Objective:
To examine differences in surgical practices between salaried and fee-for-service (FFS) surgeons for two common degenerative spine conditions. Surgeons may offer different treatments for similar conditions on the basis of their compensation mechanism.
Methods:The study assessed the practices of 63 spine surgeons across eight Canadian provinces (39 FFS surgeons and 24 salaried) who performed surgery for two lumbar conditions: stable spinal stenosis and degenerative spondylolisthesis. The study included a multicenter, ambispective review of consecutive spine surgery patients enrolled in the Canadian Spine Outcomes and Research Network registry between October 2012 and July 2018. The primary outcome was the difference in type of procedures performed between the two groups. Secondary study variables included surgical characteristics, baseline patient factors, and patient-reported outcome.
Results:For stable spinal stenosis (n = 2234), salaried surgeons performed statistically fewer uninstrumented fusion (p < 0.05) than FFS surgeons. For degenerative spondylolisthesis (n = 1292), salaried surgeons performed significantly more instrumentation plus interbody fusions (p < 0.05). There were no statistical differences in patient-reported outcomes between the two groups.
Conclusions:Surgeon compensation was associated with different approaches to stable lumbar spinal stenosis and degenerative lumbar spondylolisthesis. Salaried surgeons chose a more conservative approach to spinal stenosis and a more aggressive approach to degenerative spondylolisthesis, which highlights that remuneration is likely a minor determinant in the differences in practice of spinal surgery in Canada. Further research is needed to further elucidate which variables, other than patient demographics and financial incentives, influence surgical decision-making.
Pay to Play? How Reducing APSA Division Fees Increases Graduate Student Participation

In 2017, the American Political Science Association (APSA) Committee on the Status of Graduate Students in the Profession launched an initiative to lower the cost of Division (i.e., organized section) membership for students to promote graduate students’ professional development and to advance Division interests. This article assesses the effect of this intervention on Division membership. Using APSA membership data, we find that almost two thirds of Divisions that charged fees in 2017 reduced or eliminated student fees between 2017 and 2019, nearly halving the average student dues (i.e., from $11.57 in 2017 to $5.84 in 2019). As a result, average student membership increased by more than 300% in Divisions that reduced fees (i.e., from 79.5 in 2017 to 248.7 in 2019), compared to a marginal 30% increase in those that did not reduce fees. These outcomes of the initiative support additional efforts to reduce the costs of APSA participation for graduate students.
Compulsory admissions of patients with mental disorders: State of the art on ethical and legislative aspects in 40 European countries

Background.
Compulsory admission procedures of patients with mental disorders vary between countries in Europe. The Ethics Committee of the European Psychiatric Association (EPA) launched a survey on involuntary admission procedures of patients with mental disorders in 40 countries to gather information from all National Psychiatric Associations that are members of the EPA to develop recommendations for improving involuntary admission processes and promote voluntary care.
Methods.The survey focused on legislation of involuntary admissions and key actors involved in the admission procedure as well as most common reasons for involuntary admissions.
Results.We analyzed the survey categorical data in themes, which highlight that both medical and legal actors are involved in involuntary admission procedures.
Conclusions.We conclude that legal reasons for compulsory admission should be reworded in order to remove stigmatization of the patient, that raising awareness about involuntary admission procedures and patient rights with both patients and family advocacy groups is paramount, that communication about procedures should be widely available in lay-language for the general population, and that training sessions and guidance should be available for legal and medical practitioners. Finally, people working in the field need to be constantly aware about the ethical challenges surrounding compulsory admissions.

Background: Cervical sponylotic myelopathy (CSM) may present with neck and arm pain. This study investiagtes the change in neck/arm pain post-operatively in CSM. Methods: This ambispective study llocated 402 patients through the Canadian Spine Outcomes and Research Network. Outcome measures were the visual analogue scales for neck and arm pain (VAS-NP and VAS-AP) and the neck disability index (NDI). The thresholds for minimum clinically important differences (MCIDs) for VAS-NP and VAS-AP were determined to be 2.6 and 4.1. Results: VAS-NP improved from mean of 5.6±2.9 to 3.8±2.7 at 12 months (P<0.001). VAS-AP improved from 5.8±2.9 to 3.5±3.0 at 12 months (P<0.001). The MCIDs for VAS-NP and VAS-AP were also reached at 12 months. Based on the NDI, patients were grouped into those with mild pain/no pain (33%) versus moderate/severe pain (67%). At 3 months, a significantly high proportion of patients with moderate/severe pain (45.8%) demonstrated an improvement into mild/no pain, whereas 27.2% with mild/no pain demonstrated worsening into moderate/severe pain (P <0.001). At 12 months, 17.4% with mild/no pain experienced worsening of their NDI (P<0.001). Conclusions: This study suggests that neck and arm pain responds to surgical decompression in patients with CSM and reaches the MCIDs for VAS-AP and VAS-NP at 12 months.

