Target-based prejudices
In the early 2000s, a popular and clinically valuable class of drugs known as the nonsteroidal anti-inflammatory drug (NSAID) class was being used to treat osteoarthritis and acute pain conditions. Of the several chemical members of this class, rofecoxib was approved by the Food and Drug Administration (FDA) in May 1999, with Merck and Co. marketing it under the brand names Vioxx and Ceoxx. The drug recorded more than $2 billion a year with more than 75 million patients taking it worldwide.
In September 2004, rofecoxib was withdrawn from the market by Merck and Co. due to worries about its usage leading to increased risk of heart attack and stroke. This observation was noted in patients with long-term, high-dosage use. It has been estimated that these side effects led to the manifestation of heart disease in 100,000 cases.
The FDA intently scrutinized other NSAIDs and continued to do so. Several calls were made by concerned citizens and those affected by the rofecoxib side-effect case that the FDA needed to do more to insure public safety of post-approved drugs. Investigators began focusing on the target (known as COX-2), and it was hypothesized that the side effects observed were related directly to COX-2 inhibition. No concrete data have been observed about whether the cardiovascular effects were solely related to COX-2. However, the mindset was already created that if a side effect was observed through certain drug use then all mechanisms of physiological effects were termed again the “Vioxx effect.” This viewpoint – that all side effects and physiological effects were connected via a central target – impacted multiple other drug discovery projects as well, including the race to deliver the first CCR5 antagonists, which are a class of oral antiretroviral drugs designed to block HIV entry into cells via the CCR5 receptor. There were concerns that the novel mechanism of action of CCR5 antagonists was associated with potential risks of hepatotoxicity and malignancies, which might be adverse events that extend to all compounds in the class. The following work addressed such concerns by employing a wide range of computational approaches, utilizing pharmacology and human gene expression as well as clinical data to address these potential CCR5 antagonist class effects.

Neuropsychological assessment consists of a comprehensive evaluation of cognitive functioning and most often some evaluation of motor skills and sensory status also. Cognitive functions sampled typically include “intelligence” (IQ tests), attention, language skills, visuospatial abilities, “executive skills” and other abilities associated with frontal-lobe function, and learning and memory. Thus, the assessment samples vary widely among a variety of functions, providing a comprehensive picture of an individual's strengths and weaknesses. The resulting pattern points to the probable site of epileptic focus. Neuropsychological findings also serve to predict the risk for postsurgical cognitive decline and, when performance before and after operation is compared, they provide data on the impact of surgery upon cognitive functioning. Comprehensive evaluation of learning and memory is particularly important in this context, because of the frequency of temporal lobe epilepsy and the prominence of memory dysfunction associated with it. In addition, patients slated for elective surgery may also undergo an intracarotid amobarbital procedure (IAP), which is performed to determine the side of cerebral dominance for language and to test the memory capabilities of each hemisphere alone. All of these specialized neuropsychological tools are discussed in this paper.

Background:
Objective clinical evaluation of memory frequently requires serial testing but the issue of whether multiformed tests are equivalent and can be used interchangeably is seldom examined. An added problem in bilingual Canadian settings is the extent to which it is appropriate to measure French speakers’ performance on translations of English tests. The present work used the Rey Auditory Verbal Learning Test (RAVLT) and a nonverbal analog, the Aggie Figures Learning Test (AFLT), to examine whether a) different forms of the same test are equivalent, b) performance on the two tests is comparable, c) two language groups perform similarly, and d) the RAVLT can detect dysfunction in patients with temporal lobe epilepsy (TLE).
Methods:We compared three French versions of the RAVLT and three forms of the AFLT in 114 healthy francophone adults. We subsequently compared the performance of the same francophone subjects to a previously obtained sample of anglophones on both tests, and then administered the RAVLT to anglophone or francophone patients with TLE.
Results:For both tasks the three forms were equivalent and performance on the RAVLT was comparable to that on the AFLT. Francophone subjects performed slightly worse on the RAVLT compared to anglophones but performance of the two language groups did not differ on the AFLT. Finally, left TLE patients were impaired compared to right on the RAVLT, but no performance differences were observed across the two language groups in the patient sample.
Conclusions:The RAVLT and AFLT are useful tools for examination of learning and memory in French and English speaking populations. On the RAVLT, the lesion effect in patients is not affected by differences in performance between language groups.

