What can clinicians do to improve outcomes across psychiatric treatments: a conceptual review of non-specific components

Aims
Psychiatric treatments have specific and non-specific components. The latter has been addressed in an extensive literature on the placebo-effect in pharmacology and on common factors in psychotherapy. In the practice of mental health care, pharmacological, psychotherapeutic and social treatments are combined in complex interventions. This paper aims to review non-specific components across diverse psychiatric treatments and consider implications for practice and research.
MethodsWe conducted a non-systematic review of non-specific components across psychiatric treatments, their impact on treatment processes and outcomes, and interventions to improve them.
ResultsThe identified research is heterogeneous, both in design and quality. All non-specific components capture aspects of how clinicians communicate with patients. They are grouped into general verbal communication – focusing on initial contacts, empathy, clarity of communication, and detecting cues about unspoken concerns – non-verbal communication, the framing of treatments and decision-making. The evidence is stronger for the impact of these components on process measures – i.e. therapeutic relationship, treatment satisfaction and adherence than on clinical outcomes – i.e. symptoms and relapse. A small number of trials suggest that brief training courses and simple methods for structuring parts of clinical consultations can improve communication and subsequently clinical outcomes.
ConclusionsMethodologically, rigorous research advancing current understandings of non-specific components may increase effectiveness across different treatments, potentially benefitting large numbers of patients. Brief training for clinicians and structuring clinical communication should be used more widely in practice.

Aims
A core question in the debate about how to organise mental healthcare is whether in- and out-patient treatment should be provided by the same (personal continuity) or different psychiatrists (specialisation). The controversial debate drives costly organisational changes in several European countries, which have gone in opposing directions. The existing evidence is based on small and low-quality studies which tend to favour whatever the new experimental organisation is.
We compared 1-year clinical outcomes of personal continuity and specialisation in routine care in a large scale study across five European countries.
MethodsThis is a 1-year prospective natural experiment conducted in Belgium, England, Germany, Italy and Poland. In all these countries, both personal continuity and specialisation exist in routine care. Eligible patients were admitted for psychiatric in-patient treatment (18 years of age), and clinically diagnosed with a psychotic, mood or anxiety/somatisation disorder.
Outcomes were assessed 1 year after the index admission. The primary outcome was re-hospitalisation and analysed for the full sample and subgroups defined by country, and different socio-demographic and clinical criteria. Secondary outcomes were total number of inpatient days, involuntary re-admissions, adverse events and patients’ social situation. Outcomes were compared through mixed regression models in intention-to-treat analyses. The study is registered (ISRCTN40256812).
ResultsWe consecutively recruited 7302 patients; 6369 (87.2%) were followed-up. No statistically significant differences were found in re-hospitalisation, neither overall (adjusted percentages: 38.9% in personal continuity, 37.1% in specialisation; odds ratio = 1.08; confidence interval 0.94–1.25; p = 0.28) nor for any of the considered subgroups. There were no significant differences in any of the secondary outcomes.
ConclusionsWhether the same or different psychiatrists provide in- and out-patient treatment appears to have no substantial impact on patient outcomes over a 1-year period. Initiatives to improve long-term outcomes of psychiatric patients may focus on aspects other than the organisation of personal continuity v. specialisation.
Partitioning of Microbially Respired CO2 Between Indigenous and Exogenous Carbon Sources During Biochar Degradation Using Radiocarbon and Stable Carbon Isotopes

Pyrolized carbon in biochar can sequester atmospheric CO2 into soil to reduce impacts of anthropogenic CO2 emissions. When estimating the stability of biochar, degradation of biochar carbon, mobility of degradation products, and ingress of carbon from other sources must all be considered. In a previous study we tracked degradation in biochars produced from radiocarbon-free wood and subjected to different physico-chemical treatments over three years in a rainforest soil. Following completion of the field trial, we report here a series of in-vitro incubations of the degraded biochars to determine CO2 efflux rates, 14C concentration and δ13C values in CO2 to quantify the contributions of biochar carbon and other sources of carbon to the CO2 efflux. The 14C concentration in CO2 showed that microbial degradation led to respiration of CO2 sourced from indigenous biochar carbon (≈0.5–1.4 μmoles CO2/g biochar C/day) along with a component of carbon closely associated with the biochars but derived from the local environment. Correlations between 14C concentration, δ13C values and Ca abundance indicated that Ca2+ availability was an important determinant of the loss of biochar carbon.
Recovery practice in community mental health teams: national survey

