Epidemiology and genomics of a slow outbreak of methicillin-resistant Staphyloccus aureus (MRSA) in a neonatal intensive care unit: Successful chronic decolonization of MRSA-positive healthcare personnel

Objective:
To describe the genomic analysis and epidemiologic response related to a slow and prolonged methicillin-resistant Staphylococcus aureus (MRSA) outbreak.
Design:Prospective observational study.
Setting:Neonatal intensive care unit (NICU).
Methods:We conducted an epidemiologic investigation of a NICU MRSA outbreak involving serial baby and staff screening to identify opportunities for decolonization. Whole-genome sequencing was performed on MRSA isolates.
Results:A NICU with excellent hand hygiene compliance and longstanding minimal healthcare-associated infections experienced an MRSA outbreak involving 15 babies and 6 healthcare personnel (HCP). In total, 12 cases occurred slowly over a 1-year period (mean, 30.7 days apart) followed by 3 additional cases 7 months later. Multiple progressive infection prevention interventions were implemented, including contact precautions and cohorting of MRSA-positive babies, hand hygiene observers, enhanced environmental cleaning, screening of babies and staff, and decolonization of carriers. Only decolonization of HCP found to be persistent carriers of MRSA was successful in stopping transmission and ending the outbreak. Genomic analyses identified bidirectional transmission between babies and HCP during the outbreak.
Conclusions:In comparison to fast outbreaks, outbreaks that are “slow and sustained” may be more common to units with strong existing infection prevention practices such that a series of breaches have to align to result in a case. We identified a slow outbreak that persisted among staff and babies and was only stopped by identifying and decolonizing persistent MRSA carriage among staff. A repeated decolonization regimen was successful in allowing previously persistent carriers to safely continue work duties.

Vulture populations are in severe decline across Africa and prioritization of geographic areas for their conservation is urgently needed. To do so, we compiled three independent datasets on vulture occurrence from road-surveys, GPS-tracking, and citizen science (eBird), and used maximum entropy to build ensemble species distribution models (SDMs). We then identified spatial vulture conservation priorities in Ethiopia, a stronghold for vultures in Africa, while accounting for uncertainty in our predictions. We were able to build robust distribution models for five vulture species across the entirety of Ethiopia, including three Critically Endangered, one Endangered, and one Near Threatened species. We show that priorities occur in the highlands of Ethiopia, which provide particularly important habitat for Bearded Gypaetus barbatus, Hooded Necrosyrtes monachus, Rüppell’s Gyps rüppelli and White-backed Gyps africanus Vultures, as well as the lowlands of north-eastern Ethiopia, which are particularly valuable for the Egyptian Vulture Neophron percnopterus. One-third of the core distribution of the Egyptian Vulture was protected, followed by the White-backed Vulture at one-sixth, and all other species at one-tenth. Overall, only about one-fifth of vulture priority areas were protected. Given that there is limited protection of priority areas and that vultures range widely, we argue that measures of broad spatial and legislative scope will be necessary to address drivers of vulture declines, including poisoning, energy infrastructure, and climate change, while considering the local social context and aiding sustainable development.

This volume offers a historical and critical analysis of the emerging field of the learning sciences, which takes an interdisciplinary approach to understanding and improving how children and adults learn. It features a wide range of authors, including established scholars who founded and guided the learning sciences through the initial turbulence of forming a new line of academic inquiry, as well as newcomers who are continuing to shape the field. This diversity allows for a broad yet selective perspective on what the learning sciences are, why they came to be, and how contributors conduct their work. Reflections on the Learning Sciences serves both as a starting point for discussion among scholars familiar with the discipline and as an introduction for those interested in learning more. It will benefit graduate students and researchers in computer science, educational psychology, instructional technology, science, engineering, and mathematics.

