulatory activity (SAA) in free-ranging adolescent and young adult progeny from multi-fetal pregnancies. To test this hypothesis, we used Global Positioning System watches to continuously record SAA between 1800 and 1200 h the following day, twice during a 16-day recording period, in progeny of control (CON, n=5 males, 9 females) and PR pregnancies (n=9 males, 10 females) as adolescents (30 weeks) and as young adults (43 weeks). PR reduced size at birth overall, but not in survivors included in SAA studies. In adolescents, SAA did not differ between treatments and females were more active than males overall and during the day (each P<0.001). In adults, daytime SAA was greater in PR than CON females (P=0.020), with a similar trend in males (P=0.053) and was greater in females than males (P=0.016). Adult SAA was negatively correlated with birth weight in females only. Contrary to our hypothesis, restricted placental function and small size at birth did not reduce progeny SAA. The mechanisms for increased daytime SAA in adult female PR and low birth weight sheep require further investigation.
Aspiration pneumonia related deaths in head and neck cancer patients: a retrospective analysis of risk factors from a tertiary care centre in North India

Background:
Aspiration pneumonia is an important cause of death in head and neck cancer patients. This study therefore aimed to evaluate the risk factors associated with aspiration pneumonia in head and neck cancer patients.
Methods:Hospital death records from 12 years (2000–2012) were reviewed to obtain the number of deaths. Treatment details and cause of death were analysed. Statistical analysis was performed to identify the risk factors for aspiration pneumonia.
Results:The records revealed that aspiration pneumonia was the cause of death in 51 out of 85 patients. Primary tumour site (oropharynx and hypopharynx, odds ratio 3.3; 95 per cent confidence interval 1.17–9.4, p = 0.02) and advanced tumour stage (odds ratio 4.2, 95 per cent confidence interval 1.16–15.61, p = 0.02) had significant negative impacts on aspiration pneumonia related mortality.
Conclusion:Advanced pharyngeal cancer patients are at an increased risk of aspiration pneumonia related death. Investigations for the early detection of this condition are recommended in these high-risk patients.

We introduce a novel method for producing polystyrene (PS)-grafted multiwalled carbon nanotubes (MWCNTs), which provides a direct route to composites where carbon nanotubes (CNTs) are the major component. Infrared and Raman spectroscopies confirmed that the MWCNTs were functionalized with PS. Thermogravimetric analysis showed that CNTs increase thermal stability of the composite up to a critical loading (∼40 wt%) beyond which high nanotube loadings decrease the polymer degradation temperature, as a consequence of the thermal properties of CNTs and the composite morphology. Even at loadings as high as 80 wt% MWCNTs, the composite is an effective masterbatch material for both solution- and melt-processing. These results show that in situ polymerizations can be flexible and robust techniques for nanocomposite processing, overcoming limitations of conventional processing techniques to produce nanocomposites with very high nanotube loadings, not achieved hitherto.

Feed intake, milk production and composition of crossbred cows fed with insect-protected Bollgard II® cottonseed containing Cry1Ac and Cry2Ab proteins

Twenty crossbred lactating multiparous cows were used in a 28-day study to compare dry matter intake (DMI), milk yield, milk composition and Bacillus thuringiensis (Bt) protein concentrations in plasma when fed diets containing Bollgard II® cottonseed (BGII) or a control non-genetically modified isogenic cottonseed (CON). Bollgard II cottonseed contains the Cry1Ac and Cry2Ab insecticidal proteins that protect cotton plants from feeding damage caused by certain lepidopteran insects. Cows were assigned randomly to the BGII or CON treatments after a 2-week adjustment period. Cows consumed a concentrate containing 40% crushed cottonseed according to milk yield and green maize forage ad libitum. All cows received the same diet but with different crushed cottonseed sources. Cottonseed was included to provide approximately 2.9 kg per cow daily (dry matter basis). The ingredient composition of the concentrate was 40% crushed cottonseed, 15% groundnut cake, 20% corn, 22% wheat bran, 1% salt and 2% mineral mixture. Milk and blood plasma were analyzed for Cry1Ac and Cry2Ab proteins. DMI, BW, milk yield and milk components did not differ between cows on the BGII and CON treatments. Although milk yield and milk fat percentage were not affected by treatment, 4% fat-corrected milk (FCM) production and FCM/kg DMI for cows on the BGII treatment (14.0 kg/cow per day, 1.12 kg/kg) were significantly improved compared with cows on the CON treatment (12.1 kg/cow per day, 0.97 kg/kg). Gossypol contents in BGII cottonseed and conventional cottonseed were similar. Cry1Ac and Cry2Ab2 proteins in Bollgard II cottonseed were 5.53 and 150.8 μg/g, respectively, and were not detected in the milk or plasma samples. The findings suggested that Bollgard II cottonseed can replace conventional cottonseed in dairy cattle diets with no adverse effects on performance and milk composition.

Composition, Size, Structure, and Magnetic Characterization of Iron-Platinum Nano-particles

Iron-platinum (FePt) nano-particles were synthesized using a hot metal salt reduction reaction with iron chloride and platinum acetylacetonate as the precursor salts. The synthesis route was based upon work done previously by Sun et Al.[6]. The size and composition of the FePt nano-particles were controlled by varying the surfactant to reagent molar ratio and refluxing time during synthesis. The average size of FePt nano-particles synthesized ranged from 4.08 nm to 5.23 nm. The composition of the nano-particles ranged from Fe60Pt40 to Fe55Pt45. The size was shown to increase as the surfactant to reagent molar ratio was changed from 1:6 to 1:1. The platinum composition increased from 40 % to 45 % with an increase in the refluxing time from 30 minutes to 60 minutes. The X-ray analysis (XRD) showed that the unannealed nano-particles had a FCC crystal structure. The FCC FePt nano-particles were superparamagnetic at room temperature. A blocking temperature (Tb) of 35 K was measured for 5.23 nm Fe55Pt45 nano-particles. Annealing of the Fe55Pt45 nano-particles induced a FCT crystal structure with hard-magnetic properties. Coercivity (Hc) of approximately 13000 Oe was measured at room temperature.

We assessed the bacterial contamination of the pagers of healthcare personnel and the efficacy of disinfection with 70% isopropyl alcohol. Microorganisms were isolated from all pagers; 21% yielded Staphylococcus aureus, of which 14% were methicillin resistant. Cleaning with alcohol reduced the total colony count by an average of 94%. Bacterial load varied by healthcare worker group and service assignment.

Organic electroluminescent (EL) diodes based on fluorescent dyes and conducting polymers have attracted the interest of researchers, mainly because of their emission in the visible region and for application to large area portable flat panel display devices, driven at low voltages. Therefore, for the development of higher efficiency polymer EL diodes, the optimal combination of the merits of organic fluorescent dye molecules with that of conjugated polymer is an important approach. We report electroluminescence studies of polymer light emitting diodes (p-LEDs) fabricated with poly[2-(3-thienyl)ethanol n-butoxy carbonylmethyl urethane] (PURET) and its composite with 4-dicyanomethylene-2-methyl-6-(p-dimethylaminostyryl)-4H pyran (DCM) dye. These materials have been chosen in view of the fact that PURET exhibits a small overlap between emission and absorption spectra whereas DCM has a good efficiency of trapping both electrons as well as holes. Polyaniline has been utilized as hole injecting layer whereas tris-8-hydroxyquinoline-aluminum as electron injecting layer. Enhanced electroluminescence with bright yellow color has been observed in p-LEDs by the addition of dye.
