The effect of music to improve sleep quality in depression related insomnia

Introduction
Insomnia in depression is common and difficult to resolve. Music is commonly used as a sleep aid, and clinical trials pointing to positive effects of music as a sleep aid are increasing adding to the evidence base. There is little knowledge on the effectiveness of music for depression related insomnia.
ObjectivesA recent RCT study conducted in psychiatry at Aalborg University Hospital examined effects of a music intervention for insomnia in depression. The intervention group listened to music at bedtime for four weeks, controls were offered music intervention post-test. Primary outcome measure was Pittsburgh Sleep Quality Index (PSQI). Secondary outcomes included Actigraphy, The Hamilton depression Rating Scale (HAMD-17) and World Health Organisation well-being questionnaires (WHO-5, WHOQOL-BREF).
MethodsA two-armed randomized controlled trial (n=112) and a qualitative interview study (n=4)
ResultsThe RCT study showed signficant improvements for the music intervention group in sleep quality and quality of life at four weeks according to global PSQI scores (effect size= -2.1, 95%CI -3.3; -0.9) and WHO-5 scores (effect size 8.4, 95%CI 2.7; 14.0). Actigraphy measures showed no changes and changes in depression symptoms (HAMD-17) were not detected.
The interview study unfolded examples of the influences of music on sleep and relaxation. Music distracted, affected mood and arousal positively and supported formation of sleep habits.
Results from the trial are discussed and merged with findings from the interview study. The results from the trial suggested moderate effects of music listening for the population while findings from the interview study showed examples of individual and highly varying outcomes.
ConclusionsMusic is suggested as a low-cost, side-effect free and safe intervention in supplement to existing treatments improving sleep in depression.

Background
Alzheimer's disease (AD), the most common disease causing dementia, is linked to increased mortality. However, the effect of antipsychotic use on specific causes of mortality has not yet been investigated thoroughly.
MethodsUtilizing the Danish nationwide registers, we defined a cohort of patients diagnosed with AD. Utilizing separate Cox regressions for specific causes of mortality, we investigated the effects of cumulative antipsychotic dosage after diagnosis and current antipsychotic exposure in the time period 2000–2011.
ResultsIn total, 45,894 patients were followed for 3,803,996 person-years. A total of 6129 cardiovascular related deaths, 2088 cancer related deaths, 1620 infection related deaths, and 28 intentional self-harm related deaths are presented. Current antipsychotic exposure increased mortality rate with HR between 1.92 and 2.31 for cardiovascular, cancer, and infection related death. Cumulative antipsychotic dosages were most commonly associated with increased rates of mortality for cardiovascular and infection as cause of death, whereas the associations were less clear with cancer and intentional self-harm as cause of death.
ConclusionsWe showed that cumulative antipsychotic drug dosages increased mortality rates for cardiovascular and infection as cause of death. These findings highlight the need for further investigations of long-term effects of treatment and of possible sub-groups who could benefit from treatment.
ARE THE NEOTROPICAL SWAMP FORESTS A DISTINGUISHABLE FOREST TYPE? PATTERNS FROM SOUTHEAST AND SOUTHERN BRAZIL

This work synthesises information about plant diversity of 37 sites of coastal and inland swamp forests of southeast and southern Brazil and investigates floristic similarities and differences among them. Swamp forests often show low species richness, diversity and evenness, which are associated with the selective character of oxygen deprivation caused by soil waterlogging. However, our results pointed out some degree of site-level variation (Sobs = 5 to 110; Sjack1 = 6 to 151; H′ = 0.82 to 3.98; J′ = 0.51 to 0.87) related to local ecological conditions. Two major phytogeographical patterns emerged from our work: the strong influence of the neighbouring non-flooded vegetation on the flora of swamp forests and its high spatial heterogeneity. At least 85.9% of the 518 species sampled are found in neighbouring non-flooded vegetation. A correspondence analysis consistently separated swamp forests of the coastal plain from those of the plateau. A Mantel test indicated a significant correlation between floristic and geographical distances among sites (r = 0.45; p = 0.001), and highlighted the main effect of regional-scale changes in the flora of swamp forests. We conclude that swamp forests of southeast and southern Brazil cannot be considered a distinguishable floristic unit.

Evaluating the clinical and economic burden of healthcare-associated infections during hospitalization for surgery in France

Over 4 million patients suffer nosocomial infections annually in the European Union. This study aimed to estimate the healthcare burden associated with healthcare-associated infections (HAIs) following surgery in France, and explore the potential impact of infection control strategies and interventions on the clinical and economic burden of disease. Data on the frequency of HAIs were gathered from the 2010 Programme de Médicalisation des Systèmes d'Information (PMSI), and cost data were taken from the 2009 Echelle Nationale de Coûts à Méthodologie Commune (ENCC). It was estimated that 3% of surgical procedures performed in 2010 in France resulted in infection, resulting in an annual cost of €57 892 715. Patients experiencing a HAI had a significantly increased mortality risk (4·15-fold) and an increased length of hospital stay (threefold). Scenario analysis in which HAI incidence following surgery was reduced by 8% (based on a study of the effectiveness of triclosan-coated sutures), suggested that, annually, 20 205 hospital days and €4 588 519 could be saved. Analyses of 20% and 30% reductions in incidence (based on an estimate of the number of preventable nosocomial infections) suggested that annual savings of €11 548 057 and €17 334 696, respectively, could be made. New infection control interventions which reduce HAI incidence during hospitalization for surgery have the potential to provide valuable cost savings to healthcare providers.

