Validation of a culturally sensitive, Swahili-translated instrument to assess suicide risk among adults living with HIV in Tanzania

In Tanzania, there are high rates of suicidal thoughts and behavior among people living with HIV (PLWH), yet few instruments exist for effective screening and referral. To address this gap, we developed and validated Swahili translations of the Columbia Suicide Severity Rating Scale (C-SSRS) Screen Version and two accompanying scales assessing self-efficacy to avoid suicidal action and reasons for living. We administered a structured survey to 80 PLWH attending two HIV clinics in Moshi, Tanzania. Factor analysis of the items revealed four subscales: suicide intensity, self-efficacy to avoid suicide, fear and social concern about suicide, and family and spirituality deterrents to suicide. The area under the receiver operating curve showed only suicide intensity, and fear and social concern met the prespecified cutoff of ≥0.7 in accurately identifying patients with a plan and intent to act on suicidal thoughts. This study provides early evidence that brief screening of intensity of suicidality in the past month, assessed by the C-SSRS Screen Version, is a strong, resource-efficient strategy for identifying suicide risk in the Tanzanian setting. Patients who report little fear of dying and low concern about social perceptions of suicide may also be at increased risk.

Notes on the ecology and conservation status of key bird species in Nilo and Nguu North Forest Reserves, Tanzania

Ornithological surveys were conducted in Tanzanian submontane forest at Nilo Forest Reserve in the East Usambaras and Nguu North Forest Reserve in the Nguu Mountains in July-September 1995. Information on the ecology of poorly known bird species of conservation interest was collected and combined with previously published information to assess their conservation status. Sixteen species of conservation interest were recorded during fieldwork, including five Vulnerable (Usambara Eagle Owl Bubo vosseleri, East Coast Akalat Sheppardia gunningi, Amani Sunbird Anthreptes pallidigaster, Banded Green Sunbird Anthreptes rubritorques and Usambara Weaver Ploceus nicolli); three Near-threatened (Southern Banded Snake Eagle Circaetus fasciolatus, Fischer's Turaco Tauraco fischeri and Moreau's Sunbird Nectarinia moreaui); one regionally Endangered (Southern Hyliota Hyliota australis); one regionally Vulnerable (Uluguru Violet-backed Sunbird Anthreptes neglectus), two regionally Near-threatened (Lemon Dove Aplopelia lavata and Evergreen Forest Warbler Bradypterus barratti); and 10 in total as restricted-range species (Fischer's Turaco, Usambara Eagle Owl, Sharpe's Akalat Sheppardia sharpei, Red-capped Forest Warbler Orthotomus metopias, Spot-throat Modulatrix stictigula, Amani Sunbird, Banded Green Sunbird, Moreau's Sunbird, Kenrick's Starling Poeoptera kenricki and Usambara Weaver). New information on the ecology of these species is presented in this paper. It is recommended that the conservation status of East Coast Akalat and Uluguru Violet-backed Sunbird be changed to globally Near-threatened and regionally Near-threatened, respectively. Should further work confirm that the Usambara race of Southern Hyliota deserves specific status it is strongly recommended that it be considered globally Endangered. All other species recorded should retain their current status and continue to be monitored. The Nguus represented a new locality for several species. Sharpe's Akalat, Spot-throat, Red-Capped Forest Warbler, otherwise widely distributed throughout the east Tanzania, were absent from the Nguus.

A colony of Glossina morsitans morsitans Westw. was established in the laboratory in Tanga, Tanzania. After being expanded to the planned level of 60 000 flies, the stabilised colony was used to support a field trial using surplus males that were sterilised and released. The production system consisted of an adaptation of the in vivo rearing techniques developed at the Tsetse Research Laboratory, England. Goats were hosts for 90 % of the production; the balance was fed on rabbits. Techniques were developed to standardise the pupal off-take from stabilised colonies in each of three separate insectary units. Puparia were collected daily and kept separately until 52% emergence had occurred; these emerging flies, mostly females, were used to replenish the stock colony as needed. Further eclosion was prevented by chilling the remaining puparia (mostly males) quickly at 4°C and then storing at 10 ± 1°C for up to four days prior to irradiation and shipment to the field. About 68 % of the males produced were available for sterilisation. During the 15-month period of production in support of the field experiments, 1·3 million puparia were produced; 0·6 million of these puparia were released at a production cost of $220 per thousand.

Trypanosome infections were monitored in three species of tsetse fly (Glossina pallidipes Austen, G. morsitans centralis Machado, and G. brevipalpis News-tead) at four locations in the Kagera River region of Rwanda from May 1989 to September 1990. Two of the four areas (Mpanga Ranch and Bukora Ranch) were subjected to tsetse fly suppression operations with odour-baited traps. Proboscis infections of the Trypanosoma congolense and T. vivax types accounted for roughly equal numbers of the 207 mature infections detected (3.8%). Variation in infection rates was area-specific rather than tsetse species-specific. Order of magnitude differences in tsetse fly densities among areas were not correlated with differences in infection rates at the start of tsetse fly suppression operations. Similarly, declines in population density on both control and experimental areas were not associated with significant changes in infection rates. The prevalence of trypanosomiasis in cattle at Bukora Ranch was not affected by a roughly 90% reduction in Glossina densities. T. congolense accounted for 79% of the infections at an overall prevalence rate of 5.5%. Trypanosomiasis in cattle persisted at extremely low densities of about 0.1 fly/trap/day. Treatment of cattle with diminazene aceturate (BereniR) suggested that many T. congolense parasites were drug resistant, and hence, were cycling among cattle due to the few Glossina present.

