Evaluation of procedures for hygienic hand-disinfection: controlled parallel experiments on the Vienna test model

Controlled parallel experiments were performed on the Vienna test model for the evaluation of procedures for hygienic hand-disinfection in three laboratories (Vienna, Mainz, Birmingham). The degerming activity of four procedures, each taking 1 min, was assessed repeatedly and compared with that of a standard disinfection procedure (ST) using isopropanol 60 % (v/v). The mean log reductions (mean log RF) for each procedure were as follows: n−propanol 50% (v/v) 4·85 and 5·14 in Vienna (V) and Mainz (M) respectively, ethanol 70 % (v/v) + chlorhexidinegluconate 0·5% (w/v), 4·01 (V), 3·76 (M) and 4·00 in Birmingham (B). Washing procedures were less effective, mean log RF 's of 3·19 (V), 3·49 (M) and 3·04 (B) were obtained with povidone-iodine soap, and 2·91 (V), 3·37 (M) and 3·27 (B) with a liquid phenolic soap. Analysis of variance on the data from Vienna and Mainz revealed significant differences of means not only between procedures (‘preparations’) but also on repeat testing. To compensate for the influence of variables such as test subjects, laboratory and day, the Vienna test model provides a method of standardization by testing a ST in parallel with the test procedure (P).
Standardization of the results was obtained by pair-wise substraction, log . Analysis of variance on the resulting values demonstrated that comparability of the results between laboratories and on repeat testing was achieved. The relative variation of the measurements within the laboratories ranged from 0·9 to 4·2%. As assessed by power-analysis, a disinfection procedure will be detected as significantly (P= 0·1) inferior to the standard processes in 95 of 100 experiments if it produces a mean log RF that is at least 0·55–0·65 log units smaller than that of the standard.
Histomorphology of rabbit thigh muscles: establishment of standard control values

The thigh muscles of New Zealand White (NZW) rabbits are frequently used in experimental surgery, particularly for evaluation after reinnervation or ischaemia. Although histomorphometric analyses are regularly performed, morphological data for untreated thigh muscles in previously unoperated animals are not available. Specimens from the rectus femoris (RF), vastus medialis (VM) and adductor magnus (AM) muscles from both thighs were harvested in 7 untreated rabbits and were processed for histomorphometric evaluation. The right RF and VM were harvested in a further 5 rabbit hindlimbs after experimental denervation and reinnervation of the contralateral RF and subsequently processed for histomorphometric analysis. Muscle fibre type distribution, diameter and connective tissue content were evaluated on serial transverse cryosections reacted for ATPase and NADH tetrazolium reductase activity and statistical analysis was performed. In all untreated animals RF revealed the highest proportion of type I muscle fibres (right: 8.4±4%, left: 11.4±4.9%), whereas VM showed the highest percentage of IIa fibres (right: 31.9±5.5%, left: 28.3±7.8%) and AM the highest proportion of IIb/d fibres (right: 80.5±8.6%, left: 84.4±6.3%). Fibre type distribution and diameter in rabbits after contralateral experimental operations revealed a statistically significant difference from the data obtained in bilaterally untreated animals. Knowledge of the morphology of untreated muscles is fundamental to the understanding of changes induced by intervention to the ipsi and/or contralateral thigh muscles.

In-situ analysis of low temperature Plasma Enhanced Chemical Vapor Deposition (PECVD) SiO2 films deposited on HgCdTe, Silicon, and Aluminum substrates was performed by double modulation Fourier Transform Infrared Reflection Absorption spectroscopy (FT-IRRAS). The sensitivity and selectivity of this technique are sufficient for an in-situ assessment of the film quality and reaction conditions at any stage of film growth. An oblique angle of incidence of ca. 557deg; was chosen to yield maximum sensitivity for the 1260 cm−1 LO mode of SiO2 on Si. The peak frequency and shape of the LO mode absorption band varied with the quality of the SiO2 films. This diagnostic technique can be applied readily to in-situ analysis of dielectric thin films formed under a variety of reaction conditions as long as the gaseous ambient is partially transmissive to the IR radiation.

Parasuicide is not a single syndrome. Subtypes at present recognized are based largely on clinically derived stereotypes. When considering a series of patients, the clinician is unable to handle more than a few attributes at a time. This paper describes the application of three very different clustering algorithms to a material of 350 treated parasuicide patients. Mathematically, three types emerge. Clinically, two of these are interpretable and make sense. The types established are: I (n = 107) a group not characterized by any of the variables we examined; this group is a puzzle, mainly because the reasons for the parasuicidal act are not clear. II (n = 132) a depressed, alienated group with high life-endangerment. III (n = 111) a group whose act was highly operant: they felt alienated and were angry with others. These groups did not differ significantly on demographic variables. The usefulness of this typology, particularly for management, after-care and prevention, has now to be assessed.
