Effect of a parent agreement on return rates of Vanderbilt assessments and treatment adherence in pediatric attention-deficit/hyperactivity disorder patients

Objective
We implemented a parent–teacher Vanderbilt agreement program to increase return rates of Vanderbilt assessment scales for children in our primary care practice, and compared the assessment return rate before and after agreement signature.
MethodsWe retrospectively reviewed children diagnosed with attention-deficit/hyperactivity disorder (ADHD) who had a signed Vanderbilt agreement and were under continuous care at our clinic. Return rates were compared 1 year before and 1 year after the agreement date.
ResultsAmong 195 children, prior to the agreement, 71% returned teacher assessments, and 59% returned parent forms; after the intervention, assessment rates were not significantly different (76%, p = .255; and 65%, p = .185, respectively). The median number of returned assessments increased after the agreement.
ConclusionsLack of documented parent and teacher Vanderbilt assessments remain a barrier to appropriate management of ADHD. Improving the rate of assessments returned is an important outcome for treating ADHD in the primary care setting.
Antarctic sea anemone distribution, abundance and relationships with habitat composition, community structure and anthropogenic disturbance

Understanding the distribution, abundance and habitat preferences of species in the Southern Ocean provides a foundation for assessing the impacts of environmental change and anthropogenic disturbance on Antarctic ecosystems. In near-shore waters at Casey and Davis Stations, photoquadrat surveys were used to determine sea anemone distribution and abundance, habitat preferences, associations with other species and the impact of human disturbance on sea anemone distribution. Two distinct sea anemone morphotypes were found in this study: large sea anemones that require hard substrate for attachment and small, burrowing sea anemones found in muddy sediment. The large sea anemones were found in rocky habitats, with the exception of some sedimentary habitats where other biota were used as substrate. The large sea anemones were associated with a diverse community of epibenthic species found in rocky habitats. The burrowing sea anemones were associated with a less diverse assemblage of sediment-dwelling epibenthos. At Casey Station, sea anemones were more abundant in habitats adjacent to a former waste disposal site than at control sites. The reason for this is not yet known, but may be due to high organic matter inputs or, alternatively, a longer sea ice duration providing protection from ice scour.

OBJECTIVE
While previous work showed that the Centers for Disease Control and Prevention toolkit for carbapenem-resistant Enterobacteriaceae (CRE) can reduce spread regionally, these interventions are costly, and decisions makers want to know whether and when economic benefits occur.
DESIGNEconomic analysis
SETTINGOrange County, California
METHODSUsing our Regional Healthcare Ecosystem Analyst (RHEA)-generated agent-based model of all inpatient healthcare facilities, we simulated the implementation of the CRE toolkit (active screening of interfacility transfers) in different ways and estimated their economic impacts under various circumstances.
RESULTSCompared to routine control measures, screening generated cost savings by year 1 when hospitals implemented screening after identifying ≤20 CRE cases (saving $2,000–$9,000) and by year 7 if all hospitals implemented in a regional coordinated manner after 1 hospital identified a CRE case (hospital perspective). Cost savings was achieved only if hospitals independently screened after identifying 10 cases (year 1, third-party payer perspective). Cost savings was achieved by year 1 if hospitals independently screened after identifying 1 CRE case and by year 3 if all hospitals coordinated and screened after 1 hospital identified 1 case (societal perspective). After a few years, all strategies cost less and have positive health effects compared to routine control measures; most strategies generate a positive cost-benefit each year.
CONCLUSIONSActive screening of interfacility transfers garnered cost savings in year 1 of implementation when hospitals acted independently and by year 3 if all hospitals collectively implemented the toolkit in a coordinated manner. Despite taking longer to manifest, coordinated regional control resulted in greater savings over time.

Low aspartic acid d:l ratios and modern collagenlike concentration values indicate that amino acids in bones from the Rancho La Brea asphalt deposit, Los Angeles, California are better preserved than amino acids in bones of equivalent age that have not been preserved in asphalt. Amino acids were recovered from 10 Rancho La Brea bone samples which range in age from less than 200 to greater than 36,000 yr. The calibrated rates of aspartic acid racemization range from 2.1 to 5.0 × 10−6yr−1. Although this wide range of rate constants decreases the level of confidence for age estimates, use of the larger rate constant of 5.0 × 10−6yr−1 provides minimum age estimates which fit the known stratigraphic and chronologic records of the Rancho La Brea deposits.

