Effectiveness of medical treatment for bipolar disorder regarding suicide, self-harm and psychiatric hospital admission: between- and within-individual study on Danish national data

Background
Mood stabilisers are the main treatment for bipolar disorder. However, it is uncertain which drugs have the best outcomes.
AimsTo investigate whether rates of suicide, self-harm and psychiatric hospital admission in individuals with bipolar disorder differ between mood stabilisers.
MethodA cohort design was applied to people aged ≥15 years who were diagnosed with bipolar disorder and living in Denmark during 1995–2016. Treatment with lithium, valproate, other mood stabilisers and antipsychotics were compared in between- and within-individual analyses, and adjusted for sociodemographic characteristics and previous self-harm.
ResultsA total of 33 337 individuals with bipolar disorder were included (266 900 person-years). When compared with individuals not receiving treatment, those receiving lithium had a lower rate of suicide (hazard ratio 0.40, 95% CI 0.31–0.51). When comparing treatment and non-treatment periods in the same individuals, lower rates of self-harm were found for lithium (hazard ratio 0.74, 95% CI 0.61–0.91). Lower rates of psychiatric hospital admission were found for all drug categories compared with non-treatment periods in within-individual analyses (P<0.001). The low rates of self-harm and hospital admission for lithium in within-individual analyses were supported by results of between-individual analyses.
ConclusionsLithium was associated with lower rates of suicide, self-harm and psychiatric hospital readmission in all analyses. With respect to suicide, lithium was superior to no treatment. Although confounding by indication cannot be excluded, lithium seems to have better outcomes in the treatment of bipolar disorder than other mood stabilisers.

Background
Recently, artificial intelligence-powered devices have been put forward as potentially powerful tools for the improvement of mental healthcare. An important question is how these devices impact the physician-patient interaction.
AimsAifred is an artificial intelligence-powered clinical decision support system (CDSS) for the treatment of major depression. Here, we explore the use of a simulation centre environment in evaluating the usability of Aifred, particularly its impact on the physician–patient interaction.
MethodTwenty psychiatry and family medicine attending staff and residents were recruited to complete a 2.5-h study at a clinical interaction simulation centre with standardised patients. Each physician had the option of using the CDSS to inform their treatment choice in three 10-min clinical scenarios with standardised patients portraying mild, moderate and severe episodes of major depression. Feasibility and acceptability data were collected through self-report questionnaires, scenario observations, interviews and standardised patient feedback.
ResultsAll 20 participants completed the study. Initial results indicate that the tool was acceptable to clinicians and feasible for use during clinical encounters. Clinicians indicated a willingness to use the tool in real clinical practice, a significant degree of trust in the system's predictions to assist with treatment selection, and reported that the tool helped increase patient understanding of and trust in treatment. The simulation environment allowed for the evaluation of the tool's impact on the physician–patient interaction.
ConclusionsThe simulation centre allowed for direct observations of clinician use and impact of the tool on the clinician–patient interaction before clinical studies. It may therefore offer a useful and important environment in the early testing of new technological tools. The present results will inform further tool development and clinician training materials.

We apply the recent frame-invariant theory of separation spike formation to complex unsteady flows, including a turbulent separation bubble, an impinging jet, and flows around a freely moving cylinder and a freely rotating ellipse. We show how the theory captures the onset of material spike formation, without any assumption on the flow type (steady, periodic, unsteady) or separation type (on- or off-wall, fixed or moving boundaries). We uncover new phenomena, such as the transition from on-wall to off-wall separation, the merger of initially distinct spikes, and the presence of severe material spikes that remain hidden to previous approaches. Remarkably, even in steady flows around curved boundaries, we detect material spikes in the absence of flow reversal, the main ingredient to existing separation criteria. Together, our results unveil how an involved network of spikes arises, interacts and merges dynamically, leading to the final ejection of particles from the wall in highly transient flow separation processes.

In the frame of the COST ACTION ‘EMBOS’ (Development and implementation of a pan-European Marine Biodiversity Observatory System), coverage of intertidal macroalgae was estimated at a range of marine stations along the European coastline (Subarctic, Baltic, Atlantic, Mediterranean). Based on these data, we tested whether patterns in macroalgal diversity and distribution along European intertidal rocky shores could be explained by a set of meteo-oceanographic variables. The variables considered were salinity, sea surface temperature, photosynthetically active radiation, significant wave height and tidal range and were compiled from three different sources: remote sensing, reanalysis technique and in situ measurement. These variables were parameterized to represent average conditions (mean values), variability (standard deviation) and extreme events (minimum and maximum values). The results obtained in this study contribute to reinforce the EMBOS network approach and highlight the necessity of considering meteo-oceanographic variables in long-term assessments. The broad spatial distribution of pilot sites has allowed identification of latitudinal and longitudinal gradients manifested through species composition, diversity and dominance structure of intertidal macroalgae. These patterns follow a latitudinal gradient mainly explained by sea surface temperature, but also by photosynthetically active radiation, salinity and tidal range. Additionally, a longitudinal gradient was also detected and could be linked to wave height.

