OBJECTIVES/GOALS: Patient beliefs and goals can facilitate discussion of recovery expectations, patient-provider collaboration and maximization of goal achievement. In this study, we sought to address an evidence gap and examine the association of preoperative self-assessment of goals with preoperative and 6-week knee function and gait speed among total knee arthroplasty (TKA) patients. METHODS/STUDY POPULATION: We conducted a secondary analysis of data from the VERITAS randomized, controlled trial conducted from 11/2016-03/2018 that included adults age ≥ 18 years with scheduled and completed unilateral TKA followed by post-surgical physical therapy. Patients rated their ability to perform various activities of daily living goals scaled from 0 (unable to perform) to 10 (full performance). Patients were categorized by pre-surgical (baseline) goal rating: low = 0-2, intermediate = 3-4, and high = 5-10. Outcomes including gait speed and the KOOS were assessed within 10 days prior to surgery and 6-weeks post-surgery. Descriptive statistics and outcomes were compared for patients by preoperative goal rating using Chi-square or Fisher’s exact tests and ANOVA or Kruskal-Wallis tests as appropriate. RESULTS/ANTICIPATED RESULTS: Of 288 patients (mean age 65±8; 62.5% women; 82% white), 102 had a low goal rating (GR), 86 intermediate, and 99 high. Patients with low GR preoperatively generally had lower baseline mean scores than intermediate and high GR patients, respectively, on the KOOS (33.9/35.6/39.8; p<0.001) and lower gait speed (m/s) compared to intermediate and high GR patients at baseline (0.9/1.1/1.0; p = 0.009). The low, intermediate, and high GR groups, respectively, showed no difference across mean KOOS scores (61.0/61.2/61.9; p = 0.63) or gait speed (m/s) (1.0/1.0/1.0, p = 0.33) at 6 weeks postoperative. DISCUSSION/SIGNIFICANCE OF IMPACT: In this study, adults who perceived greater difficulty with a pre-selected activity goal, exhibited lower function prior to TKA but showed no differences in function 6-weeks after surgery. Follow-up studies will describe the association between goal-setting preoperatively and patient goal attainment and satisfaction following surgery.
Development of the Halifax Visual Scanning Test: A New Measure of Visual-Spatial Neglect for Personal, Peripersonal, and Extrapersonal Space

Objectives: Visual-spatial neglect is a common attentional disorder after right-hemisphere stroke and is associated with poor rehabilitation outcomes. The presence of neglect symptoms has been reported to vary across personal, peripersonal, and extrapersonal space. Currently, no measure is available to assess neglect severity equally across these spatial regions and may be missing subsets of symptoms or patients with neglect entirely. We sought to provide initial construct validity for a novel assessment tool that measures neglect symptoms equally for these spatial regions: the Halifax Visual Scanning Test (HVST). Methods: In Study I, the HVST was compared to conventional measures of neglect and functional outcome scores (wheelchair navigation) in 15 stroke inpatients and 14 healthy controls. In Study II, 19 additional controls were combined with the control data from Study I to establish cutoffs for impairment. Patterns of neglect in the stroke group were examined. Results: In Study I, performance on all HVST subtests were correlated with the majority of conventional subtests and wheelchair navigation outcomes. In Study II, neglect-related deficits in visual scanning showed dissociations across spatial regions. Four inpatients exhibited symptoms of neglect on the HVST that were not detected on conventional measures, one of which showed symptoms in personal and extrapersonal space exclusively. Conclusions: The HVST appears a useful measure of neglect symptoms in different spatial regions that may not be detected with conventional measures and that correlates with functional wheelchair performance. Preliminary control data are presented and further research to add to this normative database appears warranted. (JINS, 2019, 25, 490–500)

