Summary
VIGNETTE
Laura, Gary, Bridget, Chris and Theresa have been referred to social services. Laura is an 83-year-old White British lesbian. Her civil partner died last year and she now lives alone in their large multi-storey house in a rural area. She has several private pensions. Her eyesight is deteriorating and she can no longer drive. She is lonely and depressed. Gary is a single 70-year-old gay man of African Caribbean decent, living in an inner-city local authority flat and reliant on state pensions and other forms of welfare support. He is showing signs of memory loss and confusion. Bridget is a 65-year-old bi-identifying woman of White Irish Catholic origin. She lives in the suburbs with her partner, Chris, aged 69, a White British bisexual man who has multiple sclerosis, needing increasing care and support. Their daughter is supportive, but lives a long distance away. Theresa is a 61-year-old heterosexual trans woman with an Asian/White British heritage, who transitioned three years ago. She has a son, but they are estranged. She lives in a sheltered housing scheme for people with mental health issues. Theresa has complained that staff are discriminating against her.
What would good practice look like for each of these people?
Introduction
Lesbian, gay, bisexual and trans (LGBT) ageing occurs in wide-ranging socio-legal contexts. In countries where there is a lack of legal recognition and social protections, the health of LGBT people is impacted not only by the accumulated effects of discrimination but also by the fact that their older age care needs are served by health and social care systems that offer little or no recognition of their minority identities (AGE Platform Europe and ILGA-Europe, 2012). Even in more liberal countries offering some forms of legal rights, older LGBT people experience a range of health inequalities (Fish, 2007). These disadvantages (Fredriksen-Goldsen et al, 2013a, 2013b) can be clustered into four main areas:
• the cumulative physical and psychological effects of discrimination, stigma and marginalisation across the lifecourse;
• a relative lack of social capital, particularly informal social support, compared with heterosexual and cisgender older people;
• health and social care provision that is ill-equipped to recognise and meet the needs of older LGBT people (Fish, 2007; 2009);

Knowledge of both population size and genetic diversity is critical for assessing extinction risk but few studies include concurrent estimates of these two components of population biology. We conducted an investigation of population density and size, and genetic variation and past demographic events, of the Endangered grey-headed lemur Eulemur cinereiceps in south-east Madagascar. We estimated lemur density using line-transect surveys and used satellite imagery to calculate forest fragment area in three localities. We collected tissue samples from 53 individuals and used 26 polymorphic microsatellite loci to obtain measures of population structure (divergence and diversity) across these localities. We tested the probability of past bottleneck events using three models. Contrary to expectation, there were no significant differences in population density across localities. Genetic diversity decreased, but not significantly, with decreasing habitat area and population size. We found a higher likelihood of past bottleneck events in the fragmented coastal populations. The low population size and prior decline in diversity in coastal patches are consistent with their isolation, anthropogenic disturbance, and exposure to cyclone activity. The similarities in the estimates of density between continuous and fragmented sites may indicate recent population growth in the fragments but these populations nevertheless remain at risk from reduced levels of genetic variation. These patterns should be confirmed with more extensive sampling across the limited range of E. cinereiceps.

Background
Suicide prevention is a health service priority but the most effective approaches to prevention may differ between different patient groups.
AimsTo describe social and clinical characteristics in cases of suicide from different age and diagnostic groups.
MethodA national clinical survey of a 4-year (1996–2000) sample of cases of suicide in England and Wales where there had been recent (< 1 year) contact with mental health services (n=4859).
ResultsDeaths of young patients were characterised by jumping from a height or in front of a vehicle, schizophrenia, personality disorder, unemployment and substance misuse. In older patients, drowning, depression, living alone, physical illness, recent bereavement and suicide pacts were more common. People with schizophrenia were often in-patients and died by violent means. About athird of people with depressive disorder died within a year of illness onset. Those with substance dependence or personality disorder had high rates of disengagement from services.
ConclusionsPrevention measures likely to benefit young people include targeting schizophrenia, dual diagnosis and loss of service contact; those aimed at depression, isolation and physical ill-health should have more effect on elderly people.
