A paradigm shift: relationships in trauma-informed mental health services

Trauma-informed approaches emerged partly in response to research demonstrating that trauma is widespread across society, that it is highly correlated with mental health and that this is a costly public health issue. The fundamental shift in providing support using a trauma-informed approach is to move from thinking ‘What is wrong with you?’ to considering ‘What happened to you?’. This article, authored by trauma survivors and service providers, describes trauma-informed approaches to mental healthcare, why they are needed and how barriers can be overcome so that they can be implemented as an organisational change process. It also describes how past trauma can be understood as the cause of mental distress for many service users, how service users can be retraumatised by ‘trauma-uninformed’ staff and how staff can experience vicariously the service user's trauma and can themselves be traumatised by practices such as restraint and seclusion. Trauma-informed mental healthcare offers opportunities to improve service users' experiences, improve working environments for staff, increase job satisfaction and reduce stress levels by improving the relationships between staff and patients through greater understanding, respect and trust.

Aims and method
This paper describes the process of setting up and the early results from a new liaison psychiatry service in primary care for people identified as frequent general practice attenders with long-term conditions or medically unexplained symptoms. Using a rapid evidence synthesis, we identified existing service models, mechanisms to identify and refer patients, and outcomes for the service. Considering this evidence, with local contingencies we defined options and resources. We agreed a model to set up a service in three diverse general practices. An evaluation explored the feasibility of the service and of collecting data for clinical, service and economic outcomes.
ResultsHigh levels of patient and staff satisfaction, and reductions in the utilisation of primary and secondary healthcare, with associated cost savings are reported.
Clinical implicationsA multidisciplinary liaison psychiatry service integrated in primary care is feasible and may be evaluated using routinely collected data.

In 2008 it became policy that all those on the care programme approach were assessed for sexual violence/abuse. The implementation of this policy was assessed 8 years on. The findings of a survey and data request to Health and Social Care Information Centre are disappointing. We argue that this important initiative needs to be reinvigorated.

Summary
Drug metabolism is a critical determinant of therapeutic and adverse effects of many psychotropic drugs. This chapter reviews the basic concepts and definitions pertinent to pharmacogenetics of psychotropic drug metabolism from a clinical psychiatry perspective. It discusses the CYP2D6 genetic polymorphism as a classic example of a monogenic variation in drug metabolism and as a high-affinity and low-capacity elimination route. In addition to genetic contribution to interindividual differences in drug metabolism, the chapter reviews various sources of intraindividual variations and gene-environment interactions of relevance for psychopharmacology. It highlights some of the future research directions in psychotropic drug metabolism and emphasizes the need to evaluate genetic variability in drug metabolism in conjunction with other genes encoding drug transporters, receptors and ion channels, which can all influence individual's risk for adverse drug reactions or therapeutic failure.