Examining how variability in population abundance and distribution is allotted among different spatial scales can inform of processes that are likely to generate that variability. Results of studies dealing with scale issues in marine benthic communities suggest that variability is concentrated at small spatial scales (from tens of centimetres to few metres) and that spatial patterns of variation are consistent across ecosystems characterized by contrasting physical and biotic conditions, but this has not been formally tested. Here we quantified the variability in the distribution of intertidal rocky shore communities at a range of spatial scales, from tens of centimetres to thousands of kilometres, both in the NE Atlantic and the Mediterranean, and tested whether the observed patterns differed between the two basins. We focused on canopy-forming macroalgae and associated understorey assemblages in the low intertidal, and on the distribution of Patella limpets at mid intertidal levels. Our results highlight that patterns of spatial variation, at each scale investigated, were consistent between the Atlantic and the Mediterranean, suggesting that similar ecological processes operate in these regions. In contrast with former studies, variability in canopy cover, species richness and limpet abundance was equally distributed among spatial scales, possibly reflecting the fingerprint of multiple processes. Variability in community structure of low intertidal assemblages, instead, peaked at the largest scale, suggesting that oceanographic processes and climatic gradients may be important. We conclude that formal comparisons of variability across scales nested in contrasting systems are needed, before any generalization on patterns and processes can be made.

Coastal ecosystems are highly complex and driven by multiple environmental factors. To date we lack scientific evidence for the relative contribution of natural and anthropogenic drivers for the majority of marine habitats in order to adequately assess the role of different stressors across the European seas. Such relationship can be investigated by analysing the correlation between environmental variables and biotic patterns in multivariate space and taking into account non-linearities. Within the framework of the EMBOS (European Marine Biodiversity Observatory System) programme, hard bottom intertidal communities were sampled in a standardized way across European seas. Links between key natural and anthropogenic drivers and hard bottom communities were analysed using Boosted Regression Trees modelling. The study identified strong interregional variability and showed that patterns of hard bottom macroalgal and invertebrate communities were primarily a function of tidal regime, nutrient loading and water temperature (anomalies). The strength and shape of functional form relationships varied widely however among types of organisms (understorey algae composing mostly filamentous species, canopy-forming algae or sessile invertebrates) and aggregated community variables (cover or richness). Tidal regime significantly modulated the effect of nutrient load on the cover and richness of understorey algae and sessile invertebrates. In contrast, hydroclimate was more important for canopy algae and temperature anomalies and hydroclimate separately or interactively contributed to the observed patterns. The analyses also suggested that climate-induced shifts in weather patterns may result in the loss of algal richness and thereby in the loss of functional diversity in European hard bottom intertidal areas.

Within the COST action EMBOS (European Marine Biodiversity Observatory System) the degree and variation of the diversity and densities of soft-bottom communities from the lower intertidal or the shallow subtidal was measured at 28 marine sites along the European coastline (Baltic, Atlantic, Mediterranean) using jointly agreed and harmonized protocols, tools and indicators. The hypothesis tested was that the diversity for all taxonomic groups would decrease with increasing latitude. The EMBOS system delivered accurate and comparable data on the diversity and densities of the soft sediment macrozoobenthic community over a large-scale gradient along the European coastline. In contrast to general biogeographic theory, species diversity showed no linear relationship with latitude, yet a bell-shaped relation was found. The diversity and densities of benthos were mostly positively correlated with environmental factors such as temperature, salinity, mud and organic matter content in sediment, or wave height, and related with location characteristics such as system type (lagoons, estuaries, open coast) or stratum (intertidal, subtidal). For some relationships, a maximum (e.g. temperature from 15–20°C; mud content of sediment around 40%) or bimodal curve (e.g. salinity) was found. In lagoons the densities were twice higher than in other locations, and at open coasts the diversity was much lower than in other locations. We conclude that latitudinal trends and regional differences in diversity and densities are strongly influenced by, i.e. merely the result of, particular sets and ranges of environmental factors and location characteristics specific to certain areas, such as the Baltic, with typical salinity clines (favouring insects) and the Mediterranean, with higher temperatures (favouring crustaceans). Therefore, eventual trends with latitude are primarily indirect and so can be overcome by local variation of environmental factors.
The Effect of Graphene Oxide/Reduced Graphene Oxide Functionalized with Metal Nanoparticles on Dermal, Bacterial, and Cancerous/Non-Cancerous Epidermal Cells

The unique planar structures and high surface area of graphene oxide (GO) and reduced graphene oxide (rGO) has induced great interest as drug delivery platforms. Silver and platinum nanoparticles are used in medicine, biotechnology and cosmetics and have electrocatalytic properties. However, GO has been found to be toxic to a variety of cells; pure graphene is insoluble; and nanoparticles aggregate, diminishing their activity. This research functionalized GO and rGO with silver or platinum nanoparticles (Ag/PtNPs) and experimental solutions were then tested on bacteria, dermal fibroblasts (DFBC’s), and cancerous (SCC13’s) and non-cancerous (DO33’s) epidermal cells to determine toxicity and/or cell viability.
GO was functionalized with Ag or Pt salts, forming metalized-GO; then reduced with NaBH4. Ag-rGO, depending on nanoparticle size, killed either S. Aureus or K. Pneumoniae, while Pt-rGO and rGO had no effect. 1mM Ag-prGO concentrations diluted 1:100 with DMEM was toxic to SCC13 cancerous keratinocytes but showed reduced toxicity to DO33 non-cancerous keratinocytes; whereas Pt-rGO and rGO exerted little effect on SCC13’s and DO33’s at all concentrations. All test solutions adhered to DFBC’s and influenced the orientation of their growth, suggesting potential use in wound dressings to aid healing.