Influenza Vaccination among Healthcare Workers: Ten-Year Experience of a Large Healthcare Organization

Objective.
To describe the results of different measures implemented to improve adherence to the healthcare worker (HCW) influenza immunization program at BJC HealthCare during the period from 1997 through 2007.
Design.Descriptive retrospective study.
Setting.BJC HealthCare, a 13-hospital nonprofit healthcare organization in the Midwest.
Methods.We reviewed and analyzed HCW influenza vaccination data from all BJC HealthCare Occupational Health Services and hospitals during the period from 1997 through 2007. Occupational health staff, infection prevention personnel, and key influenza vaccination campaign leaders were also interviewed regarding implementation measures during the study years.
Results.At the end of 2007, BJC HealthCare had approximately 26,000 employees. With the use of multiple progressive interventions, influenza vaccination rates among BJC employees increased from 45% in 1997 to 72% in 2007 (P<.001). The influenza vaccination rate in 2007 was significantly higher than in 2006: 72%, compared with 54% (P<.001). Five hospitals had influenza vaccination rates that exceeded the target goal of 80% in 2007. The most successful interventions were adding influenza vaccination rates to the quality scorecard incentive program and the use of declination statements, both of which were implemented in 2007. The most important barriers to success identified by interview respondents were HCWs' misconceptions about influenza vaccination and a perceived lack of leadership support.
Conclusions.Influenza vaccination rates among HCWs significantly improved with multiple interventions over the years. However, the BJC HealthCare influenza vaccination target of 80% was not attained at all hospitals with these measures. More aggressive interventions, such as implementing mandatory influenza vaccination policies, are needed to achieve higher vaccination rates.
Case-Control Study of Surgical Site Infections Associated With Pacemakers and Implantable Cardioverter-Defibrillators

Objective.
In 2000, the rate of surgical site infections (SSIs) associated with pacemaker and implantable cardioverter-defibrillator (ICD) procedures performed in the cardiothoracic operating rooms of hospital A was 16% (19 of 116 procedures resulted in infections). This study investigates risks for SSI associated with these procedures in the cardiothoracic operating room.
Design.Unmatched 1: 3 case-control study performed over a 12-month period among patients who had undergone implantation of a pacemaker and/or ICD. A standardized observation scrutinized infection control practices in the area where the procedures were performed.
Setting.The cardiothoracic operating rooms of hospital A, which belongs to a hospital consortium in the midwestern United States.
Patients.Patients with SSI were identified as case patients. Control patients were chosen from the group of uninfected patients who had procedures performed during the same period as case patients.
Results.A total of 19 SSIs associated with pacemaker and ICD procedures were retrospectively identified among the patients who underwent procedures in these cardiothoracic operating rooms. Culture samples were obtained from 7 patients; 2 yielded coagulase-negative Staphylococcus on culture, 2 yielded Staphylococcus aureus, 1 yielded Serratia marcescens, and 2 showed no growth. In the case-control study, age, race, sex, diabetes mellitus, smoking history, timing of antibiotic therapy, and hair removal did not differ significantly between case patients and control patients. Case patients were more likely to have an abdominal device in place (odds ratio [OR], 5.5 [95% confidence interval {CI}, 1.6-19.3]; P = .006) and less likely to have received a new implant (OR 0.3 [95% CI, 0.1-0.8]; P = .02) or to have had new leads placed (OR, 0.2 [95% CI, 0.1-0.6]; P = .003).
Conclusions.Abdominal placement of implanted devices was associated with occurrence of an SSI after pacemaker and/or ICD procedures.

