The predictive role of symptoms in COVID-19 diagnostic models: A longitudinal insight

To investigate the symptoms of SARS-CoV-2 infection, their dynamics and their discriminatory power for the disease using longitudinally, prospectively collected information reported at the time of their occurrence. We have analysed data from a large phase 3 clinical UK COVID-19 vaccine trial. The alpha variant was the predominant strain. Participants were assessed for SARS-CoV-2 infection via nasal/throat PCR at recruitment, vaccination appointments, and when symptomatic. Statistical techniques were implemented to infer estimates representative of the UK population, accounting for multiple symptomatic episodes associated with one individual. An optimal diagnostic model for SARS-CoV-2 infection was derived. The 4-month prevalence of SARS-CoV-2 was 2.1%; increasing to 19.4% (16.0%–22.7%) in participants reporting loss of appetite and 31.9% (27.1%–36.8%) in those with anosmia/ageusia. The model identified anosmia and/or ageusia, fever, congestion, and cough to be significantly associated with SARS-CoV-2 infection. Symptoms’ dynamics were vastly different in the two groups; after a slow start peaking later and lasting longer in PCR+ participants, whilst exhibiting a consistent decline in PCR- participants, with, on average, fewer than 3 days of symptoms reported. Anosmia/ageusia peaked late in confirmed SARS-CoV-2 infection (day 12), indicating a low discrimination power for early disease diagnosis.
Bilateral collaboration between the Greenland (Kalaallit Nunaat) and United States Research Communities – from a vision to everyday practice

Each year, hundreds of international researchers enter Greenland to conduct scientific fieldwork. Historically, they have had little interaction with local communities and scientists at Greenland research institutes. Recognising that collaboration between Greenland and the United States can yield better research, consider more diverse perspectives, articulate the benefits of research to Greenland society, and train the next generation in a collaborative framework, representatives from both countries have been engaged in a series of events to cultivate bilateral relationships. Here, we describe the process of these events (workshops, conference sessions, and public dialogues), the findings, and the outcomes that have followed. Prior to this focused engagement, United States and Greenland scientists typically pursued their research independently. Since the engagement, more researchers from both countries have successfully partnered to obtain funding for collaborative research. Furthermore, development of a bilateral collaboration network is underway. The focused approach on bilateral engagement also proved essential for maintaining research and other activities during the global pandemic. When United States researchers were prevented from entering Greenland, their Greenland partners were able to continue the fieldwork, ensuring that progress was not lost. Future international projects can build on these successes to expand collaborative and interdisciplinary research in Greenland.
Examination of weight status, parity and maternal education factors on intentions to breastfeed and breastfeeding duration in an Irish cohort.

Introduction
Breastfeeding can improve health outcomes for mothers and babies and is an important factor in the reduction of obesity. However, breastfeeding rates in Ireland are amongst the lowest internationally, with rates of 59.9% in 2016.
Materials & MethodsThe objective of this study was to assess intention to breastfeed at booking, feeding status at hospital discharge, one and three months postnatal and influencing factors.
Secondary data from 124 healthy pregnant women participating in an RCT in the National Maternity Hospital who recorded their intention to breastfeed at booking were included. Observational data including demographic and lifestyle data was collected from participants.
ResultsOf the 124 women, 80.7% were Irish, 86.2% had achieved third level education, and 43.5% (n = 54) were multiparous. Mean age was 32.7years and mean BMI 24.9kg/m2. 91.1% (n = 113) reported intending to breastfeed at booking. Upon hospital discharge, 91% of those intending to breastfeed had initiated breastfeeding (n = 101/111). 78.6% (n = 77/98) and 66.7% (n = 58/87) of those who had intended to breastfeed remained breastfeeding at one and three months postnatal respectively. Intention to breastfeed in early pregnancy was significantly higher amongst women with third level education (p < 0.001). Intention to breastfeed did not differ significantly with maternal age, ethnicity, BMI, or parity, however, women who had breastfed previously were more likely to intend to breastfeed for future infants (p < 0.001). Women with higher education were more likely to breastfeed at discharge, one and three months postnatal (p < 0.05). Although maternal BMI was not associated with intention to breastfeed, women who did not initiate breastfeeding at hospital discharge had a significantly higher BMI (M = 27.22kg/m2, SD = 4.53, p = 0.018) compared to women who initiated breastfeeding (M = 24.68kg/m2, SD = 3.05). Similarly, of those intending to breastfeed, women who were not breastfeeding at three months postnatal had a significantly higher BMI (M = 26.33kg/m2, SD = 3.99, p = 0.01) than those continuing breastfeeding (M = 24.08kg/m2, SD = 2.61). There were no significant differences regarding feeding status with maternal ethnicity, age, or parity at discharge and one month postnatal (p > 0.05). However, primiparous women intending to breastfeed were significantly more likely to continue breastfeeding at three months compared to multiparous women (p < 0.05).
DiscussionMany factors are shown to influence breastfeeding intention and duration including parity, education and BMI. It is important to identify these potentially inhibiting factors during pregnancy to develop support measures to encourage initiation and continuation of breastfeeding.
The association between the maternal diet and the maternal and infant gut microbiome: a systematic review

