Geographical variation in hepatitis C-related severe liver disease and patient risk factors: a multicentre cross-sectional study

Despite promising steps towards the elimination of hepatitis C virus (HCV) in the UK, several indicators provide a cause for concern for future disease burden. We aimed to improve understanding of geographical variation in HCV-related severe liver disease and historic risk factor prevalence among clinic attendees in England and Scotland. We used metadata from 3829 HCV-positive patients consecutively enrolled into HCV Research UK from 48 hospital centres in England and Scotland during 2012–2014. Employing mixed-effects statistical modelling, several independent risk factors were identified: age 46–59 y (ORadj 3.06) and ≥60 y (ORadj 5.64) relative to <46 y, male relative to female sex (ORadj 1.58), high BMI (ORadj 1.73) and obesity (ORadj 2.81) relative to normal BMI, diabetes relative to no diabetes (ORadj 2.75), infection with HCV genotype (GT)-3 relative to GT-1 (ORadj 1.75), route of infection through blood products relative to injecting drug use (ORadj 1.40), and lower odds were associated with black ethnicity (ORadj 0.31) relative to white ethnicity. A small proportion of unexplained variation was attributed to differences between hospital centres and local health authorities. Our study provides a baseline measure of historic risk factor prevalence and potential geographical variation in healthcare provision, to support ongoing monitoring of HCV-related disease burden and the design of risk prevention measures.

The Repugnant Conclusion is an implication of some approaches to population ethics. It states, in Derek Parfit's original formulation,
For any possible population of at least ten billion people, all with a very high quality of life, there must be some much larger imaginable population whose existence, if other things are equal, would be better, even though its members have lives that are barely worth living. (Parfit 1984: 388)

New metrics are introduced in the space of random measures and are applied, with various modifications of the contraction method, to prove existence and uniqueness results for self-similar random fractal measures. We obtain exponential convergence, both in distribution and almost surely, of an iterative sequence of random measures (defined by means of the scaling operator) to a unique self-similar random measure. The assumptions are quite weak, and correspond to similar conditions in the deterministic case.
The fixed mass case is handled in a direct way based on regularity properties of the metrics and the properties of a natural probability space. Proving convergence in the random mass case needs additional tools, such as a specially adapted choice of the space of random measures and of the space of probability distributions on measures, the introduction of reweighted sequences of random measures and a comparison technique.

Introduction
In this chapter I will discuss a mathematical framework for deterministic and random (or non-deterministic) fractals. The approach we take is via scaling laws and scaling operators, the latter also being known as iterated function systems. This gives a very attractive theory with many applications, particularly to computer graphics and to image and data compression. Various applications of fractals are discussed elsewhere in this book.
The essential ideas will first be presented by means of a number of standard examples. Readers who wish to obtain a relatively informal overview of the material can accordingly restrict themselves to §5.2 and §5.3, and to the less formal parts of the later sections. §5.4, §5.5 and §5.6 develop much of the mathematics behind these ideas. I have tried to keep the mathematics self-contained, and in particular have attempted to motivate and develop from first principles the relevant notions of metric spaces, measure theory, and probability theory. The later sections may perhaps serve as a brief introduction to some aspects of these subjects.
It is perhaps worth mentioning here one point that sometimes causes confusion. A ‘mathematical’ fractal in a certain precise sense looks the same at all scales; i.e. when examined under a microscope at no matter what magnification it will appear similar to the original object. On the other hand a ‘physical’ fractal will display this ‘self-similarity’ for only a range of magnifications or scales. The mathematical object will of course only be an accurate model within this particular range.
Examples of non-integer dimensional sets with scaling properties have long been known to mathematicians.

A yield surface is proposed that can be fit to the plastic flow properties of a broad class of solids which exhibit plastic compressibility and different yield points in tension and compression. The yield surface is proposed to describe cellular solids, including foamed metals, and designed to be fit to three simple experimental results: (1) the compressive stress-strain response (including densification), (2) the difference between the tensile and compressive yield points and (3) the degree of compressibility of the foam, as measured by the lateral expansion during a uniaxial compression test. The model is implemented using finite elements and used to study the effects of plastic compressibility on two problems: the compression of a doubly notched specimen and indentation by a spherical indenter.

We prove a new energy or Caccioppoli type estimate for minimisers of the model functional ∫Ω|Du|2 + (det Du)2, where Ω ⊂ 2 and u: Ω → 2. We apply this to establish C∞ regularity for minimisers except on a closed set of measure zero. We also prove a maximum principle and use this to establish everywhere continuity of minimisers.

An ordinal in a model of set theory is truly countable if its set of predecessors is countable in the real world. We classify the order types of the sets of truly countable ordinals. Models with indiscernibles and other related results are discussed.

We prove the following extension of a result of Keisler and Morley. Suppose is a countable model of ZFC and c is an uncountable regular cardinal in . Then there exists an elementary extension of which fixes all ordinals below c, enlarges c, and either (i) contains or (ii) does not contain a least new ordinal.
Related results are discussed.
