According to Fricker’s (2007) seminal account, an epistemic injustice is done when, based on prejudice, a hearer ascribes to a speaker a level of credibility below what they deserve. When prejudice results in credibility excess, however, Fricker contends no similar injustice takes place. In this paper, I will challenge the second of these claims. Using a modified version of Zollman’s (2007) two-armed bandit model, I will show how the systematic over-ascription of credibility within a dominant group can produce epistemic advantages for that group relative to non-group members.

Arctic governance has entered a period of turmoil following the March 2022 Arctic Council pause in operations and, subsequently, the strained relations between the member states. As climate change dramatically alters the Arctic environment, opening the region to new economic possibilities and more global attention, the need for cooperation is greater than ever. This article examines the current geopolitical and environmental pressures that are undermining the Arctic Council’s legitimacy and operations at a critical juncture in Arctic governance. It contends that the Arctic Council must rethink how it engages with Arctic Council observers and the wider global community to ensure that pressing ecological, economic, and social issues are addressed judiciously to prevent potentially irreparable harm in the region. Specifically, the case is made that a shift to “inclusive regionalism” could secure the Arctic Council’s position as the pre-eminent forum to address Arctic issues and to re-establish the spirit of collaboration that reigned for a quarter century.

Background: Duchenne muscular dystrophy (DMD) causes progressive muscle wasting. The Canadian Neuromuscular Disease Registry (CNDR) previously developed a comprehensive DMD dataset in accordance with the International Classification of Functioning, Disability, and Health (ICF). Our objective was to develop a brief ICF core set that best aligns with the priorities of individuals and families affected by DMD. Methods: A literature review of best practices was prepared and reviewed by a multidisciplinary team at the CNDR. The entire process involved patient and parent input and participation and was compliant with the World Health Organization guideline to develop brief ICF core sets. Results: An eight step process was developed. In brief, these included multi-stakeholder consensus meetings, ranking surveys, mapping to international standards, further consensus meeting, evaluation of clinical feasibility in multidisciplinary clinics across Canada, an integrated literature review, and development of a finalized brief ICF core set for DMD. Conclusions: The process of identifying the priorities of those living with DMD using the brief ICF core set will support post-marketing surveillance of novel therapies. The next step in this project will be to identify the specific outcome measures that best align with the brief ICF core set for DMD, for their eventual inclusion in the CNDR registry.

Background: LC-FAOD may be missed in neuromuscular (NM) clinics due to its rarity and absence from common NM genetic panels. The Canadian Neuromuscular Disease Registry (CNDR) collects real-world patient data and includes a network of clinician-investigators. Our objective was to inform future registry work by evaluating diagnosis pathways for LC-FAOD patients and estimating the number followed at Canadian NM clinics. Methods: A questionnaire was developed with an expert committee and circulated to 111 CNDR-affiliated NM neurologists. Results: 12 neurologists in 5 provinces, primarily adult-treating (n=8) completed the survey (10.8% response rate). Eleven (91.7%) practiced for >10 years. Agreement trends existed between definition of, and tests to evaluate, rhabdomyolysis. Four clinics routinely follow LC-FAOD patients. In the last 1-2 years, respondents diagnosed approximately 91 patients with LC-FAOD (mean=7.5 per clinic). 83.3% never received continuing education on LC-FAOD, though 75% indicated interest in expert-led webinars. Further data will be presented. Conclusions: Low sample size limits conclusions about LC-FAOD clinical trends. Results suggest LC-FAOD may be under-diagnosed or not routinely followed by NM specialists, limiting viability of an LC-FAOD registry. Practitioners may be interested in LC-FAOD-specific education. Future work could include collaboration with metabolic geneticists on education initiatives to raise awareness and improve care for these patients.

Promoting healthy snacking is important in addressing malnutrition, overweight and obesity among an ageing population. However, little is known about the factors underlying snacking behaviour in older adults. The present study aimed to explore within- and between-person associations between determinants (i.e. intention, visibility of snacks, social modelling and emotions) and snacking behaviours (i.e. decision to snack, health factor of the snack and portion size) in older adults (60+). Conducting a two-part intensive longitudinal design, data were analysed from forty-eight healthy older adults consisting of (1) an event-based self-report ecological momentary assessment (EMA) diary every time they had a snack and (2) a time-based EMA questionnaire on their phone five times per day. Analysis through generalised linear mixed models indicated that higher intention to snack healthily leads to healthier snacking while higher levels of social modelling and cheerfulness promote unhealthier choices within individuals. At the between-person level, similar results were found for intention and social modelling. Visibility of a snack increased portion size at both a within- and between-person level, while the intention to eat a healthy snack only increased portion size at the between-person level. No associations were found between the decision to snack and all determinants. This is the first study to investigate both within- and between-person associations between time-varying determinants and snacking in older adults. Such information holds the potential for incorporation into just-in-time adaptive interventions, allowing for personalised tailoring, more effective promotion of healthier snacking behaviours and thus pursuing the challenge of healthy ageing.