Summary
Introduction
Fluid and electrolyte balance is important for the maintenance of normal function and wellbeing. The equilibrium of fluid balance between the body and its environment is maintained via the intake of fluids and electrolytes versus the loss of fluids through normal urine production, via the gastrointestinal tract and through insensible losses of the skin and respiratory tract.
Fluid and electrolyte balance also needs to be maintained within the different fluid compartments of the body. This includes the state of balance between the intracellular fluids (ICF) and extracellular fluids (ECF), comprising fluids in intravascular and extravascular spaces. The balance of electrolytes between the ICF and ECF is mediated via the sodium pump present in cell membranes, while the balance in the ECF (between intravascular and extravascular space) is mediated via the capillary membrane.
Infants and children are susceptible to disruptions in fluid balance and nutrition during periods of acute illness or exacerbations of existing, chronic conditions for two main reasons. First, infants and young children are physiologically immature in relation to fluid balance, and lack some of the mechanisms to protect them against disruptions to fluid balance, such as dehydration. Second, infants are dependent on parents or adult carers to provide them with fluids and food. Because of these vulnerabilities to fluid balance and nutrition, nurses working in paediatric settings need to be skilful in their assessment and monitoring of fluid balance in infants and children, and able to manage a range of therapies to correct fluid and nutritional deficits. In this chapter, you will expand your understanding and develop skills in the management of peripheral and central intravenous therapy and enteral feeding. An additional section on current insulin therapies is also included.
Differences in the fluid balance and nutrition of infants and children
There are several differences in the composition of fluids in the body of infants, particularly newborns and younger infants. These differences mean that infants and children are more susceptible to fluid losses than adults.
Newborn babies have higher total body water (75–80 per cent) than adults (60 per cent). Premature infants have higher total body water than even infants born at term. In newborn infants, there is a greater proportion of body water in the extracellular space.

Objective:
Postintubation hemodynamic instability (PIHI) is a potentially life-threatening adverse event of emergent endotracheal intubation. The objectives of this study were to determine the incidence, risk factors, and impact on patient outcomes associated with PIHI in intubations performed in emergency medicine.
Methods:A structured chart audit was performed of all consecutive adult patients requiring emergent endotracheal intubations over a 16-month period at a tertiary care emergency department (ED). Data collection included medications, comorbidities, vital signs in the 30 minutes before and after intubation, hospital length of stay, and in-hospital mortality. PIHI was defined as a decrease in systolic blood pressure (SBP) to ≤ 90 mm Hg, a decrease in SBP of ≥ 20% from baseline, a decrease in mean arterial pressure to ≥ 65 mm Hg, or the initiation of any vasopressor medication at any time in the 30 minutes following intubation.
Results:Overall, 218 patients intubated in the ED were identified, and 44% (96 of 218) developed PIHI. On multivariate analysis, increasing age (OR 1.03, 95% CI 1.01–1.05), chronic obstructive pulmonary disease (OR 3.00, CI 1.19–7.57), and pre–emergent endotracheal intubation hemodynamic instability (OR 2.52, 95% CI 1.27–4.99) were associated with the development of PIHI. The use of a neuromuscular blocking medication was associated with a decreased incidence of PIHI (OR 0.34, 95% CI 0.16–0.75).
Conclusions:Based on our data, postintubation hypotension occurs in a significant proportion of ED patients requiring emergent airway control. Further investigation is needed to confirm the factors we found to be associated with PIHI and to determine if PIHI is associated with increased morbidity and mortality.
Effect of Patterns of Transferring Patients among Healthcare Institutions on Rates of Nosocomial Methicillin-Resistant Staphylococcus aureus Transmission: A Monte Carlo Simulation

Objective.
To determine the effect of the rate and pattern of patient transfers among institutions within a single metropolitan area on the rates of methicillin-resistant Staphylococcus aureus (MRSA) transmission among patients in hospitals and nursing homes.
Methods.A stochastic, discrete-time, Monte Carlo simulation was used to model the rate and spread of MRSA transmission among patients in medical institutions within a single metropolitan area. Admission, discharges, transfers, and nosocomial transmission were simulated with respect to different interinstitutional transfer strategies and various situational scenarios, such as outlier institutions with high transmission rates.
Results.The simulation results indicated that transfer patterns and transfer rate changes do not affect nosocomial MRSA transmission. Outlier institutions with high transmission rates affect the systemwide rate of nosocomial infections differently, depending on institution type.
Conclusion.It is worth effort to understanding disease-transmission dynamics and interinstitutional transfer patterns for the management of recently introduced diseases or strains. Once endemic in a system, other strategies for transmission control need to be implemented.

