The Results of a Foresight Exercise: Future Threats and Trends in Crisis Management

Introduction:
The challenges that the health systems face in the last years increased exponentially. No matter if we are talking about the impact of the COVID-19 pandemic or the Russian military action in Ukraine, the European health ecosystem is facing new problems. In the light of these uncertainties, we assessed which could be the next trends that can impact the healthcare systems, in order to better prepare and adapt to the new contexts.
Method:Using two foresights exercises (FSE), one in 2018 and the second one in 2022, we identified the most important trends in the political, economic, social, technological, security, environmental and medical sectors that could have an impact on health.
Results:53 people participated in the first FSE and 40 in the second one. The respondents identified cyber security, an increased reliance on digital technologies for communications, CBRNE management of the patients, centrally coordinated attacks, demographic aging, reduced economic resources, violence against emergency medical staff and the increased need and demand for psychosocial support as the most important trends. Moreover, they considered that wars, hybrid threats, the fake news, pandemics and the influence of artificial intelligence could impact the healthcare systems.
Conclusion:Many of the trends identified in 2018 as having a possible impact on the health system proved to be relevant four years later. Therefore, we consider the FSE a relevant tool in foreseeing the main areas that could have an impact on health and its results could guide the preparedness for the future.
The CERAD Neuropsychological Assessment Battery Is Sensitive to Alcohol-Related Cognitive Deficiencies in Elderly Patients: A Retrospective Matched Case-Control Study

Objectives: Adverse effects of heavy drinking on cognition have frequently been reported. In the present study, we systematically examined for the first time whether clinical neuropsychological assessments may be sensitive to alcohol abuse in elderly patients with suspected minor neurocognitive disorder. Methods: A total of 144 elderly with and without alcohol abuse (each group n=72; mean age 66.7 years) were selected from a patient pool of n=738 by applying propensity score matching (a statistical method allowing to match participants in experimental and control group by balancing various covariates to reduce selection bias). Accordingly, study groups were almost perfectly matched regarding age, education, gender, and Mini Mental State Examination score. Neuropsychological performance was measured using the CERAD (Consortium to Establish a Registry for Alzheimer’s Disease). Classification analyses (i.e., decision tree and boosted trees models) were conducted to examine whether CERAD variables or total score contributed to group classification. Results: Decision tree models disclosed that groups could be reliably classified based on the CERAD variables “Word List Discriminability” (tapping verbal recognition memory, 64% classification accuracy) and “Trail Making Test A” (measuring visuo-motor speed, 59% classification accuracy). Boosted tree analyses further indicated the sensitivity of “Word List Recall” (measuring free verbal recall) for discriminating elderly with versus without a history of alcohol abuse. Conclusions: This indicates that specific CERAD variables seem to be sensitive to alcohol-related cognitive dysfunctions in elderly patients with suspected minor neurocognitive disorder. Neurofibromatosis type 1 (NF1) is a common autosomal dominant disorder caused by heterozygotic inactivation of the NF1 tumor suppressor gene at 17q11.2. The associated phenotypes are highly variable, and modifying genes have been proposed to explain at least in part the intriguing expressivity. Given that haploinsufficiency of the NF1 gene product neurofibromin is responsible for some of the clinical manifestations, variations in expression of the wildtype NF1 allele might modify the phenotype. We therefore investigated epigenetic molecular modifications that could result in variable expression of the normal NF1 allele. To exclude confounding by DNA sequence variations, we analyzed monozygotic twin pairs with NF1 who presented with several discordant features. We fine-mapped the methylation pattern of a nearly 1 kb NF1 promoter region in lymphocytes of 8 twin pairs. All twin pairs showed significant intra-pair differences in methylation, especially of specific promoter subregions such as 5'UTR, exon 1 and intron 1 (+7 to +622), transcription factor binding sites and promoter elements like NF1HCS. Furthermore, we detected significant intra-pair differences in cytosine methylation for the region from -249 to -234 with regard to discordance for optic glioma with a higher grade of methylation in glioma cases. In conclusion, our findings of epigenetic differences of the NF1 promoter in leukocytes within mono zygotic twin pairs may serve as a proof of principle for other tissues. The results point towards a role of methylation patterns of the normal NF1 allele for expression differences and for modification of the NF1 phenotype.

