To compare rates of clinical response in children with Clostridioides difficile infection (CDI) treated with metronidazole vs vancomycin.
Design:
Retrospective cohort study was performed as a secondary analysis of a previously established prospective cohort of hospitalized children with CDI. For 187 participants 2–17 years of age who were treated with metronidazole and/or vancomycin, the primary outcome of clinical response (defined as resolution of diarrhea within 5 days of treatment initiation) was identified retrospectively. Baseline variables associated with the primary outcome were included in a logistic regression propensity score model estimating the likelihood of receiving metronidazole vs vancomycin. Logistic regression using inverse probability of treatment weighting (IPTW) was used to estimate the effect of treatment on clinical response.
Results:
One hundred seven subjects received metronidazole and 80 subjects received vancomycin as primary treatment. There was no univariable association between treatment group and clinical response; 78.30% (N = 83) of the metronidazole treatment group and 78.75% (N = 63) of the vancomycin group achieved clinical response (P = 0.941). After adjustment using propensity scores with IPTW, the odds of a clinical response for participants who received metronidazole was 0.554 (95% CI: 0.272, 1.131) times the odds of those who received vancomycin (P = 0.105).
Conclusions:
In this observational cohort study of pediatric inpatients with CDI, the rate of resolution of diarrhea after 5 days of treatment did not differ among children who received metronidazole vs vancomycin.
Passions and affections, notwithstanding their potential and capacity for disorder, were fundamental to the workings of the early modern world and lay at the centre of ethical life. Yet, most scholarship ignores the inseparability of emotion and ethics for early modern society. This article reviews the current state of scholarship in three fields: history of philosophy, history of emotions, and English society and governance, highlighting the assumptions and arguments which have led to the mistaken severance of affect from morality, the exclusion of ethics from histories of emotion, and the omission of emotion from studies of English power and authority. It argues that this separation is anachronistic and impairs our ability to understand governance in early modern England. This was a world of fused affect and morality which underpinned particular notions of power and authority. That governance was located within an ethical/emotive paradigm has been given insufficient attention.
In the summer of 2022, renewed excavations were conducted at the site of Gird-i Begum in the Shahrizor Plain, Kurdistan Region of Iraq. The project aims to examine patterns of mobility, exchange, and resource acquisition practiced by the inhabitants of Gird-i Begum over time. To do so we re-examine the stratigraphic sequence, with a focus on continuities and breaks in site occupation. While the earliest occupation of the site dates to the Halaf period, with limited occupation traces attested during the Ubaid period, the settlement appears to have reached its largest extent during the Late Chalcolithic, which was one of the main foci of this year’s investigations. Our excavations confirmed the presence of Late Chalcolithic levels on the Upper Mound, with an analysis of the pottery as well as 14C dates indicating a chronological span from LC 3 to early LC 4. Work on the Lower Mound brought to light a substantial and previously undocumented Early Bronze Age occupation phase in the early third millennium B.C.E. The massive presence of snails characterizing layers of both periods additionally raised intriguing questions about subsistence strategies and potential crisis at the site.
Klebsiella pneumoniae are opportunistic pathogens which can cause mastitis in dairy cattle. K. pneumoniae mastitis often has a poor cure rate and can lead to the development of chronic infection, which has an impact on both health and production. However, there are few studies which aim to fully characterize K. pneumoniae by whole-genome sequencing from bovine mastitis cases. Here, K. pneumoniae isolates associated with mastitis in dairy cattle were identified using matrix-assisted laser desorption/ionisation time-of-flight mass spectrometry (MALDI-TOF MS) and whole-genome sequencing. Furthermore, whole-genome sequence data were used for phylogenetic analyses and both virulence and antimicrobial resistance (AMR) prediction, in parallel with phenotypic AMR testing. Forty-two isolates identified as K. pneumoniae were subject to whole-genome sequencing, with 31 multi-locus sequence types being observed, suggesting the source of these isolates was likely environmental. Isolates were examined for key virulence determinants encoding acquired siderophores, colibactin, and hypermucoidy. The majority of these were absent, except for ybST (encoding yersiniabactin) which was present in six isolates. Across the dataset, there were notable levels of phenotypic AMR against streptomycin (26.2%) and tetracycline (19%), and intermediate susceptibility to cephalexin (26.2%) and neomycin (21.4%). Of importance was the detection of two ESBL-producing isolates, which demonstrated multi-drug resistance to amoxicillin-clavulanic acid, streptomycin, tetracycline, cefotaxime, cephalexin, and cefquinome.

