COVID-19 as an Accelerator of the Implementation of Emergency Medical Teams Initiative in the AFRO Region

Objective:
This study describes the progress that the World Health Organization (WHO) African (AFRO) region has made in establishing National Emergency Medical Teams (N-EMTs), the coordination mechanisms of the EMTs, and the regional training centers.
Methods:It used a retrospective descriptive analysis of the formulation and implementation of the EMTs Initiative from an insider perspective. The analysis is based on the review of available documents such as EMTs mission reports, assessments, surveys, EMT monthly bulletins, and meeting minutes in addition to key informant interviews (n = 5) with the EMT teams’ members to validate the findings and share field experiences.
Results:The emergence of coronavirus disease 2019 (COVID-19) acted as an accelerator for the implementation of the EMT initiative in the AFRO region. A total of 18 EMT deployments were carried out in 16 countries in the AFRO region through the WHO EMT-network during COVID-19, providing support to countries in managing severe and critical COVID-19 cases.
Conclusions:A Regional Training Center for N-EMTs is being set up in Addis Ababa to train the N-EMTs and strengthen local capacity of health personnel in the region. Challenges include unavailability of mentors to support countries in implementing N-EMTs and the Regional Simulation Training Center, poor funding, and coordination in the rolling out of the N-EMTs.

Research into second language (L2) reading is an exponentially growing field. Yet, it still has a relatively short supply of comparable, ecologically valid data from readers representing a variety of first languages (L1). This article addresses this need by presenting a new data resource called MECO L2 (Multilingual Eye Movements Corpus), a rich behavioral eye-tracking record of text reading in English as an L2 among 543 university student speakers of 12 different L1s. MECO L2 includes a test battery of component skills of reading and allows for a comparison of the participants’ reading performance in their L1 and L2. This data resource enables innovative large-scale cross-sample analyses of predictors of L2 reading fluency and comprehension. We first introduce the design and structure of the MECO L2 resource, along with reliability estimates and basic descriptive analyses. Then, we illustrate the utility of MECO L2 by quantifying contributions of four sources to variability in L2 reading proficiency proposed in prior literature: reading fluency and comprehension in L1, proficiency in L2 component skills of reading, extralinguistic factors, and the L1 of the readers. Major findings included (a) a fundamental contrast between the determinants of L2 reading fluency versus comprehension accuracy, and (b) high within-participant consistency in the real-time strategy of reading in L1 and L2. We conclude by reviewing the implications of these findings to theories of L2 acquisition and outline further directions in which the new data resource may support L2 reading research.
First report of Hirnantian (Upper Ordovician) high-latitude peri-gondwanan macrofossil assemblages from Portugal

The first report of the Hirnantia high-latitude peri-Gondwanan Fauna from Portugal (Upper Ordovician, Ribeira do Braçal Formation) is presented here. The described macroassemblages are fairly diverse containing fossils of brachiopods, trilobites, echinoderms, machaeridians, and ostracodes. Among the brachiopods, the most abundant is Mirorthis mira. Plectothyrella cf. P. libyca, Paromalomena cf. P. polonica, Plectoglossa? sp., and a small indeterminate discinoid are also present. The trilobites are represented by abundant sclerites of Mucronaspis cf. M. mucronata, and an isolated cranidium and pygidium assigned to Flexicalymene. The occurrence of Mucronaspis and Flexicalymene represents the first record of these genera in Portugal. Echinoderms are dominant in the basal bed of the formation; the columnal plates tentatively ascribed to morphogenus Pentagonocyclicus are the most abundant, followed by the echinosphaeritids. Abundant disarticulated machaeridian plates, of the genus Plumulites, associated with trilobites are present in one of the localities. Ostracodes were found in one single locality and have been assigned tentatively to the genus Herrigia. Ramose and massive bryozoans also occur in the assemblage. This new macrofossil assemblage supports the assignment of an Hirnantian age for the Ribeira do Braçal Formation. Most of the brachiopod species and the dalmanitid Mucronaspis are commonly present in Hirnantian deposits globally, but the presence in the assemblage of a brachiopod close to Plectothyrella libyca, a cold-water species, previously reported only from the Hirnantian of Libya and Morocco, is noticeable. This strengthens the case for a high latitudinal setting of the present-day territory of the Portuguese Central Iberian Zone during the Late Ordovician.

