P.143 Safety of same-day discharge following incidental durotomy in tubular microdiscectomy: a retrospective cohort study

Background: Cerebrospinal fluid (CSF) leak is a common complication of minimally invasive tubular microdiscectomy (MIM). However, it is not known whether patients with CSF leak can be safely discharged home the same day. Methods: This is a retrospective cohort study of patients with incidental durotomy after MIM from January, 2009 to August, 2023. Patient demographic information, surgery information, CSF leak management, and postoperative outcomes were recorded. Results: There were 16 patients (53%) who were admitted to hospital and 14 (47%) patients discharged home the same day post CSF leak. There were no differences in patient demographics between the two groups at baseline. Twenty-nine out of 30 (97%) of the patients had onlay duraplasty, and one (3%) patient was repaired using sutures. The hospitalized group was kept on bed rest overnight or 24 hours. The discharge group was kept on best rest for 2 hours or mobilized immediately after surgery. The average length of admission for the hospitalized group was 2.4 ± 4.0 days. No patients in either group required readmission or revision surgery for CSF leak. Conclusions: Patients with CSF leak post minimally invasive tubular microdiscectomy can be safely discharged home the same day provided that duraplasty or primary repair was performed intraoperatively.

The National Environmental Isotope Facility (NEIF) Radiocarbon Laboratory at the Scottish Universities Environmental Research Centre (SUERC) performs radiocarbon measurement of a wide range of sample matrices for applications in environmental research. Radiocarbon is applied to palaeoenvironmental, palaeoceanographic, and palaeoclimatic investigations, as well as work to understand the source, fate, turnover, and age of carbon in the modern carbon cycle. The NEIF Radiocarbon Laboratory supports users in the development and deployment of novel sampling techniques and laboratory approaches. Here, we give an overview of methods and procedures used by the laboratory to support the field collection, laboratory processing, and measurement of samples. This includes in-house development of novel and/or specialized methods and approaches, such as field collection of CO2 and CH4, hydropyrolysis, and ramped oxidation. The sample types covered include organic remains (e.g., plant material, peat, wood, charcoal, proteins), carbonates (e.g., speleothems, foraminifera, mollusc shell, travertine), waters (dissolved organic and inorganic carbon), gases (CO2 and CH4), soils and sediments (including sub-fractions).
On-farm net benefit of genotyping candidate female replacement cattle and sheep

The net benefit from investing in any technology is a function of the cost of implementation and the expected return in revenue. The objective of the present study was to quantify, using deterministic equations, the net monetary benefit from investing in genotyping of commercial females. Three case studies were presented reflecting dairy cows, beef cows and ewes based on Irish population parameters; sensitivity analyses were also performed. Parameters considered in the sensitivity analyses included the accuracy of genomic evaluations, replacement rate, proportion of female selection candidates retained as replacements, the cost of genotyping, the sire parentage error rate and the age of the female when it first gave birth. Results were presented as an annualised monetary net benefit over the lifetime of an individual, after discounting for the timing of expressions. In the base scenarios, the net benefit was greatest for dairy, followed by beef and then sheep. The net benefit improved as the reliability of the genomic evaluations improved and, in fact, a negative net benefit of genotyping was less frequent when the reliability of the genomic evaluations was high. The impact of a 10% point increase in genomic reliability was, however, greatest in sheep, followed by beef and then dairy. The net benefit of genotyping female selection candidates reduced as replacement rate increased. As genotyping costs increased, the net benefit reduced irrespective of the percentage of selection candidates kept, the replacement rate or even the population considered. Nonetheless, the association between the genotyping cost and the net benefit of genotyping differed by the percentage of selection candidates kept. Across all replacement rates evaluated, retaining 25% of the selection candidates resulted in the greatest net benefit when genotyping cost was low but the lowest net benefit when genotyping cost was high. Genotyping breakeven cost was non-linearly associated with the percentage of selection candidates retained, reaching a maximum when 50% of selection candidates were retained, irrespective of replacement rate, genomic reliability or the population. The genotyping breakeven cost was also non-linearly associated with replacement rate. The approaches outlined within provide the back-end framework for a decision support tool to quantify the net benefit of genotyping, once parameterised by the relevant population metrics.
Trends of serum phospholipid fatty acids over time in rural Uganda: evidence of nutritional transition?