Brysbaert and Duyck (this issue) suggest that it is time to abandon the Revised Hierarchical Model (Kroll and Stewart, 1994) in favor of connectionist models such as BIA+ (Dijkstra and Van Heuven, 2002) that more accurately account for the recent evidence on non-selective access in bilingual word recognition. In this brief response, we first review the history of the Revised Hierarchical Model (RHM), consider the set of issues that it was proposed to address and then evaluate the evidence that supports and fails to support the initial claims of the model. Although fifteen years of new research findings require a number of revisions to the RHM, we argue that the central issues to which the model was addressed, the way in which new lexical forms are mapped to meaning and the consequence of language learning history for lexical processing, cannot be accounted for solely within models of word recognition.

Summary
Heliophysical particles: universal processes and problems
At the time the very first satellites were launched half a century ago, the space environment was portrayed by mainstream media as the science fiction home where Flash Gordon fought evil aliens. The very real threat of Earth's radiation belts was not even imagined in either the fantasy or science worlds so no consideration was given to how the very energetic particles of the belts, traveling near the speed of light and capable of penetrating solid material, might affect instrumentation. Yet it was the diminished performance of the Geiger counter designed by James Van Allen (Van Allen et al., 1958; see also Section 3.1), that led to the eventual discovery of the belts. Van Allen speculated that the unusually low flux measurements returned by his experiment were actually a sign that the instrument had saturated, overwhelmed by a previously unknown and very large population of energetic particles.
The conjecture was confirmed by the dozens of satellites launched to probe Earth's magnetosphere providing a qualitative depiction and understanding of the radiation belts. The belts consist of protons and electrons trapped in Earth's magnetic field forming torus-shaped regions extending from ˜1.5 to ˜10 Earth radii (RE) (see Fig. 11.1). The protons form only a single belt. The electrons form two belts separated at ˜2.5 RE by a minimum flux region known as the slot (Lyons and Thorne, 1973).

Objective.
To assess the impact of an institution-wide infection control education program on the rate of transmission of methicillin-resistant Staphylococcus aureus (MRSA).
Design.Before-and-after study.
Setting.A 472-bed, urban, university-affiliated hospital.
Intervention.During the period March-May 2004, all hospital staff completed a mandatory infection control education program, including the receipt of hospital-specific MRSA data and case-based practice with additional precautions.
Outcome Measure.The rate of nosocomial MRSA acquisition was calculated as the number of cases of nosocomial MRSA acquisition per 100 days that a person with MRSA colonization or infection detected at admission is present in the hospital (“admission MRSA” exposure-days) for 3 time periods: June 2002-February 2003 (before the Toronto outbreak of severe acute respiratory syndrome [SARS]), June 2003-February 2004 (after the outbreak of SARS), and June 2004-February 2005 (after education). A case of nosocomial acquisition of MRSA colonization or infection represented a patient first identified as colonized or infected more than 72 hours after admission or at admission after a previous hospitalization.
Results.The rate of nosocomial acquisition of MRSA colonization or infection was 8.8 cases per 100 admission MRSA exposure-days for the period before SARS, 3.8 cases per 100 admission MRSA exposure-days for the period after SARS (P < .001 for before SARS vs after SARS), and 1.9 cases per 100 admission MRSA exposure-days for the period after education (P = .02 for after education vs before education). The volume of alcohol-based handrub purchased was apparently stable, with 4,010 L during fiscal year 2003-2004 (April 2003-March 2004) compared with 3,780 L during fiscal year 2004—2005. The observed rate of compliance with hand washing did not change significantly (40.9% during education vs 44.2% after education; P = .23). The total number of patients screened for MRSA colonization was not different in the 3 periods.
Conclusions.The rate of nosocomial acquisition of MRSA colonization or infection decreased after SARS and was further reduced in association with a hospital-wide education program.

