Early auditory processing abnormalities alter individual learning trajectories and sensitivity to computerized cognitive training in schizophrenia

Background
Auditory system plasticity is a promising target for neuromodulation, cognitive rehabilitation and therapeutic development in schizophrenia (SZ). Auditory-based targeted cognitive training (TCT) is a ‘bottom up’ intervention designed to enhance the speed and accuracy of auditory information processing, which has been shown to improve neurocognition in certain SZ patients. However, the dynamics of TCT learning as a function of training exercises and their impact on neurocognitive functioning and therapeutic outcomes are unknown.
MethodsForty subjects (SZ, n = 21; healthy subjects (HS), n = 19) underwent comprehensive clinical, cognitive, and auditory assessments, including measurements of auditory processing speed (APS) at baseline and after 1-h of TCT. SZ patients additionally completed 30-hours of TCT and repeated assessments ~10–12 weeks later.
ResultsSZ patients were deficient in APS at baseline (d = 0.96, p < 0.005) relative to HS. After 1-h of TCT, analyses revealed significant main effects of diagnosis (d = 1.75, p = 0.002) and time (d = 1.04, p < 0.001), and a diagnosis × time interaction (d = 0.85, p = 0.02) on APS. APS learning effects were robust after 1-h in SZ patients (d = 1.47, p < 0.001) and persisted throughout the 30-h of training. Baseline APS was associated with verbal learning gains after 30-h of TCT (r = 0.51, p = 0.02) in SZ.
ConclusionsTCT learning metrics may have prognostic utility and aid in the prospective identification of individuals likely to benefit from TCT. Future experimental medicine studies may advance predictive algorithms that enhance TCT-related clinical, cognitive and functional outcomes.
Trivial aortic valve regurgitation in children and adolescents with structurally normal hearts: physiologic or pathologic?

Background:
Trivial regurgitation from a normal aortic valve is rarely seen in healthy children and adolescents. The aim of this study was to evaluate timing of presentation, associated conditions, and medium-term prognosis of this incidental finding.
Methods:Paediatric patients observed to have trivial aortic valve regurgitation with otherwise normal echocardiograms were retrospectively analysed. Clinical and echocardiographic parameters were measured and categorised on presentation and follow-up.
Results:Sixty patients (39 males) were identified over a 13-year period. Age at presentation was 14.8 years (IQR 12.9–16.0), height z-score was + 0.71 (95% CI + 0.48– + 0.94), and body mass index z-score was + 0.66 (95% CI + 0.40– + 0.92). Median aortic regurgitation vena contracta diameter was 1.0 mm (IQR 0.8–1.3). Aortic valve strands were visualised in 28% and physiologic mitral regurgitation in 32%. Aortic annulus, sinotubular junction, and mid-ascending aorta diameters were normal, and mean aortic sinus diameter was only slightly increased (z-score + 0.23, 95% CI + 0.02– + 0.44). Follow-up data were obtained in 36 patients from 1 to 6.7 years later (median 2.1). Aortic regurgitation was no longer detectable in 28%, and none exhibited worsening. Mitral regurgitation prevalence was lower in those with aortic regurgitation resolution versus persistence (10% versus 50%, p = 0.03).
Conclusions:Trivial aortic regurgitation in paediatric patients with normal hearts is more common in adolescents and is associated with an increased prevalence of aortic valve strands and physiologic mitral regurgitation. These findings do not worsen during growth and may resolve consistent with being physiologic rather than pathologic.

Background
Evidence suggests that cognitive subtypes exist in schizophrenia that may reflect different neurobiological trajectories. We aimed to identify whether IQ-derived cognitive subtypes are present in early-phase schizophrenia-spectrum disorder and examine their relationship with brain structure and markers of neuroinflammation.
Method161 patients with recent-onset schizophrenia spectrum disorder (<5 years) were recruited. Estimated premorbid and current IQ were calculated using the Wechsler Test of Adult Reading and a 4-subtest WAIS-III. Cognitive subtypes were identified with k-means clustering. Freesurfer was used to analyse 3.0 T MRI. Blood samples were analysed for hs-CRP, IL-1RA, IL-6 and TNF-α.
ResultsThree subtypes were identified indicating preserved (PIQ), deteriorated (DIQ) and compromised (CIQ) IQ. Absolute total brain volume was significantly smaller in CIQ compared to PIQ and DIQ, and intracranial volume was smaller in CIQ than PIQ (F(2, 124) = 6.407, p = 0.002) indicative of premorbid smaller brain size in the CIQ group. CIQ had higher levels of hs-CRP than PIQ (F(2, 131) = 5.01, p = 0.008). PIQ showed differentially impaired processing speed and verbal learning compared to IQ-matched healthy controls.
ConclusionsThe findings add validity of a neurodevelopmental subtype of schizophrenia identified by comparing estimated premorbid and current IQ and characterised by smaller premorbid brain volume and higher measures of low-grade inflammation (CRP).
Prominent prolapsing Chiari network: presentation and prognosis in paediatric patients