Non-communicable diseases are projected to become the most common causes of death in Africa by 2030. The impact on health of epidemiological and nutritional transitions in sub-Saharan Africa remains unclear. To assess the trends of dietary fatty acids over time in Uganda, we examined fatty acids in serum collected from individuals in rural south-west Uganda, at three time points over two decades. Independent cross-sectional samples of 915 adults and children were selected from the general population cohort in 1990 (n 281), 2000 (n 283) and 2008 (n 351). Serum phospholipid fatty acids were measured by GC. Multivariate regression analyses were performed to compare the geometric means of fatty acids by time period. Serum fatty acid profiling showed high proportions of SFA, cis-MUFA and industrial trans-fatty acids (iTFA), likely to be biomarkers of high consumption of palm oil and hydrogenated fats. In contrast, proportions of n-6 and n-3 PUFA from vegetable oils and fish were low. From 1990 to 2008, serum phospholipids showed increases in absolute amounts of SFA (17·3 % increase in adults and 26·4 % in children), MUFA (16·7 % increase in adults and 16·8 % in children) and n-6:n-3 PUFA (40·1 % increase in adults and 39·8 % in children). The amount of elaidic acid, iTFA from hydrogenated fats, increased in children (60·1 % increase). In this rural Ugandan population, we show evidence of unfavourable trends over time of dietary fatty acids.

Background
No existing models of alcohol prevention concurrently adopt universal and selective approaches. This study aims to evaluate the first combined universal and selective approach to alcohol prevention.
MethodA total of 26 Australian schools with 2190 students (mean age: 13.3 years) were randomized to receive: universal prevention (Climate Schools); selective prevention (Preventure); combined prevention (Climate Schools and Preventure; CAP); or health education as usual (control). Primary outcomes were alcohol use, binge drinking and alcohol-related harms at 6, 12 and 24 months.
ResultsClimate, Preventure and CAP students demonstrated significantly lower growth in their likelihood to drink and binge drink, relative to controls over 24 months. Preventure students displayed significantly lower growth in their likelihood to experience alcohol harms, relative to controls. While adolescents in both the CAP and Climate groups demonstrated slower growth in drinking compared with adolescents in the control group over the 2-year study period, CAP adolescents demonstrated faster growth in drinking compared with Climate adolescents.
ConclusionsFindings support universal, selective and combined approaches to alcohol prevention. Particularly novel are the findings of no advantage of the combined approach over universal or selective prevention alone.
Two-dimensional analysis of ice ridging in the Beaufort Sea using aerial photography

Aerial photography was obtained for the Beaufort Sea north of Tuktoyaktuk. The flight path covered two distinct ice zones over a 15.5 km transect extending perpendicular to the coast, yielding fifty-nine photographs at a scale of 1 : 2000. The process of ridge extraction was automated using a series of computer algorithms for image filtering, edge detection and edge linking. Examples from two different sections along the transect are chosen for presentation: (a) a heavily ridged area, and (b) an area with one dominant linear ridge feature that separates ice cover of different age. Two parameters used in the automated process, a minimum edge gradient and minimum number of connected pixels said to form a continuous ridge segment, influence the number, length and spatial pattern of extracted ridges. Direct one-to-one correlations between manually interpreted ridges from photographs and the algorithm extracted ridges from digital data are not always possible. However, results indicate that the automated ridge extraction procedure reliably characterizes the overall direction and density of the ice ridges. The distribution of the ice-ridge directions is estimated from circular (angular) histograms constructed directly from the digital data. Analysis of the Beaufort Sea transect reveals that the ice ridging is strongly anisotropic, with a principal direction parallel to the local coastline.

This paper describes the current status of knowledge regarding the impact of aviation on the atmosphere. The growth of the aviation industry is likely to continue in the future and at present there are significant concerns that this will adversely affect climate and local air quality in the vicinity of airports. Indeed, it is possible that air quality may become a constraint to increased capacity of some large hub airports. Currently the radiative forcing impact from aircraft emissions, other than CO2, are not accounted for in the International Civil Aviation Organization emission trading scheme proposals. Taking only this approach, emissions trading could, in theory, increase the radiative forcing, rather than decrease it. The study of improved ‘environmentally friendly’ flight is still in its infancy, however any proposed and actual new developments, in terms of technology and operational practice, should include an environmental assessment.