Objective
In 2013, the Philippines was struck by typhoon Haiyan, which damaged local hospitals and disrupted health care. The Belgian First Aid and Support Team erected a field hospital and water purification unit in Palo. This study aims to describe the diagnoses encountered and treatment provided.
MethodsIn this cross-sectional study, medical records of 1267 field hospital patients were reviewed for gender, age, complaints, diagnoses, and management and referral information.
ResultsAlmost 28% of the patients suffered from injury, but most presented with nonsurgical diseases (64%), particularly of respiratory (31%), dermatological (11%), and digestive (8%) origin. Only 53% presented with disaster-related pathology, and 59% showed signs of infection. Patients needed wound care (47%), pain relief (33%), or antibiotics (29%); 9% needed procedures, 8% needed fluid therapy, and 5% needed psychological support. Children under 5 years of age were more at risk for infections (OR, 18.8; CI, 10.6-33.3) and injuries (OR, 10.3; CI, 6.3-16.8). Males were more prone to injuries than females (OR, 2.1; CI, 1.6-2.6).
ConclusionsOne week after the acute phase of a typhoon, respiratory, dermatological, and digestive problems emerge to the prejudice of trauma. Only 53% of patients presented with disaster-related conditions. Young children are more at risk for injury and infectious diseases. These trends should be anticipated when composing Emergency Medical Teams and medical resources to be sent to disaster sites. (Disaster Med Public Health Preparedness. 2019;13:265-278)
Reduction of Triangular Defects on 100mm 4° off-axis 4H-SiC using a Chloride Based CVD process

Homoepitaxial layers were grown with very low surface roughness on 4", 4˚ off-axis substrates, but a new kind of large obtuse angled triangular defect that spanned 1000-2000 μm was observed. Process changes resulted in reduction of the size and concentration of these triangular defects from 3.5cm-2 to 0.13cm-2. Both large and small triangular defects were found to have a similar core structure. No degradation in the epitaxial morphology or quality was seen due to the process change. JBS diodes fabricated on wafers with large triangular defects had much higher leakage when the triangular defects were present in the active area of the diodes.

During the solidification of solder joints composed of near-eutectic Sn–Ag–Cu alloys, the Sn phase grows rapidly with a dendritic growth morphology, characterized by copious branching. Notwithstanding the complicated Sn growth topology, the Sn phase demonstrates single crystallographic orientations over large regions. Typical solder ball grid array joints, 900 μm in diameter, are composed of 1 to perhaps 12 different Sn crystallographic domains (Sn grains). When such solder joints are submitted to cyclic thermomechanical strains, the solder joint fatigue process is characterized by the recrystallization of the Sn phase in the higher deformation regions with the production of a much smaller grain size. Grain boundary sliding and diffusion in these recrystallized regions then leads to extensive grain boundary damage and results in fatigue crack initiation and growth along the recrystallized Sn grain boundaries.

Near-ternary eutectic Sn–Ag–Cu alloys are leading candidates for Pb-free solders. These alloys have three solid phases: β–Sn, Ag3Sn, and Cu6Sn5. Starting from the fully liquid state in solidifying near-eutectic Sn–Ag–Cu alloys, the equilibrium eutectic transformation is kinetically inhibited. The Ag3Sn phase nucleates with minimal undercooling, but the β–Sn phase requires a typical undercooling of 15 to 30 °C for nucleation. Because of this disparity in the required undercooling for nucleation, large, platelike Ag3Sn structures can grow rapidly within the liquid phase, before the final solidification of the solder joints. At lower cooling rates, the large Ag3Sn plates can subtend the entire cross section of solder joints and can significantly influence the mechanical deformation behavior of the solder joints under thermomechanical fatigue conditions. In this paper, it is demonstrated that the Ag3Sn plate formation can be inhibited, an important factor in assuring the reliability of solder joints composed of these alloys.