Objective.
To determine the incidence of central venous catheter (CVC)-associated bloodstream infection (CA-BSI) among patients admitted to general medical wards outside the intensive care unit (ICU).
Design.Prospective cohort study performed over a 13-month period, from April 1, 2002, through April 30, 2003.
Setting.Four selected general medical wards at Barnes-Jewish Hospital, a 1,250-bed teaching hospital in Saint Louis, Missouri.
Patients.All patients admitted to 4 general medical wards.
Results.A total of 7,337 catheter-days were observed during 33,174 patient-days. The device utilization ratio (defined as the number of catheter-days divided by the number of patient-days) was 0.22 overall and was similar among the 4 wards (0.21, 0.25, 0.19, and 0.24). Forty-two episodes of CA-BSI were identified (rate, 5.7 infections per 1,000 catheter-days). Twenty-four (57%) of the 42 cases of CA-BSI were caused by gram-positive bacteria: 10 isolates (24%) were coagulase-negative staphylococci, 10 (24%) were Enterococcus species, and 3 (7%) were Staphylococcus aureus. Gram-negative bacteria caused 7 infections (17%). Five CA-BSIs (12%) were caused by Candida albicans, and 5 infections (12%) had a polymicrobial etiology. Thirty-five patients (83%) with CA-BSI had nontunneled CVCs in place.
Conclusions.Non-ICU medical wards in the study hospital had device utilization rates that were considerably lower than those of medical ICUs, but CA-BSI rates were similar to CA-BSI rates in medical ICUs in the United States. Studies of catheter utilization and on CVC insertion and care should be performed on medical wards. CA-BSI prevention strategies that have been used in ICUs should be studied on medical wards.

Summary
This chapter illustrates how information from functional neuroimaging complements and extends the knowledge one has gained about olfaction from studying disorders of the temporal lobes. Such integration of approaches and findings promises to broaden the understanding of the 'Olfactory brain'. Olfactory functioning has been studied in various patient groups whose lesions involved the temporal lobes. Temporal lobe epilepsy (TLE) has been one of the commonly used models for the study of this relationship, because the olfactory system comprises some of the structures that are a frequent source of epileptogenic activity. Two studies showed a change in the functional neuroanatomy of the olfactory response over time. A unique feature of the olfactory system is that perception itself involves limbic structures, including the amygdala. Odour-induced activations have been reported less often in the entorhinal cortex than in the piriform cortex or the amygdala.

Eradication of Methicillin-Resistant Staphylococcus Aureus From a Neonatal Intensive Care Unit by Active Surveillance and Aggressive Infection Control Measures

Objectives:
To describe an outbreak of hospital-acquired MRSA in a NICU and to identify the risk factors for, outcomes of, and interventions that eliminated it.
Setting:An 18-bed, level III-IV NICU in a community hospital.
Methods:Interventions to control MRSA included active surveillance, aggressive contact isolation, and cohorting and decolonization of infants and HCWs with MRSA. A case–control study was performed to compare infants with and without MRSA.
Results:A cluster of 6 cases of MRSA infection between September and October 2001 represented an increased attack rate of 21.2% compared with 5.3% in the previous months. Active surveillance identified unsuspected MRSA colonization in 6 (21.4%) of 28 patients and 6 (5.5%) of 110 HCWs screened. They were all successfully decolonized. There was an increased risk of MRSA colonization and infection among infants with low birth weight or younger gestational age. Multiple gestation was associated with an increased risk of colonization (OR, 37.5; CI95, 3.9–363.1) and infection (OR, 5.36; CI95, 1.37–20.96). Gavage feeding (OR, 10.33; CI95, 1.28–83.37) and intubation (OR, 5.97; CI95, 1.22–29.31) were associated with increased risk of infection. Infants with MRSA infection had a significantly longer hospital stay than infants without MRSA (51.83 vs 21.46 days; P = .003). Rep-PCR with mec typing and PVL analysis confirmed the presence of a single common strain of hospital-acquired MRSA.
Conclusion:Active surveillance, aggressive implementation of contact isolation, cohorting, and decolonization effectively eradicated MRSA from the NICU for 2½ years following the outbreak. (Infect Control Hosp Epidemiol 2005;26:616-621)
Risk Factors for Spinal Surgical-Site Infections in a Community Hospital: A Case–Control Study