Legislative speech records from the 101st to 108th Congresses of the US Senate are analysed to study political ideologies. A widely-used text classification algorithm – Support Vector Machines (SVM) – allows the extraction of terms that are most indicative of conservative and liberal positions in legislative speeches and the prediction of senators’ ideological positions, with a 92 per cent level of accuracy. Feature analysis identifies the terms associated with conservative and liberal ideologies. The results demonstrate that cultural references appear more important than economic references in distinguishing conservative from liberal congressional speeches, calling into question the common economic interpretation of ideological differences in the US Congress.

Incorporation of Na in Low-Temperature Deposition of CIGS Flexible Solar Cells

Different methods for Na incorporation are known for the use of Na-free substrates like stainless steel or polyimide foil. In this work Cu(In,Ga)Se2 (CIGS) absorber layers with different amounts of Na are investigated. The CIGS samples were prepared via a roll-to-roll deposition process with ion beam assistance (Solarion) and by a multi-stage low temperature co-evaporation process (HZB). Na was either incorporated via in-situ co-evaporation of NaF (for roll-to-roll deposition) or by a Na-containing precursor (for multi-stage deposition). With increasing amounts of Na an increase of VOC is observed for both deposition tech-niques. In contrast, within the deposition parameters used, jSC decreases with increasing Na amount for co-evaporation of NaF while it seems unaffected when using a NaF precursor layer. The elemental depth profiles of the different CIGS thin films were studied via secondary ion mass spectroscopy and were found to depend strongly on the deposition technique. It seems that beneficial effects of the addition of Na are independent of the method of in-corporation, even if the distribution of Na in the CIGS layer is different due to different methods of incorporation and CIGS deposition processes.
Sedimentary evolution of a Palaeozoic basin and ridge system: the Middle and Upper Devonian of the Ahnet and Mouydir (Algerian Sahara)

The Ahnet and Mouydir regions of southern Algeria are part of one of the world's largest, almost undeformed exposures of Palaeozoic rocks which exemplify a hitherto poorly known early Variscan development of a Devonian basin and ridge system. This area includes a series of intracratonic basins along the northern margin of the West African Craton which consists (from W to E) of the Reggane Basin, Azel Matti Ridge, Ahnet Basin, Foum Belrem Ridge and Mouydir Basin. The depositional and palaeogeographic interpretation is based on 71 sections in this region, which for the first time were biostratigraphically calibrated by means of conodonts, goniatites and brachiopods. The structural evolution during Devonian times was probably controlled by reactivation of ancient N–S- to NW–SE-running faults in the Precambrian basement, which caused differential subsidence and uplift of a previously largely unstructured siliciclastic shelf. A hiatus during Emsian times indicates widespread emergence during this interval. The entire area was flooded during the earliest Eifelian, when the first vestiges of the Azel Matti Ridge become evident by stratigraphic condensation. The palaeogeographic differentiation is most apparent during the Givetian, when a shoal with reduced carbonate sedimentation was established on the Azel Matti Ridge passing towards the west and east into basinal environments of the Reggane and Ahnet basins, respectively. The Foum Belrem Ridge is distinguished by increased subsidence during the early Givetian and by revived uplift during the late Givetian. In the Mouydir Basin further east, up to 1000 m of shales were deposited during the Givetian. The early Frasnian is marked by the ubiquitous sedimentation of black shales and bituminous styliolinites. These lithologies occur repeatedly already during the Middle Devonian and document intermittent anoxic conditions. The basin and ridge topography is levelled by the shallowing-up sequence of up to 1400 m thick upper Frasnian and Famennian shales which grade into a deltaic sequence of uppermost Famennian/Tournaisian sandstones. The up to now only vaguely discriminated lithostratigraphic formations of the Devonian have been biostratigraphically defined in suitable type sections.