Summary
The learning sciences (LS) is positioned as an interdisciplinary approach to the study of how children and adults learn. Generally working outside of a laboratory setting, learning scientists attempt to conduct research in authentic settings to test iteratively designs for new learning environments that results in theoretical insights as well as instructional improvements. The focus of LS research has progressed beyond a sole emphasis on individual cognition to include social and cultural considerations as well as the role of the environments in which learning takes place. LS is marked by collaborations among instructional technologists, educational psychologists, content area educators, anthropologists, computer scientists, linguists, philosophers, and many more. LS is, then, an interdisciplinary approach to the study and facilitation of learning in authentic settings.
The first LS program was formed at Northwestern University in 1991 – so the field will have existed close to a quarter century as this book goes to press. Yet, in all of this time, no book has been published that analyzes “learning sciences” itself. There are now LS programs in at least thirty-five US universities and in several countries around the world, and new programs are being created as we write. Two journals dedicated to LS have been founded (The Journal of the Learning Sciences and The International Journal of Computer-Supported Collaborative Learning), and an international association, the International Society of the Learning Sciences (ISLS), holds annual meetings. An original focus on classroom learning has been supplemented by a growing interest in learning in “informal settings” such as museums, after-school programs, and the home. At the same time, LS is having an increasing impact on policy in areas such as mathematics (National Council of Teachers of Mathematics), science (National Science Education Standards), and engineering education (The Engineer of 2020: Visions of Engineering in the New Century). In addition, LS has been changing in important ways over the course of its existence and continues to do so. This book is the first to document, explore, and extrapolate from these characteristics of LS.
It is true that articles have appeared that reflect on the history of LS as a whole and of the Journal of the Learning Sciences in particular (e.g., Kirby, Hoadley, & Carr-Chellman, 2005; Kolodner, 2004). In addition, two editions of a massive Handbook have summarized the state of the art (Sawyer, 2006, 2014).

BACKGROUND
Various transmission routes contribute to spread of carbapenem-resistant Klebsiella pneumoniae (CRKP) in hospitalized patients. Patients with readmissions during which CRKP is again isolated (“CRKP readmission”) potentially contribute to transmission of CRKP.
OBJECTIVETo evaluate CRKP readmissions in the Consortium on Resistance against Carbapenems in K. pneumoniae (CRaCKLe).
DESIGNCohort study from December 24, 2011, through July 1, 2013.
SETTINGMulticenter consortium of acute care hospitals in the Great Lakes region.
PATIENTSAll patients who were discharged alive during the study period were included. Each patient was included only once at the time of the first CRKP-positive culture.
METHODSAll readmissions within 90 days of discharge from the index hospitalization during which CRKP was again found were analyzed. Risk factors for CRKP readmission were evaluated in multivariable models.
RESULTSFifty-six (20%) of 287 patients who were discharged alive had a CRKP readmission. History of malignancy was associated with CRKP readmission (adjusted odds ratio [adjusted OR], 3.00 [95% CI, 1.32–6.65], P<.01). During the index hospitalization, 160 patients (56%) received antibiotic treatment against CRKP; the choice of regimen was associated with CRKP readmission (P=.02). Receipt of tigecycline-based therapy (adjusted OR, 5.13 [95% CI, 1.72–17.44], using aminoglycoside-based therapy as a reference in those treated with anti-CRKP antibiotics) was associated with CRKP readmission.
CONCLUSIONHospitalized patients with CRKP—specifically those with a history of malignancy—are at high risk of readmission with recurrent CRKP infection or colonization. Treatment during the index hospitalization with a tigecycline-based regimen increases this risk.

On the Light Emission in GaN Based Heterostructures at High Injection

For light emitting diodes (LEDs) to be used for general lighting, high efficiencies would need to be retained at high injection levels to meet the intensity and efficiency requirements. In this regard, it is imperative to overcome the observed drop in LED efficiency at high injection levels beyond that would be expected from junction temperature. The suggested genesis of efficiency degradation includes electron overflow or spillover, also suggested to be aided by polarization induced electric field, Auger recombination, current crowding, and elevated junction temperature. Setting the junction temperature aside, the degree to which or even whether each of these mechanisms plays a role is still under debate. We have undertaken a series of experiments to isolate, whenever possible, the aforementioned processes in an effort to determine the causes of efficiency loss at high injection levels. By using 1μs pulsed electrical injection with 0.1% duty cycle, we were able to minimize the effect of the junction temperature. By changing the design of the multiple quantum well region as well as by employing or not employing electron blocking layers, we demonstrated the important role that electron overflow plays on efficiency. Furthermore, by also exploring the same on non polar surfaces and observing any lack of dispersion in terms of the effect of the electron blocking layer we can conclude that the polarization induced field does not seem to play a major role. LEDs on non polar surface with no notable efficiency degradation, up to current densities of about 2250 Acm-2 used for measurements, have been obtained which seems to imply that Auger recombination up to these injection levels is not of major importance, at least in the structures investigated. The effect of current crowding on efficiency droop was investigated by comparing semitransparent Ni/Au p-contacts and transparent conducting oxide contacts (Ga-doped ZnO). Because the latter showed notably reduced efficiency degradation at high injection levels, we can conclude that current crowding plays a role as well.