The fate of a contracting liquid filament depends on the Ohnesorge number ($Oh$), the initial aspect ratio ($\unicode[STIX]{x1D6E4}$) and surface perturbation. Generally, it is believed that there exists a critical aspect ratio $\unicode[STIX]{x1D6E4}_{c}(Oh)$ such that longer filaments break up and shorter ones recoil into a single drop. Through computational and experimental studies, we report a transitional regime for filaments with a broad range of intermediate aspect ratios, where there exist multiple $\unicode[STIX]{x1D6E4}_{c}$ thresholds at which a novel breakup mode alternates with no-break mode. We develop a simple model considering the superposition of capillary waves, which can predict the complicated new phase diagram. In this model, the breakup results from constructive interference between the capillary waves that originate from the ends of the filament.
Merging Undergraduate Teaching, Graduate Training, and Producing Research: Lessons from Three Collaborative Experiments

Teaching undergraduate students, mentoring graduate students, and generating publishable research are distinct tasks for many political scientists. This article highlights lessons for merging these activities through experiences from an initiative that sparked a series of collaborative-research projects focused on opinions about crime and punishment in the United States. This article describes three collaborative projects conducted between 2015 and 2017 to demonstrate how to merge undergraduate teaching, graduate training, and producing research. By participating in these projects, students learned about social-scientific research through hands-on experiences designing experiments, collecting and analyzing original data, and reporting empirical findings to a public audience. This approach is an effective way to engage students and generate research that can advance professional goals.

Objective:
To investigate the effectiveness and usability of automated procedural guidance during virtual temporal bone surgery.
Methods:Two randomised controlled trials were performed to evaluate the effectiveness, for medical students, of two presentation modalities of automated real-time procedural guidance in virtual reality simulation: full and step-by-step visual presentation of drillable areas. Presentation modality effectiveness was determined through a comparison of participants’ dissection quality, evaluated by a blinded otologist, using a validated assessment scale.
Results:While the provision of automated guidance on procedure improved performance (full presentation, p = 0.03; step-by-step presentation, p < 0.001), usage of the two different presentation modalities was vastly different (full presentation, 3.73 per cent; step-by-step presentation, 60.40 per cent).
Conclusion:Automated procedural guidance in virtual temporal bone surgery is effective in improving trainee performance. Step-by-step presentation of procedural guidance was engaging, and therefore more likely to be used by the participants.
Treating depression with physical activity in adolescents and young adults: a systematic review and meta-analysis of randomised controlled trials

We aimed to establish the treatment effect of physical activity for depression in young people through meta-analysis. Four databases were searched to September 2016 for randomised controlled trials of physical activity interventions for adolescents and young adults, 12–25 years, experiencing a diagnosis or threshold symptoms of depression. Random-effects meta-analysis was used to estimate the standardised mean difference (SMD) between physical activity and control conditions. Subgroup analysis and meta-regression investigated potential treatment effect modifiers. Acceptability was estimated using dropout. Trials were assessed against risk of bias domains and overall quality of evidence was assessed using GRADE criteria. Seventeen trials were eligible and 16 provided data from 771 participants showing a large effect of physical activity on depression symptoms compared to controls (SMD = −0.82, 95% CI = −1.02 to −0.61, p < 0.05, I2 = 38%). The effect remained robust in trials with clinical samples (k = 5, SMD = −0.72, 95% CI = −1.15 to −0.30), and in trials using attention/activity placebo controls (k = 7, SMD = −0.82, 95% CI = −1.05 to −0.59). Dropout was 11% across physical activity arms and equivalent in controls (k = 12, RD = −0.01, 95% CI = −0.04 to 0.03, p = 0.70). However, the quality of RCT-level evidence contributing to the primary analysis was downgraded two levels to LOW (trial-level risk of bias, suspected publication bias), suggesting uncertainty in the size of effect and caution in its interpretation. While physical activity appears to be a promising and acceptable intervention for adolescents and young adults experiencing depression, robust clinical effectiveness trials that minimise risk of bias are required to increase confidence in the current finding. The specific intervention characteristics required to improve depression remain unclear, however best candidates given current evidence may include, but are not limited to, supervised, aerobic-based activity of moderate-to-vigorous intensity, engaged in multiple times per week over eight or more weeks. The Degradation Kinetics of an Ester of Silvex and the Persistence of Silvex in Water and Sediment

