Five international consensus statements on concussion in sports have been published. This commentary argues that there is a strong need for a new approach to them that foregrounds public health expertise and patient-centered guidance. Doing so will help players, parents and practitioners keep perspective about these potentially life-altering injuries especially when they recur.
Field Expedient Vasopressors During Aeromedical Evacuation: A Case Series from the Puerto Rico Disaster Response

Introduction
Emergency physicians are using bolus-dose vasopressors to temporize hypotensive patients until more definitive blood pressure support can be established. Despite a paucity of clinical outcome data, emergency department applications are expanding into the prehospital setting. This series presents two cases of field expedient vasopressor use by emergency medicine providers for preflight stabilization during aeromedical evacuation to a hospital ship as part of the United States Navy disaster response in Puerto Rico. A critical approach and review of the literature are discussed.
Case ReportTwo critically ill patients were managed in an austere environment as a result of the devastation from Hurricane Maria (Yabucoa, Puerto Rico; 2017). They both exhibited signs of respiratory distress, hemodynamic instability, and distributive shock requiring definitive airway management and hemodynamic support prior to aeromedical evacuation.
DiscussionThe novel use of field expedient vasopressors prior to induction for rapid sequence intubation was successfully and safely employed in both cases. Both patients had multiple risk factors for peri-induction cardiac arrest given their presenting hemodynamics. Despite their illness severity, both patients were induced, transported, and ultimately admitted to the intensive care unit (ICU) in stable condition following administration of the field expedient vasopressors.
Conclusion:Field expedient vasopressors were safely and effectively employed in an austere field environment during a disaster response. This case series contributes to the growing body of literature of safe bolus-dose vasopressor use by emergency physicians to temporize hypotensive patients in resource-constrained situations.

Connecting the Interstellar Gas and Dust Properties in Distant Galaxies Using Quasar Absorption Systems

Gas and dust grains are fundamental components of the interstellar medium and significantly impact many of the physical processes driving galaxy evolution, such as star-formation, and the heating, cooling, and ionization of the interstellar material. Quasar absorption systems (QASs), which trace intervening galaxies along the sightlines to luminous quasars, provide a valuable tool to directly study the properties of the interstellar gas and dust in distant, normal galaxies. We have established the presence of silicate dust grains in at least some gas-rich QASs, and find that they exist at higher optical depths than expected for diffuse gas in the Milky Way. Differences in the absorption feature shapes additionally suggest variations in the silicate dust grain properties, such as in the level of grain crystallinity, from system-to-system. We present results from a study of the gas and dust properties of QASs with adequate archival IR data to probe the silicate dust grain properties. We discuss our measurements of the strengths of the 10 and 18 μm silicate dust absorption features in the QASs, and constraints on the grain properties (e.g., composition, shape, crystallinity) based on fitted silicate profile templates. We investigate correlations between silicate dust abundance, reddening, and gas metallicity, which will yield valuable insights into the history of star formation and chemical enrichment in galaxies.
Pseudoprogression Following Chemoradiotherapy for Glioblastoma Multiforme

Purpose:
Pseudoprogression (psPD) is now recognised following radiotherapy with concurrent temozolomide (RT/TMZ) for glioblastoma multiforme (GBM). The aim of this study was to determine the incidence of psPD following RT/TMZ and the effect of psPD on prognosis.
Materials/Methods:All patients receiving RT/TMZ for newly diagnosed GBM were identified from a prospective database. Clinical and radiographic data were retrospectively reviewed. Early progression was defined as radiological progression (RECIST criteria) during or within eight weeks of completing RT/TMZ. Pseudoprogression was defined as early progression with subsequent disease stabilization, without salvage therapy, for at least six months from completion of RT/TMZ. The primary outcome was overall survival (Kaplan-Meier) and log rank analysis was used to compare groups.
Results:Out of 111 patients analyzed, 104 were evaluable for radiological response. Median age was 58 years and median follow-up 55 weeks. Early progression was confirmed in 26% and within this group 32% had psPD. Median survival for the whole cohort was 56.7 weeks [95% CI (51.0, 71.3)]. Median survival for patients with psPD was significantly higher than for patients with true early progression (124.9 weeks versus 36.0 weeks, p=0.0286).
Conclusions:Approximately one third of patients with early progression were found to have psPD which was associated with a favourable prognosis. Maintenance TMZ should not be abandoned on the basis of seemingly discouraging imaging features identified within the first three months after RT/TMZ.

Summary
OVERVIEW
In modern industrialized societies, the experience of death is often preceded by lengthy illness. This chapter therefore commences with a study of palliative care and key aspects, including pain and symptom management and advance care planning. It proceeds to examine caregiver burden and settings of care before a final section on grief and bereavement.
Introduction
Any textbook on gerontology would not be complete without a chapter on the topic of death, dying and bereavement. Indeed, that was the intended title of this section of the book. However, a brief discussion of the context in which many older adults live today, and eventually die, will help explain the alternative title as it appears above.
In 1900, the leading causes of death in the USA were pneumonia, tuberculosis and intestinal infections. This pattern was true across much of the developed world. These three diseases are all infectious, all resulted in rapid death due to lack of effective treatments, and all affected individuals of all ages. With modern advances, first in sanitation and later in antibiotics and other medical treatments, some people with these ailments survived and others were prevented from contracting the illnesses in the first place. One of the consequences of these advances is what Robert Butler (2008) has coined the longevity revolution.

Summary
While malignancies in women of reproductive age are relatively uncommon, they are, after accidents, the most common cause of death in women aged 20-39 in the USA. Malignant melanoma was the most frequently diagnosed cancer occurring in pregnancy, followed by cervical cancer and breast cancer, reflecting the high melanoma risk in the fair-skinned Swedish population. The incidence of melanoma increases steadily from the age of 20 years and thus is a relatively common cancer in women during their reproductive years and is more common than breast cancer in the under-30s. Thyroid cancer is 2.5 times more common in women than men and the incidence varies between different populations owing to genetic and environmental factors. Gestation trophoblastic neoplasia is a malignancy intimately related to pregnancy. Improved data could be obtained by the central population-based registries formally collecting data on cancer during pregnancy.