The solar activity cycle entered a prolonged quiet phase that started in 2008 and ended in 2010. This minimum lasted for a year longer than expected and all activity proxies, as measured from Earth and from Space, reached minimum values never observed before (de Toma, 2012). The number of spotless days from 2006 to 2009 totals 800, the largest ever recorded in modern times. Solar irradiance was at historic minimums. The interplanetary magnetic field was measured at values as low as 2.9 nT and the cosmic rays were observed at records-high. While rumors spread that the Sun could be entering a grand minimum quiet phase (such as the Maunder minimum of the XVII century), activity took over in 2010 and we are now well into Solar Cycle 24 (albeit, probably, a low intensity cycle), approaching towards a maximum due by mid 2013. In addition to bringing us the possibility to observe a quiet state of the Sun and of the Heliosphere that was previously not recorded with modern instruments, the Sun has also shown us how little we know about the dynamo mechanism that drives its activity as all solar cycle predictions failed to see this extended minimum coming.
Self-Verification and Social Anxiety: Preference for Negative Social Feedback and Low Social Self-Esteem

Background: A self-verification model of social anxiety views negative social self-esteem as a core feature of social anxiety. This core feature is proposed to be maintained through self-verification processes, such as by leading individuals with negative social self-esteem to prefer negative social feedback. This model is tested in two studies. Methods: In Study 1, questionnaires were administered to a college sample (N = 317). In Study 2, questionnaires were administered to anxiety disordered patients (N = 62) before and after treatment. Results: Study 1 developed measures of preference for negative social feedback and social self-esteem, and provided evidence of their incremental validity in a college sample. Study 2 found that these two variables are not strongly related to fears of evaluation, are relatively unaffected by a treatment that targets such fears, and predict residual social anxiety following treatment. Conclusions: Overall, these studies provide preliminary evidence for a self-verification model of social anxiety.

Division II of the IAU provides a forum for astronomers studying a wide range of phenomena related to the structure, radiation and activity of the Sun, and its interaction with the Earth and the rest of the solar system. Division II encompasses three Commissions, 10, 12 and 49, and four working groups. During the last triennia the activities of the division involved some reorganization of the division and its working groups, developing new procedures for election of division and commission officers, promoting annual meetings from within the division and evaluating all the proposed meetings, evaluating the division's representatives for the IAU to international scientific organizations, and participating in general IAU business.

The relative role of the stellar radiation field, the stellar outflows and the interstellar radiation field (ISRF) in transforming the molecular ejecta into atomic gas was the subject of our ISO LWS and SWS spectroscopy study of 24 evolved stars which span the range from AGB stars to proto-planetary nebulae (PPNs) and PNs. The far-infrared (FIR) atomic fine-structure lines are powerful probes of the warm atomic gas in photodissociation regions (PDRs) and shocks. This paper summarizes and compares the ISO spectroscopy studies of carbon-rich (C-rich) and oxygen-rich (O-rich) evolved stars, published by Fong et al. (2001) and Castro-Carrizo et al. (2001), respectively. We find that photodissociation, not shocks, is responsible for the chemical change from molecular to atomic gas.

Femtosecond modelocked lasers using new ytterbium-doped borate crystals (Yb:Sr3Y(BO3)3, Yb:Ca4GdO(BO3)3 and Yb:Ca4YO(BO3)3) are demonstrated. Pulse duration as short as 69 fs has been obtained. To modelock such lasers, fast saturable absorbers need to be used. Two different types of fast saturable absorbers have been studied: low-temperature-grown semiconductor mirrors (SESAM) and high-energy-ion-implanted semiconductor Bragg reflectors (SBR). We demonstrated, for the first time to our knowledge, that ion-implanted SBR can be used to modelock oscillators using Yb-doped materials.
Proteins Encapsulated in Porous Sol-Gels: Biomolecules as Pore Structure Templates

Porous inorganic SiO2 glasses obtained by the sol-gel route represent a unique matrix for encapsulation of biomolecules wherein the pores act as enclosures for high molecular weight proteins. These hybrid materials are characterized by a pore-biomolecule interface between the pore walls and the protein surface. As a specific model protein, cytochrome c (cyt c) is used to elucidate the nature of physical and chemical interactions between the pores of the matrix and the protein. Evidence from optical absorption, and resonance Raman (RR) spectroscopy methods indicates that the dopant protein alters the structural features of the pore walls. The optical and vibrational measurements strongly suggest that the pores that contain the trapped protein undergo little or no structural change during aging and drying as compared to protein-free pores. Vibrational RR analysis of the trapped cyt c also suggests that the protein resides in a pore where the pore dimensions conform to the shape of the protein. The results indicate that noncovalent interactions between the surface of the protein and the pore walls govern the dynamics of pore formation during gelation and individual biomolecules act as structural templates to design local pore structure.
Heme Proteins Encapsulated in Sol-Gel Derived Silica Glasses and their Reaction with Ligands

Hemoglobin (Hb) and myoglobin (Mb) were encapsulated in transparent silica glasses prepared by the sol-gel method. The preparation of the silica glasses was tailored so that when proteins were entrapped in the pores of the inorganic matrix, they retained their biochemical activity, i.e. they could bind ligands reversibly. Using optical spectroscopy to monitor ligand binding, we studied the binding of O2, CO, and NO with these two heme proteins encapsulated in silica glasses and compared them to heme proteins in aqueous buffer. Both Hb and Mb in the sol-gel glass bound O2, CO, and NO, producing the same spectroscopic properties as those in aqueous buffer. In addition, silica encapsulated Mb was used to evaluate the rate of ligand (O2) transport through the pores of the glass. When varying oxygen concentration and measuring the time required for full conversion of deoxyMb to MbO2 in the silica gel, the time vs. concentration data followed an exponential trend, as expected for diffusion controlled processes.
