Partnered innovation to implement timely and personalized care: A case study

Background:
Understanding how to translate research discoveries into solutions for healthcare improvement is a priority of NIH-funded Clinical and Translational Science Awards (CTSA). This study, supported by one CTSA, aims to capture one process of shaping and implementing innovations to advance the timeliness and patient-centeredness of cardiovascular care. Specifically, we sought to understand a partnership between a private digital health startup company, a university innovation lab, and an academic health system’s cardiology program pursuing this goal.
Findings:The collaboration proceeded through clear phases to address the questions and challenges: problem definition, exploration and formalization of the partnership, innovation co-creation and pilot test, and scale-up planning. Phases were punctuated by key decisions, such as forming the partnership, negotiating terms of the partnership, iterating form and features of the innovation, and exploring sufficiency of its value-add for scale-up and sustainment. Key implementation concepts were apparent, including implementation strategies (e.g., champions and iterative trialing) and the implementation outcomes of acceptability, sustainment, and scale-up. Participants identified potential risks of collaboration, reflected on their co-creation process, and the value of engaging stakeholders in innovation design. Findings may inform subsequent collaborations between innovators and translational researchers.
Methods:We conducted a case study to understand the partnership; characterize the questions they pursued, their decision points, information and data sources; and identify the challenges and risks. Data were collected through a series of four focus groups with members of each partnering organization. A transdisciplinary research team iteratively worked to condense and synthesize data from audio recorded transcripts into a case narrative.

INTRODUCTION
While other chapters in this book address the role of national political cultures in the governance of DNA databases, in this chapter we examine how other forensic systems have provided models for the organisation of such databases. We will argue that many, but by no means all, aspects of DNA profiling followed patterns established historically by earlier techniques. In particular, we focus on fingerprint identification, the technique we view as most closely analogous to DNA profiling on several levels. Both fingerprinting and DNA profiling seek to identify particular bodies as sources of crime scene traces by examining correspondences between those traces and reference samples taken from persons in police custody. Both techniques proved useful enough from a social control perspective to warrant large government investments in developing databases of records indexed according to bodily markers. Finally, both fingerprinting and DNA profiling have enjoyed primacy as ‘gold standards’ in an imagined hierarchy of forensic techniques. Indeed, even today when DNA profiling is sometimes viewed as having supplanted fingerprinting, one recent report has noted that, ‘the more humble fingerprint retains its status as the most commonly used method of identification and is a cornerstone of forensic crime scene investigation’ (Nuffield Council on Bioethics 2007: 15).
Profiling with DNA is widely heralded as a novel and distinctively scientific technique for analysing criminal evidence that is having revolutionary impact on criminal justice systems throughout the world.
Assessing the quality of seagrass data collected by community volunteers in Moreton Bay Marine Park, Australia

Volunteer-collected data have become widely used, largely because of a perception of cost efficiency, however, the quality of these data is often questioned. This paper analyses the quality of visual estimates of seagrass cover collected by trained volunteers in Moreton Bay (Australia) which has c. 25 000 ha of seagrass habitat. Seagrass was routinely monitored by trained volunteers at 51 sites (50 × 50 m area of intertidal flat) using standardized protocols. Volunteers walked three transects at low tide and assessed the percentage of substrate covered by seagrass within quadrats (50 × 50 cm in area) using photographic guides. Of 33 samples (quadrats) taken at each site, nine (27%) were photographed and later scored by coordinating scientists. The visual estimation of per cent seagrass cover by volunteers was highly correlated with that of scientists and can therefore be used as a reliable source of base-line information about seagrasses in Moreton Bay. The qualities of this successful community-based monitoring programme include expert scientific and multi-organizational involvement, simple methods and result dissemination.

The death penalty debate in the United States has recently undergone a fundamental shift. The possibility of executing the innocent has emerged as some abolitionists' most salient argument, displacing debates over such issues as fairness, deterrence, and cost. Innocence has managed to move to the fore of the debate in part because of the success of death penalty opponents in attaching epistemological certainty to one particular category of postconviction exonerations, those vouched for by the authority of DNA evidence. We suggest that such moves are primarily rhetorical because, while DNA evidence may be more accurate and reliable than other forensic science, it still fundamentally probabilistic in nature and is prone to uncertainties at all stages of its production. Yet, because of the certainty attached to DNA evidence in public discourse, it can be used as a lever with which to challenge law's claims to truth‐making authority, and to undermine public trust in the death penalty. A few abolitionists and other scholars have expressed misgivings about the abolitionist embrace of the innocence argument. We push this concern further, suggesting that both abolitionists and death penalty reformers, who seek to promote a “scientific” death penalty centered on DNA evidence, draw upon a mythologized notion of “science” as a producer of epistemic certainty.

The Desert Migrations Project is a new interdisciplinary and multi-dimensional collaborative project between the Society for Libyan Studies and the Department of Antiquities. The geographical focus of the study is the Fazzan region of southwest Libya and in thematic terms we aim to address the theme of migration in the broadest sense, encompassing the movement of people, ideas/knowledge and material culture into and out of Fazzan, along with evidence of shifting climatic and ecological boundaries over time. The report describes the principal sub-strands of the project's first season in January 2007, with some account of research questions, methods employed and some preliminary results. Three main sub-projects are reported on. The first concerns the improved understanding of long-term climatic and environmental changes derived from a detailed palaeoenvironmental study of palaeolake sediments. This geo-science work runs alongside and feeds directly into both archaeological sub-projects, the first relating to prehistoric activity and mobility around and between a series of palaeolakes during wetter climatic cycles; the second to the excavation of burials in the Wadi al-Ajal, exploring the changing relationship between material culture, identity and ethnicity across time, from prehistory to the early Islamic period (the span of the main cemetery zones). In addition, some rock art research and a survey of historic period sites was undertaken in the Wadi ash-Shati and Ubari sand sea.

Two parallel traditions have coexisted throughout the history of modern finger print identification. One, which gave more emphasis to the rhetoric of “science,” has always been somewhat troubled by the lack of an easily articulated scientific foundation for “dactyloscopy.” The other, more concerned with practicalities, was satisfied that the method of fingerprint identification appeared to “work” and that it won widespread legal acceptance. The latter group established conser vative rules of practice to guard against errors and preserve the credibility of latent fingerprint identification in the eyes of the law. The legacy of this history is coming home to roost today, as some latent fingerprint examiners (LFPEs) are beginning to argue that the traditional practice of latent fingerprint comparison lacks a scientific foundation appropriate to contemporary forensic science. This issue raises the question of what constitutes a “scientific” method for individual ized identification in a legal setting.

In this paper we first give a brief overview of the MOCVD of high temperature superconductors and consider the general requirements of a precursor. We then present results for deposition from Y containing precursors and compare apparent enthalpies of sublimation and deposition activation energies with values in the literature. The problem of Ba containing precursors are briefly reviewed and results given for the sublimation of and deposition from a new stable and volatile precursor. Finally, some comments are made about possible deposition mechanisms.