Objective:
To characterize risk factors for surgical-site infection after spinal surgery.
Design:A case–control study.
Setting:A 113-bed community hospital.
Method:From January 1998 through June 2000, the incidence of surgical-site infection in patients undergoing laminectomy, spinal fusion surgery, or both increased at community hospital. A We compared 13 patients who acquired surgical-site infections after laminectomy, spinal fusion surgery, or both with 47 patients who were operated on during the same time period but did not acquire a surgical-site infection. Information collected included demographics, risk factors, personnel involved in the operations, length of hospital stay, and hospital costs.
Results:Of 13 case-patients, 9 (69%) were obese, 9 (69%) had spinal compression, 5 (38.5%) had a history of tobacco use, and 4 (31%) had diabetes. Oxacillin-sensitive Staphylococcus aureus (6 of 13; 46%) was the most common organism isolated. Significant risk factors for postoperative spinal surgical-site infection were dural tear during the surgical procedure and the use of glue to cement the dural patch (3 of 13 [23%] vs 1 of 47 [2.1%] ; P = .02) and American Society of Anesthesiologists risk class of 3 or more (6 of 13 [46.2%] vs 7 of 47 [15%]; P = .02). Case-patients were more likely to have prolonged length of stay (median, 16 vs 4 days; P < .001). The average excess length of stay was 11 days and the excess cost per case was $12,477.
Conclusion:Dural tear and the use of glue should be evaluated as potential risk factors for spinal surgical-site infection. Systematic observation for potential lapses in sterile technique and surgical processes that may increase the risk of infection may help prevent spinal surgical-site infection.
Preclinical prediction of AD using neuropsychological tests

Normals (N = 42) and patients with mild memory difficulty (N = 123) were given a neuropsychological test battery, and then followed annually for 3 years to determine which individuals developed sufficient functional change that they met clinical criteria for AD. Twenty-three of the 123 participants with mild memory difficulty converted to a diagnosis of probable Alzheimer's disease (AD) within 3 years of follow-up. Four of the 20 neuropsychological measures obtained at baseline, were useful in discriminating the groups on the basis of their status 3 years after the tests were given. The 4 discriminating tests pertained to assessments of memory and executive function. When the controls were compared to the individuals with memory impairments who ultimately developed AD (the converters), the accuracy of discrimination was 89%, based on the neuropsychological measures at baseline. The discrimination of the controls from the individuals with mild memory problems who did not progress to the point where they met clinical criteria for probable AD over the 3 years of follow-up (the Questionables) was 74% and the discrimination of the questionables from the converters was 80%. The specific tests that contributed to these discriminations, in conjunction with recent neuropathological and neuroimaging data from preclinical cases, have implications for which brain regions may be affected during the prodromal phase of AD. As a follow-up to a 1992 study of respiratory isolation, we studied compliance with recommendations for respiratory isolation through smokestick testing (1993) and surveys (1994). Effective negative pressure was demonstrated in 80 of 156 rooms and was associated with the presence of anterooms (P<.001). The proportion of surveyed hospitals periodically testing isolation rooms increased from 0% (1992) to 30% (1993) to 100% (1994; P<.001).

Summary
Research on code-switching in bilingual classrooms now spans almost two decades. It has been cross-disciplinary in nature and has thus reflected different currents of influence: from educational research on classroom interaction and teacher talk styles and, more recently, from conversational analysis, pragmatics and the ethnography of communication. It has also been international in nature: the early studies were carried out in the United States in bilingual education programmes for linguistic minority children. The 1970s and early 1980s saw the development of a substantial body of classroom-based research in this particular context. However, since the early 1980s, research on bilingual classroom processes has also been undertaken in other bilingual and multilingual settings such as Canada, South America, Europe, Africa and South East Asia.
Most research has been undertaken in settings where there is an ongoing debate about language education policy: in situations where a new form of language education programme has been implemented or where there has been a change in the medium of instruction or, in contrast, in situations where a change in medium needs to be considered because current policies are inappropriate. The principal motivation in undertaking most classroom-based research seems to have been to establish how language education policies are being translated into communicative practice in the day-to-day cycles of classroom life. As Merritt et al. have pointed out: ‘Determinants of language choice and code-switching in the classroom are necessarily more complex than can be “legislated” by language policy on medium of instruction’ (1992: 105).
Evaluation of Rooms with Negative Pressure Ventilation Used for Respiratory Isolation in Seven Midwestern Hospitals