Many comics are aesthetically trashy: They are immediately grasped and easily available. Historically, this trashiness is lobbed as an aesthetic defect of many comics, a defect for both their production and their appreciation. To defend these comics, some point to non-aesthetic values, like sociality. I argue that there is aesthetic value to these comics, and that it lies precisely in their trashy characteristics: their immediacy and availability. Many comics have these characteristics because many comics are cartooned. The immediacy of cartooning is precisely what makes so many ordinary comics beautiful in an ordinary way.

The Hindu Raj region of northern Pakistan is situated between the Karakoram to the east and the Hindu Kush to the west. Both the Karakoram and the Hindu Kush are better studied and have well-documented, distinct geological histories. Investigation of the Hindu Raj region has been mainly limited to reconnaissance exploration and as such little is known about its tectonometamorphic history and whether that history is similar to its neighbouring areas. Analysis of new specimens collected along the Yasin Valley within the Hindu Raj region outline mid-to-Late Cretaceous pluton emplacement (ca. 105 and 95 Ma). Some of those plutonic rocks were metamorphosed to ∼750 ± 30 °C and 0.65 ± 0.05 GPa during the ca. 80–75 Ma docking of the Kohistan arc. A record of this collisional event is well-preserved to the west in the Hindu Kush and variably so to the east in the Hunza Karakoram. A subsequent, ca. 61 Ma, thermal event is partially preserved in Rb–Sr geochronology from the Hindu Raj, which overlaps with sillimanite-grade metamorphism in the Hunza portion of the Karakoram region to the east. Finally, apatite U–Pb and in situ Rb–Sr both record a late Eocene thermal/fluid event likely related to the India-Asia collision. These new data outline a complex geological history within the Hindu Raj, one that shares similarities with both adjacent regions. The information about the tectonometamorphic development of the Hindu Raj is important to gaining a detailed view of the geological characteristics of the southern Asian margin prior to the India-Asia collision.

Background: ALS is a progressive neurodegenerative disease without a cure and limited treatment options. Edaravone, a free radical scavenger, was shown to slow disease progression in a select group of patients with ALS over 6 months; however, the effect on survival was not investigated in randomized trials. The objective of this study is to describe real-world survival effectiveness over a longer timeframe. Methods: This retrospective cohort study included patients with ALS across Canada with symptom onset up to three years. Those with a minimum 6-month edaravone exposure between 2017 and 2022 were enrolled in the interventional arm, and those without formed the control arm. The primary outcome of tracheostomy-free survival was compared between the two groups, accounting for age, sex, ALS-disease progression rate, disease duration, pulmonary vital capacity, bulbar ALS-onset, and presence of frontotemporal dementia or C9ORF72 mutation using inverse propensity treatment weights. Results: 182 patients with mean ± SD age 60±11 years were enrolled in the edaravone arm and 860 in the control arm (mean ± SD age 63±12 years). Mean ± SD time from onset to edaravone initiation was 18±10 months. Tracheostomy-free survival will be calculated. Conclusions: This study will provide evidence for edaravone effectiveness on tracheostomy-free survival in patients with ALS.

Background: Systemic corticosteroids (CS) are first-line therapy for many neuromuscular diseases. Although long-term use is associated with many adverse effects, guidelines regarding prevention and management of CS-induced (CSI) complications in neurology are lacking, introducing potential practice variation. We aimed to evaluate Canadian neuromuscular neurologist practices for screening and management of CSI complications. Methods: A web-based anonymous questionnaire was disseminated to 99 Canadian neuromuscular neurologists addressing the screening, prevention, monitoring and treatment of CSI adverse effects, such as infection and osteoporosis. Results: 71% completed the survey. Of those, 52% perform screening blood work prior to initiating CS, 56.3% screen for infections, and 18.3% for osteoporosis. The majority monitor glycemic control and blood pressure. 28.6% never use pneumocystis jiroveci pneumonia prophylaxis, and 28.6% routinely recommend vaccinations prior to CS initiation (most commonly influenza and pneumococcal). 80.0% recommended calcium supplementation to prevent osteoporosis. 36% were unaware of any existing guidelines for preventing CSI complications, and 91% endorsed a need for neurology-specific guidelines. Additional data and details of responses will be presented. Conclusions: There is substantial variability in the management of CSI adverse effects among neuromuscular neurologists. This suggests a need for neurology-specific guidelines to help standardize practice.

Howson (2000) contends that the No-Miracles argument fails as an argument in support of scientific realism because it commits the base-rate fallacy. Psillos (2009) has defended the argument by appealing to cases that involve conditional probabilities but where base-rate information can properly be ignored. I show that Psillos’s defense of the No-Miracles argument is inadequate and that the prospects for a purely probabilistic formulation of the argument are dim. I end by considering whether interpreting the argument as an inference to the best explanation might better serve the scientific realist, concluding that such an approach would limit the realist’s ability to effectively counter the Pessimistic Induction.
