Background
Individuals with treatment-resistant depression (TRD) experience a high burden of illness. Current guidelines recommend a stepped care approach for treating depression, but the extent to which best-practice care pathways are adhered to is unclear.
AimsTo explore the extent and nature of ‘treatment gaps’ (non-adherence to stepped care pathways) experienced by a sample of patients with established TRD (non-response to two or more adequate treatments in the current depressive episode) across three cities in the UK.
MethodFive treatment gaps were considered and compared with guidelines, in a cross-sectional retrospective analysis: delay to receiving treatment, lack of access to psychological therapies, delays to medication changes, delays to adjunctive (pharmacological augmentation) treatment and lack of access to secondary care. We additionally explored participant characteristics associated with the extent of treatment gaps experienced.
ResultsOf 178 patients with TRD, 47% had been in the current depressive episode for >1 year before initiating antidepressants; 53% had received adequate psychological therapy. A total of 47 and 51% had remained on an unsuccessful first and second antidepressant trial respectively for >16 weeks, and 24 and 27% for >1 year before medication switch, respectively. Further, 54% had tried three or more antidepressant medications within their episode, and only 11% had received adjunctive treatment.
ConclusionsThere appears to be a considerable difference between treatment guidelines for depression and the reality of care received by people with TRD. Future research examining representative samples of patients could determine recommendations for optimising care pathways, and ultimately outcomes, for individuals with this illness.

The first demonstration of laser action in ruby was made in 1960 by T. H. Maiman of Hughes Research Laboratories, USA. Many laboratories worldwide began the search for lasers using different materials, operating at different wavelengths. In the UK, academia, industry and the central laboratories took up the challenge from the earliest days to develop these systems for a broad range of applications. This historical review looks at the contribution the UK has made to the advancement of the technology, the development of systems and components and their exploitation over the last 60 years.

Aims and method
True Colours is an automated symptom monitoring programme used by National Health Service psychiatric services. This study explored whether patients with unipolar treatment-resistant depression (TRD) found this a useful addition to their treatment regimes. Semi-structured qualitative interviews were conducted with 21 patients with TRD, who had engaged in True Colours monitoring as part of the Lithium versus Quetiapine in Depression study. A thematic analysis was used to assess participant experiences of the system.
ResultsSix main themes emerged from the data, the most notable indicating that mood monitoring increased patients' insight into their disorder, but that subsequent behaviour change was absent.
Clinical implicationsPatients with TRD can benefit from mood monitoring via True Colours, making it a worthwhile addition to treatment. Further development of such systems and additional support may be required for patients with TRD to experience further benefits as reported by other patient groups.

Objective
To profile adults who eat less than the recommended servings of fruit and vegetables per day.
DesignAustralia-wide population telephone survey on a random sample of the Australian population, with results analysed by univariate and multivariate models.
SettingAustralia.
SubjectsOne thousand one hundred and eight interviews, respondents’ (49·3 % males) mean age was 45·12 (sd 17·63) years.
ResultsOverall 54·8 % and 10·7 % were eating the recommended number of servings of fruit and vegetables. Variables included in the multivariate model indicating low fruit consumption included gender, age, employment, education and those who were less likely to consider the safety and quality of food as important. In regard to low vegetable consumption, people who were more likely to do the food shopping only ‘some of the time’ and have a high level of trust in groups of people such as immediate family, neighbours, doctors and different levels of government were included in the final model. They were also less likely to neither consider the safety and quality of food as important nor trust organisations/institutions such as the press, television and politicians. In the final model depicting both low fruit and low vegetable servings, sex, age and a low level of importance with regard to safety and quality of food were included.
ConclusionTo increase fruit and vegetable consumption, research into a broad range of determinants associated with behaviours should be coupled with a deeper understanding of the process associated with changing behaviours. While levels of trust are related to behaviour change, knowledge and attitudes about aspects associated with safety and quality of food are also of importance.

New radiocarbon calibration curves, IntCal04 and Marine04, have been constructed and internationally ratified to replace the terrestrial and marine components of IntCal98. The new calibration data sets extend an additional 2000 yr, from 0–26 cal kyr BP (Before Present, 0 cal BP = AD 1950), and provide much higher resolution, greater precision, and more detailed structure than IntCal98. For the Marine04 curve, dendrochronologically-dated tree-ring samples, converted with a box diffusion model to marine mixed-layer ages, cover the period from 0–10.5 cal kyr BP. Beyond 10.5 cal kyr BP, high-resolution marine data become available from foraminifera in varved sediments and U/Th-dated corals. The marine records are corrected with site-specific 14C reservoir age information to provide a single global marine mixed-layer calibration from 10.5–26.0 cal kyr BP. A substantial enhancement relative to IntCal98 is the introduction of a random walk model, which takes into account the uncertainty in both the calendar age and the 14C age to calculate the underlying calibration curve (Buck and Blackwell, this issue). The marine data sets and calibration curve for marine samples from the surface mixed layer (Marine04) are discussed here. The tree-ring data sets, sources of uncertainty, and regional offsets are presented in detail in a companion paper by Reimer et al. (this issue).

Many resource managers are shifting their approach from managing resources such as fish, wildlife, and water separately to managing for the integrity of entire ecosystems (Christensen et al., 1996). Indeed, nearly all resource management agencies in the USA have recognized that informed management decisions cannot be made exclusively at the level of habitat units or local sites. It is generally accepted that ecological patterns and processes must be considered over large areas when biodiversity and ecological function must be maintained while the goods and services desired by the public are provided. For example, forest managers must determine the patterns and timing of tree harvesting while maintaining an amount and arrangement of habitats that will sustain many species. Managers of parks and nature reserves must be attentive to actions occurring on surrounding lands outside their jurisdiction. Aquatic resource managers must broaden their perspective to encompass the terrestrial and human landscape to manage stream and lake resources effectively (Hynes, 1975, widely regarded as the father of modern stream ecology and quoted above; Naiman et al., 1995). Landscape ecology also is implicit in the paradigm of ecosystem management (Grumbine,1994; Christensen et al., 1996).
Despite the acknowledged importance of a landscape perspective by both scientists and resource managers, determining how to implement management at broader scales is very much a work in progress.

C60 Cl6 can be phenylated and arylated to give derivatives of the type C60Ar5Cl, which may be readily converted to C60Ar5. The compounds C60Ar5 and various other phenylated derivatives have been isolated from the product of reaction of [60]fullerene with bromine/ferric chloride/benzene, and partially characterised.

This paper describes four key measurements of critical coating performance attributes which can affect coated optical fiber performance. The four measurement methods to be described are: 1) primary coating adhesion, 2) coating toughness, 3) primary coating equilibrium modulus, and 4) primary coating glass transition temperature.
