Summary
This chapter examines to what extent and under what conditions the Sustainable Development Goals have fostered a better inclusion and support of poor and vulnerable communities within countries, and of the least developed countries internationally. We start with a conceptualization of inclusiveness as a matter of recognition, representation, and distribution. We then review the role of the Sustainable Development Goals in steering any normative, institutional or discursive changes in favour of inclusiveness within and between countries, drawing on both a Scopus search of all articles published between 2015 and 2020 in the social sciences subject area related to inclusiveness and grey literature. Although research evidence is strikingly limited, our review indicates that rhetoric and action do not match when it comes to the impacts of the Sustainable Development Goals on inclusiveness within and between countries. While vulnerable people and countries are often discursively prioritized in the implementation of the goals, such discursive prioritization has so far not resulted in creating or reshuffling norms and institutions towards inclusiveness.

Voluntary asymptomatic severe acute respiratory coronavirus virus 2 (SARS-CoV-2) testing was provided by the NIH Clinical Center over 1 year. Among 105,927 tests, 0.2% were positive. Among eligible staff, 79% participated with variable frequency and 61% of positive individuals had symptoms at the time of testing. Saliva specimen collection was chosen as an option less frequently than midturbinate collection.

This study explored whether working within Multispecialty INterprofessional Team (MINT) memory clinics has an impact on health care professionals’ perceptions of the challenges, attitudes, and level of collaboration associated with providing dementia care. Surveys were completed by MINT memory clinic members pre- and 6-months post-clinic launch. A total of 228 pre-and-post-training surveys were matched for analysis. After working in the MINT memory clinics for 6 months, there were significant reductions in mean ratings of the level of challenge associated with various aspects of dementia care, and significant increases in the frequency with which respondents experienced enthusiasm, inspiration, and pride in their work in dementia care and in ratings of the extent of collaboration for dementia care. This study provides some insights into the effect of collaborative, interprofessional approaches on health care professionals’ perceptions of the challenges and attitudes associated with providing dementia care and level of collaboration with other health professionals.
Six Years of Admission Screening for Carbapenemase-Producing Organisms at the NIH Clinical Center

Background: Transmission of carbapenemase-producing organisms (CPO) threatens patient safety in healthcare facilities. As a result of a 2011 outbreak of blaKPC+ Klebsiella pneumoniae, the NIH Clinical Center (NIHCC) has prioritized early detection and isolation of CPO carriers, using point-prevalence surveys and targeted high-risk ward surveillance since 2011 and admission surveillance since 2013. We describe our experience over 6 years of admission surveillance. Methods: The NIHCC is a 200-bed research hospital that provides care for a highly immunocompromised patient population. From September 2013 to September 2019, perirectal swabs were ordered automatically for all patients on admission to nonbehavioral health wards. Swabs were ordered twice weekly for ICU patients, weekly in other high-risk wards, and monthly for hospital-wide point prevalence (excluding behavioral health). Patients hospitalized in the United States in the previous week or abroad in the previous 6 months were considered high risk for carriage and isolated pending results from 2 swabs. Most swabs (n = 37,526) were cultured onto HardyCHROM CRE. If gram-negative bacilli (GNB) were present, a molecular screen for carbapenemases was performed on a sweep of cultured material (day 1) pending organism isolation. GNB were identified by MALDI-TOF MS. Prior to June 2019, isolates were screened by blaKPC/blaNDM PCR. Starting in June 2019, Enterobacteriaceae and Pseudomonas aeruginosa were screened using the phenotypic modified carbapenem inactivation method (mCIM), reflexing to the GeneXpert CARBA-R molecular assay if positive; other GNB were tested directly with CARBA-R. Selected GNB underwent susceptibility testing (Sensititre). Whole-genome sequencing was used to assess relatedness among CPO isolates. Swabs from high-risk patients were tested directly by blaKPC PCR (n = 699) until August 2019 (most in parallel with culture) and thereafter by CARBA-R (n = 13). Results: Among 54,188 orders for perirectal swabs, 38,238 were collected from 14,497 patients (compliance 71%). Among 33 CPO-colonized patients identified from September 2013 through September 2019, 15 were identified on admission, 6 were identified in point-prevalence surveys, 8 were identified from high-risk ward surveillance, and 4 were identified from clinical cultures. Sequencing demonstrated no relatedness among CPO isolates. Although only 1.4% of patients sampled on admission were colonized with CPO, those meeting high-risk criteria were 21 times as likely to be colonized. Conclusion: Admission surveillance for CPO identified a low rate of colonization, but it detected nearly half of known CPO-colonized NIHCC patients over the past 6 years. Modest compliance with swab collection leaves room for improvement and likely results in missed instances of colonization. Although we cannot determine its effectiveness, we view our strategy as one of several key safety measures for our highly vulnerable patient population.

Summary
The science of studying diamond inclusions for understanding Earth history has developed significantly over the past decades, with new instrumentation and techniques applied to diamond sample archives revealing the stories contained within diamond inclusions. This chapter reviews what diamonds can tell us about the deep carbon cycle over the course of Earth’s history. It reviews how the geochemistry of diamonds and their inclusions inform us about the deep carbon cycle, the origin of the diamonds in Earth’s mantle, and the evolution of diamonds through time.

Weed seedbanks have been studied intensively at local scales, but to date, there have been no regional-scale studies of weed seedbank persistence. Empirical and modeling studies indicate that reducing weed seedbank persistence can play an important role in integrated weed management. Annual seedbank persistence of 13 summer annual weed species was studied from 2001 through 2003 at eight locations in the north central United States and one location in the northwestern United States. Effects of seed depth placement, tillage, and abiotic environmental factors on seedbank persistence were examined through regression and multivariate ordinations. All species examined showed a negative relationship between hydrothermal time and seedbank persistence. Seedbank persistence was very similar between the two years of the study for common lambsquarters, giant foxtail, and velvetleaf when data were pooled over location, depth, and tillage. Seedbank persistence of common lambsquarters, giant foxtail, and velvetleaf from October 2001 through 2002 and October 2002 through 2003 was, respectively, 52.3% and 60.0%, 21.3% and 21.8%, and 57.5% and 57.2%. These results demonstrate that robust estimates of seedbank persistence are possible when many observations are averaged over numerous locations. Future studies are needed to develop methods of reducing seedbank persistence, especially for weed species with particularly long-lived seeds.

The government publishes 3 different public report surgical site infection (SSI) metrics, all called standardized infection ratios (SIRs), that impact perceived hospital quality. We conducted a non-random cross-sectional observational pilot study of 20 California hospitals that voluntarily submitted colon surgery and SSI data. Discordant SIR values, leading to contradictory conclusions, occurred in 35% of these hospitals.