The effect of tip geometry on discharge coefficient and heat transfer is investigated both experimentally and numerically using idealised models of an unshrouded rotor blade. A flat tip was compared with two squealer-type geometries (a cavity and suction-side squealer) under the transonic conditions expected in the gas turbine engine. Heat transfer measurements were performed using a transient liquid crystal technique while a duplicate test section was used for measuring the pressure field. Computations were carried out using an unstructured, fully compressible, three-dimensional RANS (Reynolds averaged Navier Stokes) solver. Initial computations performed using a low Reynolds number k-ε model demonstrated the inability of the model to predict the Nusselt number with reasonable accuracy. Further computations performed using a low Reynolds number k-ω model improved the predictions dramatically. The computed discharge coefficient and the average Nusselt number over the blade tip agreed well with the experiments. Three upstream-total to exit-static pressure ratios were used to create a range of engine-representative Mach numbers. Both experimental and numerical studies at the lower pressure ratio of 1·3 (exit Mach number ~ 0·65) established the cavity geometry as the best performer from an aerodynamic perspective by reducing the discharge through the tip. However, from the heat transfer perspective, both the peak Nusselt number and the average heat transfer to the tip were higher than the flat tip. At the higher pressure ratios of 1·85 and 2·27 (corresponding to exit Mach numbers ~ 0·98 and 1·12) the discharge coefficient and heat transfer to the tip increases. This paper explores the fluid dynamics associated with these flows and shows that the highest heat transfer is caused by reattachment and flow impingement. The fluid dynamic computations provide insight into the experimental measurements and were successfully compared with simple analytical models.

With the changing distribution of infectious diseases, and an increase in the burden of non-communicable diseases, low- and middle-income countries, including those in Africa, will need to expand their health care capacities to effectively respond to these epidemiological transitions. The interrelated risk factors for chronic infectious and non-communicable diseases and the need for long-term disease management, argue for combined strategies to understand their underlying causes and to design strategies for effective prevention and long-term care. Through multidisciplinary research and implementation partnerships, we advocate an integrated approach for research and healthcare for chronic diseases in Africa.

Background
Most empirical studies into the covariance structure of psychopathology have been confined to adults. This work is not developmentally informed as the meaning, age-of-onset, persistence and expression of disorders differ across the lifespan. This study investigates the underlying structure of adolescent psychopathology and associations between the psychopathological dimensions and sex and personality risk profiles for substance misuse and mental health problems.
MethodThis study analyzed data from 2175 adolescents aged 13.3 years. Five dimensional models were tested using confirmatory factor analysis and the external validity was examined using a multiple-indicators multiple-causes model.
ResultsA modified bifactor model, with three correlated specific factors (internalizing, externalizing, thought disorder) and one general psychopathology factor, provided the best fit to the data. Females reported higher mean levels of internalizing, and males reported higher mean levels of externalizing. No significant sex differences emerged in liability to thought disorder or general psychopathology. Liability to internalizing, externalizing, thought disorder and general psychopathology was characterized by a number of differences in personality profiles.
ConclusionsThis study is the first to identify a bifactor model including a specific thought disorder factor. The findings highlight the utility of transdiagnostic treatment approaches and the importance of restructuring psychopathology in an empirically based manner.

Communicative Development Inventories (CDIs, parent-completed language development checklists) are a helpful tool to assess language in children who are unused to interaction with unfamiliar adults. Generally, CDIs are completed in written form, but in developing country settings parents may have insufficient literacy to complete them alone. We designed CDIs to assess language development in children aged 0;8 to 2;4 in two languages used in Coastal communities in Kenya. Measures of vocabulary, gestures, and grammatical constructions were developed using both interviews with parents from varying backgrounds, and vocabulary as well as grammatical constructions from recordings of children's spontaneous speech. The CDIs were then administered in interview format to over 300 families. Reliability and validity ranged from acceptable to excellent, supporting the use of CDIs when direct language testing is impractical, even when children have multiple caregivers and where respondents have low literacy levels.
Remarkable Symmetries in the Milky Way Disc's Magnetic Field