Objectives: Patients with mild cognitive impairment (MCI) employ compensatory cognitive processes to maintain independence in day-to-day functioning as compared to patients with Alzheimer’s dementia (AD). The dorsolateral prefrontal cortex (DLFPC) supports cognitive compensation in normal aging and MCI. Using Paired Associative Stimulation combined with Electroencephalography (PAS-EEG) we have previously shown that patients with AD have impaired DLPFC plasticity compared to healthy control (HC) individuals. The aim of this study is to examine whether DLPFC plasticity in individuals with MCI is preserved compared to those with AD and HC, serving as a potential mechanism underlying cognitive compensation in MCI.
Methods: We analyzed a combined cross-sectional data of 47 AD, 16 MCI, and 40 HC participants from three different studies that assessed their DLPFC plasticity using PAS-EEG. PAS-EEG assesses DLPFC plasticity via the induction of Long Term Potentiation (LTP)-like activity, thereby referred to as PAS-LTP. Using multiple regression, we compared PAS-LTP in MCI to PAS-LTP in AD and HCs, after adjusting for age andgender.
Results: Among the 47 participants with AD (mean [SD] age = 75.3 [7] years), 29 were women and 18 were men; among the 16 participants with MCI (mean [SD] age = 74.8 [6] years), 11 were women and 5 were men; and among the 40 HCs (mean [SD] age = 76.4 [5.1] years), 22 were women and 18 were men. After adjusting for age and gender, there was an impact of diagnostic group on PAS-LTP [F (2,95) = 4.19, p = 0.018, between-group comparison η2 = 0.81]. Post-hoc comparisons showed that participants with MCI had a higher PAS-LTP (mean [SD] = 1.31 [0.49]) than those with AD (mean [SD] = 1.09 [0.28]) (Bonferroni corrected p = 0.042) but not different from PAS-LTP in HCs (mean [SD] = 1.25 [0.33]) (Bonferroni corrected p = 1.0).
Conclusions: Our findings indicate that plasticity is preserved in the DLPFC among individuals with MCI, supporting the hypothesis that DLPFC plasticity contributes to cognitive compensation towards delaying progression to AD. Thus, further enhancement of longer preservation of DLPFC plasticity in individuals with MCI could further delay the onset of AD in this population.

This chapter examines the conceptualization and measurement of contact phenomena in the context of bilingualism across various languages. The goal of the chapter is to account for various phonetic contact phenomena in sociolinguistic analysis, as well as providing context for elaborating on quantitative methodologies in sociophonetic contact linguistics. More specifically, the chapter provides a detailed account of global phenomena in modern natural speech contexts, as well as an up-to-date examination of quantitative methods in the field of sociolinguistics. The first section provides a background of theoretical concepts important to the understanding of sociophonetic contact in the formation of sound systems. The following sections focus on several key social factors that play a major part in the sociolinguistic approach to bilingual phonetics and phonology, including language dominance and age of acquisition at the segmental and the suprasegmental levels, as well as topics of language attitudes and perception, and typical quantitative methods used in sociolinguistics.
The Market Studies discipline represents an effort to understand and unscramble the entangled knot of practices, agents, devices and infrastructures that constitute markets. Over the past two decades it has become an interdisciplinary field, with scholars from sociology, marketing, management, organization studies, economics, anthropology, geography and design – an epistemic community (Knorr Cetina 1999) studying the emergence, transformation and innovation of markets (Araujo, Finch and Kjellberg 2010; Kjellberg and Helgesson 2006, 2007). The Market Studies field understands markets as socio-material, technical, political and economic forms of organizing collectives of distributed, heterogeneous sets of expertise – not only of exchange but also of society (Çalışkan and Callon 2009; Callon 1998).