Objective:
To determine the number and efficacy of respiratory isolation facilities in St. Louis hospitals and to assess the mechanisms in place for evaluating function of hospital ventilation systems.
Design:A prospective multi-hospital surveillance study using direct observation and a standardized questionnaire.
Setting:Seven hospitals (including university-affiliated large teaching, private community, private teaching, and private nonteaching adult hospitals, and one pediatric teaching hospital) in St. Louis, Missouri.
Measurements:Actual direction of airflow in rooms designated for respiratory isolation was measured using smokesticks. Hospital demographic information, respiratory isolation policies, and frequency of ventilation tests were provided by infection control personnel.
Results:One hundred twenty-one (3.4%) of 3,574 hospital rooms were designed to have negative pressure ventilation suitable for respiratory isolation. The percentage of isolation rooms in each institution ranged from 0.4% (92 of 486) to 93% (39 of 42). Only three (43%) of seven hospitals had intensive care respiratory isolation rooms, and none had isolation rooms in the emergency department. No hospital had tested routinely the efficacy of the negative pressure ventilation, and two (28%) of seven had tested airflow for the first time in the past year.
We tested 115 (95%) of 121 isolation rooms. With the doors closed, 52 (45%) of 115 designated negative pressure rooms actually had positive airflow to the corridor. The number of negative pressure rooms and the presence or absence of anterooms did not predict correct direction of airflow. There was a significant difference among hospitals in the percentage of designated isolation rooms that had truly negative pressure (P<0.0001). Hospital age, size, and type corre-latedwith correct direction of airflow (R<.0001).
Conclusion:In the hospitals studied, only a small number of rooms were designated for respiratory isolation, and the performance of these was not tested routinely. High-risk areas including intensive care units and emergency rooms were not equipped to provide respiratory isolation. The direction of airflow in respiratory isolation rooms was not always correct and should be evaluated frequently. (Infect Control Hosp Epidemiol 1993;14:623-628.)
A Prospective Randomized Trial Comparing Manual and Automated Endoscope Disinfection Methods

Objective:
To compare the efficacy of endoscope disinfection using automated and manual systems.
Design:Prospective randomized trial.
Setting:A 1,000-bed tertiary care referral center.
Methods:All endoscopes underwent a three-stage decontamination process including brushing and cleaning with water and detergent, manual or automated disinfection with 2% glutaraldehyde, and 70% alcohol rinse with forced air drying. Cultures were obtained from endoscopes from both groups before and after alcohol rinse and then after overnight storage.
Results:Cultures from 8/30 (27%) automated and 11/30 (37%) manually disinfected (P= 0.58) endoscopes grew gram-negative bacteria and/or nontuberculous mycobacteria before the alcohol rinse. After alcohol rinse, 3 (10%) of 30 automated and 8 (27%) of 30 manually disinfected endoscopes remained contaminated (P= 0.28). Manually disinfected endoscopes were contaminated more frequently with coliform bacteria, whereas endoscopes undergoing automated disinfection were more frequently contaminated with nontuberculous mycobacteria, but the differences were not statistically significant. After alcohol rinse and forced air drying, there was no difference in contamination rates between freshly disinfected endoscopes and those stored overnight (7/30 (23%) versus 4/30 (13%), P= 0.50). Colonoscopes and duodenoscopes were contaminated more often than gastroscopes (P=0.00001).
Conclusion:The persistent endoscope contamination after manual and automated disinfection indicates the importance of developing more reliable and effective disinfection methods.