We apply a new, expanded compilation of extragalactic source Faraday rotation measures (RM) to investigate the broad underlying magnetic structure of the Galactic disk at latitudes ∣b∣ ≲15° over all longitudes l, where our total number of RMs is comparable to those in the combined Canadian Galactic Plane Survey (CGPS) at ∣b∣ < 4° and the Southern Galactic Plane (SGPS) ∣b∣<1.5°. We report newly revealed, remarkably coherent patterns of RM at ∣b∣≲15° from l∼270° to ∼90° and RM(l) features of unprecedented clarity that replicate in l with opposite sign on opposite sides of the Galactic center. They confirm a highly patterned bisymmetric field structure toward the inner disc, an axisymmetic pattern toward the outer disc, and a very close coupling between the CGPS/SGPS RMs at ∣b∣≲3° (‘mid-plane’) and our new RMs up to ∣b∣∼15° (‘near-plane’). Our analysis also shows the vertical height of the coherent component of the disc field above the Galactic disc's mid-plane—to be ∼1.5 kpc out to ∼6 kpc from the Sun. This identifies the approximate height of a transition layer to the halo field structure. We find no RM sign change across the plane within ∣b∣∼15° in any longitude range. The prevailing disc field pattern and its striking degree of large-scale ordering confirm that our side of the Milky Way has a very organized underlying magnetic structure, for which the inward spiral pitch angle is 5.5°±1° at all ∣b∣ up to ∼12° in the inner semicircle of Galactic longitudes. It decreases to ∼0° toward the anticentre.

Objective:
We sought to determine and compare rates of pediatric mental health presentations and associated costs in emergency departments (EDs) in Alberta.
Methods:We examined 16 154 presentations by 12 589 patients (patient age ≤ 17 yr) between April 2002 and March 2006 using the Ambulatory Care Classification System, a province-wide database for Alberta. The following variables of interest were extracted: patient demographics, discharge diagnoses, triage level, disposition, recorded costs for ED care, and institutional classification and location (i.e., rural v. urban, pediatric v. general EDs).
Results:A 15% increase in pediatric mental health presentations was observed during the study period. Youth aged 13-17 years consistently represented the most common age group for first presentation to the ED (83.3%). Of the 16 154 recorded presentations, 21.4% were related to mood disorders and 32.5% to anxiety disorders. Presentations for substance misuse or abuse were the most prevalent reasons for a mental health-related visit (41.3%). Multiple visits accounted for more than one-third of all presentations. Presentations for mood disorders were more common in patients with multiple compared with single visits (29.3% v. 16.9%), and substance abuse or misuse presentations were more common in patients with single compared with multiple visits (47.4% v. 30.5%). The total direct ED costs for mental health presentations during the study period was Can$3.5 million.
Conclusion:This study provides comprehensive data on trends of pediatric mental health presentation, and highlights the costs and return presentations in this population. Psychiatric and medical care provided in the ED for pediatric mental health emergencies should be evaluated to determine quality of care and its relationship with return visits and costs.
Contrasts in antigen expression in the erythrocytic and exoerythrocytic stages of rodent malaria

The time and Site of expression of five antigens, recognized by monoclonal antibodies raised against blood-stage parasites, were studied in the exoerythrocytic stage of Plasmodium berghei using indirect immunofluorescent antibody staining. Two monoclonal antibodies (W 3. 5, I 2. 6), which stain the cytoplasm of infected erythrocytes, did not stain the cytoplasm of the infected liver cell but stained the parasite itself suggesting a difference in the antigenic architecture of the erythrocytic and exoerythrocytic parasites. Another antibody (17. 6. 1) revealed a further difference in the antigenic composition of the blood and liver-stage parasites being expressed almost exclusively in the former. Two others (C139 and 17. 3. 9) showed broadly similar patterns of expression in these two stages of the malarial life-cycle.
Viability of axillary buds of white clover (Trifolium repens) in grazed pasture

Stolon nodes of white clover were sampled monthly for 18 months from continuously grazed (set stocked) and rotationally grazed pastures in New Zealand. Both pastures were stocked at 22·5 ewes plus lambs/ha. Axillary buds were classified into viability categories using an incubation technique. On average, 54% of nodes had non-viable or dormant buds, 25% had axillary buds that were viable but non-active and 0·1% bore reproductive buds. Although 21% of buds emerged, only 8·5% of nodes bore live branches. Bud activity was strongly inhibited during the spring period of both years and this inhibition could not be removed by incubation.
The greatest bud viability occurred at node 4 (from the apex) although 30–40% of the buds at this position were non-viable. This pattern of variation changed somewhat with season. Some viable buds were present at nodes more than 20 from the apex but these were of low vigour. Secondary stolons had different patterns of bud activity from those of primary stolons until they comprised more than 16 nodes.
