Background
Studying phenotypic and genetic characteristics of age at onset (AAO) and polarity at onset (PAO) in bipolar disorder can provide new insights into disease pathology and facilitate the development of screening tools.
AimsTo examine the genetic architecture of AAO and PAO and their association with bipolar disorder disease characteristics.
MethodGenome-wide association studies (GWASs) and polygenic score (PGS) analyses of AAO (n = 12 977) and PAO (n = 6773) were conducted in patients with bipolar disorder from 34 cohorts and a replication sample (n = 2237). The association of onset with disease characteristics was investigated in two of these cohorts.
ResultsEarlier AAO was associated with a higher probability of psychotic symptoms, suicidality, lower educational attainment, not living together and fewer episodes. Depressive onset correlated with suicidality and manic onset correlated with delusions and manic episodes. Systematic differences in AAO between cohorts and continents of origin were observed. This was also reflected in single-nucleotide variant-based heritability estimates, with higher heritabilities for stricter onset definitions. Increased PGS for autism spectrum disorder (β = −0.34 years, s.e. = 0.08), major depression (β = −0.34 years, s.e. = 0.08), schizophrenia (β = −0.39 years, s.e. = 0.08), and educational attainment (β = −0.31 years, s.e. = 0.08) were associated with an earlier AAO. The AAO GWAS identified one significant locus, but this finding did not replicate. Neither GWAS nor PGS analyses yielded significant associations with PAO.
ConclusionsAAO and PAO are associated with indicators of bipolar disorder severity. Individuals with an earlier onset show an increased polygenic liability for a broad spectrum of psychiatric traits. Systematic differences in AAO across cohorts, continents and phenotype definitions introduce significant heterogeneity, affecting analyses.

Measurements made at John Evans Glacier, eastern Ellesmere Island, Nunavut, Canada in 1994 and 1996 provide new insight into the internal hydrology of polythermal glaciers. During the early part of each melt season, supraglacial waters enter the glacier via a crevasse field about 4 km from the terminus and are stored in a subglacial reservoir. Release of the water from the reservoir occurs initially via an artesian fountain on the glacier surface and by upwelling of waters through subglacial sediments at the terminus (event1). Channelization of the subglacial waters then occurs and water is discharged as an outburst flood (event 2), which releases a considerably larger volume of water than event 1. Thus, drainage of the subglacial reservoir follows a cyclical pattern in which discharge oscillations increase in amplitude over time. The cycle may end with complete reservoir drainage. The total volume of water released was much greater in 1994 than in 1996, primarily because the 1994 melt season was longer and warmer than the 1996 season. Interannual differences in the form of the outflow hydrographs, and in the extent and timing of connections between the subglacial reservoir and marginal melt streams, are linked to variations in the size and rate of growth of the subglacial reservoir. This hydrological behaviour may have important implications for the dynamics of polythermal glaciers.
Effect of Hurricane Sandy on Health Care Services Utilization Under Medicaid

Objective
This investigation assessed changes in utilization of inpatient, outpatient, emergency department, and pharmacy services in the aftermath of Hurricane Sandy in 8 counties in New York affected by the storm.
MethodsMedicaid data for enrollees residing in 8 counties in New York were used to obtain aggregated daily counts of claims for 4 service types over immediate, 3-month, and 1-year periods following the storm. Negative binomial regression was used to compare service utilization in the storm year with the 2 prior years, within areas differentially affected by the storm.
ResultsChanges in service utilization within areas inside or outside the storm zone were most pronounced over the 1-year effect period. Differences in service utilization by year were the same by storm zone designation over the immediate effect period for all services.
ConclusionsResults are consistent with previous investigations demonstrating that some of the greatest effects of a disaster on health services utilization occur well beyond the initial event. One-year effects, combined with some 3-month effects, suggests that storm recovery, with its effect on health care services utilization, may have followed different paths in areas designated as inside or outside the storm zone. The evolution from a linear temperature gradient to a detonation is investigated for combustible materials whose chemistry is governed by chain-branching kinetics, using a combination of high-activation-temperature asymptotics and numerical simulations. A two-step chemical model is used, which captures the main properties of detonations in chain-branching fuels. The first step is a thermally neutral induction time, representing chain initiation and branching, which has a temperature-sensitive Arrhenius form of the reaction rate. At the end of the induction time is a transition point where the fuel is instantaneously converted into chain-radicals. The second step is the main exothermic reaction, representing chain termination, assumed to be temperature insensitive. Emphasis is on comparing and contrasting the results with previous studies that used simple one-step kinetics. It is shown that the largest temperature gradient for which a detonation can be successfully ignited depends on the heat release rate of the main reaction. The slower the heat release compared to the initial induction time, the shallower the gradient has to be for successful ignition. For example, when the rate of heat release is moderate or slow on the initial induction time scale, it was found that the path of the transition point marking the end of the induction stage should move supersonically, in which case its speed is determined only by the initial temperature gradient. For steeper gradients such that the transition point propagates subsonically from the outset, the rate of heat release must be very high for a detonation to be ignited. Detonation ignition for the two-step case apparently does not involve the formation of secondary shocks, unlike some cases when one-step kinetics is used.

Background
The prevalence of mental disorders in the community is high, yet many remain unrecognised, misdiagnosed or poorly managed within primary care. Hence, guidelines for diagnosing and managing mental disorders in primary care, ICD–10 PHC, have been developed.
AimsTo introduce the guidelines into primary care and to assess whether they improve recognition, accuracy of diagnosis and treatment standards.
MethodGPs recorded information on all patients presenting with mental disorders before and after guidelines were introduced. A10% sample of patients underwent interviews to establish a formal diagnosis. Recognition of mental disorders was assessed by screening of patients attending their GP.
ResultsThe guidelines had no impact on the overall detection of mental disorders, the accuracy of diagnosis or the prescription of antidepressants. There was a significant increase in the number of patients diagnosed with depression or unexplained somatic symptoms. The GPs also made increased use of psychological interventions.
ConclusionsThe success of the guidelines in bringing about change is by no means certain. Some areas appeared more susceptible to change than others.