An experiment was carried out in 1968 in the Lossitete Forest area of northern Tanzania to compare the fly-round and the Langridge-trap as sampling techniques for assessing the effectiveness of 0·4% pyrethrum applications on Glossina pallidipes Aust. when applied from the air. From fly-rounds, population reductions of 64%, 36% and 29% after the first, second and third applications, respectively, were estimated, whereas from Langridge traps the reductions were 11%, 7% and nil, respectively. Taking into consideration the efficiency of the Langridge-trap and the limitations of the fly-round in sampling G. pallidipes populations, it is concluded that the fly-round may give a false assessment of reduction of G. pallidipes after insecticide application.

Various ketones and 1-octen-3-ol were used as odour attractants with biconical traps at Nguruman, south-western Kenya, to assess their effects on the catch size and composition of Glossina pallidipes Austen. Acetone, methyl ethyl ketone and 1-octen-3-ol were found to be effective in increasing catch size by up to 2–4×, but the two ketones differed in their dose-response curves. Acetone was more effective for males than females, as was 1-octen-3-ol. The age composition of samples from baited and unbaited traps differed only slightly. The level of variability between traps was reduced by the use of odour. Acetone with cow urine produced increases in catch of 9–25×, and significant differences were found between samples in sex ratio and age composition. The index of increase for cow urine plus acetone, when used with a biconical trap, was correlated with temperature changes during the day. This could only be partially accounted for by variability in dose rates. Cow urine with acetone is considered to be a potentially useful combination of baits for community participation tsetse control schemes.

Sequential sowings were carried out at Dodoma, Tanzania, to examine the effect of changing climatic parameters on the growth and yield of bambara groundnut (Vigna subterranea). Sowings took place on 4 January, 4 February and 4 March 1994; 4 and 24 January, and 13 February 1995; 4 and 21 January, and 7 February 1996. Rainfall during the crop life cycle varied from 163 to 611 mm, mean photoperiod from 11.82 to 12.09 h d−1 and mean temperature from 22.6 to 24.4 °C. In 1994, the highest pod yields were achieved at the earliest sowing date, with a maximum of 2.87 and 1.42 t ha−1 for the red- and cream-seeded landraces, representing pod harvest indices of 0.56 and 0.34 respectively. A 30-d delay in sowing caused >60% reduction in pod yield, and a further 30-d delay resulted in no pods at all. Similarly, in 1995 successive delays in sowing caused dramatic yield declines, and the maximum yield was much lower, at 0.44 t ha−1. In 1996 there was no significant difference in pod yields between the two early sowing dates for the red-seeded landrace and yields were again lower than in 1994 with a maximum of 1.02 t ha−1. Differences in dry matter production between sowings and years were attributed mainly to differences in the amount and distribution of rainfall and to declining temperatures towards the end of the season; however, partitioning to pods was remarkably consistent across sowings.

The probability of a Glossina pallidipes picking up an infection from a single blood meal in Lambwe Valley was estimated to be 0.0092 for Trypanosoma vivax, 0.0028 for T. congolense, 0.00097 for T. brucei and 0.00024 for the human infective T. brucei. In cattle, the prevalence of blood protozoa was Trypanosoma spp. 5.55%, Babesia spp. 2% and Theileria spp. 33.06%. Thirty-eight point three per cent of the cattle with trypanosomes also had Theileria spp., 2.12% of the cattle with trypanosomes also had Babesia spp. and 88.28% of the cattle with babesia had also Theileria spp.

Mixed T. congolense and T. brucei appeared in mice when proboscis from 3 field collected G. pallidipes were individually inoculated into mice. One of these flies had earlier been shown by dissection, to have T. congolense and the other two had T. brucei infection. A comparison of the growth rates of T. congolense and T. brucei in mice revealed that T. brucei always overshadowed T. congolense. These observations highlight the significance of multiple trypanosome infections in the epidemiology of African trypanosomiasis.

Trypanosome infection rates in tsetse, Glossina pallidipes Austenin five areas along the Kenya coast were studied for 2 years (1980–1982). The total percentage infection rates were 12.62, 7.24, 9.50, 7.86 and 7.97 for Ukunda, Diani, Muhaka, Shimba Hills and Mwalewa, respectively. The respective values for males were 3.13, 4.16, 5.17, 5.03 and 8.13. Overall there were more trypanosome infections due to; Trypanosome congolense followed by T. vivax and T. brucei, although these varied among the localities. The age of the flies, their feeding patterns and regular use of chemoprophylaxis in local cattle were the main factors affecting infection rates.

The control of Trypanosoma rhodesiense sleeping sickness by clinical methods is often complicated by the existence of animal reservoir hosts which greatly contribute to the persistence of the disease in many parts of East Africa. The control of sleeping sickness should, therefore, include control of the vector and elimination of the reservoir hosts. Another factor which is often ignored, when planning sleeping sickness control measures, is the influence of cultural practices of the resident human population on the epidemiology of the disease in any given location in East Africa. At Lower Kitete, northern Tanzania, trypanosomiasis due to T. rhodesiense is occupational in nature so that many of the individuals are infected in Lossitete Forest while engaged in various activities. The factors favouring infection within the area, and the role played by the cultural practices of the Masai around the area in bringing about the outbreak of an epidemic, are discussed. The importance of understanding the cultural practices and occupational tendencies of the Masai in Lower Kitete, where trypanosomiasis outbreaks have occurred in the last 10 years, is stressed. With such an understanding the management of trypanosomiasis at Lower Kitete may be successful.