Chiari network is an infrequently visualized web-like structure in the right atrium that is usually thin and small. Rarely, it can be prominent and elongated with protrusion into the right ventricle during diastole and complications have been reported. Thirty-eight patients (median age 2.5 years) with prolapsing Chiari network were identified and associated cardiac abnormalities documented. Echocardiographic right and left heart parameters were measured and compared to normative data. At presentation, the extent of Chiari network prolapse below the tricuspid annulus was 9.1 ± 3.5 mm (mean ± standard deviation), mean pulmonary valve annulus diameter z-value was reduced (-0.91 ± 0.64), and mean aortic valve z-value was enlarged (+0.97 ± 0.87). Fourteen patients exhibited no other cardiac abnormality. Eight were noted to have atrial septal defects, eight demonstrated supraventricular dysrhythmias, six had mild to moderate tricuspid valve regurgitation, and one patient each had an atrial septal aneurysm, severe pulmonic valve stenosis, small perimembranous ventricular septal defect, bicuspid aortic valve, and mild right upper pulmonary vein stenosis. Fourteen patients (37%) were seen in follow-up from 1 to 8.5 years later (median 3.2 years). During that time, the magnitude of prolapse across the tricuspid valve decreased by up to 7 mm (median 2 mm). Interventions were required in three patients, but many associated cardiac abnormalities resolved spontaneously with growth. Thus, the presence of a prolapsing Chiari network has a substantial risk of associated CHDs. However, the extent of Chiari network prolapse gradually decreases and many related abnormalities resolve during growth.

Approximately, 1.7 million individuals in the United States have been infected with SARS-CoV-2, the virus responsible for the novel coronavirus disease-2019 (COVID-19). This has disproportionately impacted adults, but many children have been infected and hospitalised as well. To date, there is not much information published addressing the cardiac workup and monitoring of children with COVID-19. Here, we share the approach to the cardiac workup and monitoring utilised at a large congenital heart centre in New York City, the epicentre of the COVID-19 pandemic in the United States.

The role of Pityophthorus spp. as vectors of pitch canker affecting Pinus radiata

The fungus Fusarium circinatum Nirenberg and O’Donnell (Hypocreaceae) causes pitch canker, a disease affecting pines worldwide. In California, many native insect species have been implicated in transmission of F. circinatum. This study showed that two twig beetle species, Pityophthorus setosus Blackman and Pityophthorus carmeli Swaine (Coleoptera: Curculionidae: Corthylini), can make wounds on healthy Monterey pine (Pinus radiata D. Don (Pinaceae)) branches that are suitable for infection by the pitch canker pathogen. Because these two species are not known to engage in maturation feeding and the observed wounds were not associated with tunneling, we hypothesize that the wounds reflect “exploratory tasting” to assess the suitability of the substrate for colonization. This behavior would help to explain how twig beetles can serve as wounding agents on healthy host branches, which are not amenable to colonization by these insects. We tested two specific hypotheses: (1) two native species of Pityophthorus can create wounds on F. circinatum-contaminated trees that are sufficient for development of disease; and (2) the efficiency with which F. circinatum infects beetle wounds is affected by relative humidity. Under growth-chamber conditions, both Pityophthorus species indulged in exploratory behavior that caused wounds suitable for development of pitch canker. Field experiments did not confirm a significant effect of beetle activity on infection frequency, perhaps because of an overall low infection rate due to low temperatures. Experiments conducted under controlled conditions documented a significant effect of relative humidity on the success rate of twig beetle-initiated infections.

