Attitudes and Knowledge of Emergency Physicians Towards End-of-Life Care in the Emergency Department. A National Survey

Introduction:
With an aging population and patients on end-of-life care (EOL) pathways, emergency departments (ED) are seeing an increase in patients requiring EOL care. There is paucity of data of attitudes and knowledge of physicians providing EOL care in the ED both internationally and in Ireland. The aim of this project was to assess the attitudes and knowledge of ED physicians towards EOL care.
Method:This was a cross-sectional electronic survey of ED physicians working in Irish Eds, facilitated through the Irish Trainee Emergency Research Network (ITERN) over six weeks from September 27, 2021, to November 8, 2021. The questionnaire covered the following domains: Demographic data, Awareness of EOL Care, Views, and attitudes towards EOL care.
Results:A total of 311 completed questionnaires across 23 participant sites were analyzed, with a response rate of 45%. The majority of the respondents were under the age of 35 (62%), were male (58%) and at SHO level (36%). In terms of awareness 32% (98) of respondents were not aware of palliative care services in their hospitals while only 29% (91) were aware of national EOL guidance. Fifty-five percent (172) reported commencing EOL care in the ED, however 75.5% (234) respondents reported their knowledge of EOL care to be limited or non-existent. Few (30.2%) respondents felt comfortable commencing EOL care in the ED without speciality team input. There appears to be a lack of clarity on the roles and responsibilities of ED nurses and doctors in the care of the dying patients in ED with only 31.2% (95) being clear on this role. Significant differences were observed with regards to clinical experience and physician grade.
Conclusion:This study has highlighted a lack of awareness and knowledge of EOL care, particularly among less experienced ED physicians. However, there was a willingness to commence EOL care in the ED.

The role of the dissertation as part of academic preparation is generally accepted and unquestioned. The dissertation is intended to initiate students into serious research in their chosen field. However, what happens to the research, once completed, is another question. Apparently very few have addressed this question, and their findings are not consistent. McPhie, in a study of dissertations completed in social studies education, measured the dissemination of dissertations via library circulation. He concluded that: “These data on library circulation, even when viewed with caution, would seem to indicate a rather meager use of some potentially valuable findings.” He also found that nearly two-thirds had not published even one article from their dissertations. In a more recent study of Ph.D. dissertations in sociology, Yoels found a similar picture. He concluded, after analyzing citations to Ph.D. dissertations in the two most frequently cited journals in sociology, that “the likelihood that dissertations will be widely disseminated appears to be slim.” However, in a recent and more complete study, Hanson found that dissertations are more widely used than is indicated in the above studies. He concludes that: “When one broadens the definition of dissemination to include the media of books, monographs, chapters, presented papers, journal articles (including those not appearing in the two major journals), research bulletins, and miscellaneous forms during the professional life of the Ph.D. sociologist, a picture of more extensive transmission of dissertation results emerge. A conservative estimate would be that findings from a substantial majority of dissertations are disseminated through various media at some point in time.”

In North America, terrestrial records of biodiversity and climate change that span Marine Oxygen Isotope Stage (MIS) 5 are rare. Where found, they provide insight into how the coupling of the ocean–atmosphere system is manifested in biotic and environmental records and how the biosphere responds to climate change. In 2010–2011, construction at Ziegler Reservoir near Snowmass Village, Colorado (USA) revealed a nearly continuous, lacustrine/wetland sedimentary sequence that preserved evidence of past plant communities between ~140 and 55 ka, including all of MIS 5. At an elevation of 2705 m, the Ziegler Reservoir fossil site also contained thousands of well-preserved bones of late Pleistocene megafauna, including mastodons, mammoths, ground sloths, horses, camels, deer, bison, black bear, coyotes, and bighorn sheep. In addition, the site contained more than 26,000 bones from at least 30 species of small animals including salamanders, otters, muskrats, minks, rabbits, beavers, frogs, lizards, snakes, fish, and birds. The combination of macro- and micro-vertebrates, invertebrates, terrestrial and aquatic plant macrofossils, a detailed pollen record, and a robust, directly dated stratigraphic framework shows that high-elevation ecosystems in the Rocky Mountains of Colorado are climatically sensitive and varied dramatically throughout MIS 5.