The degradation kinetics of propylene glycol butyl ether (hereafter referred to as PGBE) ester of 2-(2,4,5-trichlorophenoxy) propionic acid (silvex) and the persistence of silvex in water and sediment were studied under impounded conditions. PGBE ester of silvex was applied to the surface of three ponds at the rate of 9 kg/ha/A. Water and sediment samples were collected at 4, 12, 24, and 48 hr after treatment and were extracted for PGBE ester of silvex immediately after collection. The concentrations of PGBE ester of silvex and silvex present in the water and sediment samples were determined by gas chromatography. Physico-chemical and mineralogical characteristics of the sediment samples collected prior to treatment were determined. The hydrolysis of PGBE ester of silvex to silvex obeyed first order reaction kinetics, the specific reaction rate constants for the three ponds being 0.09 hr−1, 0.10 hr−1, and 0.14 hr−1. Fifty percent hydrolysis of PGBE ester of silvex occurred in 5 to 8 hr, 90% hydrolysis occurred in 16 to 24 hr, and 99% hydrolysis occurred in 33 to 49 hr. The concentration of silvex in water initially increased, but decreased to zero by the end of 3 weeks. Adsorption of both the PGBE ester of silvex and silvex appeared to occur on the sediment; essentially complete disappearance of both occurred by the fifth week following treatment. Under laboratory conditions, silvex adsorption by the sediments conformed to the Freundlich adsorption equation.
Effect of Weed Management Strategy and Row Width on Nitrous Oxide Emissions in Soybean

Nitrous oxide (N2O) is a potent greenhouse gas with implication for climate change. Agriculture accounts for 10% of all greenhouse gas emissions in the United States, but 75% of the country's N2O emissions. In the absence of PRE herbicides, weeds compete with soybean for available soil moisture and inorganic N, and may reduce N2O emissions relative to a weed-free environment. However, after weeds are killed with a POST herbicide, the dead weed residues may stimulate N2O emissions by increasing soil moisture and supplying carbon and nitrogen to microbial denitrifiers. Wider soybean rows often have more weed biomass, and as a result, row width may further impact how weeds influence N2O emissions. To determine this relationship, field studies were conducted in 2013 and 2014 in Arlington, WI. A two-by-two factorial treatment structure of weed management (PRE + POST vs. POST-only) and row width (38 or 76 cm) was arranged in a randomized complete block design with four replications. N2O fluxes were measured from static gas sampling chambers at least weekly starting 2 wk after planting until mid-September, and were compared for the periods before and after weed termination using a repeated measures analysis. N2O fluxes were not influenced by the weed by width interaction or width before termination, after termination, or for the full duration of the study at P ≤ 0.05. Interestingly, we observed that POST-only treatments had lower fluxes on the sampling day immediately prior to POST application (P = 0.0002), but this was the only incidence where weed influenced N2O fluxes, and overall, average fluxes from PRE + POST and POST-only treatments were not different for any period of the study. Soybean yield was not influenced by width (P = 0.6018) or weed by width (P = 0.5825), but yield was 650 kg ha−1 higher in the PRE + POST than POST-only treatments (P = 0.0007). These results indicate that herbicide management strategy does not influence N2O emissions from soybean, and the use of a PRE herbicide prevents soybean yield loss.
Conceptual design of initial opacity experiments on the national ignition facility

Accurate models of X-ray absorption and re-emission in partly stripped ions are necessary to calculate the structure of stars, the performance of hohlraums for inertial confinement fusion and many other systems in high-energy-density plasma physics. Despite theoretical progress, a persistent discrepancy exists with recent experiments at the Sandia Z facility studying iron in conditions characteristic of the solar radiative–convective transition region. The increased iron opacity measured at Z could help resolve a longstanding issue with the standard solar model, but requires a radical departure for opacity theory. To replicate the Z measurements, an opacity experiment has been designed for the National Facility (NIF). The design uses established techniques scaled to NIF. A laser-heated hohlraum will produce X-ray-heated uniform iron plasmas in local thermodynamic equilibrium (LTE) at temperatures ${\geqslant}150$ eV and electron densities ${\geqslant}7\times 10^{21}~\text{cm}^{-3}$. The iron will be probed using continuum X-rays emitted in a ${\sim}200$ ps, ${\sim}200~\unicode[STIX]{x03BC}\text{m}$ diameter source from a 2 mm diameter polystyrene (CH) capsule implosion. In this design, $2/3$ of the NIF beams deliver 500 kJ to the ${\sim}6$ mm diameter hohlraum, and the remaining $1/3$ directly drive the CH capsule with 200 kJ. Calculations indicate this capsule backlighter should outshine the iron sample, delivering a point-projection transmission opacity measurement to a time-integrated X-ray spectrometer viewing down the hohlraum axis. Preliminary experiments to develop the backlighter and hohlraum are underway, informing simulated measurements to guide the final design.
P030: The FAN study: intranasal fentanyl and inhaled nitrous oxide for fracture reduction