– Richard Fleischer (dir), Fantastic Voyage (1966)What does it mean to represent the interior of the human body? Recent scandals around exhibitions of ‘real’ dissected human bodies, such as Gunther von Hagens's notorious Body Worlds exhibition and the recent closure of a similar type of anatomical exhibition, ‘Our Body – à corps ouvert’ by a French court in 2009, have served to keep this question alive. And yet the interior of the body has been represented in the cinema practically from the beginning of narrative films. Horror films and war films regularly expose the interior of the body for its shock value. Indeed, with increasingly sophisticated special effects now available to film directors, the interior of the body is exposed with a realism designed to turn the stomach of the most hardened spectator. However, this is not the only context in which the body's interior has been represented in fiction. The interior of the body can also be explored in more artistic and scientific ways, with such representations often playing on the wonder or even the miracle of the human ‘machine’ both to engage the audience and to convey a message about the significance of life. Yet even if one adopts this more positive attitude to the interior of the body, there are many different ways to do so, and new scientific discoveries have served to add to this panoply.
Prediction of the radiative properties of reconstructed alpha-SiC foams used for concentrated solar applications

A SiC-based ceramic foam applied in solar thermal processes was characterized in detail in terms of its textural parameters and its radiative properties. Scanning electron microscopy and x-ray µ-tomography were first performed to investigate the 3D texture of the sample at several length scales. Infrared reflectance microscopy was also applied to probe the local optical responses on the struts constituting the foam. Based on the whole set of experimental data, a numerical tool (C++) was implemented to reconstruct virtual SiC foams. A Monte Carlo Ray Tracing code (iMorphRad, C++) was then used to compute the normal spectral emittance for the real SiC foam and for another reconstructed SiC foam with similar textural features. The two numerically determined emittances were then compared with previous infrared spectroscopy experimental measurements. This numerical procedure enables us to propose a methodology for the design of SiC foams with prescribed radiative properties.

Numerous new Solar System objects, mainly asteroids, are still detected nowadays but their physical and dynamical characteristics remain not accurately determined, until dedicated observations and analysis are made. However, the Virtual Observatory is a perfect framework to search for this characterization by data mining. We are developing two studies for this goal by exploring the DENIS infrared and DFBS spectroscopic surveys. In order to scan the survey catalogues and to search for Solar System objects, we have developed a VO compliant workflow founded on the use of a recent VO tool labelled SkyBoT (Sky Bodies Tracker) (Berthier et al. 2005) and on the VizieR service provided by CDS. A public version of SkyBoT is implemented in the sky atlas Aladin since January 2006. It allows us to quickly get the precise coordinates of the asteroids, planets and natural satellites in any star field, provided we know its center, its size and the precise date of the exposure (between 1949 and 2009). Our data mining workflow performs cone search queries on SkyBoT and VizieR to extract all the known astronomical objects observed by the survey. Then the sources which are detected in the survey are correlated with the known objects to determine the matched and unmatched objects.
Tuberculosis in Hemodialysis Patients in New Jersey: A Statewide Study

Objective:
To study the incidence of tuberculosis (TB), tuberculin skin testing (TST) practices, and infection control practices at outpatient hemodialysis centers.
Design:Mail surveys performed in December 1994 and 1995.
Main Outcome Measures:The numbers of patients with incident active TB during 1994 and 1995, TST policies during 1994, and TB infection control policies in 1994.
Setting:All outpatient dialysis centers in New Jersey.
Patients or Participants:Healthcare workers and patients in dialysis centers in New Jersey.
Results:Of 47 centers, 41 provided information on TST and TB infection control policies and practices. TSTs were performed on newly hired healthcare workers at all 41 centers and on established workers at 39 centers. In contrast, only 1 center reported performing TSTs on hemodialysis patients; 5 other centers reported screening of patients for TB using chest radiographs. Active TB was reported in 3 of 4,550 chronic hemodialysis patients in 1994 (rate, 66/100,000 patient-years) and in 4 of 4,831 patients in 1995 (rate, 83/100,000 patient-years). Both rates were several times higher than the rate in the New Jersey general population during this period (10.7-10.8/100,000).
Conclusion:Although based on small numbers of patients with TB, we found a relatively high incidence of TB among hemodialysis patients in New Jersey. Most centers reported performing TSTs on workers but not on patients. These results suggest the need for improved TB screening and infection control precautions at outpatient dialysis centers.
