We analyzed efficacy of a centralized surveillance infection prevention (CSIP) program in a healthcare system on healthcare-associated infection (HAI) rates amid the coronavirus disease 2019 (COVID-19) pandemic. HAI rates were variable in CSIP and non-CSIP facilities. Central-line–associated bloodstream infection (CLABSI), C. difficile infection (CSI), and surgical-site infection (SSI) rates were negatively correlated with COVID-19 intensity in CSIP facilities.

Objective:
To develop, implement, and evaluate the effectiveness of a unique centralized surveillance infection prevention (CSIP) program.
Design:Observational quality improvement project.
Setting:An integrated academic healthcare system.
Intervention:The CSIP program comprises senior infection preventionists who are responsible for healthcare-associated infection (HAI) surveillance and reporting, allowing local infection preventionists (LIPs) a greater portion of their time to non-surveillance patient safety activities. Four CSIP team members accrued HAI responsibilities at 8 facilities.
Methods:We evaluated the effectiveness of the CSIP program using 4 measures: recovery of LIP time, efficiency of surveillance activities by LIPs and CSIP staff, surveys characterizing LIP perception of their effectiveness in HAI reduction, and nursing leaders’ perception of LIP effectiveness.
Results:The amount of time spent by LIP teams on HAI surveillance was highly variable, while CSIP time commitment and efficiency was steady. Post-CSIP implementation, 76.9% of LIPs agreed that they spend adequate time on inpatient units, compared to 15.4% pre-CSIP; LIPs also reported more time to allot to non-surveillance activities. Nursing leaders reported greater satisfaction with LIP involvement with HAI reduction practices.
Conclusion:CSIP programs are a little-reported strategy to ease burden on LIPs with reallocation of HAI surveillance. The analyses presented here will aid health systems in anticipating the benefit of CSIP programs.
Thin ice, deep snow and surface flooding in Kotzebue Sound: landfast ice mass balance during two anomalously warm winters and implications for marine mammals and subsistence hunting

The inaugural data from the first systematic program of sea-ice observations in Kotzebue Sound, Alaska, in 2018 coincided with the first winter in living memory when the Sound was not choked with ice. The following winter of 2018–19 was even warmer and characterized by even less ice. Here we discuss the mass balance of landfast ice near Kotzebue (Qikiqtaġruk) during these two anomalously warm winters. We use in situ observations and a 1-D thermodynamic model to address three research questions developed in partnership with an Indigenous Advisory Council. In doing so, we improve our understanding of connections between landfast ice mass balance, marine mammals and subsistence hunting. Specifically, we show: (i) ice growth stopped unusually early due to strong vertical ocean heat flux, which also likely contributed to early start to bearded seal hunting; (ii) unusually thin ice contributed to widespread surface flooding. The associated snow ice formation partly offset the reduced ice growth, but the flooding likely had a negative impact on ringed seal habitat; (iii) sea ice near Kotzebue during the winters of 2017–18 and 2018–19 was likely the thinnest since at least 1945, driven by a combination of warm air temperatures and a persistent ocean heat flux.

Introduction:
Many institutions are attempting to implement patient-reported outcome (PRO) measures. Because PROs often change clinical workflows significantly for patients and providers, implementation choices can have major impact. While various implementation guides exist, a stepwise list of decision points covering the full implementation process and drawing explicitly on a sociotechnical conceptual framework does not exist.
Methods:To facilitate real-world implementation of PROs in electronic health records (EHRs) for use in clinical practice, members of the EHR Access to Seamless Integration of Patient-Reported Outcomes Measurement Information System (PROMIS) Consortium developed structured PRO implementation planning tools. Each institution pilot tested the tools. Joint meetings led to the identification of critical sociotechnical success factors.
Results:Three tools were developed and tested: (1) a PRO Planning Guide summarizes the empirical knowledge and guidance about PRO implementation in routine clinical care; (2) a Decision Log allows decision tracking; and (3) an Implementation Plan Template simplifies creation of a sharable implementation plan. Seven lessons learned during implementation underscore the iterative nature of planning and the importance of the clinician champion, as well as the need to understand aims, manage implementation barriers, minimize disruption, provide ample discussion time, and continuously engage key stakeholders.
Conclusions:Highly structured planning tools, informed by a sociotechnical perspective, enabled the construction of clear, clinic-specific plans. By developing and testing three reusable tools (freely available for immediate use), our project addressed the need for consolidated guidance and created new materials for PRO implementation planning. We identified seven important lessons that, while common to technology implementation, are especially critical in PRO implementation.

