Welfare is a multidimensional construct and its quantification is of major scientific, societal and economic importance in veterinary medicine. The construction of indices that measure welfare validly and reliably remains a considerable challenge. A general methodology for constructing welfare indices can be adapted from human medicine (in particular, from methodologies to assess Quality of Life [QoL]) and modified to reflect the fact that all assessments of animal welfare must be observer-based. The methodology is based on the creation of individual, composite indices for distinct dimensions/domains of welfare such as pain, disease, or stress. The domains include behavioural, physiological and biochemical markers. We have established QoL methodologies in the assessment of acute and chronic pain in dogs and generalised this approach to farm animal welfare. We describe the development of a questionnaire with seven behavioural categories which are used to create a single pain score to assess acute pain in dogs. For chronic pain in dogs, a structured questionnaire with over 100 items has been devised, which the owner completes by indicating degree of agreement with each item using a seven-point Likert scale. The welfare measure includes pain as an integral component as well as husbandry, behavioural and physiological/biological measures. In each case, a profile of the individual indices can be studied and compared over time or among observers. These indices may also be combined to form a single composite welfare measure, should this be appropriate, using scaling models. In the welfare setting, we have both causal and indicator variables — and indeed, for farm animals, the causal variables may be sufficient cause for poor welfare (eg the presence of disease or inadequate husbandry).

Every year, there are over 200 traumatic deaths at work in Australia. A government safety inspector usually investigates each incident. The investigation may lead to prosecution of the employer or another party deemed to have breached relevant legislation. However, little systematic research has examined the needs and interests of grieving families in this process. Drawing on interviews with 48 representatives of institutions that deal with deaths at work (including regulators, unions, employers, police and coronial officers), this article examines how they view the problems and experiences of families. Notwithstanding some recent improvements, findings indicate ongoing shortcomings in meeting the needs of families regarding information provision, involvement and securing justice.

In the face of environmental uncertainty due to anthropogenic climate change, islands are at the front lines of global change, threatened by sea level rise, habitat alteration, extinctions and declining biodiversity. Islands also stand at the forefront of scientific study for understanding the deep history of human ecodynamics and to build sustainable future systems. We summarize the long history of human interactions with Polynesian, Mediterranean, Californian and Caribbean island ecosystems, documenting the effects of various waves of human settlement and socioeconomic systems, from hunter–gatherer–fishers, to agriculturalists, to globalized colonial interests. We identify degradation of island environments resulting from human activities, as well as cases of human management of resources to enhance productivity and create more sustainable systems. These case studies suggest that within a general global pattern of progressive island degradation, there was no single trajectory of human impact, but rather complex effects based on variable island physiographies, human subsistence strategies, population densities, technologies, sociopolitical organization and decision-making.

Factors Associated With Postoperative Infection

Objective:
We have developed and analyzed a large surgical prophylaxis database and now report the factors significantly associated with early infection, readmission due to infection, and death within 28 days of surgery. This study is intended to be a stepping-stone for further studies using our clinical database.
Design and Setting:A computerized database of 9,016 surgical patients from a 400-bed community hospital was examined. Multivariate logistic regression and tree-based modeling were used to identify factors associated with the outcomes. Factors considered included surgical procedure, prophylactic antibiotic, age, gender, serum creatinine, and albumin.
Results:12.6% had an early infection, 2.5% were read-mitted due to infection, and 2.5% died within 28 days. Most combination prophylactic antibiotics were associated with an increased probability of an early infection. Decreased albumin and increased age were associated with an increased probability of an early infection. Tracheostomy and amputations were associated with an increased probability of an early infection, whereas gallbladder and orthopedic procedures involving the arm were associated with a decreased probability. Factors associated with readmission due to infection included dialysis shunt, vascular repair, and an early infection. Factors associated with increased probability of death within 28 days included age, albumin, serum creatinine, and an early infection. Gallbladder procedures and obstetric-gynecologic procedures were associated with a decreased probability of death within 28 days.
Discussion:Older patients and those with a decreased albumin were most likely to have an early infection. To the extent that an early infection was a significant risk factor for readmission due to infection, the impact of age and albumin on the probability of readmission due to infection is demonstrated by their effects on early infections. Interestingly, albumin and age were significantly associated with death within 28 days, in addition to early infection, showing the predictive association between these factors and early death.
