Moderators of peak respiratory exchange ratio during exercise testing in children and adolescents with Fontan physiology

Objectives:
Many patients with Fontan physiology are unable to achieve the minimum criteria for peak effort during cardiopulmonary exercise testing. The purpose of this study is to determine the influence of physical activity and other clinical predictors related to achieving peak exercise criteria, signified by respiratory exchange ratio ≥ 1.1 in youth with Fontan physiology.
Methods:Secondary analysis of a cross-sectional study of 8–18-year-olds with single ventricle post-Fontan palliation who underwent cardiopulmonary exercise testing (James cycle protocol) and completed a past-year physical activity survey. Bivariate associations were assessed by Wilcoxon rank-sum test and simple regression. Conditional inference forest algorithm was used to classify participants achieving respiratory exchange ratio > 1.1 and to predict peak respiratory exchange ratio.
Results:Of the n = 43 participants, 65% were male, mean age was 14.0 ± 2.4 years, and 67.4% (n = 29) achieved respiratory exchange ratio ≥ 1.1. Despite some cardiopulmonary exercise stress test variables achieving statistical significance in bivariate associations with participants achieving respiratory exchange ratio > 1.1, the classification accuracy had area under the precision recall curve of 0.55. All variables together explained 21.4% of the variance in respiratory exchange ratio, with peak oxygen pulse being the most informative.
Conclusion:Demographic, physical activity, and cardiopulmonary exercise test measures could not classify meeting peak exercise criteria (respiratory exchange ratio ≥ 1.1) at a satisfactory accuracy. Correlations between respiratory exchange ratio and oxygen pulse suggest the augmentation of stroke volume with exercise may affect the Fontan patient’s ability to sustain high-intensity exercise.
A psychoeducational intervention increases use of a delirium protocol by Neurologists and Neurosurgeons in patients with brain disorders

Objectives:
Over 50% of inpatients with neurological disorders may present with a co-morbid psychiatric illness. Delirium has a reported point prevalence of 20% in hospital inpatients and is frequently undetected. We aimed to (1) examine inpatient referrals to a Liaison Neuropsychiatry service and (2) review the diagnosis and management of delirium before and after an educational intervention.
Methods:An initial 6-month audit of referrals to the inpatient Liaison Neuropsychiatry service was conducted in 2018. We then undertook a psychoeducational intervention to raise awareness of the diagnosis and management of delirium. We conducted a re-audit of referrals to the service in 2019.
Results:On initial audit, of 84 referrals, the most common referral was for mood (38%; n = 32). Just 4% (n = 3) had a specific delirium query. Following assessment by Neuropsychiatry, organic disorders (43%; n = 32), including delirium (33%; n = 25), were the most common diagnoses. On re-audit, of 86 referrals, mood assessment remained the most common reason for referral (38%; n = 33) and 2% (n = 2) were referred for possible delirium. Organic disorders remained the most common diagnoses (53%; n = 45) including delirium (38%; n = 32). We found a significant increase in the use of the delirium protocol from 12% (n = 3) on initial audit to 47% (n = 15); p < 0.01 on re-audit despite no increase in the number of specific delirium queries.
Conclusions:A psychoeducational intervention improves the management of delirium by Neurologists and Neurosurgeons in patients with brain disorders.

Clinical Enterobacteriacae isolates with a colistin minimum inhibitory concentration (MIC) ≥4 mg/L from a United States hospital were screened for the mcr-1 gene using real-time polymerase chain reaction (RT-PCR) and confirmed by whole-genome sequencing. Four colistin-resistant Escherichia coli isolates contained mcr-1. Two isolates belonged to the same sequence type (ST-632). All subjects had prior international travel and antimicrobial exposure.