This paper analyzes in detail the role of environmental and economic shocks in the migration of the 1930s. The 1940 US Census of Population asked every inhabitant where they lived five years earlier, a unique source for understanding migration flows and networks. Earlier research documented migrant origins and destinations, but we will show how short-term and annual weather conditions at sending locations in the 1930s explain those flows, and how they operated through agricultural success. Beyond demographic data, we use data about temperature and precipitation, plus data about agricultural production from the agricultural census. The widely known migration literature for the 1930s describes an era of relatively low migration, with much of the migration that did occur radiating outward from the Dust Bowl region and the cotton South. Our work about the complete United States will provide a fuller examination of migration in this socially and economically important era.
Nutritional predictors of successful chronic disease prevention for a community cohort in Central Australia

Objective
To investigate biomarkers of nutrition associated with chronic disease absence for an Aboriginal cohort.
DesignScreening for nutritional biomarkers was completed at baseline (1995). Evidence of chronic disease (diabetes, CVD, chronic kidney disease or hypertension) was sought from primary health-care clinics, hospitals and death records over 10 years of follow-up. Principal components analysis was used to group baseline nutritional biomarkers and logistic regression modelling used to investigate associations between the principal components and chronic disease absence.
SettingThree Central Australian Aboriginal communities.
SubjectsAboriginal people (n 444, 286 of whom were without chronic disease at baseline) aged 15–82 years.
ResultsPrincipal components analysis grouped twelve nutritional biomarkers into four components: ‘lipids’; ‘adiposity’; ‘dietary quality’; and ‘habitus with inverse quality diet’. For the 286 individuals free of chronic disease at baseline, lower adiposity, lower lipids and better dietary quality components were each associated with the absence at follow-up of most chronic diseases examined, with the exception of chronic kidney disease. Low ‘adiposity’ component was associated with absence of diabetes, hypertension and CVD at follow-up. Low ‘lipid’ component was associated with absence of hypertension and CVD, and high ‘dietary quality’ component was associated with absence of CVD at follow-up.
ConclusionsLowering or maintenance of the factors related to ‘adiposity’ and ‘lipids’ to healthy thresholds and increasing access to a healthy diet appear useful targets for chronic disease prevention for Aboriginal people in Central Australia.

Objective
On April 15, 2013, two improvised explosive devices (IEDs) exploded at the Boston Marathon and 264 patients were treated at 26 hospitals in the aftermath. Despite the extent of injuries sustained by victims, there was no subsequent mortality for those treated in hospitals. Leadership decisions and actions in major trauma centers were a critical factor in this response.
MethodsThe objective of this investigation was to describe and characterize organizational dynamics and leadership themes immediately after the bombings by utilizing a novel structured sequential qualitative approach consisting of a focus group followed by subsequent detailed interviews and combined expert analysis.
ResultsAcross physician leaders representing 7 hospitals, several leadership and management themes emerged from our analysis: communications and volunteer surges, flexibility, the challenge of technology, and command versus collaboration.
ConclusionsDisasters provide a distinctive context in which to study the robustness and resilience of response systems. Therefore, in the aftermath of a large-scale crisis, every effort should be invested in forming a coalition and collecting critical lessons so they can be shared and incorporated into best practices and preparations. Novel communication strategies, flexible leadership structures, and improved information systems will be necessary to reduce morbidity and mortality during future events. Simplicity of construction and operation are advantages of iTMC (ionic transition metal complex) OLEDs compared with multi-layer OLED devices. Unfortunately, lifetimes do not compare favorably with the best multi-layer devices. We have previously shown for Ru(bpy)3(PF6)2 based iTMC OLEDs that electrical drive produces emission-quenching dimers of the active species. We report evidence here that a chemical process may also be implicated in degradation of devices based on Ir(ppy)2(dtb-bpy)PF6 albeit by a very different mechanism. It appears that degradation of operating devices made with this Ir-based complex is related to current-induced heating of the organic layer, resulting in loss of the dtb-bpy ligand. (The dtb-bpy ligand is labile compared with the cyclometallated ppy ligands.) Morphological changes observed in electrically driven Ir(ppy)2(dtb-bpy)PF6 OLEDs provide evidence of substantial heating during device operation. Evidence from UV-vis spectra in the presence of an electric field as well as MALDI-TOF mass spectra of the OLED materials before and after electrical drive add support for this model of the degradation process.

Two young children were studied from the period when their expressive lexicons were approximately 30 words until they showed productive two-word positional patterns in their speech. Word combinations meeting the criteria for positional productive, positional associative and groping patterns were identified and the period during which the words appearing in these patterns were acquired was then determined. Words used in positional productive patterns had generally emerged in the children's speech before those used in associative or groping patterns. When several words that could play a particular semantic role (e.g. actor) were already well established in the child's lexicon, positional productive patterns that were somewhat broader in scope appeared as early as lexically based patterns.

