Summary
The rift setting of eastern Africa preserves exceptional records of mammalian (including hominin) fossils and archeology. The Afar region is host to multiple deposits with sediments ranging in age from>9 Ma to the present (Chorowicz, 2005; Katoh et al., 2016) and plays a major role in our understanding of human origins. The Gona project area contains fossiliferous deposits that span ca. 6.3 to <0.15 Ma (Quade et al., 2008); the duration of this record means that it can make a distinct contribution to understanding the environmental context for human evolution within the Afar and in eastern Africa (Figures 17.1 and 17.2). The primary units at Gona include the late Miocene Adu-Asa Formation, which contains fossils of Ardipithecus kaddaba; the early Pliocene Sagantole Formation with fossils of Ardipithecus ramidus; the mid- to late-Pliocene Hadar Formation; and the Busidima Formation (ca. 2.7 Ma to <0.15 Ma), which contains a record of the earliest Oldowan stone tools, fossils of Homo erectus, and Acheulean artifacts (Figure 17.2).
Use of Naloxone in 9-1-1 Patients without Respiratory Depression in Los Angeles County, California (USA)

Introduction:
Along with an increase in opioid deaths, there has been a desire to increase the accessibility of naloxone. However, in the absence of respiratory depression, naloxone is unlikely to be beneficial and may be deleterious if it precipitates withdrawal in individuals with central nervous system (CNS) depression due to non-opioid etiologies.
Objective:The aim of this study was to evaluate how effective prehospital providers were in administering naloxone.
Methods:This is a retrospective study of naloxone administration in two large urban Emergency Medical Service (EMS) systems. The proportion of patients who had a respiratory rate of at least 12 breaths per minute at the time of naloxone administration by prehospital providers was determined.
Results:During the two-year study period, 2,580 patients who received naloxone by prehospital providers were identified. The median (interquartile range) respiratory rate prior to naloxone administration was 12 (6-16) breaths per minute. Using an a priori respiratory rate of under 12 breaths per minute to define respiratory depression, only 1,232 (47.8%; 95% CI, 50.3%-54.2%) subjects who received naloxone by prehospital providers had respiratory depression.
Conclusion:This study showed that EMS providers in Los Angeles County, California (USA) frequently administered naloxone to individuals without respiratory depression.

Objective: Idiopathic normal pressure hydrocephalus (INPH) is a neurological disorder presenting with gait, cognitive, and bladder symptoms in the context of ventricular enlargement. Although gait is the primary indicator for treatment candidacy and outcome, additional monitoring tools are needed. Line Tracing Test (LTT) and Serial Dotting Test (SDT), two psychomotor tasks, have been introduced as potential outcome measures but have not been widely studied. This preliminary study examined whether LTT and SDT are sensitive to motor dysfunction in INPH and determined if accuracy and time are important aspects of performance. Methods: Eighty-four INPH subjects and 36 healthy older adults were administered LTT and SDT. Novel error scoring procedures were developed to make scoring practical and efficient; interclass correlation showed good reliability of scoring procedures for both tasks (0.997; p<.001). Results: The INPH group demonstrated slower performance on SDT (p<.001) and made a greater number of errors on both tasks (p<.001). Combined Time/Error scores revealed poorer performance in the INPH group for original-LTT (p<.001), modified-LTT (p≤.001) and SDT (p<.001). Conclusions: These findings indicate LTT and SDT may prove useful for monitoring psychomotor skills in INPH. While completion time reflects impaired processing speed, reduced accuracy may suggest planning and self-monitoring difficulties, aspects of executive functioning known to be compromised in INPH. This is the first study to underscore the importance of performance accuracy in INPH and introduce practical/reliable error scoring for these tasks. Future work will establish reliability and validity of these measures and determine their utility as outcome tools. (JINS, 2016, 22, 341–349)

Preface
Despite the increasing role of advanced cross-sectional imaging studies such as CT, MR, and PET for evaluating diseases of the abdomen and pelvis, most radiology practices continue to perform GI and GU fluoroscopic procedures on a daily basis. It therefore is important for radiologists to develop and maintain their skills in GI and GU fluoroscopy in order to provide optimal studies for their patients. In this regard, we recognized the need for a text dedicated to all aspects of performing and interpreting high-quality GI and GU fluoroscopic procedures. Our goal in writing this text was to illuminate the critical role of fluoroscopy in the diagnosis of luminal GI and GU diseases. Rather than provide an encyclopedic review of GI and GU pathology, we instead present the fundamentals and nuances for performing and interpreting all types of GI and GU fluoroscopic procedures.
Two of the authors of this work, Marc Levine and Stephen Rubesin, previously collaborated with Igor Laufer on the last edition of Double Contrast Gastrointestinal Radiology, published in 2000, and the third author, Parvati Ramchandani, collaborated with Howard Pollack on his landmark text, Clinical Urography, also last published in 2000. The three of us have been long-time colleagues and friends in the Perelman School of Medicine at the University of Pennsylvania and were delighted to collaborate on Practical Fluoroscopy of the GI and GU Tracts. It is our hope that this text will fill a major void in the radiology literature.

We evaluated the presentation, treatment, and outcome of infants who present with ventricular tachycardia in the first year of life. Seventy-six infants were admitted to our institution with a diagnosis of ventricular tachycardia between January, 1987 and May, 2006. Forty-five infants were excluded from the study because of additional confounding diagnoses including accelerated idioventricular rhythm, Wolff–Parkinson–White syndrome, supraventricular tachycardia with aberrancy, long QT syndrome, cardiac rhabdomyoma, myocarditis, congenital lesions, or incomplete data. The remaining 31 included infants who had a median age at presentation of 1 day, with a range from 1 to 255 days, and a mean ventricular tachycardia rate of 213 beats per minute, with a range from 171 to 280, at presentation. The infants were treated chronically with propranolol (38.7%), amiodarone (12.9%), mexiletine (3.2%), propranolol and mexiletine (9.7%), or propranolol and procainamide (6.5%). The median duration of treatment was 13 months, with a range from 3 to 105 months. Ventricular tachycardia resolved spontaneously in all infants. No patient died, or received catheter ablation or device therapy. Median age at last ventricular tachycardia was 59 days, with a range from 1 to 836 days. Mean follow-up was 45 months, with a range from 5 to 164 months, with a mean ventricular tachycardia-free period of 40 months. Infants with asymptomatic ventricular tachycardia, a structurally normal heart, and no additional electrophysiological diagnosis all had spontaneous resolution of tachycardia. Furthermore, log-rank analysis of the time to ventricular tachycardia resolution showed no difference between children who received chronic outpatient anti-arrhythmic treatment and those who had no such therapy. While indications for therapy cannot be determined from this study, lack of symptoms or myocardial dysfunction suggests that therapy may not be necessary.
