Many novel therapeutic options for depression exist that are either not mentioned in clinical guidelines or recommended only for use in highly specialist services. The challenge faced by clinicians is when it might be appropriate to consider such ‘non-standard’ interventions. This analysis proposes a framework to aid this decision.
Declaration of interestIn the past 3 years R.H.M.W. has received support for research, expenses to attend conferences and fees for lecturing and consultancy work (including attending advisory boards) from various pharmaceutical companies including Astra Zeneca, Cyberonics, Eli Lilly, Janssen, LivaNova, Lundbeck, MyTomorrows, Otsuka, Pfizer, Roche, Servier, SPIMACO and Sunovion. D.M.B.C. has received fees from LivaNova for attending an advisory board. In the past 3 years A.J.C. has received fees for lecturing from Astra Zeneca and Lundbeck; fees for consulting from LivaNova, Janssen and Allergan; and research grant support from Lundbeck.
In the past 3 years A.C. has received fees for lecturing from pharmaceutical companies namely Lundbeck and Sunovion. M.S. is an associate at Blueriver Consulting providing intelligence to NHS organisations, pharmaceutical and devices companies. He has received honoraria for presentations and advisory boards with Lundbeck, Eli Lilly, URGO, AstraZeneca, Phillips and Sanofi and holds shares in Johnson and Johnson. In the past 3 years P.R.A.S. has received support for research, expenses to attend conferences and fees for lecturing and consultancy work (including attending an advisory board) from life sciences companies including Corcept Therapeutics, Indivior and LivaNova. In the past 3 years P.S.T. has received consultancy fees as an advisory board member from the following companies: Galen Limited, Sunovion Pharmaceuticals Europe Ltd, myTomorrows and LivaNova. In his Farewell Address, George Washington said: “'Tis substantially true, that virtue or morality is a necessary spring of popular government. The rule indeed extends with more or less force to every species of free Government. Who that is a sincere friend to it, can look with indifference upon attempts to shake the foundation of the fabric? Promote then as an object of primary importance, Institutions for the general diffusion of knowledge. In proportion as the structure of government gives force to public opinion, it is essential that public opinion should be enlightened.” With these words in mind, I sought to examine West German institutions for the general diffusion of knowledge, specifically the schools. I tried to find out whether those institutions promote, or even speak of, virtue or morality, and whether they aim at the development of enlightened public opinion. I defined enlightened public opinion as those opinions which support free, popular government. Enlightened human beings are those “who recognize other human beings for what they are, namely, persons with rights which they must respect, in order to have their own rights recognized and respected.”

For years there was no focus within the U.S. federal government for alternatives to animal toxicity testing. Questions coming to regulatory agencies fell upon individuals to address in the best way they could. Given this void, the ad hoc Interagency Regulatory Alternatives Group was founded by staff in a number of federal agencies in the late 1980s to coalesce efforts in the field. The group sponsored two international workshops on eye irritation, the first making proposals for change in the current test method in rabbits, the second reviewing available data on in vitro alternatives. The result has been that the Organization for Economic Cooperation and Development (OECD) is considering revision of the in vivo eye irritation test guideline to incorporate a number of the workshop deliberations. However, movement of the in vitro eye irritation alternatives has been disappointing; attempts to determine their practical testing significance have thus far been unrewarding.

“Democratic caesarism,” whether by military caudillo or doctor en filosofía, has discouraged administrative efficiency and contributed to political disorganization in Latin America. Concentration of executive authority without responsibility has given free rein to the worst administrative practices of poor presidents without demanding the best from competent chief executives; and, with minority groups in congress dominated by a rubber-stamp majority—the entire legislature gasping in the shade of strong executive government—active groups have turned to revolution as an outlet for political and administrative expression. Poor organization of power and authority constitutes one of the most important problems of government in the Latin American countries, and a dovetailing of a series of approaches probably is necessary for a complete solution—better preparation of leaders, elimination of caudillismo, reform of codes and laws to conform with the spirit of the democratic constitutions, reorganization of parties along doctrinal lines rather than those of personalismo, and development of public opinion and participation in politics by the masses through education and increasing the standard of living.
