Normal and anorexic subjects' estimates of their own body width were obtained. The estimates were reliable measures, but they did not correlate with the subjects' actual body widths. The implications of this finding especially when relating anorexic patients' self-perceptions to their clinical outcome, are discussed. It seems preferable to use such estimates untransformed by division with the subjects' actual body widths.

The multi-disciplinary team of Becker and colleagues elegantly present their findings addressing aspects of the impact on Fijian schoolgirls of exposure to Western television (Becker et al, 2002, this issue). These findings support the notion that such exposure has generated disordered eating, underlying body dissatisfaction and intergenerational conflicts within the family that may, in part, be fuelling the process. Focusing on the expected escalation of such disordered eating in this population under these circumstances, their study was naturalistic in capitalising on the recent introduction of television to Fiji, with the first survey of these schoolgirls taking place within 1 month of its advent. The second survey was 3 years later in 1998. The authors point out that the traditional Fijian culture has ‘supported robust appetites and body shapes'.

Background
Recognition of the additional social handicaps and distress that people with mental illnesses experience as a result of prejudice.
AimsTo determine opinions of the British adult population concerning those with mental illnesses as baseline data for a campaign to combat stigmatisation.
MethodSurvey of adults (n=1737 interviewed; 65% response) regarding seven types of common mental disorders. Responses evaluated concerned eight specified perceptions.
ResultsRespondents commonly perceived people with schizophrenia, alcoholism and drug addiction as unpredictable and dangerous. The two latter conditions were also viewed as self-inflicted. People with any of the seven disorders were perceived as hard to talk with. Opinions about effects of treatment and prognosis suggested reasonable knowledge. About half the respondents reported knowing someone with a mental illness.
ConclusionsNegative opinions indiscriminately overemphasise social handicaps that can accompany mental disorders. They contribute to social isolation, distress and difficulties in employment faced by sufferers. A campaign against stigma should take account of the differences in opinions about the seven disorders studied.
Long-Term Mortality in Anorexia Nervosa: A 20–Year Follow-Up of the St George's and Aberdeen Cohorts

Two cohorts of anorexia nervosa patients were followed up for a mean of 20 years. All except 4% of each cohort was traced. The crude mortalities were: St George's, 4%; Aberdeen, 13%. The SMRs were: St George's, 136; Aberdeen, 471. If the untraced were assumed to be dead, crude mortalities were 7.6% and 15.9% respectively, and SMRs were 276 and 592 respectively. Causes of death were complications of the illness and suicide. Medical treatment may reduce early mortality, while comprehensive medical and psychotherapeutic treatment may reduce late mortality.

The nutritional disturbance in anorexia nervosa almost invariably leads to striking physiological changes, which include amenorrhoea, bradycardia, hypotension and relative hypothermia with reversal of the usual diurnal temperature rhythm (Bliss and Branch, 1960; Mayer-Gross et al., 1960; Crisp and Roberts, 1962; Crisp, 1967a). Crisp (1965a, 1967a) has commented on the characteristic and excessive alertness, restlessness and insomnia displayed by anorexia nervosa patients, whom he regards as showing a specific type of malnutrition associated with carbohydrate starvation. Russell (1967) has also recently demonstrated that patients with this illness restrict especially their carbohydrate intake.
