Dimensionality of the Swahili version of the General Health Questionnaire (GHQ-12) in a Kenyan population: A confirmatory factor analysis

Abstract

The current study evaluated the Kiswahili version of General Health Questionnaire (GHQ-12) in a Kenyan context comprising of women exposed to gender-based violence. Participants were randomly drawn from community sampling using household screening methods in peri-urban areas in Nairobi. A total of 1,394 participants with varying levels of literacy (years of education: mean [M] = 9.42; standard deviation [SD] = 3.73) and aged between 18 and 89 years were recruited for the study. The observed factor structure of the GHQ-12 was evaluated using six most tested models querying the dimensionality of the instrument insofar as the impacts of positive and negative wording effects in driving multidimensionality. Results from the confirmatory factor analysis supported a bifactor model, consisting of a general distress factor and two separate factors representing common variance due to the positive and negative wording of items. Overall, the findings support the use of the Kiswahili version of the GHQ-12 as a unidimensional construct with method-specific variance owing to wording effects. Importantly, GHQ-12 responses from a sample of Kenyan women with relatively low levels of literacy are congruent with the factor structure observed in other cross-cultural settings in low- and-middle-income countries.

Many thanks for submitting this paper to GMH. As you will see, your paper was reviewed by two reviewers, who both noticed significant strengths and also expressed some concerns.

They also mentioned some overlapping points, including a more thorough discussion of the previous literature validating the GHQ-12 in the introduction. I recommend to address all individual concerns raised by each reviewer in case you decide to revise and resubmit your paper to GMH.

Also, the paper would benefit from adding some discussion of the use of these instruments in measuring concepts such as general distress among culturally diverse populations, such as your sample of Kenyan females.

Comments: The paper presents novel psychometric data on a Kiswahili version of the GHQ-12 and demonstrates that a general factor accounts for the variance in scores from an urban Kenyan population to a greater extent than method-specific factors. The findings, particularly omega statistics derived from the bifactor model, offer very useful data and support the conclusion that the GHQ-12 scores can be interpreted at the level of a single construct. This is a useful contribution to the psychometric literature in Kenya and the use of the GHQ-12. My understanding of the method used here, Structural Equation Modeling, is that it affords a great degree of freedom to researchers to parameterize models in all kinds of ways. These types of analyses therefore need to be presented with clear theoretical direction and a keen awareness of the limitations and bias possible in these models in this particular way. This study needs to establish both of these with a deeper engagement with the substance of the literature, clear hypotheses and are carefully operationalized, and careful description of what has and has not been demonstrated.

Comments: One stated aim relates to describing the factor structure of a Kiswahili version of this instrument. This aim should be further motivated in the introduction by discussing prior work on translations of this instrument. This should explore remaining open questions and highlight the value of describing this factor structure in a new population. There are hints at this. A paper by Abubakar and Fischer on factor structure on an English administration of the instrument in a Kenyan population is only mentioned in the discussion.

Response: the previous paper by Abubakar and Fischer is now contextualized in the introduction within the current study’s aims. Prior work on tests of models the GHQ have been explained in the introduction.

Comments: On line 17 a paper from Goa is cited after a sentence indicating that the GHQ-2 has been translated in many places but no other papers are mentioned. The introduction should further establish what is already known and not known about translations of this instrument and its use in Kenya as well as how the present analysis contributes to

Response: We briefly acknowledge the extent of translation of the GHQ-12 to reflect the widespread use of the measure despite ongoing debate relating to its factorial structure. We then explain current arguments made in relation to the factorial structure of the GHQ-12 and position the added knowledge that this current study sets out to achieve (See lines 26 – 57)

Comments: Relatedly, a description of the translation process and inclusion of the Kiswahili items seems warranted. (Perhaps more indirectly, it seems there should be acknowledgment somewhere, if even for ethical approval, of the Kenyan institution that was involved in this research.

Response: An ethics statement with this information has been provided at the end. The translation process has been detailed in the methods (lines 128-132)

Comments: The other aim is primarily related to comparing multidimensional models described in the literature, especially clarifying the importance of a two-factor wording valence instrument. A deeper engagement with this literature would better clarify why the particular models that have been selected are needed to answer the question about multidimensionality. The authors point to Hystad & Johnsen “for an extensive review” but do not relay the findings of this study despite this paper using identical models to that one and following a similar structure. What does this analysis offer uniquely beyond the findings of Hystad & Johnsen? Or why does it not include all sorts of bifactor models like Tomás, Gutiérrez et al.? In contextualizing the present debate about existing factor solutions, it would be helpful if the authors could offer a summary of the literature that reaches beyond citations of the papers the models are derived from and grapples with the substance of this debate. This should help to establish a clear sense of why all of these models need to be included and what would be hypothesized and rejected if any one of them were to be confirmed in the analysis.

Response: The outstanding question addressed by the current paper is detailed in the introduction section (lines 78-94). The reasons for selecting target models is now explained in the data analysis plan (Lines 133-157) preceded by a summary of these detailed in the introduction section (lines 32-57).

Comments: Related to this second aim, it would be useful to further clarify how the hypotheses from the introduction are operationalized in the methods. Of particular concern is the use of comparative model fit indices. I am by no means an expert in the field of SEM. I am aware, however, that some literature has shown that bifactor models can overfit data (e.g. Mansolf & Reise, 2016). This limitation should be addressed if adjudicating between models with these indices. If that is not the purpose of the fit comparison, the role of the fit indices needs to be described more clearly.

Response: The introduction described the aims of the current study (lines 78-94). The data analysis plan now stipulates the rationale for selection of models based on the aims described in the introduction. The role of the fit indices has been elaborated in lines 158-170. To this end, we have not solely relied on model fit indices and additionally computed omega reliability estimates to validate the suitability of the bifactor model. We briefly acknowledge this in the discussion section as follows:

“To this end, we acknowledge the use of model fit indices as a means for honing upon a parsimonious explanation only as opposed arriving at definitive diagnostic conclusions (Xia & Yang, 2019). Importantly, we followed up the observation that the bifactor model may offer the most sufficient explanation of the GHQ-12 through computation of specific omega estimates”

Comment: I think inclusion of common indices AIC and BIC would also be useful.

Response: AIC and BIC values are not reported as these are commonly reported when investigating non-nested models which is not the focus of the current study.

Comments: In the discussion several claims are made that go beyond the data presented for example, on line 174, that “the findings suggest that the Swahili version of the GHQ-12 may be administered in a Kenyan population and interpreted in a similar manner to the English version of this measure.”

Again, on line 191, “and its validity in measuring psychological distress in the cultural context of Kenya is justified. My more limited interpretation is that the results to analysis demonstrate the configural invariance of the instrument in this Kenyan context. Glancing briefly at the paper by Romppel they claim on line 179 “suggested that GHQ-12 item clustering may differ across cultural contexts”, it appears that this study actually also found configural invariance but also performed other analyses (including item-level analyses) not done in this study that enabled them to conclude measurement invariance was unlikely. Could the authors instead offer a careful statement of what this study does demonstrate, engagement with external literature considering these limitations, and clear adherence to the limited aims made in the introduction?

Comments: This is an interesting article of strong technical quality. It describes a confirmatory factor analysis of the Swahili version of the GHQ-12 administered in a large sample of females affected by gender-based violence. The analyses, subsequent results and conclusions drawn from them are convincing.

However, the authors could improve the paper and make it more interesting and suitable for the readers of GMH by adding some context to the aim of the study and the interpretation of the findings. In addition, the authors should carefully read through and correct language and grammar errors occurring throughout the manuscript.
