Summary
We have previously reported in several publications the existence of a nuclease that is present in the luminal fluid of the epididymis and in higher concentrations in the lumina fluid of the vas deferens in the mouse. This luminal nuclease has the ability to digest the sperm DNA into loop-sized fragments but is restricted to digesting DNA that is not bound by protamines. In this chapter, we describe new data that show that the luminal nuclease is active in SDS for up to 30 min, complicating the analysis of sperm DNA damage that must be extracted by SDS for electrophoretic analysis on gels. High concentrations of EDTA can protect the DNA from the luminal nuclease if added before SDS is applied. This new data provides an update on our toroid loop model for sperm chromatin structure. We discuss the implications of this updated model for the study of sperm chromatin structure and for clinical assays for sperm DNA damage.

Objective
Otitis media (OM) is a common reason for children to be prescribed antibiotics and undergo surgery but a thorough understanding of disease mechanisms is lacking. We evaluate the evidence of a dysregulated immune response in the pathogenesis of OM.
MethodsA comprehensive systematic review of the literature using search terms [otitis media OR glue ear OR AOM OR OME] OR [middle ear AND (infection OR inflammation)] which were run through Medline and Embase via Ovid, including both human and animal studies. In total, 82 955 studies underwent automated filtering followed by manual screening. One hundred studies were included in the review.
ResultsMost studies were based on in vitro or animal work. Abnormalities in pathogen detection pathways, such as Toll-like receptors, have confirmed roles in OM. The aetiology of OM, its chronic subgroups (chronic OM, persistent OM with effusion) and recurrent acute OM is complex; however, inflammatory signalling mechanisms are frequently implicated. Host epithelium likely plays a crucial role, but the characterisation of human middle ear tissue lags behind that of other anatomical subsites.
ConclusionsTranslational research for OM presently falls far behind its clinical importance. This has likely hindered the development of new diagnostic and treatment modalities. Further work is urgently required; particularly to disentangle the respective immune pathologies in the clinically observed phenotypes and thereby work towards more personalised treatments.

Summary
Introduction: the malleable sperm genome
Over the last decade, our assumptions and ideas about the stability of the mammalian sperm genome have gone through a gentle and progressive change. Historically, we viewed the sperm genome as tightly packaged into a virtual crystalline state with very little biochemical accessibility. We now understand that in many cases of decreased male fertility sperm deoxyribonucleic acid (DNA) structure loses much of this inaccessibility and that the DNA is susceptible to damage. More recently, evidence has emerged from many different laboratories that spermatozoa contain nucleases that are capable of digesting their compact DNA. These data suggest that the sperm genome may be less stable than we thought and might even be flexible enough to regulate its own stability in a manner similar to somatic cells. In this review, we will explore the current status of the stability of the mammalian sperm genome in the context of what is known about sperm chromatin structure.
Sperm chromatin structure and fertility
Before discussing DNA damage as it relates to sperm chromatin structure we will briefly review those aspects of sperm chromatin, both known and suspected, that pertain to this discussion. During spermiogenesis the haploid sperm chromatin undergoes one of the most significant changes known in biology. All of the histones are replaced first by transition proteins, then by protamines (Meistrich et al., 2003).

We have recently presented evidence that the structural integrity of the mouse sperm nuclear matrix may be necessary for the proper unpackaging of sperm DNA for participation in embryogenesis. It is likely that the sperm nuclear matrix contributes to the organisation of the sperm DNA and its disturbance can seriously damage the paternal genome or its expression. In this work, we confirm our previous data and further suggest that even very subtle changes in the sperm nuclear structure may have a significant impact on embryo development. As reported previously, dithiothreitol (DTT) in the presence of an ionic detergent, ATAB, destabilised the nuclear matrix as measured by the halo assay, and oocytes injected with these nuclei failed to develop. We also discovered that omitting the protease inhibitor PMSF from the buffers used to extract spermatozoa prevented sperm injected into oocytes from participating in development. The organisation of DNA into loop domains by the nuclear matrix in these nuclei appeared normal, as measured by the halo assay. Oocytes injected with sperm nuclei that had been washed with ATAB in the presence of phenylmethylsulphonyl fluoride (PMSF) but in the absence of DTT resulted in live births. Neither DTT treatment nor the absence of PMSF would be expected to disrupt the integrity of the paternal DNA. The data therefore suggest that even very subtle alterations in the structural proteins of the nucleus are enough to deprive sperm DNA of the ability to contribute to embryonic development.

