Background:
The Single Ventricle Reconstruction Trial randomised neonates with hypoplastic left heart syndrome to a shunt strategy but otherwise retained standard of care. We aimed to describe centre-level practice variation at Fontan completion.
Methods:Centre-level data are reported as median or median frequency across all centres and range of medians or frequencies across centres. Classification and regression tree analysis assessed the association of centre-level factors with length of stay and percentage of patients with prolonged pleural effusion (>7 days).
Results:The median Fontan age (14 centres, 320 patients) was 3.1 years (range from 1.7 to 3.9), and the weight-for-age z-score was −0.56 (−1.35 + 0.44). Extra-cardiac Fontans were performed in 79% (4–100%) of patients at the 13 centres performing this procedure; lateral tunnels were performed in 32% (3–100%) at the 11 centres performing it. Deep hypothermic circulatory arrest (nine centres) ranged from 6 to 100%. Major complications occurred in 17% (7–33%). The length of stay was 9.5 days (9–12); 15% (6–33%) had prolonged pleural effusion. Centres with fewer patients (<6%) with prolonged pleural effusion and fewer (<41%) complications had a shorter length of stay (<10 days; sensitivity 1.0; specificity 0.71; area under the curve 0.96). Avoiding deep hypothermic circulatory arrest and higher weight-for-age z-score were associated with a lower percentage of patients with prolonged effusions (<9.5%; sensitivity 1.0; specificity = 0.86; area under the curve 0.98).
Conclusions:Fontan perioperative practices varied widely among study centres. Strategies to decrease the duration of pleural effusion and minimise complications may decrease the length of stay. Further research regarding deep hypothermic circulatory arrest is needed to understand its association with prolonged pleural effusion.

The pelvic shield of ankylosaurian dinosaurs refers to an area of osteoderms lacking differentiated transverse bands over the pelvic region and it is used as a diagnostic character for various ankylosaur groups. The pelvic shield character varies across ankylosaur taxa but is typically coded as a binary character or is excluded from phylogenetic analyses, which obscures evolutionary trends and relationships. This study investigates for the first time pelvic shield morphology in a stratigraphic and geographic context. This paper comprehensively reviews pelvic shield morphology with firsthand observations of specimens, and proposes three categories of pelvic shield morphology. Category 1 pelvic shields have un-fused but tightly interlocking osteoderms. Category 2 pelvic shields have fused osteoderms forming rosettes and are restricted to the Late Jurassic to mid Cretaceous of North America and Europe. Category 3 pelvic shields have fused polygonal osteoderms of similar size, and are found in the mid- to Late Cretaceous of North America. Although the pelvic shield is used to characterize the Polacanthidae, an interpretation supported by this review, the validity of such a clade is dependent upon a global parsimony analysis incorporating this character. Future analyses of the Ankylosauria should incorporate a more detailed treatment of the pelvic shield to determine its diagnostic value within the group.

The ankylosaurine Pinacosaurus is one of the best known ankylosaur to date in terms of the number and preservational quality of specimens. Juvenile to sub-adult postcrania collected by the Soviet-Mongolian Paleontological Expedition from the Upper Cretaceous Alagteeg Formation at Alag Teeg, Mongolia can be assigned to Pinacosaurus grangeri based on discrete cranial characters. One individual is significantly larger than the others and demonstrates delayed fusion of postcranial elements with the earliest occurring between dorsal ribs and vertebrae. The robustness of forelimb elements is positively allometric with respect to their length, indicating weight-bearing relationship. Such length-dependent correlations are not seen in the hind limbs. Finally, incipient cervical half rings suggest a developmental pathway of outgrowths from the underlying band combined with fusion of overlying osteoderms.
Determinants of inadequate complementary feeding practices among children aged 6–23 months in Ghana

Objective
To explore complementary feeding practices and identify potential risk factors associated with inadequate complementary feeding practices in Ghana by using the newly developed WHO infant feeding indicators and data from the nationally representative 2008 Ghana Demographic and Health Survey.
DesignThe source of data for the analysis was the 2008 Ghana Demographic and Health Survey. Analysis of the factors associated with inadequate complementary feeding, using individual-, household- and community-level determinants, was done by performing multiple logistic regression modelling.
SettingGhana.
SubjectsChildren (n 822) aged 6–23 months.
ResultsThe prevalence of the introduction of solid, semi-solid or soft foods among infants aged 6–8 months was 72·6 % (95 % CI 64·6 %, 79·3 %). The proportion of children aged 6–23 months who met the minimum meal frequency and dietary diversity for breast-fed and non-breast-fed children was 46·0 % (95 % CI 42·3 %, 49·9 %) and 51·4 % (95 % CI 47·4 %, 55·3 %) respectively and the prevalence of minimum acceptable diet for breast-fed children was 29·9 % (95 % CI 26·1 %, 34·1 %). Multivariate analysis revealed that children from the other administrative regions were less likely to meet minimum dietary diversity, meal frequency and acceptable diet than those from the Volta region. Household poverty, children whose mothers perceived their size to be smaller than average and children who were delivered at home were significantly less likely to meet the minimum dietary diversity requirement; and children whose mothers did not have any postnatal check-ups were significantly less likely to meet the requirement for minimum acceptable diet. Complementary feeding was significantly lower in infants from illiterate mothers (adjusted OR=3·55; 95 % CI 1·05, 12·02).
ConclusionsThe prevalence of complementary feeding among children in Ghana is still below the WHO-recommended standard of 90 % coverage. Non-attendance of postnatal check-up by mothers, cultural beliefs and habits, household poverty, home delivery of babies and non-Christian mothers were the most important risk factors for inadequate complementary feeding practices. Therefore, nutrition educational interventions to improve complementary feeding practices should target these factors in order to achieve the fourth Millennium Development Goal.