United Kingdom Nirex Limited develops and advises on safe, environmentally sound and publicly acceptable options for the long-term management of radioactive waste. One option Nirex has developed is a phased geological repository concept for intermediate level waste and some low level wastes that makes use of a combination of engineered and natural barriers. Physical containment of radionuclides would be achieved by immobilisation and packaging of wastes (mostly) in stainless steel containers.
Existing models of the migration of dissolved radionuclides from packaged wastes suggest that radionuclide release is determined largely by the rate of diffusion through the encapsulation grout used to immobilise the waste. The use of such models requires diffusion coefficient data for radionuclides in waste encapsulation grouts. This paper describes a programme of through-diffusion experiments, and modelling interpretation, aimed at deriving diffusion coefficients for some radionuclides in two types of encapsulation grout.
An intrinsic diffusion coefficient of HTO of around 1×10−13 to 2×10−13 m2s−1 was determined for a 3:1 mix of blast furnace slag to ordinary Portland cement, compared to around 4×10−13 to 5×10−13 m2s−1 for a 3:1 mix of pulverised fuel ash to ordinary Portland cement. These values are lower than that assumed for a non-sorbing radionuclide in an earlier modelling exercise. Porosity values around 0.3 were obtained in each case. For 36Cl as chloride, the experiments showed no significant breakthrough over the experimental timescale of about one year, suggesting an intrinsic diffusion coefficient below 5×10−13 m2s−1. One possibility is that chlorine-containing solids are precipitating in the cement. An intrinsic diffusion coefficient for 137Cs in the 3:1 mix of pulverised fuel ash to ordinary Portland cement of 4×10−15 m2s−1 was estimated, significantly lower than that determined for HTO.
The results from three of the sixteen experiments could not be fitted with a simple diffusion model, and for a further five experiments there was some doubt as to whether simple diffusion behaviour had been observed. It is suggested that this may have been due to cracks in the grouts that were sufficiently large to affect the diffusion properties of the grouts, although none was visible to the naked eye. Cracking of the waste encapsulation grouts could provide a mechanism for enhanced migration of radionuclides from waste packages, compared with diffusion in a homogeneous porous medium alone.

The prevalence and genetic diversity of hepatitis C infection in women attending antenatal clinics in two regions of England was investigated to inform future surveillance and control measures. Women booking into antenatal care are routinely offered a test for immunity to rubella. Serum residues from these tests were unlinked, anonymized and archived as part of the Unlinked Anonymous Prevalence Monitoring Programme (UAPMP). The serum specimens were tested for anti-HCV using a cost-effective pooling strategy. After taking into account differential sampling from the UAPMP serum archive, the adjusted overall prevalence of anti-HCV was 0·43% (95% CI: 0·32–0·53) in London and 0·21% (95% CI: 0·14–0·28) in the Northern and Yorkshire region. Restriction fragment length polymorphism of amplified HCV RNA identified type 3a as the most common HCV genotype in these antenatal women. The prevalence of anti-HCV in antenatal women in the UK is low and consistent with that expected from injecting drug use.

When the Indian Government banned the export of snakeskins in 1976, the Irulas – a tribal people whose traditional skills included snake-catching – lost a major source of income. The authors describe how the Irula Co-operative Venom Centre was established to replace this lost income and at the same time exploit a valuable wildlife resource in a way that is apparently sustainable. More research is needed, however, to ensure that the project is viable in the long term.
