COVID-19 vaccine knowledge, beliefs and attitudes among Oregon healthcare provider types

Background: During this pandemic, the public has struggled to navigate the abundance of COVID-19 vaccine misinformation, and it is unclear how this misinformation has affected medical providers and their recommendations for patients. We sought to understand differences in COVID-19 vaccine knowledge, beliefs, and attitudes among Oregon healthcare provider types and regions of practice (rural, suburban, urban). Methods: A 36-question survey was constructed using Qualtrics with consultation from a survey methodologist. The survey was reviewed and approved by OHSU IRB and distributed via listserv or social media posting to provider societies in Oregon, including nurse practitioners (NPs), naturopathic doctors (NDs), physician assistants (PAs), doctors of medicine (MDs), doctors of osteopathic medicine (DOs), or practioners with a bachelor of medicine–bachelor of surgery (MBBS), and via the Oregon Health Authority (OHA) immunization practice listserv. The survey accepted responses from July 9 to August 12, 2021. Participants were volunteers and responses were anonymous. Results: We collected 101 responses. Among them, 87 participants completed 100% of survey questions. Survey respondents were predominantly White females aged 41–50 years with an MD, DO, or MBBS. The overall COVID-19 vaccination rate of respondents was 94.6%. The vaccination rate was highest among the 4 NDs and 7 PAs at 100%, followed by 78 MDs, DOs, and MBBSs at 96.2%, and 12 NPs at 75%. Of NP respondents, 67% practiced rurally; 25.6% of MDs, DOs, and MBBSs practiced rurally; and 25% of NDs and 28.6% of PAs practiced rurally. In total, 22% of NPs did not feel comfortable recommending the COVID-19 vaccine to patients, compared to 1% of MDs, DOs, and MBBSs and 0% of NDs or PAs. All provider types had high rates of disagreement with the statement that the COVID-19 pandemic had increased their trust in vaccine safety: 44% of NPs; 29% of PAs; 25% of NDs; and 7% of MDs, DOs, and MBBSs. Among 19 rural providers, 19% indicated mistrust in public health to ensure that vaccines are safe versus 3% in suburban areas and 0% in urban areas. Conclusions: COVID-19 vaccine hesitancy is prevalent among healthcare providers and may be higher in NPs and those practicing rurally. Unfortunately, the response rate of NPs was low. Future research should focus on these providers to better understand their knowledge, beliefs, and attitudes about COVID-19 vaccines. These results can also inform future targeted vaccine education to healthcare providers during public health crises.

Timing dysfunction and cerebellar resting state functional connectivity abnormalities in youth at clinical high-risk for psychosis

Background
Consistent with pathophysiological models of psychosis, temporal disturbances in schizophrenia spectrum populations may reflect abnormal cortical (e.g. prefrontal cortex) and subcortical (e.g. striatum) cerebellar connectivity. However, few studies have examined associations between cerebellar connectivity and timing dysfunction in psychosis populations, and none have been conducted in youth at clinical high-risk (CHR) for psychosis. Thus, it is currently unknown if impairments in temporal processes are present in CHR youth or how they may be associated with cerebellar connectivity and worsening of symptoms.
MethodsA total of 108 (56 CHR/52 controls) youth were administered an auditory temporal bisection task along with a resting state imaging scan to examine cerebellar resting state connectivity. Positive and negative symptoms at baseline and 12 months later were also quantified.
ResultsControlling for alcohol and cannabis use, CHR youth exhibited poorer temporal accuracy compared to controls, and temporal accuracy deficits were associated with abnormal connectivity between the bilateral anterior cerebellum and a right caudate/nucleus accumbens striatal cluster. Poor temporal accuracy accounted for 11% of the variance in worsening of negative symptoms over 12 months.
ConclusionsBehavioral findings suggest CHR youth perceive durations of auditory tones as shortened compared to objective time, which may indicate a slower internal clock. Poorer temporal accuracy in CHR youth was associated with abnormalities in brain regions involved in an important cerebellar network implicated in prominent pathophysiological models of psychosis. Lastly, temporal accuracy was associated with worsening of negative symptoms across 12 months, suggesting temporal dysfunction may be sensitive to illness progression.
Ancient Cities and Landscapes in the Kurdistan Region of Iraq: The Erbil Plain Archaeological Survey 2012 Season1

In 2012, the Erbil Plain Archaeological Survey (EPAS) conducted its first season of fieldwork. The project's goal is the complete mapping of the archaeological landscape of Erbil, with an emphasis on the Neo-Assyrian and Hellenistic periods. It will test the hypothesis that the Neo-Assyrian landscape was closely planned. This first report emphasizes the project's field methodology, especially the use of a variety of satellite remote sensing imagery. Our preliminary results suggest that the plain was part of the urbanized world of Mesopotamia, with new cities of the Bronze Age, Iron Age, and Sasanian era identified.

Niedenthal et al. recognize that cultural differences are important when interpreting facial expressions. Nonetheless, many of their core observations derive more from individualistic cultures than from collectivist cultures. We discuss two examples from the latter: (1) lower rates of mutual eye contact, and (2) the ubiquity of specific “functional smiles.” These examples suggest constraints on the assumptions and applicability of the SIMS model.

Alternatives to standard acute in-patient care in England: readmissions, service use and cost after discharge

Background
Residential alternatives to standard psychiatric admissions are associated with shorter lengths of stay, but little is known about the impact on readmissions.
AimsTo explore readmissions, use of community mental health services and costs after discharge from alternative and standard services.
MethodData on use of hospital and community mental health services were collected from clinical records for participants in six alternative and six standard services for 12 months from the date of index admission.
ResultsAfter discharge, the mean number and length of readmissions, use of community mental health services and costs did not differ significantly between standard and alternative services. Cost of index admission and total 12-month cost per participant were significantly higher for standard services.
ConclusionsShorter lengths of stay in residential alternatives are not associated with greater frequency or length of readmissions or greater use of community mental health services after discharge.

In this article we measure the impact of gender on women's legislative behavior by utilizing a unique research design. We compare whether women and men of the same political party represent the same congressional district differently with respect to women's issues. Using bill sponsorship and floor remarks during the 104th to 107th sessions of the U.S. House of Representatives as measures of legislative behavior, we find that female legislators who replace men in the same district introduce more women's issues bills in Congress. Although our conclusion that women legislators represent women's issues more frequently in the House supports existing research, our results do so in a new and more effective way by controlling for the competing explanations of party identification and district opinion as factors determining a legislator's behavior.