Introduction: Recently, intranasal (IN) fentanyl and inhaled nitrous oxide/oxygen (N2O) mixture have been increasingly used for procedural sedation and analgesia (PSA) alone or in combination. There is a lack of data on the efficacy of these combined agents. Methods: The objective was to evaluate the efficacy of IN fentanyl and N2O as PSA for the reduction of mildly-to-moderately displaced fractures and dislocations. We performed a prospective, observational cohort study between September 2014 and October 2015. Patients were recruited at CHU Sainte Justine (Montréal) and Royal Children Hospital (Melbourne, Australia). Patients aged 4 to 18 years were eligible if PSA consisted of IN fentanyl and N2O for the reduction of mildly-to-moderately displaced fractures or dislocations. Patients received at least IN fentanyl 1.5 mcg/kg (100 mcg max) and at least a 50/50% mixture of N2O with oxygen. Primary outcome was the efficacy of PSA measured by the patient assigned Facial Pain Scale-Revised (FPS-R). The Face, Legs, Activity, Cry, Consolability (FLACC) scale was also recorded. Depth of sedation was evaluated using University of Michigan Sedation Scale (UMSS). Adverse events were recorded following criteria of the Consensus Panel on Sedation Research of PERC/PECARN. Additional data concerning satisfaction or discomfort were evaluated via questionnaires, and follow-up telephone calls were made to elicit information on adverse events after discharge. Results: A total of 91 patients aged 9.7 ± 3.0 years were enrolled. There was no difference between the median FPS-R score during the procedure compared to before: Median 2 and 2 (median difference 0 [95% CI 0, 0]), respectively. The FLACC score was higher during the procedure than before: Median 4 and 0 (median difference 2 [95% CI 1, 3]). UMSS was 1 (95% CI 1, 2) during the procedure. 42 (46%) patients had adverse events, all mild: vertigo (20%), nausea (16%)]or vomiting (12%). A total of 85/88 (97%) parents and 82/85 (96%) ED physicians would want the same sedation in another procedure. Conclusion: PSA with IN fentanyl and N2O seems effective in our study, as evaluated by patient assigned FPS-R. Patients were minimally sedated. Adverse events were frequent but mild. Overall, parents and medical staff would want the same agents used in another procedure. Thus, PSA with IN fentanyl and N2O appears to be an attractive option for reduction of mildly displaced fractures or dislocations.

During the Early Jurassic, cyst-forming dinoflagellates began a long-term radiation that would portend ecological importance of these taxa in the pelagic plankton community throughout the rest of the Mesozoic era. The factors that contributed to the evolutionary success of dinoflagellates are poorly understood. Here we examine the relationship between oceanographic and climatic conditions during the Hettangian–Toarcian interval in relation to the radiation of dinoflagellates and other organic-walled phytoplankton taxa in the Tethys Ocean. Our analysis is based on two data sets. The first includes δ13Ccarb, δ13Corg, total organic carbon (TOC), and quantitative palynological observations derived from the Mochras Core (Wales, U.K.), which spans the complete Early Jurassic. The second is a coupled Mg/Ca and δ18O record derived from analyses of belemnite calcite obtained from three sections in northern Spain, covering the upper Sinemurian to Toarcian. From these two data sets we reconstructed the influence of sea level, trophism, temperature, and salinity on dinoflagellate cyst abundance and diversity in northwest Europe. Our results suggest that organic-walled phytoplankton (acritarchs, prasinophytes, and dinoflagellates) diversity increased through the Early Jurassic. The radiation coincides with a long-term eustatic rise and overall increase in the areal extent of continental shelves, a factor critical to cyst germination. On shorter timescales, we observed short bursts of dinoflagellate diversification during the late Sinemurian and late Pliensbachian. The former diversification is consistent with the opening of the Hispanic Corridor during the late Sinemurian, which apparently allowed the pioneer dinoflagellate, Liasidium variabile, to invade the Tethys from the Paleo-Pacific. A true radiation pulse during the late Pliensbachian, with predominantly cold-water taxa, occurred during sea level fall, suggesting that climate change was critical to setting the evolutionary tempo. Our belemnite δ18O and Mg/Ca data indicate that late Pliensbachian water masses cooled (ΔT ≈ −6°C) and became more saline (ΔS ≈ +2 psu). Cooling episodes during generally warm and humid Early Jurassic climate conditions would have produced stronger winter monsoon northeast trade winds, resulting in hydrographic instability, increased vertical mixing, and ventilation of bottom waters. During the late Pliensbachian, dinoflagellates replaced green algae, including prasinophytes and acritarchs, as primary producers. By producing benthic resting cysts, dinoflagellates may have been better adapted to oxidized ocean regimes. This hypothesis is supported by palynological data from the early Toarcian ocean anoxic event, which was marked by highly stratified anoxic bottom water overlain by low-salinity, warm surface waters. These conditions were advantageous to green algae, while cyst-producing dinoflagellates temporarily disappeared. Our results suggest that the rise in dinoflagellate diversity later in the Jurassic appears to correspond to deep water ventilation as a result of the opening of the Atlantic seaway, conditions that appear to have simultaneously led to a loss of prasinophyte dominance in the global oceans.

