Predictors of olfactory improvement after endoscopic sinus surgery in chronic rhinosinusitis with nasal polyps

Objective
This study aimed to determine the predictors of olfactory improvement after endoscopic sinus surgery among patients with chronic rhinosinusitis with nasal polyps.
MethodThis prospective cohort study included patients admitted to a university hospital between 2006 and 2012. Assessment using odour identification testing, a sinonasal symptom questionnaire, the Rhinosinusitis Disability Index and mucus biomarker levels was performed at various time points. Correlation of variables with identification score differences at six post-operative time points and at baseline was performed, followed by multiple linear regression to determine significant predictors at each of the six post-operative time points.
ResultsBaseline absence of acute sinusitis, elevated serpin F2 and anterior rhinorrhoea predict early olfactory improvement, whereas baseline allergic rhinitis predicts late olfactory improvement. Baseline odour identification score was the strongest predictor across all time points.
ConclusionPatients with chronic rhinosinusitis and nasal polyps with worse disease or baseline olfactory function may benefit more from endoscopic sinus surgery in terms of olfactory improvement.
Detection and first characterization of an uncommon haptoglobin in porcine saliva of pigs with rectal prolapse by using boronic acid sample enrichment

Salivary glycoprotein profiles, obtained after boronic acid enrichment, were studied for the first time in pigs in order to search for specific overall alterations related to acute inflammatory condition. Five healthy pigs and five pigs suffering from rectal prolapse were used, and the levels of acute phase proteins were measured to determine the degree of inflammation of the animals. The enriched glycoprotein profiles, achieved by two-dimensional gel electrophoresis (2DE) were statistically evaluated and spots that appeared differentially regulated between states were subjected to MS analysis for protein identification. Spots from three unique proteins were identified: carbonic anhydrase VI (CA VI), α-1-antichymotrypsin and haptoglobin (Hp). CA VI appeared as two adjacent horizontal spot trains in the glycoprotein profile of healthy animals in its regular isoelectric points (pI). One spot of α-1-antichymotrypsin was found in saliva from pigs with rectal prolapse in an unusual basic pI, and was considered as a breakdown product. Hp was identified as several spot trains in saliva from pigs with rectal prolapse in an unusual alkaline pI and was consequently further investigated. SDS-PAGE and 2DE of paired serum and saliva samples combined with Western blot analysis showed that the unusual Hp position observed in saliva samples was absent in serum. Furthermore, N-glycans from serum and saliva Hp glycopatterns were evaluated from SDS-PAGE Hp bands and showed that the serum N-glycan distribution in Hp β-chain was comparable in quantity and quality in both groups of animals. In saliva, no Hp β-chain derived N-glycans could unambiguously be identified from this sample set, thus needing further detailed investigations in the future.

A photochromic tracer method has been used to record pulsatile flow velocity profiles simultaneously at three axial locations along a flow channel. Two major advantages of this multiple-trace method are that it enables velocity data to be acquired in an efficient non-invasive manner and that it provides a detailed description of the spatial relationship of the flow field. The latter is found to be particularly useful in the investigation of transitional type flows; for example, in describing coherent flow structures. Studies of the flow patterns in tubes with mild to moderate degrees of vessel constriction were performed using a 2.9 Hz sinusoidal flow superimposed on a steady flow (frequency parameter of 7.5; mean and modulation Reynolds numbers of 575 and 360, respectively). With mild constrictions (< 50% area reduction), isolated regions of vortical and helical structures were observed primarily during the deceleration phase of the flow cycle and in the vicinity of the reattachment point. As expected, these effects were accentuated when the constriction was asymmetric. For moderate constrictions (50%–80%), transition to turbulence was triggered just before peak flow through the breakdown of waves and streamwise vortices that were shed in the high-shear layer. During this vortex generation phase of the flow cycle, the wall shear stress fluctuated quite intensely, especially in the vicinity of the reattachment point, and its instantaneous value increased by at least a factor of eight. Such detailed descriptions of the transition to turbulence and of the spatial and temporal variation of the wall shear stress, particularly near the reattachment point, have not been previously reported for pulsatile flow through constricted tubes. The observed wall shear stress variations support a proposal by Mao & Hanratty (1986) of an interaction of the imposed flow oscillation with the turbulent fluctuations within the viscous sublayer.
S5 0836+710 A Kelvin-Helmholtz Unstable Jet on Parsec Scales?