Background
There is consensus about the importance of ‘recovery’ in mental health services, but the link between recovery orientation of mental health teams and personal recovery of individuals has been underresearched.
AimsTo investigate differences in team leader, clinician and service user perspectives of recovery orientation of community adult mental health teams in England.
MethodIn six English mental health National Health Service (NHS) trusts, randomly chosen community adult mental health teams were surveyed. A random sample of ten patients, one team leader and a convenience sample of five clinicians were surveyed from each team. All respondents rated the recovery orientation of their team using parallel versions of the Recovery Self Assessment (RSA). In addition, service users also rated their own personal recovery using the Questionnaire about Processes of Recovery (QPR).
ResultsTeam leaders (n = 22) rated recovery orientation higher than clinicians (n = 109) or patients (n = 120) (Wald(2) = 7.0, P = 0.03), and both NHS trust and team type influenced RSA ratings. Patient-rated recovery orientation was a predictor of personal recovery (b = 0.58, 95% CI 0.31–0.85, P<0.001). Team leaders and clinicians with experience of mental illness (39%) or supporting a family member or friend with mental illness (76%) did not differ in their RSA ratings from other team leaders or clinicians.
ConclusionsCompared with team leaders, frontline clinicians and service users have less positive views on recovery orientation. Increasing recovery orientation may support personal recovery.
A conceptual framework for improving well-being in people with a diagnosis of psychosis

Background.
Well-being is important for people with severe mental illness, such as psychosis. So far, no clear concept of well-being exists for this client group. A recent systematic review and narrative synthesis developed a static framework of well-being components. The present study aims to validate the static framework and to illuminate the processes by which well-being is experienced by people with psychosis.
Methods.Semi-structured interviews were conducted with 23 service users with psychosis exploring their experience of well-being. Thematic analysis was used to analyse the data employing techniques taken from grounded theory to enhance the rigour of the analysis. Respondent validation was undertaken with 13 of the 23 participants.
Results.Three superordinate categories of well-being were identified: current sense of self; transition to enhanced sense of self and enhanced sense of self. In the dynamic process of improving well-being the current sense of self undergoes a transition to an enhanced sense of self. The four factors influencing the transition are consistent with the static framework of well-being, hence validating the static framework. In addition, we identified three determinants of current sense of self and seven indicators of enhanced sense of self, which represent the achievement of improved well-being.
Conclusions.This study provides an empirically defensible framework for understanding well-being in terms of determinants, influences and indicators. The influences are targets for interventions to improve well-being, and the indicators are outcome domains to assess the effectiveness of services in supporting well-being.
International differences in understanding recovery: systematic review

Aims.
Mental health policy internationally varies in its support for recovery. The aims of this study were to validate an existing conceptual framework and then characterise by country the distribution, scientific foundations and emphasis in published recovery conceptualisations.
Methods.Update and modification of a previously published systematic review and narrative synthesis of recovery conceptualisations published in English.
Results.A total of 7431 studies were identified and 429 full papers reviewed, from which 105 conceptualisations in 115 papers were included and quality assessed using established rating scales. Recovery conceptualisations were identified from 11 individual countries, with 95 (91%) published in English-speaking countries, primarily the USA (47%) and the UK (25%). The scientific foundation was primarily qualitative research (53%), non-systematic literature reviews (24%) and position papers (12%). The conceptual framework was validated with the 18 new papers. Across the different countries, there was a relatively similar distribution of codings for each of five key recovery processes.
Conclusions.Recovery as currently conceptualised in English-language publications is primarily based on qualitative studies and position papers from English-speaking countries. The conceptual framework was valid, but the development of recovery conceptualisations using a broader range of research designs within other cultures and non-majority populations is a research priority.

Corynetoxins, the toxic glycolipids produced by Corynebacterium rathayi colonizing bacterial galls induced by the seed gall nematode Anguina agrostis in annual ryegrass (Loliurn rigidum), did not affect embryogenesis or larval development of A. agrostis although they did inhibit the rate of egg hatching. Corynetoxins were not toxic to several species of nematode which are parasites of roots nor to various stages of the free-living nematode Caenorhabditis elegana and they did not appear to influence the numbers of C. rathayi that became attached to the surface of the cuticle of infective larvae of A. agrostis.
The relationship of soil mineral NO3-N with stem NO3-N concentration, and of fertilizer-N with the amount of nitrogen taken up by winter wheat, in experiments testing nitrogen fertilizer in combination with aphicide and fungicides, from 1980 to 1982

Experiments on winter wheat were made from 1980 to 1982 to test fungicide and aphicide sprays in factorial combination with four amounts of nitrogen fertilizer, applied in either one or two dressings in spring. The wheat was grown on three farms with contrasting calcareous clay soils from three soil series; each year it followed a 2-year break on one farm, a cereal rotation on the second and continuous wheat on the third. Soils were sampled to a depth of 0·9 m at seedling emergence in autumn, and again in February and April, to determine the NO3-N and NH4-N in each 0·3 m horizon. Crops were sampled for growth analysis at monthly intervals from March onwards and analysed for nitrogen content. Measurements of stem sap NO3-N concentration were also made at 2-weekly intervals from February or March to late June.
Measurements of soil mineral N were used to calculate the fertilizer nitrogen dressings used in the experiments. The concentration of NO3-N in the stem sap was related to NO3-N in soil; concentiations remained high until most of the soil NO3-N had been removed by the crop. The time at which stem sap NO3-N concentration declined therefore acted as an index of soil N supply, and the data showed that fertilizer-N was needed when the NO3-N concentration fell below a 200 μg/ml threshold. Yields benefited from N applied in February or March only when stem sap NO3-N concentration fell below the threshold at this time.
Apparent fertilizer nitrogen efficiency exceeded 70 % where yields were very large, but ranged between 53 and 64% where yields were smaller because either soil physical problems or disease restraints were present.
A severe attack by take-all (Gaeumannomyces cerealis) caused premature senescence at one centre in 1980; this apparently prevented previously assimilated nitrogen from moving into the grain.