The average somatic nucleus is only 10 microns in diameter, yet contains 2 meters of DNA coiled within its volume. This compaction must be accomplished in such a way so that the DNA remains accessible to the machinery of transcription, replication and repair. Sperm nuclei also have compacted DNA; however, structurally and functionally the arrangement of DNA in sperm is vastly different from that of somatic DNA. Somatic DNA is initially compacted by coiling around histone octamers, while sperm DNA is bound to elongate, highly basic proteins called protamines. Protamines bind the major groove of the double helix and allow adjacent DNA strands to come in close contact by neutralizing the negative charge of the sugar-phosphate backbone. Figure 1 illustrates the two most widely accepted models of DNA compaction in both somatic and sperm nuclei.
The arrangement of DNA within sperm nuclei is difficult to study because it is so highly compressed.
Can alcohol retain the reproductive and genetic potential of sperm nuclei? Chromosome analysis of mouse spermatozoa stored in alcohol

Alcohol is known to preserve genomic DNA and the primary structure of sperm protamines. To determine whether alcohol can retain the genetic and reproductive potential of mammalian sperm nuclei, mature mouse spermatozoa were stored in 70% ethanol or propanol for up to 2 months before injection into oocytes. Live offspring were obtained after injection of spermatozoa stored in 70% ethanol for 1 day at -20 °C. About 20% of the spermatozoa stored under this condition had normal chromosomes. The remaining 80% of spermatozoa and all the spermatozoa stored in 70% ethanol for 2 months had structurally aberrant chromosomes, and none could support the development of normal embryos. High concentrations of alcohol do not alter the primary structure of either DNA or small-molecular-weight protamines. However, alcohol may modify protamine—protamine or protamine—DNA interactions in a manner that results in the induction of DNA strand breaks during sperm chromatin decondensation within the oocyte. The limited success in obtaining normal offspring with ethanol-stored spermatozoa is encouraging. It may be possible to overcome these problems and develop a simple method for preserving mammalian spermatozoa without freezing.

The average sperm nucleus is only 8 micrometers long, but contains 1 meter of DNA. We have previously described how the sperm cell is able to package this DNA into the small volume of the sperm head. The only DNA structure that exists in both somatic cells and sperm nuclei is the organization of DNA loop domains that are attached at their bases to the nuclear skeleton, the nuclear matrix. We compared the structure of one gene in sperm nuclei to that of spermatogenic and adult cells.
Individual loop domains of the 5S rRNA gene cluster were visualized by fluorescent in situ hybridization (FISH). Nuclei were isolated from hamster spermatozoa, spermatogonia, pachytene spermatocytes, round spermatids, and adult liver cells. They were then extracted to remove the protamines or histones, fixed, hybridized to a biotinylated 5S rDNA probe, then viewed by epifluorescence microscopy. The 5S rDNA in liver nuclei was organized into a single large loop domain (Fig. 1),

Cholestyramine, the first bile acid sequestrant to be marketed, has been in use for over 20 years. Despite its low potency, requiring 16-24 g of polymer to achieve 20% LDL cholesterol reduction in hypercholesterolemic individuals, only one other sequestrant, colestipol, has come to market in the ensuing period. GelTex Pharmaceuticals has been involved for over six years in the discovery and development of new, more potent polymeric sequestrants. Two binding mechanisms are presented — one that operates via an aggregate binding structure and one that is effective via a defined site binding structure. These two binding mechanisms are compared and contrasted through bile acid binding isotherms. The best of these new sequestrants bind bile acids through a combination of hydrophobicity and ion exchange. Optimization and balancing of each of these interactions led us to more potent materials. The first of these, colesevelam hydrochloride is expected to be three to four times more potent than cholestyramine. A third generation product is still in research at GelTex. With another twofold increase in potency possible, single tablet therapy may become a reality.
