Recent experiences with lay public cardiopulmonary resuscitation and use of ambulatory external defibrillators have been demonstrated to improve survival from out-of-hospital cardiac arrests from an abysmal 10% or less to as high as 30%. Though rare, sudden cardiac arrest in presumably healthy young people have been highly publicised over the last 25 years and have motivated the institution of school- and community-based programmes that facilitate first-responder resuscitation, including defibrillation. These efforts often begin at a grass roots level (usually parent-inspired advocacy groups) and through collaborative activities involving the schools, local businesses, and local administrative agencies are now having a meaningful impact in selected communities. The current focus of these activities is on school- and sports venue-based ambulatory external defibrillator programmes and formal cardiopulmonary resuscitation education in high schools. The extension of these programmes to the level of state mandates has been slower, and even when enacted, public funding has usually not been approved.

Congenital coronary arterial abnormalities as isolated lesions are exceedingly rare. The electrocardiogram, while a reasonable adjunct in the diagnosis of coronary arterial abnormalities, should not supplant a good history and physical examination. Careful attention must be devoted to any signs or symptoms of ischaemic pain in the chest or syncope, which must not be overlooked. Exertional pain in the chest and exertional syncope should prompt an extensive evaluation by both the echocardiographer and the electrophysiologist. Clearance for participation in sports should be curtailed until a complete evaluation has ruled out the presence of any of the following disorders: a channelopathic mutation, a cardiomyopathy, or a congenital coronary arterial anomaly. Major abnormalities in the coronary arteries may present in the first few months of life or remain dormant until the exertional demands of adolescence unmask symptoms of myocardial ischaemia. Congenital coronary arterial anomalies may be analysed in the following major diagnostic groups: anomalous origin of the left coronary artery from the pulmonary artery, anomalous aortic origin of a coronary artery from the wrong aortic sinus of Valsalva, atresia of the left main coronary artery, myocardial bridges, and coronary arterial fistulas. The advent of state-of-the-art modalities of imaging seems, at times, to have supplanted the electrocardiogram in making the diagnosis of potentially serious coronary artery abnormalities, especially in asymptomatic patients. However, as is also the case for a detailed history and physical examination, the electrocardiogram provides a potentially insightful look at the coronary arteries. Furthermore, the past decade has witnessed an increase in the use of the electrocardiogram as a screening tool in the assessment of the risk of sudden cardiac death in athletes in high school.

In recent years it has become much more accepted in Germany to consider aspects of the Second World War in which Germans were not perpetrators, but victims: the Allied bombing campaign, expulsions of 'ethnic' Germans, mass rapes of German women, and postwar internment and persecution. An explosion of literary fiction on these topics has accompanied this trend. Sebald's 'The Air War and Literature' and Grass's 'Crabwalk' are key texts, but there are many others; the great majority seek not to revise German responsibility for the Holocaust but to balance German victimhood and German perpetration. This book of essays is the first in English to examine closely the variety of these texts. An opening section on the 1950s - a decade of intense literary engagement with German victimhood before the focus shifted to German perpetration - provides context, drawing parallels but also noting differences between the immediate postwar period and today. The second section focuses on key texts written since the mid-1990s shifts in perspectives on the Nazi past, on perpetration and victimhood, on 'ordinary Germans,' and on the balance between historical empathy and condemnation. Summary
TWENTY YEARS AFTER THE FALL of the Berlin Wall in November 1989, how the Nazi past is discussed, represented, and commemorated in what has come to be known as the Berlin Republic now appears somewhat different from the “memory culture” of the “old” Federal Republic (FRG, or West Germany) and certainly distinct from the state-directed memorialization of the former East Germany. In place of a rather rigid opposition between a West German “official” culture of Holocaust remembrance, modesty in foreign affairs, and ritualized gestures of contrition — at least from the early 1970s — and an uneasiness on the part of many private citizens with repeated proclamations of German guilt and the “repression” of “German suffering,” — most likely vocalized from the early 1980s by conservative challenges to the centrality of the Nazi past to public debate — we now see the emergence of a more fluid, less monolithic, and often more fragmented discourse on the years 1933 to 1945. This is a form of multifaceted engagement with a past that can never be grasped in its totality that accepts gaps in knowledge and understanding and explicitly attempts to capture the complexity of the period — above all, the reality that German perpetrators might also have been victims — as well as the bewilderingly contingent nature of individual motivations, personal circumstances, and the array of opportunities for both cowardice and courage. In a hitherto seldomly encountered fashion, then, much of today's debate on Nazism centers on the effort to reconcile empathy with “real” historical actors, their choices and limitations (individual choices and objectively “given”), with more abstract notions of historical justice, universal ethical imperatives, and personal responsibility.