Background
Findings from family and twin studies support a genetic contribution to the development of sexual orientation in men. However, previous studies have yielded conflicting evidence for linkage to chromosome Xq28.
MethodWe conducted a genome-wide linkage scan on 409 independent pairs of homosexual brothers (908 analyzed individuals in 384 families), by far the largest study of its kind to date.
ResultsWe identified two regions of linkage: the pericentromeric region on chromosome 8 (maximum two-point LOD = 4.08, maximum multipoint LOD = 2.59), which overlaps with the second strongest region from a previous separate linkage scan of 155 brother pairs; and Xq28 (maximum two-point LOD = 2.99, maximum multipoint LOD = 2.76), which was also implicated in prior research.
ConclusionsResults, especially in the context of past studies, support the existence of genes on pericentromeric chromosome 8 and chromosome Xq28 influencing development of male sexual orientation.

Background
Previous studies have shown that genetic risk for externalizing (EXT) disorders is greater in the context of adverse family environments during adolescence, but it is unclear whether these effects are long lasting. The current study evaluated developmental changes in gene–environment interplay in the concurrent and prospective associations between parent–child relationship problems and EXT at ages 18 and 25 years.
MethodThe sample included 1382 twin pairs (48% male) from the Minnesota Twin Family Study, participating in assessments at ages 18 years (mean = 17.8, s.d. = 0.69 years) and 25 years (mean = 25.0, s.d. = 0.90 years). Perceptions of parent–child relationship problems were assessed using questionnaires. Structured interviews were used to assess symptoms of adult antisocial behavior and nicotine, alcohol and illicit drug dependence.
ResultsWe detected a gene–environment interaction at age 18 years, such that the genetic influence on EXT was greater in the context of more parent–child relationship problems. This moderation effect was not present at age 25 years, nor did parent-relationship problems at age 18 years moderate genetic influence on EXT at age 25 years. Rather, common genetic influences accounted for this longitudinal association.
ConclusionsGene–environment interaction evident in the relationship between adolescent parent–child relationship problems and EXT is both proximal and developmentally limited. Common genetic influence, rather than a gene–environment interaction, accounts for the long-term association between parent–child relationship problems at age 18 years and EXT at age 25 years. These results are consistent with a relatively pervasive importance of gene–environmental correlation in the transition from late adolescence to young adulthood.

PILOT (the Pathfinder for an International Large Optical Telescope) is a proposed 2.5-m optical/infrared telescope to be located at Dome C on the Antarctic plateau. The atmospheric conditions at Dome C deliver a high sensitivity, high photometric precision, wide-field, high spatial resolution, and high-cadence imaging capability to the PILOT telescope. These capabilities enable a unique scientific potential for PILOT, which is addressed in this series of papers. The current paper presents a series of projects dealing with the nearby Universe that have been identified as key science drivers for the PILOT facility. Several projects are proposed that examine stellar populations in nearby galaxies and stellar clusters in order to gain insight into the formation and evolution processes of galaxies and stars. A series of projects will investigate the molecular phase of the Galaxy and explore the ecology of star formation, and investigate the formation processes of stellar and planetary systems. Three projects in the field of exoplanet science are proposed: a search for free-floating low-mass planets and dwarfs, a program of follow-up observations of gravitational microlensing events, and a study of infrared light-curves for previously discovered exoplanets. Three projects are also proposed in the field of planetary and space science: optical and near-infrared studies aimed at characterising planetary atmospheres, a study of coronal mass ejections from the Sun, and a monitoring program searching for small-scale Low Earth Orbit satellite debris items.