Objective
As numerous factors in the home environment have been related to children’s fruit and vegetable (F&V) consumption as a component of a healthy diet, the purpose of the present systematic review was to examine these factors specifically for children aged 6–12 years.
DesignRelevant observational studies published in English between January 2007 and December 2015 were obtained through electronic database searches. Studies were included if the researchers reported on a potentially modifiable measure of the home physical, political and sociocultural environment related to child F&V consumption.
ResultsOf the thirty-three articles reviewed, overall methodological quality was poor with twenty studies rated as weak, mainly due to cross-sectional design (majority of studies), selection bias, convenience sampling and voluntary participation. Half of the studies had strong–moderate ratings for using valid and/or reliable tools while for the other half, psychometric properties were either not reported or weak. The most consistent evidence for children’s combined F&V consumption was found for availability and accessibility of F&V, parental role modelling of F&V and maternal intake of F&V.
ConclusionsA vast array of home environment components and their influence on children’s consumption of fruits and/or vegetables have been studied in recent years. Specific components of the home environment may have more influence than others, but more compelling evidence is needed to draw strong conclusions. Recommendations are made for future studies to be based upon conceptual/theoretical models to provide consistency in defining the home environment and investigation of potential moderators, such as personal or contextual factors.

Ultramicrotomy, the technique of cutting nanometers-thin slices of material using a diamond knife, was applied to prepare transmission electron microscope (TEM) specimens of nanoporous poly(methylsilsesquioxane) (PMSSQ) thin films. This technique was compared to focused ion beam (FIB) cross-section preparation to address possible artifacts resulting from deformation of nanoporous microstructure during the sample preparation. It was found that ultramicrotomy is a successful TEM specimen preparation method for nanoporous PMSSQ thin films when combined with low-energy ion milling as a final step. A thick, sacrificial carbon coating was identified as a method of reducing defects from the FIB process which included film shrinkage and pore deformation.

Compressive yield stresses have been measured for pastes (0.35 ≤ w/c ≤ 0.50) of portland cement, calcium aluminate cement, and weakly and strongly flocculated alumina (Φ0 = 0.20) using the centrifuge sediment height technique. Equilibrium sediment heights are reached quickly, allowing all measurements to be taken during the cement's induction period. The compressive behavior showed little dependence on the compressive history. Compressive yield stress was, however, dependent upon initial volume fraction, decreasing as the initial volume fraction increases. This behavior was observed in both the cements and alumina suspensions, implying that strong dependencies on initial structure may be a general property of the compressive behavior of flocculated suspensions.

Summary
The beginning of the Civil War marked the beginning of the end of slavery in the American South. At first, most white Americans denied what would eventually seem self-evident. With President Abraham Lincoln in the fore, federal authorities insisted that the nascent conflict must be a war to restore the national union, and nothing more. Confederate leaders displayed a fuller comprehension of the importance of slavery, which Vice-President Alexander Stephens called the cornerstone of the Southern nation. But if Stephens and others grasped slavery's significance, they assumed that the Confederate struggle for independence would require no change in the nature of the institution. A Southern victory would transform the political status, not the social life, of the slave states; black people would remain in their familiar place. Despite a vigorous dissent from Northern abolitionists, most white people – North and South – saw no reason to involve slaves in their civil war.
Slaves had a different understanding of the sectional struggle. Unmoved by the public pronouncements and official policies of the federal government, they recognized their centrality to the dispute and knew that their future depended upon its outcome. With divisions among white Americans erupting into open warfare, slaves watched and waited, alert for ways to turn the military conflict to their own advantage, stubbornly refusing to leave its outcome to the two belligerents.

Summary
Freedom came to most American slaves only through force of arms. The growing Northern commitment to emancipation availed nothing without victory on the battlefield. But once federal policy makers had committed the Union to abolishing slavery, the Northern armies that eroded Confederate territory simultaneously expanded the domain of freedom. The Union army perforce became an army of liberation, and as it did, both the Northern public and the freed slaves themselves demanded that the direct beneficiaries of freedom join the battle against the slaveholders' rebellion. The incorporation of black soldiers into Union ranks at once turned to Northern advantage a vast source of manpower that the Confederacy proved incapable of tapping and enhanced the antislavery character of the war. The liberating force of black enlistments weakened slavery in the loyal border states and the Union-occupied South no less than in the Confederacy, thereby extending the nation's commitment to freedom beyond the limits of the Emancipation Proclamation. Black enlistees in the border states received their freedom, and, eventually, their enlistment also guaranteed the liberty of their immediate families. Throughout the slave states, black enlistment and slave emancipation advanced together and, indeed, became inseparable.
Black men coveted the liberator's role, but soldiering remained a complex, ambiguous experience. If most free blacks and slaves rushed to join the Union army, others entered federal service only at the point of a bayonet.