Field studies were conducted to evaluate the influence of herbicide application timing on weed control in no-till soybean production. Row spacing generally had no effect on weed control. Herbicide treatments containing chlorimuron plus metribuzin applied as many as 45 days prior to planting in 1988 and 1989 controlled broadleaf weeds throughout the growing season. Imazaquin applied 45 and 30 days prior to planting provided poor control of common cocklebur in 1989. Giant foxtail control was inconsistent with all herbicide treatments. Soybean yields subsequent to early preplant herbicide applications were greater than or equal to those in which applications were made at planting when late-season weed control was adequate. Herbicides applied preemergence did not control high densities of common lambsquarters in 1989.

Objective:
The aetiology and outcomes for patients with acquired subglottic stenosis are highly variable. This study aimed to identify risk factors for subglottic stenosis and patient characteristics that predict long-term clinical outcomes.
Methods:A retrospective review was performed on 63 patients with subglottic stenosis and 63 age-matched controls. Patient demographics and clinical characteristics were compared. Subglottic stenosis patients were further grouped according to tracheostomy status (i.e. tracheostomy never required, tracheostomy initially required but patient eventually decannulated, and tracheostomy-dependent). Patient factors from these three groups were then compared to evaluate risk factors for long-term tracheostomy dependence.
Results:Compared to controls, patients with subglottic stenosis had a significantly higher body mass index (30.8 vs 26.0 kg/m2; p < 0.001) and were more likely to have diabetes (23.8 per cent vs 7.94 per cent; p = 0.01). Comparing tracheostomy outcomes within the subglottic stenosis group, body mass index trended towards significance (p = 0.08). Age, gender, socio-economic status, subglottic stenosis aetiology and other co-morbidities did not correlate with outcome.
Conclusion:Obesity and diabetes are significant risk factors for acquiring subglottic stenosis. Further investigations are required to determine if obesity is also a predictor for failed tracheostomy decannulation in subglottic stenosis.
Anticholinergic medication use is associated with globus pharyngeus

Background:
Globus pharyngeus has been linked to salivary hypofunction. We hypothesise that a considerable portion of the globus experienced by patients is due to a drying effect secondary to anticholinergic medication use; this study aimed to determine their association.
Methods:A cross-sectional study was conducted of 270 patients who presented to a laryngology practice over 6 months. Participants rated globus sensation on a 5-point severity scale, with those scoring 0 considered as controls (non-globus). Participants were excluded if they had a likely cause of globus. Scores were compared with participants’ medication lists, co-morbidities, age and gender, and evaluated using multivariate analysis, with significance set at p < 0.05.
Results:Any participant taking at least 2 anticholinergic medications had a 3.52 increased odds (p = 0.02) of experiencing globus. A previous diagnosis of gastroesophageal reflux disease was also significantly associated with globus (p = 0.004), with an odds ratio of 3.75.
Conclusion:A substantial portion of idiopathic globus may be due to anticholinergic use or reflux. The findings implicate medication use as a risk factor for globus. An awareness of these associations is invaluable for identifying cause and treating globus.

Disk-Resolved Photometry of Cometary Nuclei: Results from DIXI and Stardust-NExT

Previous comet flyby missions enabled detailed studies of the photometric properties of several cometary nuclei from disk-resolved images, including 9P/Tempel 1, 19P/Borrelly, and 81P/Wild 2. Two recent missions, DIXI and Stardust-NExT, encountered Comets 103P/Hartley 2 and Tempel 1 respectively, expanding the pool of sampled cometary nuclei in their unique ways: Hartley 2 is a hyperactive comet; Tempel 1 was visited and impacted by the Deep Impact dual-spacecraft during its previous perihelion passage. Photometric modeling shows that the global photometric properties of the nuclei of Hartley 2 and Tempel 1 are similar to those of other cometary nuclei. The photometric variation of the hyperactive nucleus of Hartley 2 is about 15%, similar to that of weakly active comets Tempel 1 and Wild 2. The photometric properties of Tempel 1 measured by NExT suggest little change from those measured by DI. These results, together with the photometric properties of Wild 2 and Borrelly, indicate that the photometric properties of cometary nuclei are independent of the activity level and gross geomorphology of cometary nuclei. Instead, cometary nucleus photometric properties might be determined by its outgassing, which leaves low-albedo deposit on the surface and forms similar photometric texture. The time scale for the photometric alteration on cometary nuclei due to outgassing should be much shorter than the dynamic time scale.