During pregnancy, changes occur to influence the maternal gut microbiome, and potentially the fetal microbiome. Diet has been shown to impact the gut microbiome. Little research has been conducted examining diet during pregnancy with respect to the gut microbiome. To meet inclusion criteria, dietary analyses must have been conducted as part of the primary aim. The primary outcome was the composition of the gut microbiome (infant or maternal), as assessed using culture-independent sequencing techniques. This review identified seven studies for inclusion, five examining the maternal gut microbiome and two examining the fetal gut microbiome. Microbial data were attained through analysis of stool samples by 16S ribosomal RNA gene-based microbiota assessment. Studies found an association between the maternal diet and gut microbiome. High-fat diets (% fat of total energy), fat-soluble vitamins (mg/d) and fibre (g/d) were the most significant nutrients associated with the gut microbiota composition of both neonates and mothers. High-fat diets were significantly associated with a reduction in microbial diversity. High-fat diets may reduce microbial diversity, while fibre intake may be positively associated with microbial diversity. The results of this review must be interpreted with caution. The number of studies was low, and the risk of observational bias and heterogeneity across the studies must be considered. However, these results show promise for dietary intervention and microbial manipulation in order to favour an increase of health-associated taxa in the gut of the mother and her offspring.

The Wisconsin Twin Project comprises multiple longitudinal studies that span infancy to early adulthood. We summarize recent papers that show how twin designs with deep phenotyping, including biological measures, can inform questions about phenotypic structure, etiology, comorbidity, heterogeneity, and gene–environment interplay of temperamental constructs and mental and physical health conditions of children and adolescents. The general framework for investigations begins with rich characterization of early temperament and follows with study of experiences and exposures across childhood and adolescence. Many studies incorporate neuroimaging and hormone assays.

To address growing concern over the effects of fisheries non-target catch on elasmobranchs worldwide, the accurate reporting of elasmobranch catch is essential. This requires data on a combination of measures, including reported landings, retained and discarded non-target catch, and post-discard survival. Identification of the factors influencing discard versus retention is needed to improve catch estimates and to determine wasteful fishing practices. To do this, retention rates of elasmobranch non-target catch in a broad subset of fisheries throughout the world were compared by taxon, fishing country, and gear. A regression tree and random forest analysis indicated that taxon was the most important determinant of retention in this dataset, but all three factors together explained 59% of the variance. Estimates of total elasmobranch removals were calculated by dividing the Food and Agriculture Organization of the United Nations (FAO) global elasmobranch landings by average retention rates, and suggest that total elasmobranch removals may exceed FAO reported landings by as much as 400%. This analysis is the first effort to directly characterize global drivers of discards for elasmobranch non-target catch. The results highlight the importance of accurate quantification of retention and discard rates to improve assessments of the potential impacts of fisheries on these species.

Findings from an online behavioural weight management programme provided with or without a fortified diet beverage

The present multi-centre randomised weight-loss trial evaluated the efficacy of a low-intensity 12-week online behavioural modification programme, with or without a fortified diet beverage using a 2 × 2 factorial design. A total of 572 participants were randomised to: (1) an online basic lifestyle information (OBLI) intervention, consisting of one online informational class about tips for weight management; (2) an online behavioural weight management (OBWM) intervention, entailing 12 weekly online classes focused on weight-loss behaviour modification; (3) an OBLI intervention plus a fortified diet cola beverage (BEV) containing green tea extract (total catechin 167 mg), soluble fibre dextrin (10 g) and caffeine (100 mg) (OBLI+BEV); (4) OBWM+BEV. Assessments included height, weight, dual-energy X-ray absorptiometry-derived body composition, and waist circumference (WC). Attrition was 15·7 %. Intention-to-treat (ITT) models demonstrated a main effect for type of Internet programme, with those assigned to the OBWM condition losing significantly more weight (F= 7·174; P= 0·008) and fat mass (F= 4·491; P= 0·035) than those assigned to the OBLI condition. However, there was no significant main effect for the OBWM condition on body fat percentage (F= 2·906; P= 0·089) or WC (F= 3·351; P= 0·068), and no significant main effect for beverage use or significant interactions between factors in ITT models. A 12-week, low-intensity behaviourally based online programme produced a greater weight loss than a basic information website. The addition of a fortified diet beverage had no additional impact.

Visual–spatial neglect in a child following sub-cortical tumor resection

Although clinical syndromes of visual–spatial neglect have been well described in adults, clinical features of neglect associated with subcortical dysfunction are infrequently reported in children and have not been described in detail. Unilateral visual–spatial neglect in a 7-year-old male following removal of a right subcortical juvenile pilocytic astrocytoma is reported. Preoperative baseline neurocognitive assessment of the patient established intact attentional and intellectual functioning. Postoperatively visual–spatial neglect was observed that was not accounted for by the patient's visual field deficit. Consistent with classic features of attentional neglect, increases in attentional demands led to greater errors in performance. The risk of unilateral neglect following resection of subcortical tumors that abut the thalamus or disrupt thalamo-cortical projections was confirmed in a retrospective analysis of patients referred for neurocognitive testing at our site. It was concluded that the ventral thalamus may play a role in visual–spatial attention early in development.