The aim of this study was to uncover factors accounting for the ability of children with specific language impairment (SLI) to learn agreement morphemes in intervention. Twenty-five children with SLI who participated in a six-month intervention program focused on teaching third person singular -s or auxiliary is/are/was showed a wide range of use of the target morpheme after intervention. Regression analyses showed that age and two factors expected to be related to agreement – the use of noun plural -s and subject/verb constructions prior to intervention – significantly predicted progress in the acquisition of agreement morphemes. In contrast, the pretreatment use of morphemes hypothesized to be unrelated to agreement was not a significant predictor of progress. The results indicate that the ability of children with SLI to learn agreement morphemes relies on their prior ability to use noun plural and subject/verb constructions.

The processes underlying degradation of organic light emitting diodes (OLEDs) are gradually becoming understood. In ruthenium-based ionic transition metal complex (iTMC) OLEDs, a dimeric species forms during device operation that quenches light emission [1]. Water has been implicated in this degradation process [2]. We report recent studies on degradation of OLEDs fabricated with Ir(ppy)2(dtb-bpy)PF6 [ppy = 2-phenylpyridine, dtb-bpy = 4,4'-di-tert-butyl 2,2'-bipyridine [3]. We have found that applying a thicker-than-usual metal electrode results in shorter turn-on times and higher light emission, though little improvement in lifetime. It appears that the degradation of these devices occurs by a different mechanism from that of the ruthenium-based devices and may involve local heating leading to chemical decomposition of the organic material.
Observation of recurring but often transient dark-colored substances in both the Ru(bpy)3(PF6)2 and Ir(ppy)2(dtb-bpy)PF6 systems, seen both in the solid state and in solution samples, may also be indicative of decomposition.

Postprandial glycaemic control is important in the prevention and therapy of type 2 diabetes and related diseases. Agents that may reduce postprandial glycaemia and/or insulinaemia, such as consumption of high-amylose foods, are considered beneficial; however, little is known about the dose–response relationship and the effects of cooking. The aim of the present study was to define the dose–response curve for postprandial glycaemic and insulinaemic excursions following meals of different amylose content and to compare the dose–response curves for meals containing cooked and uncooked starches. Following an overnight fast, rats ingested a test meal and blood was sampled over 2 h. The meal was given at 1.0 g carbohydrate/kg body weight, with an amylose content of 0, 270, 600 or 850 g/kg total starch. The area under the glucose curve did not differ under any condition investigated. For the uncooked-starch diets, area under the insulin curve was higher for the 0 g amylose/kg total starch meal than all other meals (P = 0·0001). For the cooked-starch diets, area under the insulin curve was higher in the O than the 600 and 850 g amylose/kg total starch groups (P < 0·01), but did not differ from the 270 amylose/kg total starch group. These results suggest that even a relatively small proportion of uncooked amylose (270 g/kg total starch) is sufficient to achieve a maximal attenuating effect on postprandial insulin concentrations as compared with 0 g amylose/kg total starch. Following cooking, however, a much higher proportion of amylose (≥ 600 g/kg total starch) is needed to achieve a similar effect.

Single crystal thin films with compositions from the A1N-InN-GaN system were grown via metal-organic chemical vapor deposition (MOCVD) on single crystal 6H-SiC substrates. Blue light emitting (LED) and laser diode (LD) structures were fabricated. The conducting buffer layer LEDs employed an AlGaN buffer layer which provides a conduction path between SiC and the active device region. The external quantum efficiency of the LEDs was 3% at 20 mA- 3.6V and peak emission wavelength of 430 nm. Violet and blue LDs were fabricated and consisted of an 8-well InGaN/GaN multiple quantum well (MQW) active region in a separate confinement heterostructure (SCH) design. Lasing was obtained both on structures using an insulating buffer layer, and also on structures using a conducting buffer layer. The resulting lasers operated at room temperature using pulsed and continuous wave operation with an emission wavelength of 404-435 rim. The lowest threshold current density obtained for lasing was 11 kA/cm2.

This paper reports the events at NCSU leading up to and including those of June 5, 1997 which produced the first demonstration of a nitride laser diode on silicon carbide – and the very first nitride laser demonstration outside of Japan. All of the laser diode samples tested at NCSU were designed, grown, and fabricated into cleaved cavity test structures at Cree Research. Laser testing at NCSU consisted of spectral emission versus current measurements, light output power versus current (L-I) measurement, and light output polarization measurements versus current. The first successful laser on silicon carbide emitted at 402.6 nm. Subsequently, lasers displaying outputs ranging from 402.6 to 430.2 have been successfully tested at NCSU.
