Rett Syndrome: Investigation of Nine Patients, including PET Scan

Background:
We describe nine females with Rett Syndrome (RS), aged 14 to 26 years. All had had developmental delay before the end of their first year and had subsequently regressed to profound dementia with apraxia, ataxia, irregular respirations and often also seizures.
Methods:The Revised Gesell developmental assessment and Alpern-Boll Developmental Profile were used in modified form. Volumetric measurements of basal ganglia using MRI were compared with the findings in nine age-matched volunteer females. Positron emission scans with [18F]-6-fluorodopa and [11C]-raclopride were performed under light anesthesia with intravenous Propofol, and the findings were compared with those in healthy control girls. Bidirectional sequencing of the coding regions of the MECP2 gene was investigated in blood samples for mutational analyses.
Results:The RS females functioned at a mental age level ranging from about 4 to 15 months. The scores correlated with height, weight and head circumference. Magnetic resonance scans of basal ganglia showed a significant reduction in the size of the caudate heads and thalami in the Rett cases. Positron emission scans demonstrated that the mean uptake of fluorodopa in RS was reduced by 13.1% in caudate and by 12.5% in putamen as compared to the controls, while dopamine D2 receptor binding was increased significantly by 9.7% in caudate and 9.6% in putamen. Mutations in the coding regions of the MECP2 gene were present in all nine patients. No significant correlation between type and location of mutation and volumetric changes or isotope uptake was demonstrable.
Conclusion:Our findings suggest a mild presynaptic deficit of nigrostriatal activity in Rett syndrome.

Deep Tissue Fluorescent Imaging in Scattering Specimens Using Confocal Microscopy

In scattering specimens, multiphoton excitation and nondescanned detection improve imaging depth by a factor of 2 or more over confocal microscopy; however, imaging depth is still limited by scattering. We applied the concept of clearing to deep tissue imaging of highly scattering specimens. Clearing is a remarkably effective approach to improving image quality at depth using either confocal or multiphoton microscopy. Tissue clearing appears to eliminate the need for multiphoton excitation for deep tissue imaging.

It is widely believed that people can remember the age at which they first had sexual intercourse. Questions about age at onset are routinely asked in population sexual behaviour surveys and in clinical history-taking. However, there are limited test–retest data, especially with regard to individual differences in unreliable recall. In this study, telephone interviews and follow-ups an average of 15 months later were conducted with 570 non-virgin subjects aged between 28 and 73 years. Test–retest correlations for recalled age at first intercourse were 0·85 for females and 0·91 for males. Consistency was slightly lower among older people and women with a history of sexual abuse. There were no significant associations between consistency of recall and measures of personality, educational background or history of alcohol dependence and depression.

The majority of musculoskeletal injuries involve the soft connective tissues including cartilage, meniscus, tendon, and ligament. In contrast to bone tissue, which regenerates after injury, these soft tissues tend to heal poorly through formation of weak, disorganized scar tissue. In fact certain soft-tissue injuries cannot heal on their own so surgical repair or complete reconstruction is required to restore function. Biological grafts (autografts and allografts) and “permanent” synthetic prostheses are currently used to reconstruct large soft-tissue defects. Autografts (a patient's own tissue transferred to another site) can be successful, but as you “rob Peter to pay Paul,” complications at the donor site can be significant. Allografts (transplants from another human) are in short supply and carry the risk of rejection or disease transmission. In general, permanent synthetic prostheses fail to withstand long-term mechanical loading associated with the musculoskeletal system. New strategies are clearly needed to manage musculoskeletal soft-tissue reconstruction.
Several tissue-engineering strategies are potentially useful to enhance repair of these troublesome injuries or to induce regeneration. One interesting approach (developed for skin and cartilage regeneration) combines scaffolds, cells, and cell signals to grow living tissue analogues in the laboratory. Scaffolds are natural or synthetic biomaterials upon which the cells attach and grow. The cells may be fibroblasts, chondrocytes, etc., depending on the specific application. Influential cell signals can include attachment factors, growth factors, and mechanical loads.
Our laboratory is developing a similar approach for ligament reconstruction. Ligaments are the strong, flexible bands of collagenous tissue that connect bone-to-bone to provide a delicate balance of stability and flexibility to the joints of the body. The anterior cruciate ligament (ACL) is the primary stabilizer of the knee and is frequently injured in sporting activities and accidents. Serious ACL injuries can lead to disability and progressive degeneration of cartilage and meniscus that normally serve as shock absorbers in the joint. Anterior-cruciate-ligament ruptures do not heal without surgical intervention, due to a poor intrinsic healing response and poor vascularity. To restore normal joint function, ruptured ACLs can be surgically reconstructed using a patellar-tendon autograft or allograft. While significant progress has been made in understanding ACL anatomy, structure, biomechanics, and healing, there is still no biological graft or biomaterial ideally suited for ACL reconstruction.