Objective.
To determine healthcare-associated infection (HAI) prevalence in 9 hospitals in Jacksonville, Florida; to evaluate the performance of proxy indicators for HAIs; and to refine methodology in preparation for a multistate survey.
Design.Point prevalence survey.
Patients.Acute care inpatients of any age.
Methods.HAIs were defined using National Healthcare Safety Network criteria. In each facility a trained primary team (PT) of infection prevention (IP) staff performed the survey on 1 day, reviewing records and collecting data on a random sample of inpatients. PTs assessed patients with one or more proxy indicators (abnormal white blood cell count, abnormal temperature, or antimicrobial therapy) for the presence of HAIs. An external IP expert team collected data from a subset of patient records reviewed by PTs to assess proxy indicator performance and PT data collection.
Results.Of 851 patients surveyed by PTs, 51 had one or more HAIs (6.0%; 95% confidence interval, 4.5%–7.7%). Surgical site infections (n = 18), urinary tract infections (n = 9), pneumonia (n = 9), and bloodstream infections (n = 8) accounted for 75.8% of 58 HAIs detected by PTs. Staphylococcus aureus was the most common pathogen, causing 9 HAIs (15.5%). Antimicrobial therapy was the most sensitive proxy indicator, identifying 95.5% of patients with HAIs.
Conclusions.HAI prevalence in this pilot was similar to that reported in the 1970s by the Centers for Disease Control and Prevention's Study on the Efficacy of Nosocomial Infection Control. Antimicrobial therapy was a sensitive screening variable with which to identify those patients at higher risk for infection and reduce data collection burden. Additional work is needed on validation and feasibility to extend this methodology to a national scale.

There have been fundamental changes in the intergenerational family, and yet families continue to be an important part of people's lives. We use the convoy model to describe the factors that influence supportive relations within intergenerational families, beginning with a description of the changing structure of the intergenerational family. We next outline support exchanges, detailing how personal characteristics, especially gender, race, age and socio-economic status, and situational characteristics, in particular family structure and intergenerational context, influence support exchanges. Instrumental and emotional family exchanges are described, with special attention to the unique circumstances of care-giving in intergenerational families. We also examine the importance of recognising differences in the quality of intergenerational relations, again noting the influence of personal and situational characteristics. Variations in support quality, e.g. positive, negative and ambivalent, and its influence on wellbeing are discussed. As families and individuals change, differences emerge at the individual, family and societal levels. We consider the implications of changes and stability in intergenerational relations and make recommendations about how best to envisage and plan future intergenerational family support. Societies with fewer resources as well as individuals and families with diverse individual histories must be innovative and creative in meeting the needs of older people as well as those of all family members.

Summary
Case narrative
Annie is born at full term with good Apgar scores, but weighs only 2350 grams. On physical exam, she is alert but has somewhat low tone. She has abnormal facial features, a short sternum, and over-riding digits. After failure to pass a nasogastric tube, a tracheal–esophageal fistula (TEF) is diagnosed. Chromosome studies confirm trisomy 18, and an echocardiogram shows a moderate ventricular septal defect (VSD) and moderate pulmonary valvular dysplasia.
Ellen, the neonatologist, and Mary, the surgeon, meet Annie’s parents, Bill and Liz, to discuss treatment options. They explain that trisomy 18 is a lethal condition and that most children die within the first few weeks to months. The team does not recommend surgery, explaining that surgery is not in the child’s best interest and will result in unnecessary suffering for Annie. They offer the services of the neonatal palliative care team and reassure Bill and Liz that the palliative care team can assist the family in the grieving process. They can provide comfort care for Annie by keeping an IV in place for hydration and pain relief.