Recent work in social epistemology has drawn attention to various problematic social epistemic phenomena that are common within online networks. Nguyen (2020) argues that it is important to distinguish epistemic bubbles from echo chambers. An epistemic bubble is an information structure that merely lacks information or sources that would be relevant or important to the user. An echo chamber is a structure in which dissenting opinions are, not necessarily absent, but actively undermined, for example by instilling attitudes of distrust towards their adherents. Because of this, echo chambers are thought to be especially difficult to escape. In contrast, according to Nguyen, it is relatively easy to shatter an epistemic bubble: one simply introduces the missing information. In this paper, I argue that it is more difficult to shatter an epistemic bubble than has been recognised in the literature. The reason for this is the relationship between epistemic bubbles and interpretative resources. Despite their epistemic drawbacks, it is comparatively easy to gain knowledge from sources inside one's epistemic bubble because agents within a bubble share common ground. In contrast, it can be very difficult to gain knowledge from sources outside of one's bubble because interlocutors on the outside are less likely to have the shared context needed to facilitate communicative success. I argue that this problem suggests a different way to understand the nature of epistemic bubbles and our prospects for escaping them.
Following an outbreak of Salmonella Typhimurium in Wales in July 2021 associated with sheep meat and offal, further genetically related cases were detected across the UK. Cases were UK residents with laboratory-confirmed Salmonella Typhimurium in the same 5-single-nucleotide polymorphism (SNP) single-linkage cluster with specimen date between 01/08/2021–2031/12/2022. We described cases using routine (UK) and enhanced (Wales only) surveillance data. Exposures in cases in Wales were compared with non-Typhimurium Salmonella case–controls. Environmental Health Practitioners and the Food Standards Agency investigated supply chains of food premises reported by ≥2 cases. Animal, carcass, and environmental samples taken for diagnostic or monitoring purposes for gastrointestinal pathogens were included in microbiological investigations. We identified 142 cases: 75% in England, 23% in Wales and 3% in Scotland. Median age was 32 years, and 59% were male. Direct contact with sheep was associated with becoming a case (aOR: 14, 95%CI: 1.4–145) but reported by few (6/32 cases). No single food item, premises, or supplier linked all cases. Multi-agency collaboration enabled the identification of isolates in the same 5-SNP single-linkage cluster from a sheep carcass at an English abattoir and in ruminant, wildlife, poultry, and environmental samples, suggesting multiple vehicles and pathways of infection.
Recent reviews and meta-analyses of metacognitive therapy for schizophrenia-spectrum disorder (SSD) have included uncontrolled studies, single-session interventions, and/or analyses limited to a single form of metacognitive therapy. We sought to evaluate the efficacy of metacognitive therapies more broadly based on controlled trials (CT) of sustained treatments. We conducted a pre-registered meta-analysis of controlled trials that investigated the effects of meta-cognitive therapies on primary positive symptom outcomes, and secondary symptom, function and/or insight measures. Electronic databases were searched up to March 2022 using variants of the keywords, ‘metacognitive therapy’, ‘schizophrenia’, and ‘controlled trial’. Studies were identified and screened according to PRISMA guidelines. Outcomes were assessed with random effects models and sample, intervention, and study quality indices were investigated as potential moderators. Our search identified 44 unique CTs with usable data from 2423 participants. Data were extracted by four investigators with reliability >98%. Results revealed that metacognitive therapies produced significant small-to-moderate effects on delusions (g = 0.32), positive symptoms (g = 0.30) and psychosocial function (g = 0.31), and significant, small effects on cognitive bias (g = 0.25), negative symptoms (g = 0.24), clinical insight (g = 0.29), and social cognition (g = 0.27). Findings were robust in the face of sample differences in age, education, gender, antipsychotic dosage, and duration of illness. Except for social cognition and negative symptoms, effects were evident even in the most rigorous study designs. Thus, results suggest that metacognitive therapies for SSD benefit people, and these benefits transfer to function and illness insight. Future research should modify existing treatments to increase the magnitude of treatment benefits.