Questions remain as to the integrity of hydroxyapatite (HA) coatings on orthopaedic implants and the possibility of failure at the coating/implant interface in vivo. Previous investigations of resorbable HA coated implants showed bone bonding strengths equal to non-resorbable HA coated surfaces in interference fit surgical model. If the concept of a resorbing coating will alleviate concerns of interface failure, further investigation as to the mode of coating resorption and the cellular events at the interface are required. The effect of coating these implants with Matrigel, as a means to enhance bone cell attachment and growth was studied. Fibroblast cell behavior on resorbable, non-resorbable HA coated and uncoated rough titanium implants were studied at 4 and 24 hours for attachment and spreading. Postnatal rat calvarial cell attachment, growth and morphology studies were also performed on these three implants and on tissue culture plates with and without Matrigel at 2, 7, and 10 days. Resorbable HA coatings showed greater surface activity and cell spreading, but not greater cell attachment at 4 and 24 hours. Greater alkaline phosphatase production per cell was seen on Matrigel coated tissue culture plates than uncoated wells. Cytoplasmic processes similar to osteocytic canalicular networks were seen. Matrigel coating on the three types of implants showed slightly greater alkaline phosphatase production of cells than without Matrigel coating.

Previous studies in our laboratory showed that acellular collagen scaffold implants induce tissue ingrowth and perform similar to autografts following reconstruction of rabbit Achilles tendon or anterior cruciate ligament (ACL). We chronologically review these and related studies, and report preliminary development of fibroblast-seeded collagen scaffolds potentially useful for ACL reconstruction. The ‘healing potential’ of fibroblasts was measured within collagen scaffolds in vitro, as a function of fibroblast source. Aligned collagen scaffolds were seeded with fibroblasts from rabbit ACL, synovium, patellar tendon, or skin. Fibroblast viability, adherence, spreading, proliferation, and protein and collagen deposition were measured on collagen scaffolds. The fibroblasts attached to the scaffolds, and spread along the long axis of the collagen fibers. ACL fibroblasts adhered better than other fibroblast types; however, the ACL fibroblasts proliferated at the slowest rate. Patellar tendon fibroblasts proliferated at the most rapid rate. All four of the fibroblast types secreted protein and collagen within the collagen scaffolds.
Preliminary in vivo studies suggest that fibroblasts seeded onto collagen scaffolds can remain viable following reimplantation into the donor rabbit. Ongoing studies will elucidate the role of autogenous seeded fibroblasts in neoligament formation/remodeling. These ‘ligament analogs’ are potentially useful for clinical ACL reconstruction: fibroblasts would be obtained from biopsy, cultured, seeded onto a collagen scaffold, and implanted as an ACL substitute into the same patient.

Results of implantation studies in a variety of animal tissue models demonstrate that the rate of biogradation of a collagen scaffold should parallel the rate of wound healing observed in particular anatomic sites. This rapid degradation maximizes tissue regeneration and minimizes encapsulation of the implant. The following paper reviews the effects of crosslinking on the rate of tissue ingrowth and regeneration. In addition, preliminary mechanical data on newly developed soluble type I collagen fibers is presented as a possible advance in the production of high strength collagen based tissue scaffolds.

The use of hedging with commodity futures markets to reduce the price risk in corn production is examined. Both intra-year and inter-year risk are evaluated with different hedging strategies. Strategies involve no hedge, hedge and hold, controlled hedge placement and hold, and in and out hedging. Both technical and forecasting criteria are used to place hedges in the more active strategies. Substantial risk reduction is possible, often without a reduction in price received. Considerable basis risk diminishes the risk reducing properties of a hedge and hold strategy.
