Enhancing return to work or school after a first episode of schizophrenia: the UCLA RCT of Individual Placement and Support and Workplace Fundamentals Module training

Background
This study evaluated in a rigorous 18-month randomized controlled trial the efficacy of an enhanced vocational intervention for helping individuals with a recent first schizophrenia episode to return to and remain in competitive work or regular schooling.
MethodsIndividual Placement and Support (IPS) was adapted to meet the goals of individuals whose goals might involve either employment or schooling. IPS was combined with a Workplace Fundamentals Module (WFM) for an enhanced, outpatient, vocational intervention. Random assignment to the enhanced integrated rehabilitation program (N = 46) was contrasted with equally intensive clinical treatment at UCLA, including social skills training groups, and conventional vocational rehabilitation by state agencies (N = 23). All patients were provided case management and psychiatric services by the same clinical team and received oral atypical antipsychotic medication.
ResultsThe IPS–WFM combination led to 83% of patients participating in competitive employment or school in the first 6 months of intensive treatment, compared with 41% in the comparison group (p < 0.005). During the subsequent year, IPS–WFM continued to yield higher rates of schooling/employment (92% v. 60%, p < 0.03). Cumulative number of weeks of schooling and/or employment was also substantially greater with the IPS–WFM intervention (45 v. 26 weeks, p < 0.004).
ConclusionsThe results clearly support the efficacy of an enhanced intervention focused on recovery of participation in normative work and school settings in the initial phase of schizophrenia, suggesting potential for prevention of disability.

In large, impersonal moral orders many of us wish to maintain good will toward our fellow citizens only if we are reasonably sure they will maintain good will toward us. The mutual maintaining of good will, then, requires that we somehow communicate our intentions to one another. But how do we actually do this? The current paper argues that when we engage in moral responsibility practices—that is, when we express our reactive attitudes by blaming, praising, and resenting—we communicate a desire to maintain good will to those in the community we are imbedded in. Participating in such practices alone will not get the job done, though, for expressions of our reactive attitudes are often what economists call cheap talk. But when we praise and blame in cases of moral diversity, expressions of our reactive attitudes act as costly signals capable of solving our social dilemma.

It is a privilege and a pleasure to have this opportunity to talk to you about progress in vertical and short take-off and landing (V/STOL) aircraft technology. I must admit at the outset to some trepidation in accepting the invitation of the Society to speak on a subject that was successfully pioneered in the United Kingdom and brought to its current operational state largely because of the enduring faith of the Harrier team. However, I hope that the more modest progress that has been made on a variety of STOL and VTOL concepts will also be of interest to you; at the very least, this talk will identify some of the blind alleys to be avoided and, with a little luck, point out some new directions for future research. I will confine myself largely to the V/STOL technology with which I have been involved over the 1970–1980 period at Ames Research Center.

Editors' introduction The Women's Health Initiative Memory Study (WHIMS) has been much criticized and it has been suggested that subgroups of women exist for whom hormone therapy (HT) might improve or impair cognitive function. Possible modifying variables could be age, smoking, body mass index, and menopausal symptoms.
These were included in artificial neural networks (ANN) analyses, which allow testing of complex non-linear higher order interactions of variables to predict outcomes. Artificial neural networks analyses without hidden units could predict responders and non-responders to treatment as well as logistic regression models that included only main effects, which indicated that higher order interactions were not necessary and did not add to the value of the models. There seemed to be no subgroups (e.g., older women who smoke and have a high body mass) for whom HT has a worse or better effect on cognitive function over time. This study also showed that cross-validation is essential in building robust models with many independent variables and should be applied as a standard technique in complex multivariate analyses.

Over the past few years there has been a heightened interest in small scale agriculture. Increasing food costs, concern for the quality of food, a concept of self-sufficency, and a reawakening of interest in the positive attributes of rural life are among the reasons for this new attitude. At the national level, the problems of small farms has been explicitly recognized in the 1977 Farm Bill. The following amendments to the Rural Development Act of 1972 were enacted (U.S. Congress, 1977):
“(c) SMALL FARM RESEARCH PROGRAMS – Small farm research programs shall consist of programs of research to develop new approaches for initiating and upgrading small farmer operations through management techniques, agricultural production techniques, farm machinery technology, new products, new marketing techniques, and small farm finance.”
“(d) SMALL FARM EXTENSION PROGRAMS – Small farm extension programs shall consist of extension programs to improve operations of small farmers using, to the maximum extent practicable, paraprofessional personnel to work with small farmers on an intensive basis to initiate and improve management techniques, agricultural production techniques, farm machinery technology, marketing techniques, and small farm finance, and to increase utilization by small farmers of existing services offered by the United States Department of Agriculture and other public and private agencies and organizations.”