Objective:
To determine the impact of the screening test, nursing workload, handwashing rates, and dependence of handwashing on risk level of patient visit on methicillin-resistant Staphylococcus aureus (MRSA) transmission among hospitalized patients.
Setting:General medical ward.
Methods:Monte Carlo simulation was used to model MRSA transmission (median rate per 1,000 patient-days). Visits by healthcare workers (HCWs) to patients were simulated, and MRSA was assumed to be transmitted among patients via HCWs.
Results:The transmission rate was reduced from 0.89 to 0.56 by the combination of increasing the sensitivity of the screening test from 80% to 99% and being able to report results in 1 day instead of 4 days. Reducing the patient-to-nurse ratio from 4.3 in the day and 6.8 at night to 3.8 and 5.7, respectively, reduced the number of nosocomial infections from 0.89 to 0.85; reducing the ratio to 1 and 1, respectively, further reduced the number of nosocomial infections to 0.32. Increases in handwashing rates by 0%, 10%, and 20% for high-risk visits yielded reductions in nosocomial infections similar to those yielded by increases in handwashing rates for all visits (0.89, 0.36, and 0.24, respectively). Screening all patients for MRSA at admission reduced the transmission rate to 0.81 per 1,000 patient-days from 1.37 if no patients were screened.
Conclusion:Within the ranges of parameters studied, the most effective strategies for reducing the rate of MRSA transmission were increasing the handwashing rates for visits involving contact with skin or bodily fluid and screening patients for MRSA at admission. Objective:
To obtain accurate data regarding the handwashing behavior and patterns of visits to patients by healthcare workers (HCWs).
Methods:All visits by HCWs to selected patient rooms were recorded for 3 days and 2 nights. Additionally, 5 nurses were observed for 1 day each and 2 nurses were observed for 1 night each. Nurses were observed for their entire shifts and all of their activities were recorded.
Setting:A general medical ward in a tertiary-care hospital.
Participants:Convenience samples of HCWs and patients.
Results:Patients were visited every 25 minutes on average. Monitoring rooms and observing nurses resulted in similar rates of patient visits. The highest level of risk was contact with body fluids in 11% of visits and skin in 40% of visits. The overall rate of handwashing was 46%; however, the rate was higher for visits involving contact with body fluids (81%) and skin (61%). Nurses returned immediately to the same patient 45% of the time. The rate of handwashing was higher for the last of a series of visits to a patient's room (53% vs 30%, P < .0001).
Conclusions:Nurses adjusted their handwashing rates in accordance with the risk level of each visit. Monitoring patient rooms and observing nurses yielded similar estimates of patient visits and proportions of visits involving contact with skin or body fluids. Education programs about hand hygiene may be more effective if patterns of care and levels of risk are incorporated into recommendations.

Many studies on colloidal nanoparticles have focused on the control of nanoparticle size and correlated this to the catalytic activity. Recently, our group has reported for the first time a technique that controlled the shape distribution of Pt nanoparticles. This was done by varying the concentration of the capping polymer and the platinum ion ratio used in the reductive synthesis of colloidal nanoparticles at room temperature. Cubic, tetrahedral and truncated octahedral (TO) particles have been prepared, making it possible to study the catalytic activities of nanoparticles with different shapes and facets.
Using transmission electron microscopy (TEM), we imaged the shapes and determined the shape distribution of platinum nanoparticles at different stages of their growth as a function of time. The small nanoparticles formed during the early stages of growth or at high polymer concentration displayed distributions with a dominance of tetrahedral shapes (see Figure la).

Background
The growth in the USA of ‘recovered memory therapy’ for past sexual abuse has caused great public and professional concern. It became apparent that the polarisation of views and fierce controversy within the American psychiatric community was in danger of bringing psychotherapy into disrepute and it seemed important to examine objectively the scientific evidence before such polarisation developed in the UK.
MethodA small working group reviewed their own experience, visited meetings and centres with expertise in this field, interviewed ‘retractors' and accused parents, and then began a comprehensive review of the literature.
ResultsThere is a vast literature but little acceptable research. Opinions are expressed with great conviction but often unsupported by evidence.
ConclusionsThe issue of false or recovered memories should not be allowed to confuse the recognition and treatment of sexually abused children. We concluded that when memories are ‘recovered’ after long periods of amnesia, particularly when extraordinary means were used to secure the recovery of memory, there is a high probability that the memories are false, i.e. of incidents that had not occurred. Some guidelines which should enable practitioners to avoid the pitfalls of memory recovery are offered.