Background
In first-episode schizophrenia, longer duration of untreated psychosis (DUP) predicts poorer outcomes.
AimsTo address whether the relationship between DUP and outcome is a direct causal one or the result of association between symptoms and/or cognitive functioning and social functioning at the same time point.
MethodSymptoms, social function and cognitive function were assessed in 98 patients with first-episode schizphrenia at presentation and 1 year later.
ResultsThere was no significant clinical difference between participants with short and long DUP at presentation. Linear regression analyses revealed that longer DUP significantly predicted more severe positive and negative symptoms and poorer social function at 1 year, independent of scores at presentation. Path analyses revealed independent direct relationships between DUP and social function, core negative symptoms and positive symptoms. There was no significant association between DUP and cognition.
ConclusionsLonger DUP predicts poor social function independently of symptoms. The findings underline the importance of taking account of the phenomenological overlap between measures of negative symptoms and social function when investigating the effects of DUP.
Comorbid substance use and age at onset of schizophrenia

Background
Substance use may be a risk factor for the onset of schizophrenia.
AimsTo examine the association between substance use and age at onset in a UK, inner-city sample of people with recent-onset schizophrenia.
MethodThe study sample consisted of 152 people recruited to the West London First-Episode Schizophrenia Study. Self-reported data on drug and alcohol use, as well as information on age at onset of psychosis, were collected. Mental state, cognition (IQ, memory and executive function) and social function were also assessed.
ResultsIn total, 60% of the participants were smokers, 27% reported a history of problems with alcohol use, 35% reported current substance use (not including alcohol), and 68% reported lifetime substance use (cannabis and psychostimulants were most commonly used). Cannabis use and gender had independent effects on age at onset of psychosis, after adjusting for alcohol misuse and use of other drugs.
ConclusionsThe strong association between self-reported cannabis use and earlier onset of psychosis provides further evidence that schizophrenia may be precipitated by cannabis use and/or that the early onset of symptoms is a risk factor for cannabis use.

Background
Studies of chronic schizophrenia suggest that there are subgroups with different profiles of cognitive impairment.
AimsTo determine whether such heterogeneity is present at illness onset and any relationship to clinical variables.
MethodNinety-three community patients with first-episode schizophrenia and 50 healthy volunteers were assessed for premorbid (Revised National Adult Reading Test) and current IQ, memory and executive function.
ResultsHalf of those with schizophrenia had preserved IQ in the normal range but there was evidence of a specific impairment in spatial working memory even in those with high/average IQ; 37 out of 93 (40%) had generalised cognitive decline. Those with low premorbid IQ were significantly younger at illness onset. For the entire group, age at onset correlated positively with premorbid but not current IQ.
ConclusionsAt illness onset, cognitive heterogeneity is present in people with schizophrenia, with a high proportion having undergone general cognitive decline. However, working memory impairment may be a common feature. Lower premorbid IQ is a risk factor for an earlier onset.

Studies in schizophrenia suggest that a longer initial period of untreated illness is associated with a poorer clinical outcome.
AimsTo determine whether, in first-episode schizophrenia, a longer duration of untreated psychosis (DUP) or of untreated illness (DUI) (DUP plus any prodrome) is associated with clinical variables that could mediate a poor prognosis.
MethodClinical, social, neuropsychological and oculomotor function data on 53 patients with first-episode schizophrenia were related to the DUP and DUI.
ResultsComparing short and long DUP groups split around the median showed no statistically significant differences (except age); patients in the latter group tended to perform worse on an executive attentional set-shifting task, and were more likely to be unemployed, and living alone or homeless.
ConclusionsThere was little evidence of any association between either DUP or DUI and progressive deterioration in the schizophrenic illness or the development of resistance to initial drug treatment. Social variables that augur a poor prognosis may be associated with delayed presentation of schizophrenia to psychiatric services.

Recently, there has been considerable interest in advanced materials and processing techniques for practical applications. V2O5 xerogels have generated much attention because they are layered materials that undergo reversible redox intercalation with lithium. The sol-gel process has been used to intercalate V2O5 xerogels with the polymer electrolyte, oxymethylene linked poly(ethylene oxide) - lithium triflate [(a-PEO)n(LiCF3SO3)]. The resulting nanocomposite is a mixed ionic-electronic conductor in which the ionic charge carriers in the polymer electrolyte are in intimate contact with the electronic charge carriers in the V205 xerogel. Variable-temperature electronic conductivity and thermoelectric power measurements have been performed to examine the charge transport properties.

Part of the female reproductive system of Glossina morsitans morsitans Westwood (Diptera: Glossinidae) consists of two spermathecal glands each with a duct and receptacle. The receptacle is composed of secretory cells with the normal secretory apparatus (secretory vesicles, rough endoplasmic reticulum, mitochondria and microvilli), as well as bundles of microtubules. The extensive rough endoplasmic reticulum is indicative of the production of proteinaceous substances. On the basis of histochemical tests, the material in the epithelium and lumen of the spermathecal receptacle appears to be a mixture of carbohydrate and protein. These materials accumulate in cavities within the epithelial secretory cells, and are then transferred to the main lumen of the receptacle in which sperm are also stored.