Soil NO3-N and NH4-N were measured to 90 cm depth in autumn and again in spring, under several sets of winter wheat experiments, on contrasting sites. Crop samples were taken throughout the growing season, both before and after the fertilizer N was applied, to measure N uptake. The amount of NO3-N in soil at the outset of growth in autumn was related to the uptake of N by wheat not given any fertilizer N until April.
The effect of sowing date (September v. October) on both crop and soil N was compared, as also was the effect of soil type (retentive of NO3-N v. readily leached) and previous crop (potatoes v. oats and wheat v. beans).
The amounts of NO3-N in the soils in autumn related well with previous crop and declined gradually during winter on the heavier soils, but rapidly on the sandy soil, in the latter case as a consequence of leaching. On the heavier soils, where little leaching occurred, the decline in soil NO3-N related well with the amount of N taken up by September-sown wheat during autumn and winter, but not with that taken up by October-sown wheat, where NO3-N accumulated in the soil during winter, because uptake was so small. Hence delayed sowing enhanced the likelihood of losses of NO3-N by leaching or by denitrification. On the sandy soil at Woburn, whilst the September-sown wheat removed more N than the October-sown, losses of NO3-N by leaching were severe, so that late winter growth was restricted by shortage of N in soil, and the amount of N taken up was far smaller than at Rothamsted.
The soil measurements distinguished between the NO3-N residues remaining after beans or wheat in the same field and between residues after oats or potatoes on soils of the same soil series, but in different fields on the same farm.
The amount of NO3-N in soil and the N taken up by wheat in February-March were together used to adjust the amount of fertilizer N applied in April, using a balance sheet approach to meet a specific yield objective. Some of the N uptake data from these experiments are presented. This should aid the calculation of N requirement during specific growth periods and thus help improve the prediction of fertilizer N dressings in spring.

INTEGRAL is the first gamma-ray astronomy mission with a sufficient sensitivity and angular resolution combination appropriate to the detection and identification of considerable numbers of gamma-ray emitting sources. The large field of view enables INTEGRAL to survey the Galactic Plane on a regular ($\sim$weekly) basis as part of the core programme. The first source catalogue, based on the 1st year of core programme data ($\sim$5 Msec) has been completed and published (Bird et al. 2004). It contained 123 $\gamma$-ray sources (24 HMXB, 54 LMXB, 28 “unknown”, plus 17 others) - sufficient numbers for a reasonable statistical analysis of their global properties. The detection of previously unknown $\gamma$-ray emitting sources generally exhibiting high intrinsic absorption, which do not have readily identifiable counterparts at other wavelengths, is intriguing. The substantial fraction of unclassified $\gamma$-ray sources suggests they must constitute a significant family of objects. In this paper we review the global characteristics of the known galactic sources as well as the unclassified objects.
Radiocarbon Dating of Wood Using Different Pretreatment Procedures: Application to the Chronology of Rotoehu Ash, New Zealand

We compare radiocarbon accelerator mass spectrometry (AMS) ages of wood samples subjected to a conventional acid-base-acid pretreatment with stepped combustion (ABA-SC) with results from the same samples subjected to an acid-base-wet oxidation pretreatment with stepped combustion (ABOX-SC) and cellulose extraction with stepped combustion (CE-SC). The ABOX-SC procedure has been shown previously to lead to lower backgrounds for old charcoal samples. Analyses of relatively uncontaminated “14C-dead” samples of wood suggest that backgrounds of 0.11 ± 0.04 pMC are obtainable for both the ABOX-SC and ABA-SC procedures. Where wood is significantly contaminated the ABOX-SC technique provides significantly better decontamination than either the ABA-SC technique or cellulose extraction alone, although CE-SC can produce comparably low backgrounds to the ABOX-SC procedure.
We also report the application of the ABOX-SC, ABA-SC and CE-SC procedures to wood samples associated with the chronologically controversial Rotoehu Ash eruption, New Zealand. New 14C-AMS dates from wood sampled from below the Rotoehu Ash span an age range of 43–50 ka BP consistent with recently presented OSL dates of 42–44 ka obtained for palaeosols beneath the ash.

A series of designed experiments was performed in an attempt to evaluate and quantify the historically “anecdotal” benefits of conformal cooling for injection molding tooling. The study considered different generic part geometries, gating schemes, mold materials, plastic resins and cooling approaches. This paper provides an overview of the mold design approach, cooling simulation, tool fabrication via the 3DP™ process, as well as part molding and inspection results.