The highly redshifted quasar S5 0836+710 (z = 2.172) displays an outstanding one sided jet with complex morphology. The continuous jet shows several remarkable lateral displacements (kinks) of its ridgeline on various length scales. Despite the strength of these distortions the jet is not destroyed by them and remains well collimated. We investigated the possibility to explain the displacements by Kelvin-Helmholtz instabilities.

We derive the temperature and density structure of the accretion disk of the dwarf nova U Gem in quiescence from 3D radiative line and continuum transport calculations of a differentially rotating disk.

Interferometry offers an improvement in the accuracy with which astrometric measurements can be made. Using this technique, radio astronomers together with geodeticists have established a global inertial reference frame that is accurate to 0.1 milliarcseconds. At optical wavelengths, interferometry was first developed by Michelson at the turn of the twentieth century, but due to the complexities of precise beam combination at high speeds, it has lagged in its development. Now, with the availability of lasers, detectors and computers that allow path length compensation on millisecond time scales and distance determination between light collectors with a precision of 0.01 μm, interferometry at optical wavelengths will achieve the results in astrometry comparable to those at radio wavelengths.

We present 3-D LTE radiative transfer calculations [1] for H, He and Ca in accretion disks (AD) of dwarf novae in quiescence. The model disk is assumed to be in hydrostatic equilibrium vertically, and to rotate with Keplerian velocities. Calculated emission lines are fitted to phase-averaged, continuum-subtracted spectra of U Gem (Fig. 1) and T Leo (Fig. 2). Up to four parameters of the AD have been fitted: distance D, baryonic number density N, isotropic turbulence Vtu and disk temperature T; the latter two are assumed to be constant throughout the disk. Geometrical parameters are from [2] and [3].

Significant progress has been made in building multilevel copper interconnection systems for advanced microelectronics. In this article, we examine some of the materials science issues underlying this progress, and indicate where significant materials challenges remain. It is probable that several approaches to process integration will be developed for copper interconnections, as has been the case with aluminum systems. The first successful demonstration of a fully integrated 4-level copper/polyimide (Cu/PI) interconnection system has been described by Luther et al. of IBM. A schematic cross section of this interconnection system is shown in Figure 1, indicating multiple layers of BPDA-PDA polyimide (PI 5810), Cu lines and studs, and layers of Ta and Si3N4 which serve as diffusion barriers, adhesion layers, and stopping layers in the patterning and planarization processes. This system demonstrates excellent planarity, as shown in the SEM cross section in Figure 2. The electromigration lifetime of this Cu/PI system is greatly improved relative to state-of-the-art aluminum-based systems, and the dielectric integrity appears adequate. Signal propagation studies also confirm the performance improvements anticipated for copper as a low-resistivity conductor and the use of Cu may allow significant capacitance reduction (≃ 25%) simply by scaling Cu lines to equal the resistance of Al lines. In parallel with efforts to introduce Cu metallization for its low resistivity, extensive efforts are under way to replace SiO2 with lower dielectric constant insulators.
Bulk Relativistic Motion in a Complete Sample of Flat Spectrum Radio Sources

As part of a multi-epoch and multi-wavelength study of the physics of core dominated radio sources we have investigated the occurrence of apparent superluminal motion and other indications for bulk relativistic motion (b.r.m) in a statistically complete, flux density limited (S5GHz ≥ 1Jy) sample of 13 objects with flat radio spectra These sources come from the S5-survey (Kühr et al., 1981) and are optically identified as 7 quasars and 6 BL-Lac objects. They have been observed over a wide range of frequencies, from radio through X-rays (s. Eckart et al., 1986 and references therein). Radiomaps have been obtained at frequencies from 327 MHz to 22.2 GHz with resolutions from arcseconds to 0.2 mas, using the VLA, MERLIN and telescopes of the US- and European-VLBI networks. A recent status report on the VLBI-observations is given by Witzel (1987). In this paper we summarize the results on the direct evidence for b.r.m. in this sample as derived from repeated VLBI-observations at 5 GHz, as well as supporting evidence from SSC-calculations and flux density variability of the 5GHz VLBI core components (Table 1).
